Case summary A female kitten with two ectopic supernumerary pelvic limbs arising from the umbilicus was born in our clinic. Five weeks after the kitten’s birth, she showed an acute deterioration in condition, with clinical signs of an ileus in the intestines entering the supernumerary body part. We then performed a surgical separation of the supernumerary limbs and intestines. Based on clinical findings and radiography, the kitten was classified as a case of parasitic twinning-omphalopagus or a subtype of asymmetrical caudal duplication, rather than a case of polymelia. Parasitic twinning and caudal duplication are rare congenital malformations in many animal species and in humans, which can often have a poor outcome. Almost 1 year after the separation, the cat appeared to be living a normal life without complications. Relevance and novel information A good outcome is rare in parasitic twinning because many of these animals present with other concurrent health issues, such as congenital heart malformation or cleft palate, or they are stillborn. However, this pathological entity is scarce in small animal medicine, and there is no documented survival rate. To our knowledge, this is the first officially documented case of parasitic twinning-omphalopagus in a cat in the past decade.

Case Report: The use of a 3D printed alignment guide system for correction of a delayed union/nonunion/malunion femoral fracture in a dog

A two-year-old neutered male mixed-breed dog was presented with a history of chronic lameness of the right pelvic limb. Physical examination and diagnostic imaging report revealed a delayed union with suspected progression toward nonunion/malunion of the femur. Further assessment showed the presence of a multiplanar deformity with a distal femoral valgus, a caudomedial translation of 26 mm, a femoral torsion angle of 35° compared to 21° to the left, and an excessive limb shortening of 34% in the frontal plane/ 31% in the sagittal plane. A 3D-printed alignment guide (3DPAG) was designed, printed, and successfully used to reduce the delayed union/nonunion fracture. The pre-contoured LCP plate, secured with seven screws, provided stable fracture fixation. The 3DPAG contributed significantly to achieving accurate alignment and implant placement.

Successful use of a 3D-printed surgical guide to facilitate transiliosacral fixation in a cat with bilateral sacroiliac luxation

Case summary A 7-year-old male castrated domestic shorthair cat was presented for treatment of a bilateral sacroiliac luxation (SIL). CT was performed and the data were extracted in a stereolithography (STL) file, after which a 3D-printed drill guide (3DPDG) was devised, using computer-aided design (CAD) software, and printed. Using an open surgical approach, the guide was used as an aid for drilling the sacrum. The ilial wings were drilled free-hand later and a transiliosacral pin (TP) was inserted to realign and stabilise the SIL. The cat exhibited an early return to normal limb function and a CT scan performed at the postoperative follow-up showed early signs of bone remodelling at the sacroiliac joint. Relevance and novel information To the authors’ knowledge, this is the first report using a 3DPDG for implant placement in the feline sacrum without intraoperative imaging.

Paraparesis in a cat caused by idiopathic sterile pyogranulomatous inflammation and necrosis of epidural fat

A five-month-old Maine Coon tomcat was presented with acute non-ambulatory paraparesis and severe spinal hyperaesthesia. Neurological examination was consistent with a spinal cord lesion between the third thoracic and the third lumbar segment. Diagnostic imaging and further laboratory work revealed an extradural mass consisting of sterile pyogranulomatous inflammation and necrosis of the epidural adipose tissue of unknown origin compressing the spinal cord dorsally at the 12th thoracic vertebra. The cat recovered well after surgical decompression and regained ambulation over the next three months. To the authors’ knowledge, this is the first report of an idiopathic sterile pyogranulomatous inflammation of epidural fat in a cat. Although rare, this disease should be included as a possible differential diagnosis for a paraparetic cat.
