We investigate the ecological and evolutionary variables that best explain spatial diversity patterns of anuran amphibians in three of South America's most diverse and geographically widespread biomes: the Cerrado, Amazonia, and Atlantic Rainforest. We used Conditional Autoregressive Models to assess the potential influence of present‐day climate (temperature and precipitation), historical climate (stability over the last 120,000 years), potential evapotranspiration (PET), and topography (slope, aspect, and rugosity) on spatial variation in taxonomic, functional, and phylogenetic diversity at a resolution of 0.5 × 0.5 degrees. Both taxonomic diversity and phylogenetic diversity increased with long‐term climatic stability in all regions. By contrast, functional diversity was negatively impacted by precipitation in the driest quarter. However, the relative importance of each predictor variable differed among diversity metrics and biomes. In the Atlantic Rainforest, potential evapotranspiration was positively correlated with functional diversity but negatively associated with taxonomic and phylogenetic diversity. In Amazonia, precipitation and relief slopes were positively associated with functional and phylogenetic diversity, respectively, whereas relief slopes were negatively correlated with taxonomic diversity. In the Cerrado, precipitation was negatively correlated with functional diversity, but climatic stability was more strongly associated with phylogenetic and taxonomic diversity. These findings indicate that present‐day climatic factors are critical in forested biomes, whereas a combination of historical and current variables is more relevant in Cerrado's savanna mosaics. Notably, ecotones exhibit significantly higher functional diversity (except for Amazon), reflecting the encounter of faunas from adjacent biomes having distinct ecological regimes and thus associated organismal traits. Characterizing the drivers of heterogeneous biodiversity distribution will offer insights into the assembly of ecological communities in other tropical and transitional ecosystems and can potentially guide conservation strategies globally. This study identifies the key drivers of anuran amphibian diversity (taxonomic, functional, and phylogenetic) across South America's Cerrado, Amazonia, and Atlantic Rainforest. Although taxonomic and phylogenetic diversity consistently increased with long‐term climatic stability in all regions, functional diversity was primarily influenced by current precipitation and showed divergent regional responses to predictors like potential evapotranspiration and topography. The findings reveal distinct combinations of historical and contemporary factors shaping each diversity dimension per domain, with notably higher functional diversity in ecotones where adjacent faunas meet.

Extreme environments filter functionally rich communities of Atlantic Forest treefrogs along altitudinal and latitudinal gradients

Understanding how environmental filtering and biotic interactions structure communities across elevational and latitudinal gradients is still a matter of debate. To provide insight into their relative importance, we explore the mismatch between three dimensions of biodiversity (taxonomic, functional and phylogenetic), and compare their patterns to null models of random community assembly. We focus on a highly diverse tropical group: the tree frogs (hylids) of the Atlantic Forest of Brazil. For that, we combined geographic distribution data with phylogenetic and trait information for 158 species to produce maps of the three dimensions of diversity. We show that communities of tree frogs of the Hylidae family tend to have high functional richness along the Atlantic Forest, potentially driven by niche partitioning. However, environmental filtering gains importance in extreme environments, both at high elevations and higher latitudes. In these areas of the forest, clustering of both functional and phylogenetic diversity is observed. Trait overdispersion is found in areas of contact between vegetation types, or of adjacent biomes, reflecting the encounter of amphibian species adapted to distinct environmental conditions and landscape configurations. Considering all three dimensions of diversity along with multiple axes of environmental variation allows for a more comprehensive understanding of the processes shaping amphibian community assembly in this biodiversity hotspot.

We provide novel data on the biology of Proceratophrys melanopogon and describe its larval external morphology and internal oral features on the basis of specimens from the Serra do Mar, southeastern Brazil. We also review the larval internal oral features of the genus, including descriptions and comparisons with P. cururu and P. moratoi. The external morphology of tadpoles of P. melanopogon is very similar to that of closely related species. The internal oral features are very conserved in this genus, with the majority of species having lingual papillae unusually bifurcated, four infralabial papillae, and a transverse ridge on the prenarial arena. Our results also provide data for phylogenetic studies on the genus Proceratophrys.

We describe the larval external morphology and internal oral features of Proceratophrys mantiqueira from its type locality and provide novel natural history information for adults. The tadpoles of P. mantiqueira (Stage 37) exhibit a depressed body with low tail fins, eyes positioned dorsally, and a ventral oral disc bordered by a single row of marginal papillae, with a wide gap on the anterior labium; a few submarginal papillae may be present. The labial tooth row formula is 2(2)/3(1). The shape and amount of papillae in the buccal floor, especially the infralabial and lingual papillae, can distinguish the tadpole of P. mantiqueira from other congeners. Our results add important new data about the larval morphology of this species and may help future studies on the intrageneric relationships of Proceratophrys .

Serine integrases (Ints) are a family of site-specific recombinases (SSRs) encoded by some bacteriophages to integrate their genetic material into the genome of a host. Their ability to rearrange DNA sequences in different ways including inversion, excision, or insertion with no help from endogenous molecular machinery, confers important biotechnological value as genetic editing tools with high host plasticity. Despite advances in their use in prokaryotic cells, only a few Ints are currently used as gene editors in eukaryotes, partly due to the functional loss and cytotoxicity presented by some candidates in more complex organisms. To help expand the number of Ints available for the assembly of more complex multifunctional circuits in eukaryotic cells, this protocol describes a platform for the assembly and functional screening of serine-integrase-based genetic switches designed to control gene expression by directional inversions of DNA sequence orientation. The system consists of two sets of plasmids, an effector module and a reporter module, both sets assembled with regulatory components (as promoter and terminator regions) appropriate for expression in mammals, including humans, and plants. The complete method involves plasmid design, DNA delivery, testing and both molecular and phenotypical assessment of results. This platform presents a suitable workflow for the identification and functional validation of new tools for the genetic regulation and reprogramming of organisms with importance in different fields, from medical applications to crop enhancement, as shown by the initial results obtained. This protocol can be completed in 4 weeks for mammalian cells or up to 8 weeks for plant cells, considering cell culture or plant growth time.

•The use of carvedilol is recommended by guidelines for the prevention of anthracycline-induced cardiotoxicity. However, the level of evidence behind this recommendation is weak.•CARDIOTOX is the largest clinical trial ever conducted to evaluate the potential role of carvedilol in preventing anthracycline-induced cardiotoxicity, including several types of cancer.•The monitoring of cardiotoxicity through systematic echocardiography with GLS and biomarkers (during chemotherapy and follow-up) might serve as a guidance for future strategies in managing these patients.
Patients with cancer undergoing chemotherapy with an anthracycline-based regimen are at increased risk of cardiotoxicity, predisposing to heart failure, arrhythmias and death. Whether carvedilol may confer benefit to prevent anthracycline-induced cardiotoxicity remains to be determined.
CARDIOTOX is a double-blind, placebo controlled randomized clinical trial that plan to enroll 1,018 patients across 25 study sites in Brazil. Patients with active cancer scheduled to undergo an anthracycline-based chemotherapy regimen are eligible. Patients with prior HF or cardiomyopathy are excluded. Patients are randomized in 1:1 ratio to carvedilol (starting dose 6.25mg BID up titrated to 25mg BID or maximum tolerated dose) or placebo, stratified by site and use of renin-angiotensin blockers at baseline. Study drug is administered through the duration of chemotherapy and up to 30 days after the last dose of anthracycline. Patients are scheduled to undergo echocardiographic evaluations at baseline and at 3, 6, and 12 months. The study primary endpoint is the composite of new left ventricle ejection fraction (LVEF) reduction by at least 10% leading to an LVEF <50%, cardiovascular death, myocardial infarction, urgent care visit or hospitalization for heart failure, or clinically significant arrhythmias at 12 months. Echocardiographic images will be analyzed by a central core lab, clinical outcomes will be adjudicated, and safety endpoints include serious adverse events and adverse events of special interest (symptomatic bradycardia, hypotension, syncope and bronchospasm).
The CARDIOTOX trial is the largest trial to date analyzing the potential role of beta-blockers as prophylactic therapy to prevent cardiotoxicity induced by anthracyclines.

Angiotensin-converting enzyme-2 (ACE2) expression may increase due to upregulation in patients using angiotensin-converting enzyme inhibitors (ACEI) and angiotensin receptor blockers (ARBs). Because renin-angiotensin system blockers increase levels of ACE2, a protein that facilitates coronavirus entry into cells, there is concern that these drugs could increase the risk of developing a severe and fatal form of COVID-19. The impact of discontinuing ACEI and ARBs in patients with COVID-19 remains uncertain.
BRACE CORONA is a pragmatic, multicenter, randomized, phase IV, clinical trial that aims to enroll around 500 participants at 34 sites in Brazil. Participants will be identified from an ongoing national registry of suspected and confirmed cases of COVID-19. Eligible patients using renin-angiotensin system blockers (ACEI/ARBs) with a confirmed diagnosis of COVID-19 will be randomized to a strategy of continued ACEI/ARB treatment versus temporary discontinuation for 30 days. The primary outcome is the median days alive and out of the hospital at 30 days. Secondary outcomes include progression of COVID-19 disease, all-cause mortality, death from cardiovascular causes, myocardial infarction, stroke, transient ischemic attack, new or worsening heart failure, myocarditis, pericarditis, arrhythmias, thromboembolic events, hypertensive crisis, respiratory failure, hemodynamic decompensation, sepsis, renal failure, and troponin, B-type natriuretic peptide (BNP), N-terminal-proBNP, and D-dimer levels.
BRACE CORONA will evaluate whether the strategy of continued ACEI/ARB therapy compared with temporary discontinuation of these drugs impacts clinical outcomes among patients with COVID-19.

Prognosis of the outcome of severe diabetic foot ulcers with multidisciplinary care

Approximately 2-4% of individuals worldwide with diabetes mellitus have foot ulcers. This study aims to assess the factors affecting the outcomes of severe foot ulcers in diabetic individuals.
An analytical prospective cohort study was conducted from March 1st, 2015, to March 1st, 2017. A total of 34 individuals was selected. The study included patients with foot ulcers below the ankle who were at risk of amputation. All tests used a <5% level of significance and confidence interval of 95%. A Pearson's chi-squared test and binary multiple regression were performed to assess the factors related to healing.
Only 11.7% of the individuals required amputation; ulcers classified as 2/B according to the University of Texas Diabetic Foot Ulcer Classification System healed before the 1/B ulcers. Neuropathic ulcers were the most prevalent (58.8%); 61.8% healed after 1 year. Most of the individuals were overweight, 47.1% had reduced glomerular filtration rates, and 78.8% had glycated hemoglobin >7%. Body mass index and osteomyelitis were the two significant variables in logistic regression.
In this study, osteomyelitis was the main complication related to the risk of amputation, and elevated body mass index and osteomyelitis were the significant factors that induced a slower healing time.

Discontinuing vs continuing ACEIs and ARBs in hospitalized patients with COVID-19 according to disease severity: Insights from the BRACE CORONA trial

We explored the effect of discontinuing versus continuing angiotensin-converting enzyme inhibitors (ACEIs) and angiotensin II receptor blockers (ARBs) on clinical outcomes in patients with COVID-19 according to baseline disease severity.
We randomized 659 patients with a confirmed diagnosis of COVID-19 and classified them as having mild or moderate COVID-19 disease severity at hospital presentation using blood oxygen saturation and lung imaging. The primary outcome was the mean ratio of number of days alive and out of the hospital at 30 days according to disease severity.
At presentation, 376 patients (57.1%) had mild and 283 (42.9%) had moderate COVID-19. In patients with mild disease, there was no significant difference in the number of days alive and out of the hospital between ACEI/ARB discontinuation (mean 23.5 [SD 6.3] days) and continuation (mean 23.8 [SD 6.5] days), with a mean ratio of 0.98 (95% CI 0.92-1.04). However, in patients with moderate disease, there were fewer days alive and out of the hospital with ACEI/ARB discontinuation (mean 19.6 [SD 9.5] days) than continuation (mean 21.6 [SD 7.6] days), with a mean ratio of 0.90 (95% CI 0.81-1.00; P-interaction = .01). The impact of discontinuing versus continuing ACEIs/ARBs on days alive and out of hospital through 30 days differed according to baseline COVID-19 disease severity.
Unlike patients with mild disease, patients with moderate disease who continued ACEIs/ARBs had more days alive and out of hospital through 30 days than those who discontinued ACEIs/ARBs. This suggests that ACEIs/ARBs should be continued for patients with moderate COVID-19 disease severity.
