Centella Asiatica Extract Containing Bilayered Electrospun Wound Dressing
2020
Innovative and bioactive wound dressings prepared by electrospinning mimicking the native structure of the extracellular matrix (ECM) have gained significant interest as an alternative to conventional wound care applications. In this study, bilayered wound dressing material was produced by sequential electrospinning of quaternized poly(4-vinyl pyridine) (upper layer) on the Centella Asiatica (CA) extract containing electrospun poly(D, L-lactide-co-glycolide) (PLGA)/poly(3-hydroxybutyrate-co-3-hydroxy valerate) (PHBV) blend membrane (lower layer). Scanning electron microscopy (SEM) was utilized to show a uniform and bead-free fiber structure of electrospun membranes. The average diameter of CA extract containing electrospun PLGA/PHBV blend membrane was calculated 0.471±0.11 µm, whereas the average fiber diameter of electrospun poly(Q-VP) membranes was in the range of 0.460±0.057 µm. Chemical, thermal, mechanical properties, and adsorption capacity of electrospun membranes, as well as the cumulative release of CA from the electrospun PLGA/PHBV membrane, were investigated. Viability, adhesion, and attachment of human fibroblast cells on the electrospun membranes on pre-set days were evaluated by the colorimetric CellTiter 96® Aqueous One Solution Cell Proliferation Assay (MTS assay) and SEM. Results revealed that CA loaded bilayered electrospun wound dressing showed promoted attachment and proliferation of fibroblasts. Hence, it can be concluded that CA extract containing bilayered electrospun wound dressing prepared in this study has a promising potential for wound healing applications.

The views of associate degree students from child development on distance education after the earthquake

This study aims to investigate the satisfaction and experiences of associate degree students from the child development department on distance education after the earthquake in Türkiye. The research was carried out in a mixed method embedded design with the participation of 133 associate degree students who studied in the child development department in a vocational school affiliated with Bursa Uludağ University in the spring term of the 2022-2023 academic year. The quantitate data were collected through \"The Distance Education Satisfaction Scale of University Students.” The qualitative data were obtained through a semi-structured interview with 20 students from the same study group. Descriptive statistics, Independent samples t-test, Mann-Whitney U test and Kruskal-Wallis H Test, and content analysis were used to analyze the data obtained. According to the findings, online education was less effective than face-to-face education due to the earthquake. The student’s satisfaction with distance education differs significantly depending on having technological tools. The students stated that they had difficulties due to the adverse psychological effects of the earthquake and the infrastructure problems. It was determined that distance education's most significant advantage is having a flexible environment and listening to the course recordings. However, they were disadvantaged in not making enough applications during the lessons in online education.

Annelerin kişisel değerleri ile beş-alti yaş çocuklarinin değer kazanimlari arasindaki ilişkiler

Bu araştırmanın amacı; annelerin kişisel değerleri ile 5-6 yaş grubu çocuklarının değer kazanımları ve çocuklarına aktarmak istedikleri değerler arasındaki ilişkileri saptamaktır. Araştırmanın çalışma grubunu Bursa ilinde 11 farklı ilköğretim okulunun anasınıflarında okul öncesi eğitimi almakta olan toplam 390 çocuk ve anneleri oluşturmuştur. Buna göre, araştırmadaki annelerin kişisel değerlerini saptamak için Schwartz ve diğerleri (2001) tarafından geliştirilen Portre Değerler Anketi; çocuklarına aktarmak istedikleri değerleri saptamak için Demirutku (2007) tarafından geliştirilen Değer Aktarımı Anketi uygulanmıştır. Ayrıca araştırmadaki çocukların değer önceliklerini saptamak için Döring (2008) tarafından geliştirilen Çocuklar İçin Resimlerle Değer Anketi (PBVS-C Ölçeği) uygulanmıştır. Araştırmada mevcut durumu tespit etmek için ilişkili tarama modeli kullanılmıştır. Çocuklar İçin Resimlerle Değer Anketi ile okul öncesi yaş dönemi çocukları ile Türkiye'de ilk kez çalışılacağı için öncelikle ölçeğin geçerlik ve güvenirlik analizleri yapılmıştır. Annelere uygulanan Portre Değerler Anketi'nden ve Değer Aktarımı Anketi'nden elde edilen annelerin kişisel değerlerinin, değişkenlere göre farklı olup olmadığını belirlemek amacıyla Kruskal Wallis-H testi kullanılmış, farklılıkların hangi gruplar arasında olduğunu belirlemek üzere de Mann Whitney-U testi kullanılmıştır. Daha sonra çocuklara uygulanan Çocuklar İçin Resimlerle Değer Anketi'nden elde edilen çocukların değer önceliklerinin, değişkenlere göre farklarını incelemek için yine aynı non parametrik testler kullanılmıştır. Ayrıca annelerin kişisel değerleri ile çocukların değer öncelikleri arasındaki ilişkiyi görebilmek için de veriler üzerinde Spearman's Rank Order Korelâsyon Testi uygulanmıştır. Anneler kendi kişisel değerlerinde en çok Evrensellik değerini önemserken, çocuklarına değer aktarma konusunda en çok Başarı değeri üzerinde durmuşlardır. Çocuklar da değer öncelikleri bakımından ilk sırada annelerin aktarmak istedikleri Başarı değerini belirtmişlerdir. Annelerin kişisel değerleri yaş ve çalışma durumuna göre farklılık gösterirken, çocukların değer önceliklerinde cinsiyet, annelerin eğitim durumu, çalışma durumu ve sosyoekonomik durum değişkenlerine göre anlamlı farklar ortaya çıkmıştır. Sonuç olarak, annelerin ve çocukların değerleri arasındaki ilişkiler incelendiğinde Geleneksellik değeri dışındaki diğer değerler arasında çocuklar değer önceliklerini belirlerken çoğunlukla annelerinin aktarmak istedikleri değerler yerine, annelerinin kişisel değerlerini model aldıkları saptanmıştır.

Dramanın çocukların dil gelişimine etkisinin incelendiği bu araştırma; okulöncesi eğitime devam eden altı yaş grubundaki çocuklara anadili etkinlikleri sırasında uygulanacak olan yaratıcı drama etkinliklerinin onların dil gelişimleri üzerindeki etkisini saptamak amacıyla gerçekleştirilmiştir.Araştırmanın evrenini Bursa ilinde ilköğretime bağlı bir anasınıfına ve bağımsız bir anaokuluna giden altı yaş grubundaki okulöncesi çocukları oluşturmuştur.Araştırmanın örneklemini ise, 2008?2009 eğitim-öğretim yılında Bursa ili Osmangazi ilçesine bağlı Emine Hasan Özatav İlköğretim Okulu'nun anasınıfından 30 erkek ve 22 kız çocuk, Mürüvvet Baş Anaokulu'ndan ise 26 erkek ve 22 kız çocuk olmak üzere toplam 100 çocuk oluşturmuştur.Bu araştırmada öntest-sontest kontrol gruplu deneysel yöntem kullanılmıştır. Araştırmada veri toplama aracı olarak Dunn (1959) tarafından geliştirilen ve Katz ve arkadaşları (1974) tarafından Türkçe'ye uyarlanan ?Peabody Resim-Kelime Testi ? hem öntest hem de sontest şeklinde kullanılmıştır. Ayrıca deney grubuna öntest sonrasında anadili etkinliklerine yönelik haftada bir gün olmak üzere toplam 12 haftalık drama uygulaması yapılmıştır. Verilerin analizinde t- Testi, Varyans analizi ve Tukeys HSD testi kullanılmış, ulaşılan sonuçlar yorumlanmıştır.Araştırmanın sonucunda deney ve kontrol gruplarının sontest puanları arasında deney grubu lehine anlamlı bir fark ortaya çıkmıştır. Bunun yanında deney grubundaki çocukların öntest ve sontest puanları arasında istatistikî açıdan anlamlı bir fark görülürken, kontrol grubundaki çocukların öntest ve sontest puanları arasında istatistikî açıdan anlamlı bir fark tespit edilememiştir. Ayrıca, deney grubundaki çocukları oluşturan anaokulu ve anasınıfı grupları kendi aralarında karşılaştırılmış ve deney grubundaki anasınıfına giden çocukların öntest puanları yüksekken, drama uygulaması sonunda deney grubundaki anaokuluna giden çocukların sontest puanlarının daha yüksek olduğu saptanmıştır. Buna ek olarak, çocukların öntest ve sontest puanları üzerinde cinsiyet değişkeninin herhangi bir etkisinin olmadığı saptanmıştır. Diğer bulgular araştırmada detaylı olarak sunulmuş ve yorumlanmıştır.

The effect of colchicine on micronutrients in children with newly diagnosed familial Mediterranean fever

Colchicine is an essential component of familial Mediterranean fever (FMF) treatment. It is suggested that it may affect vitamin B12 levels by affecting intestinal absorption. This research focused on examining the impact of colchicine therapy on anemia parameters, as well as levels of vitamin B12 and folate in patients who have recently been diagnosed with FMF.
This prospective cohort investigation assessed children diagnosed with FMF between October 2016 and February 2017, evaluating anemia parameters as well as vitamin B12 and folate levels at baseline and at the third and sixth months of colchicine therapy. The demographic data were recorded from patient files.
Forty-six children were involved. M694V was the most common mutation. The most common clinical findings were fever (87%), peritonitis (76.1%), and pleuritis (23.9%). The mean hemoglobin (p 0.027), iron (p < 0.001) levels were significantly increased, while the mean ferritin (p 0.002), vitamin B12 (p < 0.001) and folate (p 0.001) levels were decreased at the 6th month of colchicine treatment.
Although it has been determined that the suppressive effect of colchicine on inflammation improves anemia by increasing hemoglobin levels, it should also be kept in mind that it may cause a decrease in the levels of other variables that can cause anemia, such as vitamin B12 and folate.

Approach to switching biologics in juvenile idiopathic arthritis: a real-life experience

The primary aim of the treatment of juvenile idiopathic arthritis (JIA) is complete remission and minimizing the development of complications. Though biologic agents (BAs) provide better disease control, data related to BA switching patterns in JIA patients are scarce. This study aimed to determine the BA switching patterns in JIA patients. The study included children with JIA that received ≥ 1 BAs. Disease activity was evaluated based on the juvenile arthritis disease activity score 71 (JADAS71). Demographic data, clinical and laboratory findings, BA switching patterns, and the rationales for BA switching were recorded. The study included 177 (82 female and 95 male) JIA patients that received ≥ 1 BAs. Mean age at diagnosis of JIA was 9.1 ± 4.9 years. BAs were prescribed a median of 14 months (range: 3–66 months) after diagnosis. Among the 177 patients, 31 (17.5%) required BA switching a median 10.5 months (range: 3–38 months) after initiation of the first BA. Among all the BAs that were switched to after administration of the first BA, tocilizumab was the most commonly switched (n = 15). The most common reason for BA switching was inadequate response (n = 29). BAs were switched 2 times in 5 patients and 3 times in 1 patient. When patients that switched BAs 1 time were compared to those that switched 2 and 3 times there were not any differences in terms of JIA types, whereas those that switched 2 and 3 times had a higher active joint count and JADAS71 score after 6 months of initiation of the first BA. As some of the JIA patients could not achieve remission despite using the prescribed BA, BA switching was required. Herein, we provide data on both BA switching patterns and requirements, which may improve the management of JIA patients.

Comparison of the effects of submucosal hyaluronidase and dexamethasone on postoperative edema, pain, trismus, and infection following impacted third molar surgery

Background
Limiting postoperative edema, pain, trismus, and infection is crucial for smooth healing. This prospective, controlled clinical trial investigated and compared the effectiveness of dexamethasone and hyaluronidase in relieving these complications.
Methods
In groups Ia and IIa, 8 mg of dexamethasone and 150 IU of hyaluronidase were administered following the removal of impacted teeth, respectively. The contralateral sides (groups Ib and IIb) were determined as control groups. Edema, pain, trismus, and infection were clinically evaluated on the 1st, 2nd, 3rd, and 7th postoperative days.
Results
60 patients were enrolled in the study. Hyaluronidase provided significantly more edema relief than dexamethasone on the 1st, 2nd, 3rd, and 7th postoperative days (
P
= 0.031, 0.002, 0.000, and 0.009, respectively). No statistical difference was found between dexamethasone and hyaluronidase in VAS and rescue analgesic intake amount values for all time points. Hyaluronidase was more effective in reducing trismus than dexamethasone on the 2nd and 3rd postoperative days (
P
= 0.029, 0.024, respectively). Neither of the agents significantly increased the postoperative infection rate.
Conclusions
Hyaluronidase can be selected when postoperative excessive edema and trismus are anticipated. Dexamethasone may be a cost–effective option if postoperative pain control is merely targeted.

Drug reactions in children with rheumatic diseases receiving parenteral therapies: 9 years’ experience of a tertiary pediatric rheumatology center

Parenteral treatments (either subcutaneous or intravenous) are frequently used in rheumatology practice. In this study, drug side effects in patients who were followed up with a rheumatic disease and treated with parenteral administration methods were evaluated. The drug side effects in children who were followed up with a rheumatic disease and treated with parenteral treatments between 2010 and 2019 were recorded, retrospectively. All parenteral treatments are applied by a clinical nurse specialist (CNS) who is experienced in pediatric rheumatology for 10 years. Four hundred and thirteen patients were evaluated in this study. The mean age was 12.09 ± 5.05 years. Most of them were diagnosed with juvenile idiopathic arthritis (n = 317) and colchicine-resistant familial Mediterranean fever (n = 57). Among the patients, 287 was treated with methotrexate, 130 with etanercept, 90 with adalimumab, 71 with anakinra, 64 with canakinumab, 55 with tocilizumab, seven with rituximab, six with infliximab, and four with abatacept. Two of the patients had a history of drug allergy (ceftriaxone = 1, ranitidine = 1). The most common adverse reactions were as follows: nausea–vomiting in 52, rash in 11, itching in three, chest tightening in two, bruising in two, headache in two, and abdominal pain in one of the patients. Drug side effects were observed after an average of three (1–4) administrations. Antihistaminic and steroids (tocilizumab = 3, infliximab = 1, methotrexate = 1) were administered to five patients due to hypersensitivity reactions. Considering the possible side effects and preparation protocols of parenteral treatments, experienced physicians and nurses are required in the field.

Evaluation of childhood malignancies presenting with musculoskeletal manifestations from two different divisions: a multicenter study

Background. The aim of the study was to evaluate the approaches of pediatric rheumatologists and pediatric hematologists to patients with similar musculoskeletal (MSK) complaints and to highlight the differences that general pediatricians should consider when referring patients to these specialties. Methods. This is a cross-sectional study involving the patients who applied to pediatric rheumatology centers with MSK complaints and were diagnosed with malignancy, as well as patients who were followed up in pediatric hematology centers with a malignancy diagnosis, and had MSK complaints at the time of admission. Results. A total of 142 patients were enrolled in the study. Of these patients, 83 (58.4%) applied to pediatric rheumatology centers, and 59 (41.6%) applied to pediatric hematology centers. Acute lymphoblastic leukemia (ALL) was the most common diagnosis among the patients who applied to both centers, with 80 cases (56.3%). The median age of diagnosis was 87 (interquartile range, IQR: 48-140) months. The most common preliminary diagnosis in pediatric rheumatology centers was juvenile idiopathic arthritis (JIA), with 37 cases (44.5%). MSK involvement was mainly seen as arthralgia, and bone pain. While arthralgia (92.7%) was the most common complaint in rheumatology centers, bone pain (88.1%) was more common in hematology centers. The most frequently involved joints were the knee (62.9%), ankle (25.9%), hip (25%), and wrist (14%). The most common laboratory abnormalities were high lactate dehydrogenase (LDH), high C-reactive protein (CRP), anemia, and high erythrocyte sedimentation rate (ESR). Thrombocytopenia, neutropenia, and high LDH were statistically significantly more frequent in patients admitted to hematology centers than in patients admitted to rheumatology centers (p<0.001, p=0.014, p=0.028, respectively). Patients who applied to rheumatology clinics were found to have statistically significantly higher CRP levels (p=0.032). Conclusions. Malignancies may present with only MSK system complaints in childhood. Therefore, malignancies should be included in the differential diagnosis of patients presenting with MSK complaints.

Evaluation of Sinus Membrane Perforation in Osteotome Sinus Floor Elevation With and Without Grafting

The purpose of this study was to evaluate, using cone-beam computerized tomography (CBCT), the rate of sinus membrane perforation in osteotome sinus floor elevation (OSFE) performed with and without a graft material. Thirty patients with 52 OSFE sites were included in the study. Patients were divided into the control group (OSFE performed without graft material) and test groups (OSFE performed with autograft or xenograft). The autograft was harvested from the maxillary tuberosity using bone forceps. The xenograft was a commercial product originating from bovine bone. Graft volume was measured using the water displacement method. CBCT was performed at the initial examination and immediately after surgery to measure the residual bone height and to evaluate the endo-sinus bone gain and membrane perforation. The rate of sinus membrane perforation was 15.4%. Of the 52 OSFE procedures, 26.9% were performed without grafting and 34.6% and 38.5% were performed with autografts and xenografts, respectively. Membrane perforation was significantly higher in the autograft group ( P = .033). The median volume of graft materials was 0.3 mL. The difference in graft volume between the autograft and xenograft was not statistically significant ( P = .768). The mean endo-sinus bone gain was 6.55 mm in patients without membrane perforation and 8.71 mm in patients with membrane perforation; this difference was statistically significant ( P = .035). The volume and physical properties of graft materials are important factors in membrane perforation. Further clinical studies with larger and standardized samples are needed to confirm the effect of graft materials on sinus membrane perforation in OSFE.
