Current Status and Future Prospects for Esophageal Cancer
Esophageal cancer (EC) is the ninth most common cancer and the sixth leading cause of cancer deaths worldwide. Esophageal squamous cell carcinoma (ESCC) and esophageal adenocarcinoma (EAC) are the two main histological subtypes with distinct epidemiological and clinical features. While the global incidence of ESCC is declining, the incidence of EAC is increasing in many countries. Decades of epidemiologic research have identified distinct environmental exposures for ESCC and EAC subtypes. Recent advances in understanding the genomic aspects of EC have advanced our understanding of EC causes and led to using specific genomic alterations in EC tumors as biomarkers for early diagnosis, treatment, and prognosis of this cancer. Nevertheless, the prognosis of EC is still poor, with a five-year survival rate of less than 20%. Currently, there are significant challenges for early detection and secondary prevention for both ESCC and EAC subtypes, but Cytosponge™ is shifting this position for EAC. Primary prevention remains the preferred strategy for reducing the global burden of EC. In this review, we will summarize recent advances, current status, and future prospects of the studies related to epidemiology, time trends, environmental risk factors, prevention, early diagnosis, and treatment for both EC subtypes.
Cost-effectiveness of fixed-dose combination pill (Polypill) in primary and secondary prevention of cardiovascular disease: A systematic literature review
Background A significant proportion of cardiovascular disease (CVD) morbidity and mortality could be prevented via the population-based and cost-effective interventions. A fixed-dose combination treatment is known as the polypill for the primary and secondary prevention of CVD has come up in recent years. Purpose In order to provide recommendations for future economic evaluations, this systematic review aimed to review and assess the quality of published evidence on the cost-effectiveness of polypill in primary and secondary prevention of CVD, to identify the key drivers that impact the cost-effectiveness Methods A systematic review of literature, following the PRISMA guidelines, was undertaken in the electronic databases. Two researchers identified the relevant studies according to inclusion and exclusion criteria. Consolidated Health Economic Evaluation Reporting Standards (CHEERS) checklist was used to quality assessment of included studies. ICERs value adjusted to 2020 United States Dollar using consumer price index (CPI) and purchasing power parity (PPP). Finally, data were summarized via a narrative synthesis. Results In total, 24 articles were identified based on the determined inclusion criteria. All studies met more than 50% of the CHEERS criteria. Adjusted incremental cost-effectiveness ratios varied from 24 $ to 31000$ (2020 US dollar) among the studies. The polypill resulted in the improved adherence and quality of life, at a price equal to or lower than multiple monotherapies. This price is typically below the commonly accepted thresholds or cost saving in both, primary and secondary prevention of CVD. The main identified cost-effectiveness drivers were the polypill price, adherence, age, CVD risk, and drug combination. Conclusions This systematic review found that the polypill seemed to be a cost-effective intervention in primary and secondary prevention of CVD. However, it is necessary to conduct more economic evaluation studies based on the long-term clinical trials with large populations. Also, studies should consider how the polypill interacts with other primary and secondary preventive strategies as a complementary health strategy.
Manipulation of light spectrum can improve the performance of photosynthetic apparatus of strawberry plants growing under salt and alkalinity stress
Strawberry is one of the plants sensitive to salt and alkalinity stress. Light quality affects plant growth and metabolic activities. However, there is no clear answer in the literature on how light can improve the performance of the photosynthetic apparatus of this species under salt and alkalinity stress. The aim of this work was to investigate the effects of different spectra of supplemental light on strawberry (cv. Camarosa) under salt and alkalinity stress conditions. Light spectra of blue (with peak 460 nm), red (with peak 660 nm), blue/red (1:3), white/yellow (1:1) (400–700 nm) and ambient light were used as control. There were three stress treatments: control (no stress), alkalinity (40 mM NaHCO 3 ), and salinity (80 mM NaCl). Under stress conditions, red and red/blue light had a positive effect on CO 2 assimilation. In addition, blue/red light increased intrinsic water use efficiency ( WUE i) under both stress conditions. Salinity and alkalinity stress decreased OJIP curves compared to the control treatment. Blue light caused an increase in its in plants under salinity stress, and red and blue/red light caused an increase in its in plants under alkalinity. Both salt and alkalinity stress caused a significant reduction in photosystem II (PSII) performance indices and quantum yield parameters. Adjustment of light spectra, especially red light, increased these parameters. It can be concluded that the adverse effects of salt and alkalinity stress on photosynthesis can be partially alleviated by changing the light spectra.
Supplemental light application can improve the growth and development of strawberry plants under salinity and alkalinity stress conditions
The use of complementary light spectra is a potential new approach to studying the increase in plant resilience under stress conditions. The purpose of this experiment was to investigate the effect of different spectra of complementary light on the growth and development of strawberry plants under salinity and alkalinity stress conditions. Plants were grown in the greenhouse under ambient light and irradiated with blue (460 nm), red (660 nm), blue/red (1:3), and white/yellow (400–700 nm) light during the developmental stages. The stress treatments were as follows: control (non-stress), alkalinity (40 mM NaHCO 3 ), and salinity (80 mM NaCl). Our results showed that salinity and alkalinity stress decreased fresh and dry weights and the number of green leaves, and increased chlorotic, tip burn, and dry leaves. The blue and red spectra had a greater effect on reducing the effects of stress compared to other spectra. Stress conditions decreased SPAD and RWC, although blue light increased SPAD, and blue/red light increased RWC under stress conditions. Blue/red and white/yellow light had the greatest effect on reproductive traits. Stress conditions affected fruit color indicators, and red and blue light had the most significant effect on these traits. Under stress conditions, sodium uptake increased, while K, Ca, Mg, and Fe uptake decreased, markedly. Blue and red light and their combination alleviated this reducing effect of stress. It can be concluded that the effects of salinity and alkalinity stresses can be reduced by manipulating the supplemental light spectrum. The use of artificial light can be extended to stresses.
Red Meat Consumption and Risk of Nonalcoholic Fatty Liver Disease in a Population With Low Meat Consumption: The Golestan Cohort Study
Nonalcoholic fatty liver disease (NAFLD), as the most common liver disease in the world, can range from simple steatosis to steatohepatitis. We evaluated the association between meat consumption and risk of NAFLD in the Golestan Cohort Study (GCS). The GCS enrolled 50,045 participants, aged 40-75 years in Iran. Dietary information was collected using a 116-item semiquantitative food frequency questionnaire at baseline (2004-2008). A random sample of 1,612 cohort members participated in a liver-focused study in 2011. NAFLD was ascertained through ultrasound. Total red meat consumption and total white meat consumption were categorized into quartiles based on the GCS population, with the first quartile as the referent group. Multivariable logistic regression models were used to estimate odds ratios (ORs) and 95% confidence intervals (CIs). The median intake of total red meat was 17 and total white meat was 53 g/d. During follow-up, 505 individuals (37.7%) were diagnosed with NAFLD, and 124 of them (9.2%) had elevated alanine transaminase. High total red meat consumption (ORQ4 vs Q1 = 1.59, 95% CI = 1.06-2.38, P trend = 0.03) and organ meat consumption (ORQ4 vs Q1 = 1.70, 95% CI = 1.19-2.44, P trend = 0.003) were associated with NAFLD. Total white meat, chicken, or fish consumption did not show significant associations with NAFLD. In this population with low consumption of red meat, individuals in the highest group of red meat intake were at increased odds of NAFLD. Epidemiologic profile of inflammatory bowel disease in Eastern Mediterranean Region (EMRO) countries: a systematic review and meta-analysis
Background Inflammatory bowel disease (IBD) consists of two main types: Crohn’s disease (CD) and ulcerative colitis (UC). The epidemiology of IBD patients has not been comprehensively studied in EMRO countries; therefore, we conducted this meta-analysis to study the epidemiology of this disease in these countries. Methods We searched four international databases, namely Scopus, Web of Knowledge (ISI), Medline/PubMed, and ProQuest, from inception up to the end of May 2023. The Preferred Reporting Items for Systematic Review and Meta-analysis (PRISMA) guideline was used to carry out this systematic review and meta-analysis investigation. Using the Joanna Briggs Institute (JBI) Critical Appraisal Checklist, the quality of the selected papers was assessed. Results Based on the results of this study, the incidence of UC in EMRO countries was 2.65 per 100,000 (95% CI: 1.39–3.90), and the incidence of CD was 1.16 per 100,000 (95% CI: 0.73–1.59). The most commonly involved intestinal segment in CD was the terminal ileum (44.7%, 95% CI: 34.7–55.2), followed by the ileum (29.8%, 95% CI: 22.2–38.6), and colon (18.7%, 95% CI: 10.8–30.4). However, in UC patients, extensive colitis was the most common finding (32.3%, 95% CI: 26.4–38.8), followed by proctosigmoiditis (27.9%, 95% CI: 21.1–35.8), left-sided colitis (27.4%, 95% CI: 22.7–32.7), and proctitis (22.6%, 95% CI: 17.5–28.5). Conclusion As a result, we were able to establish the traits of IBD patients in EMRO nations. UC patients had a higher incidence than CD patients. The most common regions of involvement in CD and UC patients, respectively, were the colon and pancolitis. Compared to UC patients, CD patients had a higher history of appendectomy.
The effect of supplementary light on the photosynthetic apparatus of strawberry plants under salinity and alkalinity stress
Considering the destructive effect of stresses on the photosynthetic apparatus of plants and the important role of light in photosynthesis, we investigated the effect of complementary light on the photosynthetic apparatus under salinity and alkalinity stress conditions. Light-emitting diodes (LEDs) in monochromatic blue (460 nm), monochromatic red (660 nm), dichromatic blue/red (1:3), white/yellow (400–700 nm) at 200 μmol m −2 S −1 , and without LED treatment were used. The stress treatments were in three stages: Control (no stress), Alkalinity (40 mM NaHCO 3 ), and Salinity (80 mM NaCl). Our results showed that salinity and alkaline stress reduced CO 2 assimilation by 62.64% and 40.81%, respectively, compared to the control treatment. The blue light spectrum had the highest increase in water use efficiency (54%) compared to the treatment without supplementary light. Under salinity and alkalinity stress, L, K, and H bands increased and G bands decreased compared to the control treatment, with blue/red light causing the highest increase in L and K bands under both stress conditions. In salinity and alkalinity stress, white/yellow and blue/red spectra caused the highest increase in H bands. Complementary light spectra increased the G band compared to the treatment without complementary light. There was a significant decrease in power indices and quantum power parameters due to salt and alkalinity stress. The use of light spectra, especially blue, red, and blue/red light, increased these parameters compared with treatment without complementary light. Different light spectra have different effects on the photosynthetic apparatus of plants. It can be concluded that using red, blue spectra and their combination can increase the resistance of plants to stress conditions and be adopted as a strategy in planting plants under stress conditions.
A cohort study protocol to analyze the predisposing factors to common chronic non-communicable diseases in rural areas: Fasa Cohort Study
Background Non-communicable diseases (NCDs) have become the main causes of morbidity and mortality even in rural areas of many developing countries, including Iran. In view of this increased risk, Fasa Cohort Study (FACS) has been established to assess the risk factors for NCDs with the ultimate goal of providing optimal risk calculators for Iranian population and finding grounds for interventions at the population level. Methods In a population-based cohort, at least 10,000 people within the age range of 35 to 70 years old from Sheshdeh, the suburb of Fasa city and its 24 satellite villages are being recruited. A detailed demographic, socioeconomic, anthropometric, nutrition, and medical history is obtained for each individual besides limited physical examinations and determination of physical activity and sleep patterns supplemented by body composition and electrocardiographic records. Routine laboratory assessments are done and a comprehensive biobank is compiled for future biological investigations. All data are stored online using a dedicated software. Discussion FACS enrolls the individuals from rural and little township areas to evaluate the health conditions and analyze the risk factors pertinent to major NCDs. This study will provide an evidence-based background for further national and international policies in preventive medicine. Yearly follow ups are designed to assess the health events in the participating population. It is believed that the results would construct a contemporary knowledge of Iranian high risk health characteristics and behaviors as well as the platform for further interventions of risk reduction in a typical Iranian population. Constantly probing for future advances in NCDs prevention and management, the accumulated database and biobank serves as a potential for state of the art research and international collaborations.
Coniochaeta fungus benefits from its intracellular bacteria to form biofilm and defend against other fungi
During cultivation of a gastric fungus, Coniochaeta polymorpha, growth of Nocardia colonies on top of the fungal culture raised the question whether bacteria originated from inside of fungus. In this study, the likelihood of intracellular origin of bacteria as well as interaction of two microorganisms was assessed. Fluorescence and electron microscopy showed occurrence of several bacterial cells in fungal cytoplasm. A thick biofilm was observed on the surface of co-culture compared with thin one on bacterial and none on fungal monocultures. Field emission scanning electron microscopy (FESEM) micrographs of co-culture showed a dense network of fungal and bacterial cells embedded in a slime-like layer. Dual cultures revealed antagonistic activity of both fungus and bacteria against three Candida species. These findings indicate that Nocardia isolate identified in this study originated from the inside of fungus C. polymorpha. Intracellular bacteria could benefit the fungal host by producing a rigid biofilm and an antifungal compound.
