Prevalence and Genetic Characteristics of Staphylococcus aureus and Staphylococcus argenteus Isolates Harboring Panton-Valentine Leukocidin, Enterotoxins, and TSST-1 Genes from Food Handlers in Myanmar
Asymptomatic carriers of toxigenic Staphylococcus aureus are potential source of diseases, including food poisoning. Toxigenic potential and genetic traits of colonizing S. aureus were investigated for 563 healthy food handlers in Myanmar. Carriage of S. aureus was found in 110 individuals (19.5%), and a total of 144 S. aureus isolates were recovered from nasal cavities (110 isolates) and hands (34 isolates). Panton-Valentine leucocidin genes (pvl) were detected in 18 isolates (12.5%), among which 11 isolates were classified into coa-VIa, agr type III, and ST1930 (CC96) that had been also detected in pvl-positive clinical isolates in Myanmar. A pvl-positive, ST2250 nasal isolate was identified as S. argenteus, a novel coagulase-positive staphylococcus species. Toxic shock syndrome toxin-1 (TSST-1) gene was detected in five pvl-negative isolates. All of the 144 isolates harbored at least one of the 21 enterotoxin(-like) gene(s). The most prevalent enterotoxin(-like) gene was selw (98%), followed by selx (97%), sei (28%), sely (28%), sem (26%), sel (24%), and sea and sec (22% each). Considerable genetic diversity with five groups was detected for selw. The present study revealed the relatively high rate of pvl, as well as the wide distribution of enterotoxin(-like) genes among colonizing S. aureus in Myanmar.
An integrated EPMA-EBSD study of metamorphic histories recorded in garnet
Growth histories recorded in garnet grains in metasedimentary rocks from the Sanbagawa belt in Japan and the Mogok belt in Myanmar were analyzed using an effective combination of electron backscatter diffraction (EBSD) and electron probe microanalysis (EPMA) data. Garnet in the Sanbagawa metapelite has inner and outer zones that formed in the eclogite and epidote-amphibolite facies stages, respectively. Based on EPMA element mapping, this garnet appears to have grown as a single crystal with a temporal break in growth between the inner and outer zones that occurred during exhumation. The EBSD data, however, document that the garnet grain is composed of four domains. The misorientation angles of crystallographic orientations between the domains are as large as 59°, and domain boundaries crosscut the growth zoning and the compositional boundary between the inner and outer zones. Sets of quartz grains included in the garnets on either side of the domain boundaries sometimes share the same crystallographic orientation with misorientation angles less than 4°. The garnet grains formed via a three-step process of prograde crystallization of polycrystalline garnet during the eclogite facies stage (inner zone)→resorption around garnet rims and along domain boundaries during exhumation→crystallization of the outer zone and in the domain boundaries during the prograde epidote-amphibolite facies stage. The garnet porphyroblasts in the Mogok pelitic gneisses, which formed during prograde metamorphism to the upper amphibolite-granulite facies (0.6-1.0 GPa/780-850°C), are now separated into segments of various sizes by mosaic or symplectite aggregates of biotite, plagioclase, and quartz or monomineralic biotite veins. The segment texture formed at about 0.3-0.4 GPa/610-650°C or lower-grade conditions. The EBSD analysis shows that most of the segments share the same crystallographic orientation with misorientation angles less than 4° and show no evidence of deformation and/or rotation processes after segmentation. These data suggest that the Mogok sample did not experience dynamic deformation of the garnet grains after the resorption and segmentation stage and may have been exhumed under static conditions from depths of 9-12 km.
Prevalence of Extended-Spectrum Beta-Lactamase and Carbapenemase Genes in Clinical Isolates of Escherichia coli in Myanmar: Dominance of blaNDM-5 and Emergence of blaOXA-181
The increasing trend of Escherichia coli producing extended-spectrum beta-lactamases (ESBLs) and carbapenemases is a global public health concern. In this study, prevalence and molecular characteristics of E. coli harboring ESBL and carbapenemase genes were investigated for 426 isolates derived from various clinical specimens in a teaching hospital in Yangon, Myanmar, for the 1-year period beginning January 2016. A total of 157 isolates (36.9%) were ESBL producers and harbored CTX-M-1 group genes (146 isolates; bla CTX-M-15 , bla CTX-M55 ) or CTX-M-9 group genes (11 isolates; bla CTX-M-14 , bla CTX-M-27 ). Carbapenem resistance was detected in 35 isolates (8.2%), among which 26 isolates had carbapenemase genes encoding NDM-1 (2 isolates), NDM-4 (6 isolates), NDM-5 (14 isolates), NDM-7 (3 isolates), and OXA-181 (2 isolates). bla NDM-5 was identified in phylogenetic groups A, B1, and D isolates belonging to various genotypes (ST101, ST354, ST405, ST410, ST1196) associated with bla TEM-1 , bla CTX-M-15 , bla OXA-181 , bla CMY-2 , bla CMY-6 , bla CMY-42 , qnrB , qnrS , or aac6′-Ib-cr . While two isolates with bla OXA-181 belonged to phylogenetic group A-ST410, one isolate had also bla NDM-5 , as well as bla CTX-M-15 and bla CMY-2 , and the other harbored bla CMY-42 and aac6′-Ib-cr , showing different resistance patterns. Phylogenetic group B2 isolates examined were classified into mostly ST131 and had solely bla CTX-M-15 or bla CTX-M-27 , harboring more virulence factors than other phylogenetic groups. The present study revealed high prevalence of ESBL genes represented by bla CTX-M-15 and dominance of bla NDM-5 among NDM genes, disseminating to various E. coli clones. Notably, carbapenemase gene encoding OXA-181 was first identified in Myanmar, suggesting its spread together with NDM genes.
Diversity of rotifer in fishery ponds from Amarapura Township (Mandalay Division) Myanmar
Study of the diversity of rotifer in fishery ponds from Amarapura Township near Taungthaman Lake was carried out from December 2013 to March 2014. The rotifer samples were collected by plankton nylon net with a mouth diameter of 12 cm and 50 μm mesh size. Field sampling were conducted bimonthly from the two sampling ponds. A total of 16 species of freshwater rotifers were recorded. Brachionus angularis was the most dominant species and second dominant species was Polyarthra vulgaris in both ponds. The least dominant species was Brachionus forficula in pond I and Filinia terminalis in pond II. The Shannon- weaver diversity index value (H') ranged from (1.44-1.87) in pond I (1.58-1.95) in pond II. In this result, although H' value were no significant differences between these two ponds, the relative abundance of the rotifer population is higher in pond I (69.36%) than pond II (40.20%).
Characteristics of Suppressor Macrophages Induced by Mycobacterial and Protozoal Infections in relation to Alternatively Activated M2 Macrophages
In the advanced stages of mycobacterial infections, host immune systems tend to change from a Th1-type to Th2-type immune response, resulting in the abrogation of Th1 cell- and macrophage-mediated antimicrobial host protective immunity. Notably, this type of immune conversion is occasionally associated with the generation of certain types of suppressor macrophage populations. During the course of Mycobacterium tuberculosis (MTB) and Mycobacterium avium-intracellulare complex (MAC) infections, the generation of macrophages which possess strong suppressor activity against host T- and B-cell functions is frequently encountered. This paper describes the immunological properties of M1- and M2-type macrophages generated in tumor-bearing animals and those generated in hosts with certain microbial infections. In addition, this paper highlights the immunological and molecular biological characteristics of suppressor macrophages generated in hosts with mycobacterial infections, especially MAC infection.
Impacts of Microphysics Schemes and Topography on the Prediction of the Heavy Rainfall in Western Myanmar Associated with Tropical Cyclone ROANU (2016)
The impacts of different microphysics and boundary schemes and terrain settings on the heavy rainfall over western Myanmar associated with the tropical cyclone (TC) ROANU (2016) are investigated using the Weather Research and Forecasting (WRF) model. The results show that the microphysics scheme of Purdue Lin (LIN) scheme produces the strongest cyclone. Six experiments with various combinations of microphysics and boundary schemes indicated that a combination of WRF Single-Moment 6-class (WSM6) scheme and Mellor-Yamada-Janjic (MYJ) best fits to the Joint Typhoon Warning Center (JTWC) data. WSM6-MYJ also performs the best for the track and intensity of rainfall and obtains the best statistics skill scores in the range of maximum rainfall intensity for 48-h. Sensitivity experiments on different terrain settings with Normal Rakhine Mountain (NRM), with Half of Rakhine Mountain (HRM), and Without Rakhine Mountain (WoRM) are designed with the use of WSM6-MYJ scheme. The track of TC ROANU moved northwestward in WoRM and HRM. Due to the presence of Rakhine Mountain, TC track moved into Myanmar and the peak rainfall occurred on the leeward side of the Mountain. In the absence of Rakhine Mountain, a shift in peak rainfall was observed in north side of the Mountain.
The Influence of Modernization and Disease on the Gastric Microbiome of Orang Asli, Myanmars and Modern Malaysians
The present study explored the differences in gastric microbiome between three distinct populations of Southeast Asia. These include the isolated Orang Asli population and modern Malaysians, as well as patients from Myanmar, the least developed country in the region. All 79 subjects recruited in this study had Helicobacter pylori infection. Based on alpha diversity analysis, Orang Asli had the richest and most diverse gastric microbiome, followed by Myanmar and modern Malaysian groups. Beta diversity analysis revealed significant separation of samples between different populations. These observations are likely to be associated with the level of modernization of each population. Our data further suggested increased bacterial species richness and diversity of the gastric microbiome in individuals who were less modernized, particularly in the Orang Asli group, could suppress the growth of H. pylori. In addition, there were significant variations in the gastric microbiome between modern Malaysians with different types of gastric diseases. Notably, Cutibacterium acnes was present at significantly greater abundance level in patients with non-ulcerative dyspepsia than those with peptic-ulcer diagnosis. This suggests that C. acnes may also play a role in gastritis besides H. pylori, which merits further investigation.
2022 American College of Rheumatology/European Alliance of Associations for Rheumatology Classification Criteria for Eosinophilic Granulomatosis with Polyangiitis
ObjectiveTo develop and validate revised classification criteria for eosinophilic granulomatosis with polyangiitis (EGPA).MethodsPatients with vasculitis or comparator diseases were recruited into an international cohort. The study proceeded in five phases: (1) identification of candidate criteria items using consensus methodology, (2) prospective collection of candidate items present at the time of diagnosis, (3) data-driven reduction of the number of candidate items, (4) expert panel review of cases to define the reference diagnosis and (5) derivation of a points-based risk score for disease classification in a development set using least absolute shrinkage and selection operator logistic regression, with subsequent validation of performance characteristics in an independent set of cases and comparators.ResultsThe development set for EGPA consisted of 107 cases of EGPA and 450 comparators. The validation set consisted of an additional 119 cases of EGPA and 437 comparators. From 91 candidate items, regression analysis identified 11 items for EPGA, 7 of which were retained. The final criteria and their weights were as follows: maximum eosinophil count ≥1×109/L (+5), obstructive airway disease (+3), nasal polyps (+3), cytoplasmic antineutrophil cytoplasmic antibody (ANCA) or anti-proteinase 3–ANCA positivity (−3), extravascular eosinophilic predominant inflammation (+2), mononeuritis multiplex/motor neuropathy not due to radiculopathy (+1) and haematuria (−1). After excluding mimics of vasculitis, a patient with a diagnosis of small- or medium-vessel vasculitis could be classified as having EGPA if the cumulative score was ≥6 points. When these criteria were tested in the validation data set, the sensitivity was 85% (95% CI 77% to 91%) and the specificity was 99% (95% CI 98% to 100%).ConclusionThe 2022 American College of Rheumatology/European Alliance of Associations for Rheumatology Classification Criteria for Eosinophilic Granulomatosis with Polyangiitis demonstrate strong performance characteristics and are validated for use in research.

The final criteria and their weights were as follows: bloody nasal discharge, nasal crusting or sino-nasal congestion (+3); cartilaginous involvement (+2); conductive or sensorineural hearing loss (+1); cytoplasmic antineutrophil cytoplasmic antibody (ANCA) or anti-proteinase 3 ANCA positivity (+5); pulmonary nodules, mass or cavitation on chest imaging (+2); granuloma or giant cells on biopsy (+2); inflammation or consolidation of the nasal/paranasal sinuses on imaging (+1); pauci-immune glomerulonephritis (+1); perinuclear ANCA or antimyeloperoxidase ANCA positivity (−1); and eosinophil count ≥1×109 /L (−4). After excluding mimics of vasculitis, a patient with a diagnosis of small- or medium-vessel vasculitis could be classified as having GPA if the cumulative score was ≥5 points. When these criteria were tested in the validation data set, the sensitivity was 93% (95% CI 87% to 96%) and the specificity was 94% (95% CI 89% to 97%).ConclusionThe 2022 American College of Rheumatology/European Alliance of Associations for Rheumatology classification criteria for GPA demonstrate strong performance characteristics and are validated for use in research.
