Incidence Rates of Medically Certified Long-term Sickness Absence Among Japanese Employees: A Focus on Sex Differences

Background: Long-term sickness absence (LTSA) is an important public health challenge, yet limited data exist on its incidence in Japan. We aimed to describe the incidence of all-cause and cause-specific LTSA by sex and age using 10-year data from a large Japanese working population, with a focus on sex differences.Methods: The study participants were employees from 16 worksites in the Japan Epidemiology Collaboration on Occupational Health Study between April 2012 and March 2022. LTSA, defined as sickness absence from work lasting 30 days or more, was recorded at each worksite. The causes of LTSA were classified using the International Classification of Diseases, 10th Revision. Incidence rates for all-cause and cause-specific LTSA were calculated based on sex and age.Results: During 730,391 and 161,513 person-years of follow-up, 6,518 and 1,866 spells of LTSA were recorded in males and females, respectively. Females had higher incidence rates of all-cause LTSA than males (115.5 vs 89.2 per 10,000 person-years), especially among females in their 20s and 30s. This was partly attributed to younger females experiencing higher LTSA incidence rates due to mental disorders, neoplasms, and pregnancy-related illnesses. In older age, females had higher LTSA incidence rates than males for musculoskeletal diseases and injuries/external causes, whereas LTSA incidence rates due to circulatory diseases were lower than those in males.Conclusion: The incidence of total and cause-specific LTSA varied greatly by sex and age, highlighting the need to consider employees’ characteristics in the prevention and management of LTSA.

Serum sodium and risk of hypertension: a cohort study

This study aimed to investigate the cross-sectional association between serum sodium and blood pressure at baseline and, more importantly, investigate the prospective association between serum sodium and the risk of incident hypertension. We used data from 1 638 workers aged 18 to 71 years who participated in 2015-2016 survey of the Furukawa Nutrition and Health Study. During a maximum follow-up of 3 years, 229 participants developed hypertension. Multivariable linear regression models were used to evaluate the cross-sectional association. The Cox proportional hazards model was used to calculate the hazard ratio and 95% confidence interval of incident hypertension across quartiles of serum sodium (137-140, 141-142, 143, and 144-147 mmol/L). In the cross-sectional analysis, we did not observe a significant association between serum sodium and blood pressure at baseline. In the prospective analysis, the multivariable-adjusted hazard ratios (95% confidence intervals) for incident hypertension were 1.03 (0.71-1.51), 1.35 (0.87-2.08), and 1.46 (0.97-2.20) for the upper three quartiles of the serum sodium levels compared with the lowest quartile (P for trend=0.02). When serum sodium was treated as a continuous variable, the multivariable-adjusted hazard ratio for hypertension was 1.10 (1.03-1.18). The association was slightly attenuated after additionally adjusting for baseline blood pressure, with a hazard ratio of 1.08 (1.00-1.16) for a 1 mmol/L increase in serum sodium. In conclusion, an elevated serum sodium level was associated with an increased risk of developing hypertension, suggesting that serum sodium could be a potential risk factor for hypertension.

Dietary fiber intake and depressive symptoms in Japanese employees: The Furukawa Nutrition and Health Study

Dietary fiber may play a favorable role in mood through gut microbiota, but epidemiologic evidence linking mood to dietary fiber intake is scarce in free-living populations. We investigated cross-sectionally the associations of dietary intakes of total, soluble, insoluble, and sources of fiber with depressive symptoms among Japanese workers.
Participants were 1977 employees ages 19–69 y. Dietary intake was assessed via a validated, brief self-administered diet history questionnaire. Depressive symptoms were assessed using the Center for Epidemiologic Studies Depression Scale. Logistic regression was used to estimate odds ratios of depressive symptoms adjusted for a range of dietary and non-dietary potential confounders.
Dietary fiber intake from vegetables and fruits was significantly inversely associated with depressive symptoms. The multivariable-adjusted odds ratios (95% confidence intervals) for the lowest through the highest tertile of vegetable and fruit fiber were 1.00 (reference), 0.80 (0.60–1.05), and 0.65 (0.45–0.95), respectively (P for trend = 0.03). Dietary intake of total, soluble, insoluble, and cereal fiber was not associated with depressive symptoms.
Higher dietary fiber intake from vegetables and fruits may be associated with lower likelihood of having depressive symptoms.
•We examined the association between fiber intake and depressive symptoms.•Vegetable and fruit fiber was inversely associated with depressive status.•The associations were significant after adjusting for various confounders.

Objectives: This study examines the cross-sectional association between relatively lower-intensity physical activity (LIPA) during leisure time and presenteeism, accounting for relatively higher-intensity physical activity (HIPA) during leisure time.Methods: Data were derived from 11 438 workers from 6 worksites of large companies in Japan, which participated in a questionnaire survey conducted between fiscal years 2018 and 2020. Frequency and duration per occasion were assessed for leisure-time physical activities at 3 intensities as determined by shortness of breath. LIPA was defined as activity not causing shortness of breath. The participants were divided into 3 groups according to LIPA volume (none, <60 min/wk, or ≥60 min/wk) and into 2 groups according to HIPA volume (none or engaged). A single question assessed the participants’ presenteeism through self-ratings of their work performance. A multivariable Poisson regression model with a robust variance estimator estimated prevalence ratios (PRs) of presenteeism and their 95% CIs across the categories for both LIPA and HIPA.Results: The prevalence of presenteeism tended to decrease with increasing amounts of LIPA and HIPA (P for trend <.001). Among those who did not engage in HIPA, a significantly lower prevalence of presenteeism was observed among individuals who engaged in LIPA for ≥60 min/wk compared with those who did not (adjusted PR = 0.74; 95% CI = 0.68-0.81).Conclusions: This study supports the protective role of LIPA during leisure time that does not cause shortness of breath against presenteeism among workers who do not engage in HIPA during leisure time.

Dietary intake of minerals in relation to depressive symptoms in Japanese employees: The Furukawa Nutrition and Health Study

Although intake of minerals has been suggested to be beneficial against depression, epidemiologic data from free-living settings are limited. The aim of this study was to determine the cross-sectional associations between the intake of magnesium, calcium, iron, and zinc and the prevalence of depressive symptoms in Japanese employees.
Participants were 1792 men and 214 women ages 19 to 69 y. Dietary intake was assessed using a validated, brief self-administered diet history questionnaire. Participants with depressive symptoms were defined as those with a scale score of ≥16 on the Center for Epidemiologic Studies Depression Scale.
The prevalence of depressive symptoms was 27.8%. Intakes of magnesium, calcium, iron, and zinc were inversely associated with the prevalence of depressive symptoms. The multivariate adjusted odds ratios (95% confidence interval) of having depressive symptoms were 0.63 (0.44–0.91), 0.64 (0.47–0.88), 0.59 (0.40–0.87), and 0.63 (0.45–0.87) in the highest versus lowest tertiles of magnesium, calcium, iron, and zinc, respectively.
Results suggest that higher dietary intake of magnesium, calcium, iron, and zinc is associated with lower prevalence of depressive symptoms in Japanese employees.
•In this study, we investigated the association between dietary intake of minerals and depressive symptoms among Japanese employees.•Higher intakes of magnesium, calcium, iron, and zinc were each found to be associated with a lower prevalence of depressive status.•The associations remained statistically significant after adjustment for potential dietary and non-dietary confounders.

Background: Diabetes and prediabetes have been linked with morbidity or mortality from cardiovascular disease, cancer, or other physical disorders among working-age populations, but less is known about outcomes directly related to labor loss (eg, Tlong-term sickness absence [LTSA] or pre-retirement death due to physical disorders).This prospective study aimed to examine the association of diabetes and prediabetes with the risk of a composite outcome of LTSA and pre-retirement death due to physical disorders. The present study also examined the associations of severe outcomes (LTSA or death) due to specific physical disorders or injuries/external causes in relation to diabetes and prediabetes.Methods: Data were derived from the Japan Epidemiology Collaboration on Occupational Health study. A total of 60,519 workers from 12 companies were followed for 8 years. Diabetes and prediabetes were defined based on the American Diabetes Association criteria. A Cox proportional hazards regression model was used to examine the association between diabetes/prediabetes and severe outcomes due to physical disorders or injuries/external causes.Results: The adjusted hazard ratios of severe outcomes due to all physical disorders were 1.22 (95% confidence interval [CI], 1.02–1.45) and 2.32 (95% CI, 2.04–2.64) for prediabetes and diabetes, respectively. In cause-specific analyses, an increased risk was observed for severe outcomes due to cancers, cardiovascular diseases, diseases of the musculoskeletal system, and injuries/external causes in relation to either or both diabetes and prediabetes.Conclusion: Diabetes and prediabetes were associated with an increased risk of severe outcomes due to physical disorders or injuries/external causes among Japanese workers.

Loss of Working Life Years Due to Mortality, Sickness Absence, or Ill-health Retirement: A Comprehensive Approach to Estimating Disease Burden in the Workplace

Background: While much effort has focused on quantifying disease burden in occupational health, no study has simultaneously assessed disease burden in terms of mortality and morbidity. We aimed to propose a new comprehensive method of quantifying the disease burden in the workplace. Methods: The data were obtained from the Japan Epidemiology Collaboration on Occupational Health (J-ECOH) Study, a large-scale prospective study of approximately 80,000 workers. We defined disease burden in the workplace as the number of working years lost among the working population during a 6-year period (April 2012 to March 2018). We calculated the disease burden according to consequences of health problems (ie, mortality, sickness absence [SA], and ill-health retirement) and disease category. We also calculated the age-group- (20–39 and 40–59 years old) and sex-specific disease burden. Results: The largest contributors to disease burden in the workplace were mental and behavioural disorders (47.0 person-years lost per 10,000 person-years of working years; ie, per myriad [proportion]), followed by neoplasms (10.8 per myriad) and diseases of the circulatory system (7.1 per myriad). While mental and behavioural disorders made a greater contribution to SA and ill-health retirement compared to mortality, the latter two disorders were the largest contributors to the disease burden in the workplace due to mortality. The number of working years lost was greater among younger versus older female participants, whereas the opposite trend was observed in males. Conclusions: Our approach is in contrast to those in previous studies that focused exclusively on mortality or morbidity.

Background: While a growing body of research suggests a protective role of healthy lifestyle against depression, evidence from prospective studies is scarce. We constructed a healthy lifestyle index (HLI) and examined its prospective association with depressive symptoms in a Japanese working population. Methods: Participants were 917 employees (19–68 years old) who were free from depressive symptoms at baseline in 2012–2013 and attended the 3-year follow-up survey. The HLI (range: 0–7 points) was constructed by assigning 1 point to each healthy lifestyle factor, namely, (1) normal body mass index (18.5–24.9 kg/m2), (2) non-smoking, (3) no or moderate alcohol intake (≤23 g ethanol/day), (4) adequate physical activity (≥7.5 metabolic equivalent-hours/week), (5) high vegetable intake (≥350 g/day), (6) high fruit intake (≥200 g/day), and (7) adequate sleep duration (6–8.9 hours/day), which was categorized into three groups (low: 0–2 points; middle: 3–4 points; and high: 5–7 points). Depressive symptoms were assessed using the Center for Epidemiologic Studies Depression Scale. Results: A total of 155 incident cases (17.0%) of depressive symptoms were identified at the follow-up survey. Compared with the low HLI group, multivariable-adjusted odds ratios of depressive symptoms were 0.74 (95% confidence interval, 0.48–1.15) and 0.55 (95% confidence interval, 0.31–0.99) for the middle and high HLI groups, respectively (P-trend = 0.041). Conclusion: The present study suggests the importance of adherence to multiple healthy lifestyle factors in prevention of depressive symptoms.

Background: While it is essential to understand how long is sufficient for return-to-work when designing paid sick-leave systems, little attempt has been done to collect cause-specific information on when and how many of sickness absentees returned to work, became unemployed, or passed away.Methods: We studied the first sick-leave episode of ≥30 consecutive days in those ≤55 years of age during 2012–2013 among employees of 11 Japanese private companies (n = 1,209), which were followed until 2017. Overall and disease-specific cumulative incidences of return-to-work, resignations, and deaths were estimated using competing risk analysis.Results: During the 3.5-year period (follow-up rate: 99.9%), 1,014 returned to work, 167 became unemployed, and 27 died. Overall, return-to-work occurred within 1 year in 74.9% of all absentees and in 89.3% of those who successfully returned to work. Resignation occurred within 1 year in 8.7% of all absentees and in 62.9% of all subjects who resigned. According to ICD-10 chapters, the cumulative incidence of return-to-work ranged from 82.1% for mental disorders (F00–F99) to 95.3% for circulatory diseases (I00–I99). The cumulative incidence of return-to-work due to mental disorders ranged from 66.7% in schizophrenia (F20) to 95.8% in bipolar affective disorders (F31). Death was rarely observed except for cases of neoplasms (C00–D48), of which the cumulative incidence of death reached 14.2% by 1.5 years.Conclusion: Return-to-work and resignations occurred commonly within 1 year of sick leave among long-term sickness absentees in the Japanese private companies. Our findings may assist occupational physicians and employers in developing effective social protection schemes.

Serum 25-hydroxyvitamin D and metabolic syndrome in a Japanese working population: The Furukawa Nutrition and Health Study

Increasing evidence has suggested a protective role of vitamin D on metabolic syndrome (MetS). However, studies addressing this issue are limited in Asia and it remains unclear whether calcium could modify the association. We examined the association of serum 25-hydroxyvitamin D (25(OH)D) status with MetS, and the potential effect modification by calcium intake in a Japanese working population.
Study subjects were 1790 workers, ages 18 to 69 y, who participated in a health survey at the time of periodic checkup. MetS was defined according to the joint interim statement. Serum 25(OH)D was measured by a protein binding assay. Multilevel logistic regression was used to estimate the odds ratio (OR) with adjustment for potential confounding variables.
An inverse trend was observed between 25(OH)D and MetS. Compared with those with a 25(OH)D of <20 ng/mL, multivariable adjusted OR (95% confidence interval) for MetS was 0.79 (0.55–1.15) and 0.52 (0.25–1.04) for those with a 25(OH)D of 20 to 29 ng/mL and ≥30 ng/mL, respectively (P for trend = 0.051). Similar association was observed in the analysis using quartile categories of 25(OH)D; the OR in the highest quartile of 25(OH)D compared with the lowest quartile was 0.61 (0.36–1.01) (P for trend = 0.046). This association was noted only in older subjects (≥44 y). The inverse association between serum 25(OH)D and MetS was more pronounced in subjects with high calcium intake. The inverse association between 25(OH)D and MetS appears to be linear according to restricted cubic spline regression. There was inverse, but statistically nonsignificant, associations between 25(OH)D and each component of MetS.
Our results suggest that higher circulating vitamin D is associated with decreased likelihood of having MetS among Japanese adults.
