Social Stigma during COVID-19 and its Impact on HCWs Outcomes
The COVID-19 emergency has significantly transformed the working environment and job demands. Providing care was emotionally difficult for healthcare workers. Uncertainty, stigmatisation, and potentially exposing their families to infection were prominent themes for healthcare workers (HCWs) during the crisis, which first broke out in China at the end of 2019, and then in Italy in early 2020. This study examined the effects of stigma, job demands, and self-esteem, and the consequences of working as a “frontline care provider” with patients infected with the coronavirus (COVID-19). A correlational design study involved 260 healthcare workers (HCWs) working in a large hospital in southern Italy. The following questionnaires were administered: (1) the Job Content Questionnaire (JCQ), for assessing psychological and physical demands; (2) the Professional Quality of Life Scale (ProQOL) to measure the quality individuals feel in relation to their work as “frontline care providers”, through three dimensions: compassion fatigue (CF), burnout (BO), and compassion satisfaction (CS); (3) the Rosenberg Self-Esteem Scale, for evaluating individual self-esteem; (4) a self-administered multiple-choice questionnaire developed by See et al. about attitudes of discrimination, acceptance, and fear towards HCWs exposed to COVID-19. The findings suggest that stigma has a high impact on workers’ outcomes. Stigma may influence worker compliance and can guide management communication strategies relating to pandemic risk for HCWs.
Night-Time Shift Work and Related Stress Responses: A Study on Security Guards
Work-related stress can induce a break in homeostasis by placing demands on the body that are met by the activation of two different systems, the hypothalamic–pituitary–adrenal axis and the sympathetic nervous system. Night-shift work alters the body’s exposure to the natural light–dark schedule and disrupts circadian (daily) rhythms. The greatest effect of night-shift work is the disruption of circadian rhythms. The impact that these disruptions may have on the pathogenesis of many diseases, including cancer, is unknown. This study aims to discover the relationship among three different job activities of security guards and their stress-related responses by evaluating salivary cortisol levels and blood pressure. Methods: Ninety security guards, including night-time workers and night-time and daily-shift workers, were recruited for this study. Each security guard provided two saliva samples before and after three scheduled time points: (i) at 22:00, (ii) at 06:30, and (iii) at 14:00. Results: The results of the study showed a significant alteration in cortisol levels. Night-time shift cortisol levels significantly increased before and after the work shifts. A physiological prevalence of the vagal tone on the cardiocirculatory activity was found during night-shift work. Conclusions: This study indicates that cortisol levels and blood pressure are sensitive markers of biological responses to severe work stress. Shift-change consequences may occur at the end of the night shift when there is a significant increase in the cortisol level and a significant variation in cardiovascular parameters.
Breaking Barriers: The Role of the Bone Marrow Microenvironment in Multiple Myeloma Progression
Multiple myeloma (MM) is an incurable malignancy characterized by the proliferation of abnormal plasma cells within the bone marrow, followed by potential dissemination to extramedullary sites. The bone marrow barrier (BMB) plays a pivotal role in plasma cell homing and disease progression. Bone marrow endothelial cells (BMECs) and bone marrow stromal cells (BMSCs), through their interactions with MM cells, secrete adhesion molecules, angiogenic cytokines, anti-apoptotic factors, and growth-promoting signals that support MM cell survival and proliferation. This review examines the components of the BMB and the major pathways involved in MM pathogenesis. Targeting the interactions between MM cells and the BMB may offer novel therapeutic opportunities.
Oxidative stress and DNA damage in agricultural workers after exposure to pesticides
Background Recent epidemiological studies on workers describe that exposure to pesticides can induce oxidative stress by increased production of free radicals that can accumulate in the cell and damage biological macromolecules, for example, RNA, DNA, DNA repair proteins and other proteins and/or modify antioxidant defense mechanisms, as well as detoxification and scavenger enzymes. This study aimed to assess oxidative stress and DNA damage among workers exposed to pesticides. Methods For this purpose, 52 pesticide exposed workers and 52 organic farmers were enrolled. They were assessed: the pesticide exposure, thiobarbituric acid reactive substances (TBARS), total glutathione (TG), oxidized glutathione levels (GSSG), and 8-oxo-7,8-dihydro-2′-deoxyguanosine (8-oxodG), levels. Results Correlation between pesticide exposure was positively associated with high TBARS and 8-oxodG levels ( p  <  0.001). A negative association was founded with TG and GSSG and pesticide exposure. Conclusions The present investigation results seem to indicate a mild augment in oxidative stress associated with pesticide exposure, followed by an adaptive response to increase the antioxidant defenses to prevent sustained oxidative adverse effects stress.
Wearables in Healthcare Organizations: Implications for Occupational Health, Organizational Performance, and Economic Outcomes
Background: Healthcare organizations face major challenges in protecting staff health and ensuring business continuity, particularly in high-risk settings. Wearable technologies are emerging tools to monitor occupational health indicators, improve staff safety, and strengthen organizational resilience. Objectives: This scoping review aimed to map the current evidence on wearable technologies in healthcare, focusing on their impact on occupational health, staff safety, and economic outcomes, as well as barriers and facilitators to their adoption. Methods: A systematic search was conducted in PubMed, Scopus, Web of Science, Embase, CINAHL, sources from inception to July 2025. Studies were included if they evaluated wearable technologies used by healthcare workers and assessed outcomes related to occupational health, organizational resilience, absenteeism, presenteeism, or cost-effectiveness. The review followed the Arksey and O’Malley framework and PRISMA-ScR guidelines. Results: 31 studies met the inclusion criteria. Most were pilot or feasibility studies; only two randomized controlled trials were identified. The wearable technologies evaluated included continuous physiological monitoring devices, real-time location systems, hands-free communication tools, and consumer-grade devices. Evidence suggests potential benefits in improving staff safety, reducing stress and burnout, and enhancing workflow efficiency. However, economic evidence was limited and outcomes varied considerably. Barriers included alert fatigue, privacy concerns, interoperability challenges, and limited staff engagement. Facilitators included leadership support, user-centered design, and adequate infrastructure. Conclusions: Wearable technologies show promise for supporting occupational health and organizational resilience in healthcare, but evidence remains fragmented.
High HOMA-IR Index in Healthcare Shift Workers
Background and objectives: Evidence shows that shift work may be correlated with insulin resistance (IR). Therefore its estimation in clinical and prevention practice is of great significance. A cross-sectional study was performed to examine the Homeostasis Model Assessment-Insulin Resistance (HOMA-IR) Index among healthcare shift workers (HCSW). Materials and Methods: A total of 272 healthcare workers (HCWs) were invited to participate in the study within an occupational surveillance framework, 137 were HCSW while 135 were healthcare non-shift workers (HCNSW). Fasting glucose, insulin, and HOMA-IR Index were evaluated in each participant and correlated with shift workers. Results: Indicators of glucose metabolism were significantly higher in HCSW p < 0.001, and logistic regression analysis confirmed a significant positive association between increased values of HOMA-IR Index and shift workers (p < 0.05). Conclusions: Shift work could be a risk factor in developing insulin resistance and metabolic syndrome.
The Italian Version of the Adult Vaccine Hesitancy Scale (aVHS) for the Working-Age Population: Cross-Cultural Adaptation, Reliability, and Validity
The adult Vaccine Hesitancy Scale (aVHS) is valid and reliable for evaluating attitudes toward vaccine preventable diseases (VPDs). The aim of the present study was to evaluate the cross-cultural adaptation, reliability, and validity of the Italian version of the aVHS. After cross-cultural adaptation of the aVHS, internal consistency (IC), intra-class correlation (ICC), and content validity (S-CVI) were evaluated through a survey on 160 workers. Results of the ICC were analyzed on questionnaires administered twice at a distance of two months and revealed a satisfactory reproducibility (0.87). The IC of the aVHS was assessed by the Cronbach alpha coefficient test, with a result of 0.94, demonstrating an excellent IC reliability. The S-CVI calculated for the total scale was 0.97. The aVHS is a valid and reliable tool for evaluating vaccine hesitancy toward adult vaccinations. We suggest the use of this scale in upcoming surveys on opinions and perceptions of adult vaccinations.
Biomarkers and Human Biomonitoring in Occupational Medicine
Over the last years, biomarkers have increasingly expanded and become early and specific end points for monitoring cellular responses to various disease states and exposures to drugs and chemical agents. They have enjoyed some success as predictors of health outcomes for a number of clinical diseases, but their application for chemical exposure risk assessments has been more limited. Biomarkers may be classified into markers of exposure, effect, and susceptibility. To be useful in providing linkage to human exposure, biomarkers should be as specific as possible to the exposure agents and highly sensitive, so to detect even low levels of exposure.
COVID-19 Pandemic: New Prevention and Protection Measures
As of the end of February 2021, more than 420,000,000 confirmed cases of COVID-19 have been reported worldwide, with 5,856,224 deaths. Transmission of the different genetically engineered variants of SARS-CoV-2, which have been isolated since the beginning of the pandemic, occurs from one infected person to another by the same means: the airborne route, indirect contact, and occasionally the fecal–oral route. Infection is asymptomatic or may present with flulike symptoms such as fever, cough, and mild to moderate and severe respiratory distress, requiring hospitalization and assisted ventilation support. To control the spread of COVID-19, the World Health Organization (WHO) and Centers for Disease Control and Prevention (CDC) have indicated that the appropriate use of personal protective equipment (PPE), as well as the adoption of effective hygiene systems, is one of the primary prevention measures for the entire population. Companies and institutions around the world are therefore trying to find the best ways to reorganize their operations, minimizing the risk of infection among their employees, in order to protect their health and prevent internal outbreaks of SARS-CoV-2, including through the development of new technologies that could also be an innovative and driving factor for the relaunch of companies in a more sustainable, ethically correct, and safe for the health of employees perspective. On the basis of the above premises, in view of the coexistence with SARS-CoV-2 that will most likely accompany us in the coming years, and in view of the vaccination campaign adopted worldwide, the purpose of our narrative review is to update the previous operational protocols with the latest scientific knowledge to be adopted in the workplace even when the emergency crisis is over.
Attitude of Health Care Workers (HCWs) toward Patients Affected by HIV/AIDS and Drug Users: A Cross-Sectional Study
Caring for HIV/AIDS patients and/or drug users requires health care workers (HCWs) to have good knowledge of the issues. Cultural differences in HCWs, combined with professional ethics and personal beliefs, could also result in conflicting attitudes, leading to difficulties related to looking after people affected by HIV/AIDS or drug users. A cross-sectional study was carried out to assess the attitude towards HIV/AIDS patients and/or drug users in a sample of workers operating in a large university hospital in southern Italy. A total of 736 workers were surveyed from May to November 2016. During the periodic occupational health surveillance, a questionnaire was administered about attitudes of discrimination, acceptance and fear towards these patients. Respondents showed average levels of acceptance to HIV/AIDS and drug user patients. As years of experience and professional training increased, scores for discrimination, acceptance of HIV/AIDS, acceptance of drug users and fear decreased. Factors positively influencing levels of attitudes were being female and younger. Supplementary education is needed to strengthen the awareness of HCWs.
