Serum and ascites tumor markers in the diagnostic and prognostic prediction for appendiceal pseudomyxoma peritonei
Background To investigate the expression of carcinoembryonic antigen (CEA), cancer antigen 199 (CA199) and CA125 in serum and ascites of appendiceal pseudomyxoma peritonei (PMP) patients relative to their diagnostic and predictive value. Methods The study comprised 183 patients with pathologically confirmed appendiceal PMP, enrolled from May 2012 to June 2020, in Aerospace Center Hospital. Serum and ascites tumor markers were obtained, and their diagnostic values were compared by receiver operating characteristic (ROC) curves. The prognostic factors of appendiceal PMP with different pathologic subgroups were calculated by univariate and multivariate Cox proportional hazard regression models. Results There were significant differences between the numbers of patients with positive CEA and CA199 in serum vs. ascites: p  = 0.034 in CEA and p  = 0.006 in CA199, respectively. The sensitivities with optimal cut-off values for ascites markers of CEA, CA199 and CA125 were 83.5%, 88.9% and 72.6%, respectively. CEA in ascites showed significant difference in the diagnosis of appendiceal PMP ( p  = 0.000); the areas under the ROC curves (AUROCs) and specificity were 0.725, 70.7%, respectively. Univariate analysis showed that the higher the ascites tumor markers, the poorer the survival ( p  = 0.014). Multivariate analysis indicated that completeness of cytoreduction (CCR), ascites CEA and pathological grade were independent risk factors for overall survival (OS). Conclusion CEA in ascites can be used to help specify the origin of PMP. Furthermore, elevation of ascites CEA, high pathological grade and incomplete cytoreduction predicted poor prognosis of appendiceal PMP.
Antibacterial Activity of Epigallocatechin Gallate (EGCG) against Shigella flexneri
Shigella flexneri (S. flexneri), a major intestinal pathogen, is a global public health concern. The biofilms formed by S. flexneri threaten environmental safety, since they could promote the danger of environmental contamination and strengthen the disease-causing properties of bacteria. Epigallocatechin gallate (EGCG) is an important catechin in tea, which has a high antibacterial activity. However, its antibacterial mechanism is still unclear. This research aims to quantify the antibacterial function and investigate the possible mechanism of EGCG inhibition of S. flexneri. The minimum inhibitory concentration (MIC) of EGCG against planktonic S. flexneri in the investigation was measured to be 400 μg/mL. Besides, SDS-PAGE and field emission scanning electron microscopy showed that EGCG interfered with protein synthesis and changed bacteria morphology. Through controlling the expression of the mdoH gene, EGCG was found to be able to prevent an S. flexneri biofilm extracellular polysaccharide from forming, according to experiments utilizing the real-time PCR test. Additional research revealed that EGCG might stimulate the response of S. flexneri to oxidative stress and prevent bacterial growth. These findings suggest that EGCG, a natural compound, may play a substantial role in S. flexneri growth inhibition.
Sensorless Capability Expansion for SPMSM Based on Inductance Parameter Identification
Pulsating high-frequency voltage injection can be used for the sensorless control of a surface-mounted permanent magnet synchronous motor (SPMSM) at zero- and low-speed ranges. However, the sensorless capability still faces challenges to the requirements of industrial application, especially at heavy load status. Aiming at this issue, this article proposes a sensorless capability expansion method for an SPMSM based on inductance parameter identification. Firstly, incremental inductances at the d-q-axis and cross-coupling inductance are identified offline by three steps combining the rotating high-frequency voltage injection and pulsating high-frequency voltage injection. Using a polynomial curve fitting algorithm, apparent inductances are calculated. Secondly, positive DC current injection at the d-axis is proposed to enhance the saliency ratio based on the analysis of parameter identification results. Compared with the conventional id = 0 or id < 0 method, the saliency ratio is enhanced obviously when a positive DC current is injected at the d-axis. Then, the convergence region of the sensorless control method at heavy load status is expanded and the accuracy of rotor position estimation is improved using the proposed method. Finally, the experimental results validate that the sensorless capability of the SPMSM is expanded.
Nomogram to Predict Incomplete Cytoreduction for Pseudomyxoma Peritonei
BackgroundsThe completeness of cytoreduction is one of the most important prognostic factors for patients with pseudomyxoma peritonei (PMP). To date, no nomograms have been established to predict incomplete cytoreduction (IC) for patients with PMP. The current study therefore proposed a nomogram to predict individual IC risk for PMP patients.MethodsBetween 1 June 2013, and 22 November 2019, 144 consecutive PMP patients who underwent cytoreductive surgery (CRS) combined with hyperthermic intraperitoneal chemotherapy (HIPEC) for the first time in our center were included in a retrospective study. Possible predictors of cytoreducibility were analyzed using logistic regression modeling to predict IC for PMP patients. A nomogram was developed based on the multivariate analysis and further investigated for internal validation.ResultsAfter CRS, the 144 participants were divided into complete CRS (CCRS) (n = 46) and IC (n = 98) subgroups. Four independent predictors (sex, disease duration, anemia, and carbohydrate antigen 19-9 (CA 199)) were included in the prediction model. Then, a nomogram predicting IC was established based on the aforementioned variables, which demonstrated good predictive accuracy (C-index, 0.837; 95 % confidence interval [CI], 0.764–0.894). The predicted probability was close to the actual observed outcome according to the calibration plot.ConclusionsThe current work led to the development of a nomogram capable of predicting IC for PMP patients who demonstrated good performance. Risk stratification by the established nomogram had ability to optimize individual IC prediction and help physicians to establish meticulous preoperative plans.
Research progress in the diagnosis and treatment of pseudomyxoma peritonei
Pseudomyxoma Peritonei (PMP) is a rare and complex clinical syndrome characterized by the extensive implantation and metastasis of tumor cells from different tissues within the abdominal cavity, accompanied by the continuous secretion of large amounts of jelly-like mucin. PMP primarily originates from appendiceal mucinous tumors, and in rare cases, can also arise from organs or tissues such as the ovary, colon, gallbladder, pancreas, urachus, small intestine, stomach, intestinal duplications, and teratomas. PMP has diverse clinical manifestations and lacks specificity, making diagnosis difficult, and usually requires comprehensive evaluation in combination with imaging, surgery, and pathological examination. At present, Cytoreductive Surgery (CRS) combined with Hyperthermic Intraperitoneal Chemotherapy (HIPEC) has become its standard treatment regimen. Although this comprehensive therapeutic strategy has significantly improved the overall survival rate of PMP patients, the high postoperative recurrence rate remains a critical factor adversely affecting patient prognosis. This article aims to summarize recent research advances in this field through a literature review, exploring its pathogenesis, clinical manifestations, diagnostic methods, and treatment strategies, with the goal of deepening the understanding of this disease and providing a reference for clinical diagnosis and treatment planning.
Parameter identification method for brushless DC motors of electrical hoist application
Brushless DC motors (BLDCMs) have been widely adopted across various fields. However, implementing advanced algorithms for high-performance torque control typically relies on accurate mathematical models, requiring precise parameters including inductance, resistance, and back electromotive force (EMF) coefficients. Unlike permanent magnet synchronous motors (PMSMs) that achieve unified mathematical models through coordinate transformation, BLDCMs exhibit distinct mathematical models during commutation and non-commutation phases due to their \"three-phase six-step\" driving mode. Addressing this challenge, this paper conducts a comprehensive investigation into the mathematical models of BLDCMs across different operational phases and modulation methods, ultimately establishing a unified mathematical framework. Building upon this foundation, a model reference adaptive algorithm-based parameter identification method is proposed. This innovative approach enables offline identification of inductance and resistance parameters, while simultaneously performing online identification of inductance and back EMF coefficients. Experimental results have verified that this method can achieve high identification accuracy for stator inductance, resistance, and back EMF coefficients. Particularly suited for hoist motors requiring frequent start-stop operations, this methodology shows significant practical value in applications demanding robust parameter identification under dynamic operating conditions.
Predicting the potential distribution of Astragali Radix in China under climate change adopting the MaxEnt model
Astragali Radix is the dried root of Astragalus mongoliae or Astragalus membranaceus, a leguminous plant. Since ancient times, Astragali Radix has been widely used in Chinese traditional Chinese medicine. As people become more health-conscious, the market demand for Astragali Radix grows and its popularity is increasing in the international market. As an important medicinal plant, the growth of Astragali Radix is strongly influenced by environmental conditions. In order to meet the market demand for high quality Astragali Radix herbs, it is necessary to search and find areas suitable for the growth of Astragali Radix. In this study, we assessed the potential impacts of climate change on the distribution of the Chinese medicinal plant Astragali Radix using the maximum entropy (MaxEnt) model in combination with a geographic information system(GIS). Distribution data and environmental variables were analyzed to predict suitable areas for Astragali Radix under the SSP126, SSP245 and SSP585 scenario for current and future (2041-2060, 2061-2080, 2081-2100). Jackknife is used to assess the importance of environmental variables, and environmental variables with a model contribution greater than 5% were considered to be the main drivers. The results showed that the current area of suitable area for Astragali Radix is 188.41 km , and the three climate scenarios show an increasing trend in the future, with a maximum of 212.70 km . North China has always been the main suitable area, while the area of suitable area in Southwest China is decreasing, and Xinjiang will be developed as a new suitable area in the future. Annual precipitation (41.6%), elevation (15.9%), topsoil calcium carbonate (14.8%), annual mean temperature (8.3%), precipitation seasonality (8%) and topsoil pH (6%) contributed more to the model and were the main environmental influences on the distribution of Astragali Radix. In addition, the centroids of the suitable areas shifted northward under all three climate scenarios, indicating a migratory response to global warming. Our study found that suitable area of Astragali Radix has been expanding for most of the time in each period of the three climate scenarios compared with the current situation. In the future, humans can focus on enhancing the cultivation techniques of Astragali Radix in these suitable areas. This study provide a scientific basis for the development of planting strategies and spatial distribution management of Astragali Radix. It helps to optimize the selection of planting areas and resource conservation of Chinese herbs.
Hyperthermic intraperitoneal chemotherapy in patients with incomplete cytoreduction for appendiceal pseudomyxoma peritonei: a 10-year treatment experience in China
Background To explore the application value of hyperthermic intraperitoneal chemotherapy (HIPEC) in patients with incomplete cytoreduction for appendiceal pseudomyxoma peritonei (PMP). Methods We retrospectively analyzed the clinical data of 526 patients with incomplete cytoreduction for appendiceal PMP to discover its prognostic factors, and the therapeutic value of HIPEC. Results The 5-year and 10-year overall survival rates of patients after cytoreductive surgery (CRS) treated with HIPEC were significantly higher than those without HIPEC (5y-OS: 58% vs. 48%, 10y-OS: 37% vs. 16%, P = 0.032). The median progression-free survival (PFS) following CRS was 20 months, with a 20% 3-year PFS. The median PFS following CRS + HIPEC was 33 months, with a 60% 3-year PFS (P = 0.000). Univariate analysis indicated that HIPEC, gender, completeness of cytoreduction (CCR) and pathological grade had statistical difference. Multivariate analysis showed that CRS without HIPEC and high pathological grade were independent risk factors for poor prognosis and rapid tumor progression. Conclusions HIPEC may prolong the survival in patients with incomplete cytoreduction for low-grade appendiceal PMP. High pathological grade indicates poor survival and rapid tumor progression.
Radiomics analysis based on CT’s greater omental caking for predicting pathological grading of pseudomyxoma peritonei
The objective of this study was to predict the preoperative pathological grading and survival period of Pseudomyxoma peritonei (PMP) by establishing models, including a radiomics model with greater omental caking as the imaging observation index, a clinical model including clinical indexes, and a combined model of these two. A total of 88 PMP patients were selected. Clinical data of patients, including age, sex, preoperative serum tumor markers [CEA, CA125, and CA199], survival time, and preoperative computed tomography (CT) images were analyzed. Three models (clinical model, radiomics model and combined model) were used to predict PMP pathological grading. The models’ diagnostic efficiency was compared and analyzed by building the receiver operating characteristic (ROC) curve. Simultaneously, the impact of PMP’s different pathological grades was evaluated. The results showed that the radiomics model based on the CT’s greater omental caking, an area under the ROC curve ([AUC] = 0.878), and the combined model (AUC = 0.899) had diagnostic power for determining PMP pathological grading. The imaging radiomics model based on CT greater omental caking can be used to predict PMP pathological grading, which is important in the treatment selection method and prognosis assessment.
Management and prognostic prediction of appendiceal mucinous adenocarcinoma with peritoneal metastasis: a single center study in China
Background To investigate the clinical and pathological characteristics of appendiceal mucinous adenocarcinoma with peritoneal metastasis and analyze the prognostic factors. Methods A retrospective analyses of clinicopathological features of 50 patients with appendiceal mucinous adenocarcinoma with peritoneal metastasis from January, 2013 to December, 2017 in Aerospace Central Hospital, Beijing, China. Survival data calculation and comparison were respectively performed with the Kaplan-Meier method and the log-rank test. The Cox proportional hazards regression method was used for multivariate survival analyses. Results Cytoreduction for appendiceal mucinous adenocarcinoma was conducted on 50 patients (24 males and 26 females), with a median age of 52.5 years at the time of surgery (range 31–71 years). The median overall survival (OS) time was 24 months, with 2-,3- and 5-year survival rates of 53, 24 and 8%, respectively. At the last follow-up in December 2018, 13 patients were still alive. Multivariate analysis revealed that patients who had low Ki-67 expression (less than 50%) and CCR (completeness of cytoreduction) 0/1/2 score had significantly better OS rate than their respective counterparts. Conclusions Ki-67 expression statue and CCR score could be employed as the prognosis prediction in patients with appendiceal mucinous adenocarcinoma.
