The neural correlates of semantic control revisited

Semantic control, the ability to selectively access and manipulate meaningful information on the basis of context demands, is a critical component of semantic cognition. The precise neural correlates of semantic control are disputed, with particular debate surrounding parietal involvement, the spatial extent of the posterior temporal contribution and network lateralisation. Here semantic control is revisited, utilising improved analysis techniques and a decade of additional data to refine our understanding of the network. A meta-analysis of 925 peaks over 126 contrasts illuminated a left-focused network consisting of inferior frontal gyrus, posterior middle temporal gyrus, posterior inferior temporal gyrus and dorsomedial prefrontal cortex. This extended the temporal region implicated, and found no parietal involvement. Although left-lateralised overall, relative lateralisation varied across the implicated regions. Supporting analyses confirmed the multimodal nature of the semantic control network and situated it within the wider set of regions implicated in semantic cognition.

Abstract
Study Objectives
To identify systematic biases across groups in objectively and subjectively measured sleep duration.
Methods
We investigated concordance of self-reported habitual sleep duration compared with actigraphy- and single-night in-home polysomnography (PSG) across white, black, Hispanic, and Chinese participants in the Multi-Ethnic Study of Atherosclerosis.
Results
Among 1910 adults, self-reported sleep duration, determined by differences between bed and wake times, was overestimated in all racial groups compared with PSG and actigraphy. Compared with whites (ρ = 0.45), correlations were significantly lower only in blacks (ρ = 0.28). Self-reporting bias for total sleep time compared with wrist actigraphy was 66 min (95% confidence interval [CI]: 61–71) for whites, 58 min (95% CI: 48–69) for blacks, 66 min (95% CI: 57–74) for Hispanics, and 60 min (95% CI: 49–70) for Chinese adults. Compared with PSG, self-reporting bias in whites at 73 min (95% CI: 67–79) was higher than in blacks (54 min [95% CI: 42–65]) and Chinese (49 min [95% CI: 37–61]) but not different from Hispanics (67 min [95% CI: 56–78]). Slight agreement/concordance was observed between self-reported and actigraphy-based total sleep time (kw = 0.14 for whites, 0.10 for blacks, 0.17 for Hispanics, and 0.11 for Chinese) and PSG (kw = 0.08 for whites, 0.04 for blacks, 0.05 for Hispanics, and 0.01 for Chinese) across race/ethnicity.
Conclusions
Self-reported sleep duration overestimated objectively measured sleep across all races, and compared with PSG, overestimation is significantly greater in whites compared with blacks. Larger reporting bias reduces the ability to identify significant associations between sleep duration and health among blacks compared with whites. Sleep measurement property differences should be considered when comparing sleep indices across racial/ethnic groups.

North Atlantic overturning and water mass transformation in CMIP6 models

Climate models are important tools for investigating how the climate might change in the future, however recent observations have suggested that these models are unable to capture the overturning in subpolar North Atlantic correctly, casting doubt on their projections of the Atlantic Meridional Overturning Circulation (AMOC). Here we compare the overturning and surface water mass transformation in a set of CMIP6 models with observational estimates. There is generally a good agreement, particularly in the recent conclusion from observations that the mean overturning in the east (particularly in the Iceland and Irminger seas) is stronger than that in the Labrador Sea. The overturning in the Labrador Sea is mostly found to be small, but has a strong relationship with salinity: fresh models have weak overturning and saline models have stronger mean overturning and stronger relationships of the Labrador Sea overturning variability with the AMOC further south.We also find that the overturning reconstructed from surface flux driven water mass transformation is a good indicator of the actual overturning, though mixing can modify variability and shift signals to different density classes.

A Comparative Effectiveness Meta-Analysis of Drugs for the Prophylaxis of Migraine Headache

To compare the effectiveness and side effects of migraine prophylactic medications.
We performed a network meta-analysis. Data were extracted independently in duplicate and quality was assessed using both the JADAD and Cochrane Risk of Bias instruments. Data were pooled and network meta-analysis performed using random effects models.
PUBMED, EMBASE, Cochrane Trial Registry, bibliography of retrieved articles through 18 May 2014.
We included randomized controlled trials of adults with migraine headaches of at least 4 weeks in duration.
Placebo controlled trials included alpha blockers (n = 9), angiotensin converting enzyme inhibitors (n = 3), angiotensin receptor blockers (n = 3), anticonvulsants (n = 32), beta-blockers (n = 39), calcium channel blockers (n = 12), flunarizine (n = 7), serotonin reuptake inhibitors (n = 6), serotonin norepinephrine reuptake inhibitors (n = 1) serotonin agonists (n = 9) and tricyclic antidepressants (n = 11). In addition there were 53 trials comparing different drugs. Drugs with at least 3 trials that were more effective than placebo for episodic migraines included amitriptyline (SMD: -1.2, 95% CI: -1.7 to -0.82), -flunarizine (-1.1 headaches/month (ha/month), 95% CI: -1.6 to -0.67), fluoxetine (SMD: -0.57, 95% CI: -0.97 to -0.17), metoprolol (-0.94 ha/month, 95% CI: -1.4 to -0.46), pizotifen (-0.43 ha/month, 95% CI: -0.6 to -0.21), propranolol (-1.3 ha/month, 95% CI: -2.0 to -0.62), topiramate (-1.1 ha/month, 95% CI: -1.9 to -0.73) and valproate (-1.5 ha/month, 95% CI: -2.1 to -0.8). Several effective drugs with less than 3 trials included: 3 ace inhibitors (enalapril, lisinopril, captopril), two angiotensin receptor blockers (candesartan, telmisartan), two anticonvulsants (lamotrigine, levetiracetam), and several beta-blockers (atenolol, bisoprolol, timolol). Network meta-analysis found amitriptyline to be better than several other medications including candesartan, fluoxetine, propranolol, topiramate and valproate and no different than atenolol, flunarizine, clomipramine or metoprolol.
Several drugs good evidence supporting efficacy. There is weak evidence supporting amitriptyline's superiority over some drugs. Selection of prophylactic medication should be tailored according to patient preferences, characteristics and side effect profiles.

Immune activation caused by vascular oxidation promotes fibrosis and hypertension

Vascular oxidative injury accompanies many common conditions associated with hypertension. In the present study, we employed mouse models with excessive vascular production of ROS (tg(sm/p22phox) mice, which overexpress the NADPH oxidase subunit p22(phox) in smooth muscle, and mice with vascular-specific deletion of extracellular SOD) and have shown that these animals develop vascular collagen deposition, aortic stiffening, renal dysfunction, and hypertension with age. T cells from tg(sm/p22phox) mice produced high levels of IL-17A and IFN-γ. Crossing tg(sm/p22phox) mice with lymphocyte-deficient Rag1(-/-) mice eliminated vascular inflammation, aortic stiffening, renal dysfunction, and hypertension; however, adoptive transfer of T cells restored these processes. Isoketal-protein adducts, which are immunogenic, were increased in aortas, DCs, and macrophages of tg(sm/p22phox) mice. Autologous pulsing with tg(sm/p22phox) aortic homogenates promoted DCs of tg(sm/p22phox) mice to stimulate T cell proliferation and production of IFN-γ, IL-17A, and TNF-α. Treatment with the superoxide scavenger tempol or the isoketal scavenger 2-hydroxybenzylamine (2-HOBA) normalized blood pressure; prevented vascular inflammation, aortic stiffening, and hypertension; and prevented DC and T cell activation. Moreover, in human aortas, the aortic content of isoketal adducts correlated with fibrosis and inflammation severity. Together, these results define a pathway linking vascular oxidant stress to immune activation and aortic stiffening and provide insight into the systemic inflammation encountered in common vascular diseases.

Rapid versus traditional qualitative analysis using the Consolidated Framework for Implementation Research (CFIR)

Background
Qualitative approaches, alone or in mixed methods, are prominent within implementation science. However, traditional qualitative approaches are resource intensive, which has led to the development of rapid qualitative approaches. Published rapid approaches are often inductive in nature and rely on transcripts of interviews. We describe a deductive rapid analysis approach using the Consolidated Framework for Implementation Research (CFIR) that uses notes and audio recordings. This paper compares our rapid versus traditional deductive CFIR approach.
Methods
Semi-structured interviews were conducted for two cohorts of the Veterans Health Administration (VHA) Diffusion of Excellence (DoE). The CFIR guided data collection and analysis. In cohort A, we used our traditional CFIR-based deductive analysis approach (directed content analysis), where two analysts completed independent in-depth manual coding of interview transcripts using qualitative software. In cohort B, we used our new rapid CFIR-based deductive analysis approach (directed content analysis), where the primary analyst wrote detailed notes during interviews and immediately “coded” notes into a MS Excel CFIR construct by facility matrix; a secondary analyst then listened to audio recordings and edited the matrix. We tracked time for our traditional and rapid deductive CFIR approaches using a spreadsheet and captured transcription costs from invoices. We retrospectively compared our approaches in terms of effectiveness and rigor.
Results
Cohorts A and B were similar in terms of the amount of data collected. However, our rapid deductive CFIR approach required 409.5 analyst hours compared to 683 h during the traditional deductive CFIR approach. The rapid deductive approach eliminated $7250 in transcription costs. The facility-level analysis phase provided the greatest savings: 14 h/facility for the traditional analysis versus 3.92 h/facility for the rapid analysis. Data interpretation required the same number of hours for both approaches.
Conclusion
Our rapid deductive CFIR approach was less time intensive and eliminated transcription costs, yet effective in meeting evaluation objectives and establishing rigor. Researchers should consider the following when employing our approach: (1) team expertise in the CFIR and qualitative methods, (2) level of detail needed to meet project aims, (3) mode of data to analyze, and (4) advantages and disadvantages of using the CFIR.

Beta-blockers for the prevention of headache in adults, a systematic review and meta-analysis

Headaches are a common source of pain and suffering. The study's purpose was to assess beta-blockers efficacy in preventing migraine and tension-type headache.
Cochrane Register of Controlled Trials; MEDLINE; EMBASE; ISI Web of Science, clinical trial registries, CNKI, Wanfang and CQVIP were searched through 21 August 2018, for randomized trials in which at least one comparison was a beta-blocker for the prevention of migraine or tension-type headache in adults. The primary outcome, headache frequency per month, was extracted in duplicate and pooled using random effects models.
This study included 108 randomized controlled trials, 50 placebo-controlled and 58 comparative effectiveness trials. Compared to placebo, propranolol reduced episodic migraine headaches by 1.5 headaches/month at 8 weeks (95% CI: -2.3 to -0.65) and was more likely to reduce headaches by 50% (RR: 1.4, 95% CI: 1.1-1.7). Trial Sequential Analysis (TSA) found that these outcomes were unlikely to be due to a Type I error. A network analysis suggested that beta-blocker's benefit for episodic migraines may be a class effect. Trials comparing beta-blockers to other interventions were largely single, underpowered trials. Propranolol was comparable to other medications known to be effective including flunarizine, topiramate and valproate. For chronic migraine, propranolol was more likely to reduce headaches by at least 50% (RR: 2.0, 95% CI: 1.0-4.3). There was only one trial of beta-blockers for tension-type headache.
There is high quality evidence that propranolol is better than placebo for episodic migraine headache. Other comparisons were underpowered, rated as low-quality based on only including single trials, making definitive conclusions about comparative effectiveness impossible. There were few trials examining beta-blocker effectiveness for chronic migraine or tension-type headache though there was limited evidence of benefit.

Phenotype switching in a global method for agent-based models of biological tissue

Agent-based models (ABMs) are an increasingly important tool for understanding the complexities presented by phenotypic and spatial heterogeneity in biological tissue. The resolution a modeler can achieve in these regards is unrivaled by other approaches. However, this comes at a steep computational cost limiting either the scale of such models or the ability to explore, parameterize, analyze, and apply them. When the models involve molecular-level dynamics, especially cell-specific dynamics, the limitations are compounded. We have developed a global method for solving these computationally expensive dynamics significantly decreases the computational time without altering the behavior of the system. Here, we extend this method to the case where cells can switch phenotypes in response to signals in the microenvironment. We find that the global method in this context preserves the temporal population dynamics and the spatial arrangements of the cells while requiring markedly less simulation time. We thus add a tool for efficiently simulating ABMs that captures key facets of the molecular and cellular dynamics in heterogeneous tissue.

Global and European climate impacts of a slowdown of the AMOC in a high resolution GCM

The impacts of a hypothetical slowdown in the Atlantic Meridional Overturning Circulation (AMOC) are assessed in a state-of-the-art global climate model (HadGEM3), with particular emphasis on Europe. This is the highest resolution coupled global climate model to be used to study the impacts of an AMOC slowdown so far. Many results found are consistent with previous studies and can be considered robust impacts from a large reduction or collapse of the AMOC. These include: widespread cooling throughout the North Atlantic and northern hemisphere in general; less precipitation in the northern hemisphere midlatitudes; large changes in precipitation in the tropics and a strengthening of the North Atlantic storm track. The focus on Europe, aided by the increase in resolution, has revealed previously undiscussed impacts, particularly those associated with changing atmospheric circulation patterns. Summer precipitation decreases (increases) in northern (southern) Europe and is associated with a negative summer North Atlantic Oscillation signal. Winter precipitation is also affected by the changing atmospheric circulation, with localised increases in precipitation associated with more winter storms and a strengthened winter storm track. Stronger westerly winds in winter increase the warming maritime effect while weaker westerlies in summer decrease the cooling maritime effect. In the absence of these circulation changes the cooling over Europe’s landmass would be even larger in both seasons. The general cooling and atmospheric circulation changes result in weaker peak river flows and vegetation productivity, which may raise issues of water availability and crop production.
