Self-sampling to increase participation in cervical cancer screening: an RCT comparing home mailing, distribution in pharmacies, and recall letter
Background: We performed a multicentre randomised controlled trial to evaluate the effect on participation in organised screening programmes of a self-sampling device mailed home or picked up at a pharmacy compared with the standard recall letter. Methods: Women aged 30–64 non-responding to screening invitation were eligible. Response rate to first invitation ranged from 30% to 60% between centres. The control was the standard reminder letter to undergo the test used by the programme (Pap test in three centres and HPV DNA test in three other centres). Home mailing of the self-sampler was preceded by a letter with a leaflet about HPV. The analysis was intention-to-treat. Results: In all, 14 041 women were randomised and recruited: 5012 in the control arm, 4516 to receive the self-sampler at home, and 4513 to pick up the self-sampler at a pharmacy. Participation was 11.9% in the control, 21.6% (relative participation: 1.75; 95% CI 1.60–1.93) in home, and 12.0% (relative participation: 0.96; 95% CI 0.86–1.07) in the pharmacy arms, respectively. The heterogeneity between centres was high (excess heterogeneity of that expected due to chance, i.e., I 2 , 94.9% and 94.1% for home and pharmacy arm, respectively). The estimated impact on the overall coverage was +4.3% for home mail self-sampling compared with +2.2% for standard reminder. Conclusions: Home mailing of self-sampler proved to be an effective way to increase participation in screening programmes, even in those with HPV as primary testing. Picking up at pharmacies showed effects varying from centre to centre.
The role of selenium in autoimmune thyroiditis
Selenium (Se) is an essential trace element of fundamental importance for human health. Se is incorporated into selenoproteins (SPs) which are endowed with pleiotropic effects including antioxidant and anti-inflammatory effects and active production of thyroid hormones. It has also been suggested that Se plays a crucial role in the pathogenesis of various human diseases. The therapeutic effects of supplementation with Se have already been described in various thyroid diseases. However, there are still conflicting results regarding the optimal dose of Se to administer and the duration of treatment, efficacy, and safety. The highly beneficial effects of supplementation with Se have been observed in subjects with thyroid disease in the hyperthyroid phase. In line with these observations, clinical studies have shown that in patients with Basedow's disease (BD) and autoimmune thyroiditis (AT), treatment with a combination of antithyroid drugs and Se restores the euthyroid state faster than administration of antithyroid drugs alone. However, the efficacy of this therapeutic approach remains to be better evaluated.
Metaproteomic characterization of Vitis vinifera rhizosphere
The rhizosphere is a hotspot of microbial activity where the release of root exudates stimulates bacterial density and diversity. The majority of the bacterial cells in soil are viable, unculturable, but active. Proteomic tools could be useful in gaining information about microbial community activity and to better understand the real interactions between roots and soil. The aim of this work was to characterize the bacterial community associated with Vitis vinifera cv. Pinot Noir roots using a metaproteome approach. Our results confirmed the large potential of proteomics in describing the environmental microbial communities and their activities: in particular, we showed that bacteria belonging to Streptomyces, Bacillus, Bradyrhizobium, Burkholderia and Pseudomonas genera are the most active in protein expression. Concerning the biological activity of these genera in the rhizosphere, we observed the exclusive presence of the phosphorus metabolic process and the regulation of primary metabolic processes. To our knowledge, this is the first study reporting the rhizosphere proteome of V. vinifera, describing the bacterial community structure and activity of an important ecosystem for the Italian landscape, agriculture and economy.
The burden of the variability introduced by the HEp-2 assay kit and the CAD system in ANA indirect immunofluorescence test
According to the recent recommendations of the American College of Rheumatology, ANA Task Force, IIF technique should be considered the gold standard in antinuclear antibodies (ANAs) testing. To overcome the lack of standardization, biomedical industries have developed several computer-aided diagnosis (CAD) systems. Two hundred and sixty-one consecutive samples with suspected autoimmune diseases were tested for ANA by means of IIF on routinely HEp-2 assay kit (Euroimmun AG). Assignment of result was made if consensus for positive/negative was reached by at least 2 out of 3 expert physicians. ANA-IIF was also carried out using 3 CAD systems: Zenit G-Sight ( n  = 84), Helios ( n  = 85) and NOVA View ( n  = 92); human evaluation was repeated on the same substrate of each CAD system (Immco, Aesku and Inova HEp-2 cells, respectively). To anonymize the results, we randomly named these three systems as A, B and C. We ran a statistical analysis computing several measures of agreement between the ratings, and we also improved the evaluation by using the Wilcoxon’s test for nonparametric data. Agreement between the human readings on routinely HEp-2 assay kit and human readings on CAD HEp-2 assay was substantial for A ( k  = 0.82) and B ( k  = 0.72), and almost perfect for C ( k  = 0.89). Such readings were statistically different only in case A. Comparing experts’ readings with the readings of CAD systems, when the samples were prepared using CAD HEp-2 assay kits, we found almost perfect agreement for B and C ( k  = 0.86; k  = 0.82) and substantial agreement for A ( k  = 0.73). Again, human and CAD readings were statistically different only in A. When we compared the readings of medical experts on routinely HEp-2 assay kit with the output of the CAD systems that worked using their own slides, we found substantial agreement for all the systems (A: k  = 0.62; B: k  = 0.65; C: k  = 0.71). Such readings were not statistically different. The change of the assay kit and/or the introduction of a CAD system affect the laboratory reporting, with an evident impact on the autoimmune laboratory workflow. The CAD systems may represent one of the most important novel elements of harmonization in the autoimmunity field, reducing intra- and inter-laboratory variability in a new vision of the diagnostic autoimmune platform.
Bioactive effects of citrus flavonoids and role in the prevention of atherosclerosis and cancer
Citrus fruits are the main fruits of the Mediterranean diet and have been long recognized for their beneficial effects on human health. Observational studies have shown a significant association between dietary flavonoid intake and reduced risk of cardiovascular and malignant diseases. The beneficial effects of citrus fruits on human health appear to be due to their high content in vitamins, minerals and fibers. In particular, the antioxidant and anti-inflammatory activities have been indicated as some of the mechanisms through which citrus fruits may thwart the development of chronic degenerative diseases such as atherosclerosis and cancer. This review would critically examine the results from numerous experimental and clinical studies carried out in order assess the contribute of citrus flavonoids to the prevention of chronic pathological conditions including atherosclerosis and cancer.
Effects of a rehabilitative whole-body resistance band wrap on equine gait, posture, cortisol, and muscular function
Resistance bands used while horses are exercised with their handlers have shown benefits, but it is unknown if whole-body resistance bands used independently have therapeutic benefits. This study hypothesized that horses with varying gait asymmetries would experience improvements in lameness, muscular function, range of motion, posture, and cortisol following short-term use of a whole-body resistance band wrap (RBW). In this study, nine lame adult horses were evaluated with and without the RBW. The assessment included: objective gait analysis, acoustic myography, postural analysis, gait kinematics, and salivary cortisol concentrations. Statistical analyses included: Shapiro–Wilk Test, Paired Student T-Tests or non-parametric Wilcoxon Signed-Rank Tests (significant at p  < 0.05). There was no difference in lameness, velocity, or stride length. Cortisol levels were lower while wearing the RBW ( p  = 0.03). The RBW decreased semitendinosus muscle efficiency (EST Score, p  = 0.008), and increased carpal ( p  = 0.03), tarsal ( p  = 0.03), and shoulder ( p  = 0.03) joint range of motion (ROM). Back angle increased when wearing the RBW ( p  = 0.04). These findings indicate the RBW has short-term effects on decreasing cortisol, improving joint ROM, and decreasing semitendinosus muscle function. Future studies exploring the use of the RBW with different exercise protocols are needed to further clarify its use for equine rehabilitation.
Percutaneous puncture during PCNL: new perspective for the future with virtual imaging guidance
ContextLarge and complex renal stones are usually treated with percutaneous nephrolithotomy (PCNL). One of the crucial steps in this procedure is the access to the collecting system with the percutaneous puncture and this maneuver leads to a risk of vascular and neighboring organs’ injury. In the last years, the application of virtual image-guided surgery has gained wide diffusion even in this specific field.ObjectivesTo provide a short overview of the most recent evidence on current applications of virtual imaging guidance for PCNL.Evidence acquisitionA non-systematic review of the literature was performed. Medline, PubMed, the Cochrane Database and Embase were screened for studies regarding the use virtual imaging guidance for PCNL.Evidence synthesis3D virtual navigation technology for PCNL was first used in urology with the purpose of surgical training and surgical planning; subsequently, the field of surgical navigation with different modalities (from cognitive to augmented reality or mixed reality) had been explored. Finally, anecdotal preliminary experiences explored the potential application of artificial intelligence guidance for percutaneous puncture.ConclusionNowadays, many experiences proved the potential benefit of virtual guidance for surgical simulation and training. Focusing on surgery, this tool revealed to be useful both for surgical planning, allowed to achieve a better surgical performance, and for surgical navigation by using augmented reality and mixed reality systems aimed to assist the surgeon in real time during the intervention.
Use of acoustic myography to evaluate forelimb muscle function in retriever dogs carrying different mouth weights
ObjectivesTo evaluate the effect of mouth weight on gait and relative function of forelimb muscles in retriever hunting dogs as a possible explanation for biceps tendinopathy.MethodsTen sound retriever dogs underwent acoustic myography, measuring efficiency (E-score), spatial summation (S-score), and temporal summation (T-score) during walk and trot on a pressure-sensitive walkway while carrying a 0 lb (0 kg), 1 lb (0.45 kg), and 3.2 lb (1.45 kg) mouth weight. Gait data included total pressure index (TPI), step length, and stance time. Statistics included a mixed effects model significant at p < 0.05.ResultsForelimb TPI increased with increasing weight. There was no significant change in individual muscle parameters in response to weight. Significance was found in between-muscle comparisons. For walk, T-score was significantly lower in triceps vs. brachiocephalicus with 1 lb, not with 3.2 lb., S-score was significantly lower in the biceps at 0, 1 lb, and triceps at 0 lb. when compared to brachiocephalicus, E-score was significantly lower in deltoideus vs. brachiocephalicus at trot with l and 3.2 lb. There was an overall significant effect of muscle on T-score at trot, but no individual muscle comparison was significant.ConclusionForelimb load increases with mouth weight. Deltoideus had a longer contraction time in response to increasing weight at trot when compared to brachiocephalicus. The biceps muscle did not show increased work in response to increasing weight.Clinical relevanceThe underlying etiology of biceps tendinopathies in retriever dogs remains uncertain but is not due to increasing weight.
Toxoplasma gondii Antibodies in Bulk Tank Milk Samples of Caprine Dairy Herds
A major public health issue, Toxoplasma gondii infection can affect humans mainly via the consumption of animal products from certain species, including small ruminants. Therefore, a regular monitoring of the infection in ovine and caprine populations is advisable for the control of human and animal toxoplasmosis. Antibody detection in individual and bulk tank milk (BTM) may represent a valid alternative to serological analysis, in that its collection is easy and does not affect animal welfare. Many serological tools for milk analysis have already been validated for several parasites, including Apicomplexa. Thus, the aim of the present study was to obtain epidemiological data on T. gondii infection through the detection of antibodies in BTM of dairy goat herds from an important area for caprine dairy production (northern Italy). The performance of a commercial ELISA was first evaluated for analysis of caprine milk samples, using a panel of serum–milk pairs of goats naturally infected by T. gondii. The analysis of BTM confirmed the presence of anti-T. gondii antibodies in 59% of the samples. Toxoplasma gondii antibody positivity was more frequently found in goats reared on farms under extensive (64.9%) or semi-intensive systems (68.7%) in comparison with intensive farms (51.1%). Analysis of milk was a valid alternative to serological tests, being easily applied in large-scale epidemiological surveys and for continuous monitoring of T. gondii infection.
