Umbilical remnant disorders are common in neonatal foals and may require surgical intervention when complicated by infection or urinary involvement. Abdominal surgery in this population is often considered high risk, particularly due to postoperative complications and adhesion formation. The aim of this retrospective observational study was to evaluate the short- and long-term outcomes of foals undergoing omphalectomy with peritonealization of the umbilical arterial stumps. Medical records of foals younger than 30 days treated surgically between 2005 and 2025 were reviewed. Data regarding signalment, diagnosis, comorbidities, postoperative complications, and short-term survival were collected. Long-term follow-up was obtained by owner interview to assess post-discharge colic episodes and if they started their intended athletic activity. Seventy-one foals met the inclusion criteria. Overall, 83.1% survived to hospital discharge. Mortality was significantly associated with the presence of concurrent comorbidities and postoperative complications, but not with age or sex. Long-term follow-up was available for a subset of foals; post-discharge colic was reported in 6.4% of cases, with only one foal developing colic considered compatible with postoperative adhesions. Most foals with available follow-up successfully started their expected athletic activity. These findings suggest that omphalectomy with peritonealization of the umbilical arterial stumps is associated with favorable short- and long-term outcomes and may represent a safe surgical option in neonatal foals.

Dystocia in the Standardbred Mare: A Retrospective Study from 2004 to 2020

Dystocia as a prolonged stage II parturition (>30 min) was associated with a higher risk of complications. The hypothesis of the study was that any type of dystocia could affect the foal’s health, even when the stage II was <30 min. Clinical reports on 222 Standardbred mares and their foals hospitalized at the Veterinary Teaching Hospital of the University of Bologna from 2004 to 2020 were reviewed. Mares were divided into the Eutocia Group (165, eutocic delivery) and the Dystocia Group (57, dystocic delivery). The incidence of dystocia was 4.9%. Stage II was longer in the Dystocia Group (median 20 min) than in the Eutocia Group (median 12 min). All occurrences of dystocia were retrospectively classified into three categories of severity: mild, moderate and severe dystocia. The occurrence of postpartum complications in mares and neonatal diseases and failure of passive transfer of immunity in foals was higher in the Dystocia Group. Foal venous lactatemia and serum creatine kinase were significantly higher in the Dystocia Group (median 3.9 mmol/L; 262 UI/L respectively) than in the Eutocia Group (median 3.1 mmol/L; 187 UI/L respectively). The APGAR score was lower in the Dystocia Group (median 8) than in the Eutocia Group (median 10) and significantly lower in severe dystocia (median 3). The duration of stage II should not be considered the only parameter of dystocia in mares: even a rapid resolution of dystocia could pose health risks to the foal and the mare.

The Systemic Inflammatory Response Syndrome and Predictors of Infection and Mortality in 1068 Critically Ill Newborn Foals

Abstract
Background
Sepsis has been defined in humans as the concurrent proven or suspected presence of microbial infection and the systemic inflammatory response syndrome (SIRS). Sepsis is the leading cause of morbidity and mortality in neonatal foals. The clinical utility of using SIRS or its individual components to predict infection and mortality in critically ill foals is currently unknown.
Objectives
Assess the ability of history and signalment, clinical findings, laboratory results, and SIRS-related indices to predict infection and mortality in critically ill foals.
Animals
Retrospective, multi-center, cross-sectional study using a convenience sample of 1068 critically ill foals < 3 days of age admitted to 16 veterinary referral hospitals in 4 countries.
Methods
Data were retrieved from medical records. Infection was defined as the presence of bacteremia (positive blood culture) or clinical identification of an infected focus on admission. Univariate non-parametric and categorical methods, multivariate logistic regression, and classification tree methods were used for statistical analysis.
Results
Foal age at admission and presence of toxic neutrophils were independent predictors of infection, whereas SIRS-related indices were not predictive of infection. In-hospital mortality was 24%. Independent predictors for mortality were hypokinetic pulses, cold extremities, presence of seizures, blood L-lactate concentration > 6.0 mmol/L, and increased serum potassium and total bilirubin concentrations.
Conclusions and Clinical Importance
The presence of infection in critically ill newborn foals was not predicted by SIRS indices. Cardiovascular dysfunction was strongly associated with mortality, suggesting that maintaining adequate perfusion and pulse pressure should be important treatment goals.

Molecular Diagnosis and Identification of Equine Piroplasms: Challenges and Insights from a Study in Northern Italy

Equine piroplasmosis is a tick-borne disease caused by Babesia and Theileria species. Despite its presence in Europe, no laboratory testing is required for animal movement, even though some countries remain free of this disease. Differentiating between species and genotypes is crucial to determine the most effective treatment, as dosage, active compounds, and duration vary. However, diagnosis is often challenging due to genetic variability and the limited sensitivity of molecular methods. The aims of this study were to compare the performances of different molecular diagnostic tests to identify the most effective assay for piroplasm DNA detection and to genetically characterize the piroplasms identified in horses in Northern Italy from 2016 to 2022. Among 63 horses tested, 24 (38.1%) were positive in at least one of the tests used. Four horses tested positive for Babesia caballi with identical nucleotide sequences, while 22 horses tested positive for genetically different Theileria species, including Theileria equi, Theileria haneyi-like species, and Theileria sp. Africa. Two horses were coinfected by Babesia caballi and Theileria haneyi-like species. The best diagnostic approach to avoid false negative results was a combination of different assays. Further studies will be necessary to better assess the prevalence and genetic diversity of piroplasms involved in equine piroplasmosis.

Assay of steroid hormones in hair has become an attractive alternative for studies focusing on the perinatal period in equine medicine. The aim of the present study was to evaluate mares’ and foals’ hair ALLO concentrations and their ratio in relation to clinical conditions and selected clinical parameters. The 37 mare–foal pairs were categorized into healthy (group H; n = 15) and sick (group S; n = 22) groups. ALLO from hair was measured using a commercial ELISA kit. Foal ALLO and foal/mare ALLO ratio were lower in group S compared to group H (p < 0.001). Moderate positive correlations were found between both the foal ALLO and foal/mare ALLO ratio and the mare’s gestation length (p = 0.003; r = 0.476 and p = 0.002; r = 0.487), between the foal ALLO and foal’s weight (p = 0.042; r = 0.336), and between the foal/mare ALLO ratio and foal’s Apgar score (p = 0.047; r = 0.410). Based on a logistic regression model, a strong relationship (R2 = 0.75) emerged between ALLO concentrations and foals’ clinical outcome, with concentrations of the hormone predicting foals’ clinical outcome with high accuracy (86.8%). Decreased foal ALLO and foal/mare ALLO ratio in sick foals appear to be potential biomarkers of prenatal disease toward the end of pregnancy.

Hair Cortisol and DHEA-S in Foals and Mares as a Retrospective Picture of Feto-Maternal Relationship under Physiological and Pathological Conditions

Equine fetal hair starts to grow at around 270 days of pregnancy, and hair collected at birth reflects hormones of the last third of pregnancy. The study aimed to evaluate cortisol (CORT) and dehydroepiandrosterone-sulfate (DHEA-S) concentrations and their ratio in the trichological matrix of foals and mares in relation to their clinical parameters; the clinical condition of the neonate (study 1); the housing place at parturition (study 2). In study 1, 107 mare-foal pairs were divided into healthy (group H; n = 56) and sick (group S; n = 51) foals, whereas in study 2, group H was divided into hospital (n = 30) and breeding farm (n = 26) parturition. Steroids from hair were measured using a solid-phase microtiter radioimmunoassay. In study 1, hair CORT concentrations measured in foals did not differ between groups and did not appear to be influenced by clinical parameters. A correlation between foal and mare hair CORT concentrations (p = 0.019; r = 0.312, group H; p = 0.006; r = 0.349, group S) and between CORT and DHEA-S concentrations in foals (p = 0.018; r = 0.282, group H; p < 0.001; r = 0.44, group S) and mares (p = 0.006; r = 0.361, group H; p = 0.027; r = 0.271, group S) exists in both groups. Increased hair DHEA-S concentrations (p = 0.033) and decreased CORT/DHEA-S ratio (p < 0.001) appear to be potential biomarkers of chronic stress in the final third of pregnancy, as well as a potential sign of resilience and allostatic load in sick foals, and deserve further attention in the evaluation of prenatal hypothalamus-pituitary-adrenal (HPA) axis activity in the equine species. In study 2, hormone concentrations in the hair of mares hospitalized for attended parturition did not differ from those that were foaled at the breeding farm. This result could be related to a too brief period of hospitalization to cause significant changes in steroid deposition in the mare’s hair.

Antimicrobial resistance (AMR) is one of the most complex global health challenges today: decades of overuse and misuse in human medicine, animal health, agriculture, and dispersion into the environment have produced the dire consequence of infections to become progressively untreatable. Infection control and prevention (IPC) procedures, the reduction of overuse, and the misuse of antimicrobials in human and veterinary medicine are the cornerstones required to prevent the spreading of resistant bacteria. Purified drinking water and strongly improved sanitation even in remote areas would prevent the pollution from inadequate treatment of industrial, residential, and farm waste, as all these situations are expanding the resistome in the environment. The One Health concept addresses the interconnected relationships between human, animal, and environmental health as a whole: several countries and international agencies have now included a One Health Approach within their action plans to address AMR. Improved antimicrobial usage, coupled with regulation and policy, as well as integrated surveillance, infection control and prevention, along with antimicrobial stewardship, sanitation, and animal husbandry should all be integrated parts of any new action plan targeted to tackle AMR on the Earth. Since AMR is found in bacteria from humans, animals, and in the environment, we briefly summarize herein the current concepts of One Health as a global challenge to enable the continued use of antibiotics.

BackgroundAdrenal gland ultrasonographic measurements are useful in clinical evaluation of patients with adrenal dysfunction in several species. In human healthy neonates, the ultrasonographic size of the adrenal glands decreases during the first days of life. Ultrasonography of adrenal glands was demonstrated to be feasible in neonatal foals. The aims of this study were to describe a technique for ultrasonographic measurement of adrenal gland size to test its reliability in neonatal foals, and to assess any variation of ultrasonographic measurements during the first five days of life in healthy foals.MethodsFirst, measurements of the adrenal glands were retrospectively obtained by three observers in 26 adrenal gland images of 13 healthy and sick neonatal foals. The interobserver and intraobserver agreement were tested. Later, adrenal gland ultrasonographic images and measurements were acquired by one operator in 11 healthy neonatal foals at one, three and five days of life and differences among the measurements obtained at the different time points were assessed.ResultsInterobserver agreement ranged from fair to excellent (0.48–0.92), except for cortex width (<0.4); intraobserver agreement ranged from good to excellent (0.52–0.98). No significant differences were found among the measurements obtained at one, three and five days of life.ConclusionAdrenal glands ultrasonographic measurements can be obtained consistently in equine neonates, and in contrast to people they do not vary during the first five days of life in healthy foals.

A complex feedback of growth factors, secreted by a variety of cell types, is responsible for the mediation of skin healing. Despite the recent advances in wound healing management, this fails up to 50% and skin wounds can still be considered one of the main causes of morbidity, both in human and veterinary medicine. Regenerative medicine, involving mesenchymal stromal cells (MSCs), is nowadays a promising solution for skin wound healing. Indeed, MSCs are involved in the modulation of the inflammatory local response and cell replacing, by a paracrine mode of action. Local application of equine umbilical cord Wharton's jelly MSCs (WJMSCS) was carried out in a 6-months-old filly with a non-healing skin wound. Heterologous WJMSCs were applied four times using a carboxymethylcellulose (CMC) gel, produced dissolving CMC in autologous plasma. At first application the mean wound area was 7.28 ± 0.2 cm
. Four days after the last application of WJMSCs, the mean wound area was 1.90 ± 0.03 cm
, and the wound regression rate was +74%. No local or systemic side effects were registered after WJMSCs application and no evident exuberant scar was observed after wound healing. At discharge, the mean wound area was 0.38 ± 0.01 cm
and the total regression rate was +80%. Five days later, the wound was completely healed. In the present clinical case report, the use of WJMSCs led to promising clinical results, paving the way for possible future applications in the treatment of chronic wounds in horses.

Assessment of the microvascular perfusion using sidestream dark‐field imaging in healthy newborn foals

Background Different methods to measure tissue perfusion are available in equine neonatology, but they are not representative of microvascular derangements. Objective To evaluate the feasibility of the sidestream dark‐field (SDF) capillaroscopy to visualize the capillary microvasculature in conscious newborn foals, the differences between two imaging sites and times of measurements, and the measurements' reproducibility. Methods Seventeen healthy newborn foals were enrolled. Three sites at the upper and lower lip mucosa were assessed by SDF, using a hand‐held capiscope, at 24 h and at 4–5 days after birth. Videos were assessed independently by two observers for quality and for semiquantitative calculation of microvascular parameters, including vascular density (VD), microvascular flow index (MFI), proportion of perfused vessels (PPV), and functional capillary density (FCD). Data were analyzed using unpaired and paired Student's T‐tests to assess differences between sites and time‐points. Bland–Altman plots and intraclass correlation coefficient (ICC) were used to assess measurement reproducibility. Results Differences were found between the upper and the lower lip for VD at both 24 h and 4–5 days, and for FCD at 24 h, and between the two time‐points for PPV at the lower lip. ICC for measurement reproducibility was good for all parameters (0.64–0.79) for the lower lip, and was good for VD and FCD (0,76–0,79) and fair to moderate for MFI and PPV (0.31–0.41) for the upper lip. Conclusions Measurement of the capillary microvasculature is feasible in the conscious newborn foal. The lower lip has the best measurement reproducibility. Further investigations are warranted in cardiovascularly compromised cases, particularly in septic foals. This study evaluated the feasibility and reproducibility of sidestream dark‐field (SDF) imaging to visualize the capillary microvasculature in conscious newborn foals. Seventeen healthy newborn foals were assessed on the upper and lower lip mucosa at 24 h and at 4–5 days after birth, and semiquantitative microvascular parameters calculated. The SDF is feasible in the conscious newborn foal. The lower lip performed best with reproducibility; there were differences in microvascularization between sites and time‐points. This technique could detect microvascular changes in normal and cardiovascularly compromised foals.
