Patient preferences concerning the efficacy and side-effect profile of schizophrenia medications: a survey of patients living with schizophrenia
Background Despite the availability of numerous antipsychotic medications, many patients with schizophrenia continue to experience side effects that contribute to the overall burden of the illness. The present survey of patients with schizophrenia and schizoaffective disorder aimed to assess patient attitudes toward antipsychotic treatment, and understand key factors about willingness to try a new medication. Methods A cross-sectional survey was administered to 250 patients with a primary clinical diagnosis of a schizophrenia spectrum disorder across five outpatient clinics in the United States. The survey included self-reported gender, age, weight, and height, and questions about the importance of efficacy and side effects on the decision to take a prescribed antipsychotic medication. Results Patients rated efficacy and side effects as important attributes of antipsychotic treatment, with 93.6% and 83.6% of patients listing these as “very” or the “most” important factors in taking prescribed medication. A total of 87.6% of respondents identified the ability to think more clearly as an important property of their medication. Patients identified weight gain, physical restlessness, and somnolence as important side effects of current treatments (“very” or “most” important by 61.6%, 60.8%, and 58.8%, respectively). When asked about willingness to change antipsychotic medication, anticipated weight gain had a negative influence on willingness to try the new treatment, with 22.0% declining to try a medication that would lead to weight gain of 2.7–4.5 kg (6–10 lb), 34.0% declining for anticipated weight gain of 5.0–9.1 kg (11–20 lb), and 52.4% declining for anticipated weight gain greater than 9 kg (20 lbs). Conclusion Patients living with schizophrenia spectrum disorders are influenced by many factors when considering whether to take their medication, including efficacy and side effects. It is important for clinicians to assess specific patient concerns to develop a comprehensive treatment plan that maximizes adherence to the prescribed therapy.

21 Patient Preferences Concerning the Efficacy and Side-effect Profile of Schizophrenia Medication: A Survey of Patients Living with Schizophrenia
Study objectivePatient-reported outcomes and preferences rely on reports of the status of a patient's health condition that comes directly from the patient, without interpretation or qualification by clinicians or investigators. Patient-reported outcomes and preferences have become an accepted approach in drug development. As part of this effort, we assessed the relative importance to patients with schizophrenia of trying a new antipsychotic that might improve symptoms in the context of common antipsychotic side effects, especially weight gain. Information from surveys such as this one can provide pilot guidance about what might be acceptable versus unacceptable trade-offs when considering new therapies for schizophrenia. We prospectively administered a cross-sectional survey to 250 patients with clinical diagnoses of schizophrenia or schizoaffective disorder, aged ≥18 years, from five US outpatient community clinics, regarding the importance of efficacy and side effects on treatment decisions involving medications. Sixty-four percent (n=160) of the patients were male; mean age was 43 years (range: 18-72 years); mean weight was 91 kg (range: 49-182 kg); and mean body mass index was 30.3 kg/m2 (range: 15.3-63.3 kg/m2). Patients rated both efficacy and side effects as important attributes of medication for schizophrenia treatment, with 88.5% identifying the ability to think more clearly as an important property of their medication. Patients identified efficacy and side effects as important drivers to take their prescribed medicine (endorsed as very or most important by 94.3% and 84.0% of patients, respectively). Patients identified weight gain, physical restlessness and somnolence as significant side effects of current treatments for schizophrenia (very/most important by 61.5%, 60.4%, and 58.9%, respectively). When asked about willingness to change antipsychotics, anticipated weight gain had a strong negative influence on willingness to try a new antipsychotic, with 44.9% of patients declining to try a medication that would lead to a weight gain of 3-5 kg, and 70.8% of patients declining for an anticipated weight gain of 5-9kg. Patients living with schizophrenia or schizoaffective disorder are influenced by many factors when considering whether to take their prescribed medication, including efficacy and side effects. It is important for clinicians to assess patient-specific concerns and develop a comprehensive treatment plan to maximize adherence to prescribed therapies.Funding Acknowledgements: This study was funded by Alkermes, Inc.
The American Civil War and other 19th century influences on the development of nursing
The Industrial Revolution created sweeping cultural and technological changes in 19th century American society. During this era, nursing evolved from an unskilled to a skilled form of work. Changes in manufacturing, communication, and transportation occurred differentially in America, which favored the growth of different regional economies. Sectionalism erupted into the first modern war in American history. The Civil War created the conditions in which nursing, medicine, and the hospital formed organizational structures, roles, and boundaries that would later form the template for the modern healthcare system. The purpose of this research was to study how the context and culture of mid-nineteenth century American life affected the evolution of nursing during the Civil War, and the later affect it would have on skilled nursing knowledge, roles, education, and practice. The overall goal of the work is to contribute to the body of research on parallel historic processes that had an influence over the formation of early skilled nursing practice and the evolution of the nursing role. The effect of parallel processes associated with the Industrial Revolution and the advent of modern warfare on the development of skilled nursing were the particular focus of this research. A social history methodology was utilized to examine texts and discourse from the Civil War period. It was found that advances in transportation, communication, and manufacturing were both integral to the advent of modern war and modern nursing, and that the advent of these was highly integrated. It was also found that the industrialization of the hospital in response to wartime was highly influential on the development of skilled nursing programs later in the century. The role that nurses would take in the postbellum hospital, however, reflected the mass media image of nursing generated during the war rather than actual wartime practice.

Special Branch, a wartime intelligence unit, raided the flat where the surrealist publication, Free Unions, Unions Libres, was being prepared and took away a hoard that included [Conroy Maddox]'s hard- hitting collages. They were kept under lock and key in police custody until the war had ended. Then, in 1945, he went on to live at 11 Highfield Road, Edgbaston, with the parents of Nick Mason, the drummer of Pink Floyd. When Conroy met his future wife, Nan Burton, he set up home first at 81 Islington Row, Five Ways and then at 29 Speedwell Road. It was there that he created a hub of surrealist activity, often having wild, allnight parties. Among the guests were the young Desmond Morris, who was studying at Birmingham University and George Melly. Maddox saw himself as having the social responsibility of revealing such scandals. During his 70 years as a surrealist, Maddox toiled against the very fabric of reason and brought dreams into waking life. His goal was never in doubt.
Heat stress increases immune cell function in Hexacorallia
Climate change induced heat stress has increased coral bleaching events worldwide. Differentially regulated immune genes are one of the primary responses to heat stress suggesting that immune activation is critical. However, the cellular immune mechanisms of coral bleaching is currently unknown, and it is still not known if the immune response documented during heat stress is a consequence of bleaching or is directly caused by the heat stress itself. To address this question, we have used two model system sea anemones (Order: Actiniaria): Exaiptasia diaphana and Nematostella vectensis . E. diaphana is an established sea anemone model for algal symbiont interaction, while N. vectensis is an established sea anemone model that lacks the algal symbiont. Here, we examined the effect of increased temperature on phagocytic activity, as an indication of immune function. Our data shows that immune cell activity increases during heat stress, while small molecule pinocytosis remains unaffected. We observed an increase in cellular production of reactive oxygen species with increasing temperatures. We also found that the cellular immune activity was not affected by the presence of the Symbiodiniaceae. Our results suggest that the immune activity observed in heat-stress induced bleaching in corals is a fundamental and basic response independent of the bleaching effect. These results establish a foundation for improving our understanding of hexacorallian immune cell biology, and its potential role in coral bleaching.
Respiratory infection- and asthma-prone, low vaccine responder children demonstrate distinct mononuclear cell DNA methylation pathways
Background Infants with frequent viral and bacterial respiratory infections exhibit compromised immunity to routine immunizations. They are also more likely to develop chronic respiratory diseases in later childhood. This study investigated the feasibility of epigenetic profiling to reveal endotype-specific molecular pathways with potential for early identification and immuno-modulation. Peripheral blood mononuclear cells from respiratory infection allergy/asthma-prone (IAP) infants and non-infection allergy/asthma prone (NIAP) were retrospectively selected for genome-wide DNA methylation and single nucleotide polymorphism analysis. The IAP infants were enriched for the low vaccine responsiveness (LVR) phenotype (Fisher's exact p -value = 0.02). Results An endotype signature of 813 differentially methylated regions (DMRs) comprising 238 lead CpG associations (FDR < 0.05) emerged, implicating pathways related to asthma, mucin production, antigen presentation and inflammasome activation. Allelic variation explained only a minor portion of this signature. Stimulation of mononuclear cells with monophosphoryl lipid A (MPL), a TLR agonist, partially reversed this signature at a subset of CpGs, suggesting the potential for epigenetic remodeling. Conclusions This proof-of-concept study establishes a foundation for precision endotyping of IAP children and highlights the potential for immune modulation strategies using adjuvants for future investigation.

Considering the development and ongoing influence of Black thought From 1900 to the present, people of African descent living in the United States have drawn on homegrown and diasporic minds to create a Black intellectual tradition engaged with ideas on race, racial oppression, and the world. This volume presents essays on the diverse thought behind the fight for racial justice as developed by African American artists and intellectuals; performers and protest activists; institutions and organizations; and educators and religious leaders. By including both women's and men's perspectives from the U.S. and the Diaspora, the essays explore the full landscape of the Black intellectual tradition. Throughout, contributors engage with important ideas ranging from the consideration of gender within the tradition, to intellectual products generated outside the intelligentsia, to the ongoing relationship between thought and concrete effort in the quest for liberation. Llama-Nemotron: Efficient Reasoning Models
We introduce the Llama-Nemotron series of models, an open family of heterogeneous reasoning models that deliver exceptional reasoning capabilities, inference efficiency, and an open license for enterprise use. The family comes in three sizes -- Nano (8B), Super (49B), and Ultra (253B) -- and performs competitively with state-of-the-art reasoning models such as DeepSeek-R1 while offering superior inference throughput and memory efficiency. In this report, we discuss the training procedure for these models, which entails using neural architecture search from Llama 3 models for accelerated inference, knowledge distillation, and continued pretraining, followed by a reasoning-focused post-training stage consisting of two main parts: supervised fine-tuning and large scale reinforcement learning. Llama-Nemotron models are the first open-source models to support a dynamic reasoning toggle, allowing users to switch between standard chat and reasoning modes during inference. To further support open research and facilitate model development, we provide the following resources: 1. We release the Llama-Nemotron reasoning models -- LN-Nano, LN-Super, and LN-Ultra -- under the commercially permissive NVIDIA Open Model License Agreement. 2. We release the complete post-training dataset: Llama-Nemotron-Post-Training-Dataset. 3. We also release our training codebases: NeMo, NeMo-Aligner, and Megatron-LM.
