The global, regional, and national burden of acute pancreatitis in 204 countries and territories, 1990–2019

Background
Acute pancreatitis is a common and potentially lethal gastrointestinal disease, but literatures for the disease burden are scarce for many countries. Understanding the current burden of acute pancreatitis and the different trends across various countries is essential for formulating effective preventive intervenes. We aimed to report the incidence, mortality, and disability-adjusted life-years (DALYs) caused by acute pancreatitis in 204 countries and territories between 1990 and 2019.
Methods
Estimates from the Global Burden of Disease Study 2019 (GBD 2019) were used to analyze the epidemiology of acute pancreatitis at the global, regional, and national levels. We also reported the correlation between development status and acute pancreatitis’ age-standardized DALY rates, and calculated DALYs attributable to alcohol etiology that had evidence of causation with acute pancreatitis. All of the estimates were shown as counts and age-standardized rates per 100,000 person-years.
Results
There were 2,814,972.3 (95% UI 2,414,361.3–3,293,591.8) incident cases of acute pancreatitis occurred in 2019 globally; 1,273,955.2 (1,098,304.6–1,478,594.1) in women and 1,541,017.1 (1,307,264.4–1,814,454.3) in men. The global age-standardized incidence rate declined from 37.9/100,000 to 34.8/100,000 during 1990–2019, an annual decrease of 8.4% (5.9–10.4%). In 2019, there were 115,053.2 (104,304.4–128,173.4) deaths and 3,641,105.7 (3,282,952.5–4,026,948.1) DALYs due to acute pancreatitis. The global age-standardized mortality rate decreased by 17.2% (6.6–27.1%) annually from 1.7/100,000 in 1990 to 1.4/100,000 in 2019; over the same period, the age-standardized DALY rate declined by 17.6% (7.8–27.0%) annually. There were substantial differences in the incidence, mortality and DALYs across regions. Alcohol etiology attributed to a sizable fraction of acute pancreatitis-related deaths, especially in the high and high-middle SDI regions.
Conclusion
Substantial variation existed in the burden of acute pancreatitis worldwide, and the overall burden remains high with aging population. Geographically targeted considerations are needed to tailor future intervenes to relieve the burden of acute pancreatitis in specific countries, especially for Eastern Europe.

Street Dust Heavy Metal Pollution Source Apportionment and Sustainable Management in A Typical City—Shijiazhuang, China
Street dust is repeatedly raised by the wind as a secondary suspension, helping heavy metals therein to enter the human body through the respiratory system, harming human health. A detailed investigation was conducted to determine levels and sources of Cd (cadmium), Cr (chromium), Cu (copper), Pb (lead), Zn (zinc), Ni (nick), and Hg (mercury) contamination in street dust from Shijiazhuang, China. The average concentrations of these metals were: Cd, 1.86 mg·kg−1; Cr, 131.7 mg·kg−1; Ni, 40.99 mg·kg−1; Cu, 91.06 mg·kg−1; Pb, 154.78 mg·kg−1, Hg, 0.29 mg·kg−1; and Zn, 496.17 mg·kg−1—all of which were greater than the local soil reference values. The concentrations of the heavy metals were mapped for the three Shijiazhuang ring roads, with the results showing significant differences between each ring. Application of enrichment factors and geoaccumulation indexes showed that there was significant enrichment and accumulation of Cd, Pb, Zn, and Hg. Multivariate statistical analyses showed that Cd, Pb, Zn, and Hg levels were mainly controlled by human activities, while Cr, Ni, and Cu levels were associated with natural sources. Absolute principal component scores with multiple linear regression (APCS-MLR) were applied to facilitate source apportionment. The results showed that the mixed (traffic and industry) group contributed 53.55%, 59.7%, and 62.25% of the Cd, Pb, and Zn concentration, respectively, while the natural sources group contributed 58.01%, 65.09%, and 66.91% of the Cu, Ni, and Cr concentration, respectively. The burning coal group was found to be responsible for 63.38% of the Hg present in the samples. These results provide a useful theoretical basis for Shijiazhuang authorities to address heavy metal pollution management.

Whole exome sequencing has been increasingly used in human disease studies. Prioritization based on appropriate functional annotations has been used as an indispensable step to select candidate variants. Here we present the latest updates to dbNSFP (version 4.1), a database designed to facilitate this step by providing deleteriousness prediction and functional annotation for all potential nonsynonymous and splice-site SNVs (a total of 84,013,093) in the human genome. The current version compiled 36 deleteriousness prediction scores, including 12 transcript-specific scores, and other variant and gene-level functional annotations. The database is available at

Multiple computational approaches have been developed to improve our understanding of genetic variants. However, their ability to identify rare pathogenic variants from rare benign ones is still lacking. Using context annotations and deep learning methods, we present pathogenicity prediction models, MetaRNN and MetaRNN-indel, to help identify and prioritize rare nonsynonymous single nucleotide variants (nsSNVs) and non-frameshift insertion/deletions (nfINDELs). We use independent test sets to demonstrate that these new models outperform state-of-the-art competitors and achieve a more interpretable score distribution. Importantly, prediction scores from both models are comparable, enabling easy adoption of integrated genotype-phenotype association analysis methods. All pre-computed nsSNV scores are available at
