Locally advanced squamous cervical carcinoma (M0): management and emerging therapeutic options in the precision radiotherapy era
2024
Squamous cervical carcinoma (SCC) requires particular attention in diagnostic and clinical management. New diagnostic tools, such as (positron emission tomography–magnetic resonance imaging) PET–MRI, consent to ameliorate clinical staging accuracy. The availability of new technologies in radiation therapy permits to deliver higher dose lowering toxicities. In this clinical scenario, new surgical concepts could aid in general management. Lastly, new targeted therapies and immunotherapy will have more room in this setting. The aim of this narrative review is to focus both on clinical management and new therapies in the precision radiotherapy era.

Enrichment of Apple Slices with Bioactive Compounds from Pomegranate Cryoconcentrated Juice as an Osmodehydration Agent

Pomegranate juice is an important source of bioactive compounds, and cryoconcentrated juice is an interesting osmodehydration agent to enrich a vegetal matrix. This investigation aimed to incorporate bioactive compounds from pomegranate cryoconcentrated juice into apple slices using osmodehydration (OD) assisted by pulse vacuum (PV) and ohmic heating (OH). The apple slices (3 × 4 × 0.5 cm) were osmodehydrated using a 47 °Brix pomegranate cryoconcentrated juice at 30, 40, or 50°C for 180 min with an electric field of 6.66 V/cm (50 V) and a 5 min pulsed vacuum. Over the time, all of the treatments applied to the apple slices increased the soluble solids and bioactive compounds compared with the fresh sample. PVOD/OH at 50°C had the highest content of total anthocyanin during processing, and the best results for the total flavonoids were obtained with OD/OH at 50°C and 40°C. The osmodehydration assisted by pulse vacuum and ohmic heating using a cryoconcentrated juice is a useful combined technique to acquire enriched vegetal samples with bioactive compounds.

Protecting and enhancing inherited assets is a duty of every age; ours requires disclosure through the services of the interconnected network, the only one, to date, capable of reaching a wide audience and with it attracting adequate economic resources for the implementation of programs. In keeping with the international definition of \"Cultural Heritage\", the paper describes the methodology that guided the construction in 3D of the monumental building sculpted in the iconographies between 52 and 62 AD on the marble slab now preserved at the archaeological museum of Perugia. An informative workflow on what has been collected is proposed to then discuss the potential of its uses. The focus lies in particular on the possibility of the model to act as an interoperable collector to compare the reconstructive hypotheses.The final objective looks at the opportunity to create multimedia, multimodal and cross modal collaboration spaces to remedy aspects that, by affecting a wider audience of users, encourage socio-economic policies.

Long-term effects of repeated autologous transplantation of bone marrow cells in patients affected by peripheral arterial disease
Long-term effects of autologous mononuclear bone marrow cell transplantation were studied in patients with severe peripheral arterial disease (PAD) and critical limb ischemia. Ten patients with end-stage disease were infused twice with autologous bone marrow cells and they completed the 12-month follow-up study. Substantial improvement of blood flow and increasing capillary densities were seen when compared with a concomitant control group comprising patients who did not enroll in the study. The ankle–brachial index (ABI) and pain-free walking distance improved significantly in treated patients. The improvement was sustained 12 months after treatment. These results confirm that the autologous bone marrow transplantation is an effective therapeutic strategy in critical limb ischemia.

We consider three inviscid, incompressible, irrotational fluids that are contained between the rigid walls y=−h1 and y=h+H and that are separated by two free interfaces η1 and η2. A generalized nonlocal spectral (NSP) formulation is developed, from which asymptotic reductions of stratified fluids are obtained, including coupled nonlinear generalized Boussinesq equations and (1+1)-dimensional shallow water equations. A numerical investigation of the (1+1)-dimensional case shows the existence of solitary wave solutions which have been investigated for different values of the characteristic parameters.

The dynamic Kirchhoff equations, describing a thin elastic rod of infinite length, are considered in connection with the study of the conformations of polymeric chains. A novel special traveling wave solution that can be interpreted as a conformational soliton propagating at constant speed is obtained, featuring arbitrary non-constant curvature and torsion of the rod, in the simple case of constant cross-section, homogeneous density and elastic isotropy. This traveling wave corresponds to a specific constraint on the twist-to-bend ratio of the constant stiffness parameters, which in turn appears to be compatible with the experimental evidence for the mechanical properties of real polymeric chains. Due to such a constraint, the square of the velocity of the solitary wave is directly proportional to the bending stiffness and inversely proportional to the density and to the principal momentum of inertia of the rod. Several applications to the study of conformational changes in polymeric chains are given.

A major winter storm took place over Mexico during 7 to 11 March 2016, impacting 28 states and leaving four million families without power. Extensive agricultural damage and livestock deaths were also reported with widespread snow across central and northern Mexico. North of the border, this system resulted in record-breaking flooding and severe weather in Texas and Louisiana. The event was due to a trough that deepened and cut off over central Mexico with 500-hPa heights that were nine standard deviations below normal, well beyond previous records! Motivated by the societal impacts of this event, this study investigates factors that contributed to the extreme trough and influenced its predictability in forecast models. A strong El Niño provided the antecedent conditions, with enhanced tropical convection over the central Pacific, a strengthened subtropical anticyclone, and poleward Rossby wave dispersion. However, unlike past strong El Niños, the North Pacific preceding this event was characterized by significant synoptic-scale Rossby wave activity on the midlatitude jet stream including multiple wave packets tracking around the globe during February and March. The interaction of one of these packets with the subtropical anticyclone aloft resulted in a large anticyclonic wave break over the east Pacific, leading to the amplification of the downstream trough over Mexico. The ability of numerical weather prediction to capture this extreme trough is directly related to the predictability of the Rossby wave packet. These results are also discussed within the context of the relationship between El Niño, Rossby wave activity, and extreme events in western North America.

To evaluate if topical support therapy during static-intensity modulated radiotherapy (sIMRT) course is able to equal the characteristic minimum risk for radiation proctitis of Image-guided volumetric modulated arc therapy (IG-VMAT) treatment among localized prostate cancer patients.
Rectal toxicity data of the above patients were retrospectively collected throughout three different clinical periods at our Radiotherapy Deparment: from October 2011 to December 2012, prostate cancer patients were treated with sIMRT and in advance supported by means of daily topical corticosteroids; from January 2013 to November 2016, topical corticosteroids were replaced by daily hyaluronic acid enemas; from December 2016 to May 2018 eligible patients were treated with newly introduced IG-VMAT supported by only on-demand topical corticosteroids.
Among 359 eligible patients, IG-VMAT was proven generally more effective than sIMRT supported by topical medications in terms of proctitis reduction, although without clinical and practical relevance.
Topical medications might have a role in radiation proctitis prevention.

Existence of energy minimums for thin elastic rods in static helical configurations

We characterize families of solutions of the static Kirchhoff model of a thin elastic rod physically. These families, which are proved to exist, depend on the behavior of the so-called register and also on the radius and pitch. We describe the energy densities for each of the solutions in terms of the elastic properties and geometric shape of the unstrained rod, which allows determining the selection mechanism for the preferred helical configurations. This analysis promises to be a fundamental tool for understanding the close connection between the study of elastic deformations in thin rods and coarse-grained models with widespread applications in the natural sciences.

We solve a half-line problem for a nonlinear diffusion equation with a given time-dependent thermal conductivity at the origin. The problem reduces to a linear Volterra integral equation, which is solvable by Picard’s process of successive approximations. We analyze some explicit examples numerically.
