Clinical response to 2 protocols of aerosolized gentamicin in 46 dogs with Bordetella bronchiseptica infection (2012‐2018)
Background Bordetella bronchiseptica (Bb) infection commonly causes respiratory disease in dogs. Gentamicin delivered by aerosol maximizes local drug delivery without systemic absorption but clinical response to protocols remains undetermined. Objectives To compare the clinical response to 2 protocols of aerosolized delivery of gentamicin in bordetellosis. Animals Forty‐six dogs with Bb infection confirmed by culture or quantitative polymerase chain reaction on bronchoalveolar lavage. Methods Retrospective study. Administration of aerosolized gentamicin for ≥10 minutes q12h for ≥3 weeks using 4 mg/kg diluted with saline (group 1) or undiluted 5% solution (group 2). Clinical response firstly assessed after 3‐4 weeks and treatment pursued by 3‐weeks increments if cure not reached. Cure defined as absence of cough persisting at least a week after treatment interruption. Results Demographic data were similar between both groups. Clinical cure at 3‐4 weeks was more frequently observed with the use of undiluted solution (19/33 vs 3/13 dogs, P = .03) in association with a shorter median duration of treatment (4 vs 6 weeks, P = .01). Dogs from group 2 having less than 1000 cells/μL in lavage were also more likely to be cured at 3‐4 weeks than dogs with more than 1000 cells/μL [9/9 vs 10/19, P = .006] and median duration of treatment in that subgroup of animals was reduced (3 vs 5 weeks, P = .02). Conclusion and Clinical Importance Aerosolized delivery of gentamicin seems effective for inducing clinical cure in Bb infection. Clinical response appears better using undiluted 5% solution, particularly in the subgroup of dogs having less than 1000 cells/μL in lavage.
Comparison of C‐reactive protein concentrations in dogs with Bordetella bronchiseptica infection and aspiration bronchopneumonia
Background C‐reactive protein (CRP) is a well‐known acute‐phase protein in dogs that may discriminate bacterial bronchopneumonia from other pulmonary conditions. Bronchopneumonia caused by Bordetella bronchiseptica (Bb) is common but the associated increase in CRP concentration in naturally infected dogs has not been fully explored. Objective To compare CRP concentrations of dogs with Bb infection, with or without radiographic pulmonary lesions, to dogs with aspiration bronchopneumonia (ABP). Animals Sixteen dogs with Bb infection and 36 dogs with ABP. Methods Retrospective study. C‐reactive protein concentrations and thoracic radiographs were available for each dog. Results Eleven dogs with Bb infection had alveolar lesions. In all dogs, CRP concentration was mildly increased (14‐38 mg/L). In the 5 dogs without alveolar lesions, CRP concentration was within the reference range in all but 1 dog, in which it was slightly increased. Median CRP concentration was significantly higher in dogs with alveolar lesions (20 mg/L) compared with dogs without alveolar lesions (5 mg/L; p < .002). In dogs with Bb infection, median duration of clinical signs was not different between dogs with normal CRP concentration and dogs with increased concentration. In dogs with Bb infection either with or without alveolar lessions, median CRP concentration was significantly lower (20 mg/L) than in dogs with ABP (118 mg/L; p < .001). Conclusions and Clinical Importance In contrast to dogs with APB, CRP was not a good marker for the diagnosis of dogs suspected to have bordetellosis. Confirmation of Bb infection still requires lower airway sampling.
Outcome after allogeneic stem cell transplantation with haploidentical versus HLA-matched donors in patients with higher-risk MDS
Allogeneic hematopoietic stem cell transplantation remains the best curative option for higher-risk myelodysplastic syndrome. The presence of monosomal karyotype and/or complex karyotype abnormalities predicts inferior survival after allo-SCT in MDS patients. Haploidentical allo-SCT has been increasingly used in acute leukemia (AL) and has similar results as using HLA-matched donors, but data on higher-risk MDS is sparse. We compared outcomes in 266 patients with higher-risk MDS after HLA-matched sibling donor (MSD, n = 79), HLA-matched unrelated donor (MUD, n = 139) and HLA haploidentical donor (HID, n = 48) from 2010 to 2019. Median donor age differed between the three groups (p < 0.001). The overall survival was significantly different between the three groups with a better OS observed in the MUD group (p = 0.014). This observation could be explained by a higher progression-free survival with MUD (p = 0.014). The cumulative incidence of grade 2–4 acute GvHD was significantly higher in the HID group (p = 0.051). However, in multivariable analysis, patients transplanted using an HID had comparable mortality to patients transplanted using a MUD (subdistribution hazard ratio [sHR]: 0.58 [0.32–1.07]; p = 0.080) and a MSD ([sHR]: 0.56 [0.28–1.11]; p = 0.094). MUD do not remain a significant positive predictor of survival, suggesting that beyond the donor-recipient HLA matching, the donor age might impact recipient outcome.
Fast-moving features in the debris disk around AU Microscopii
High-contrast imaging of the nearby, young, active late-type star AU Microscopii reveals five mysterious large-scale features in the southeast side of its debris disk, moving away from the star. The so-called debris disks found around stars in the 1980s were thought to be byproducts of planet formation as they often exhibited morphological and brightness asymmetries that may have resulted from gravitational perturbation by planets. This assumption was proven correct for the β Pictoris system, but the exact nature and origin of the fast-moving features in the AU Mic disk are unknown. In the 1980s, excess infrared emission was discovered around main-sequence stars; subsequent direct-imaging observations revealed orbiting disks of cold dust to be the source 1 . These ‘debris disks’ were thought to be by-products of planet formation because they often exhibited morphological and brightness asymmetries that may result from gravitational perturbation by planets. This was proved to be true for the β Pictoris system, in which the known planet generates an observable warp in the disk 2 , 3 , 4 , 5 . The nearby, young, unusually active late-type star AU Microscopii hosts a well-studied edge-on debris disk; earlier observations in the visible and near-infrared found asymmetric localized structures in the form of intensity variations along the midplane of the disk beyond a distance of 20 astronomical units 6 , 7 , 8 , 9 . Here we report high-contrast imaging that reveals a series of five large-scale features in the southeast side of the disk, at projected separations of 10–60 astronomical units, persisting over intervals of 1–4 years. All these features appear to move away from the star at projected speeds of 4–10 kilometres per second, suggesting highly eccentric or unbound trajectories if they are associated with physical entities. The origin, localization, morphology and rapid evolution of these features are difficult to reconcile with current theories.
Prenatal Intestinal Obstruction Affects the Myenteric Plexus and Causes Functional Bowel Impairment in Fetal Rat Experimental Model of Intestinal Atresia
Intestinal atresia is a rare congenital disorder with an incidence of 3/10,000 birth. About one-third of patients have severe intestinal dysfunction after surgical repair. We examined whether prenatal gastrointestinal obstruction might effect on the myenteric plexus and account for subsequent functional disorders. We studied a rat model of surgically induced antenatal atresia, comparing intestinal samples from both sides of the obstruction and with healthy rat pups controls. Whole-mount preparations of the myenteric plexus were stained for choline acetyltransferase (ChAT) and nitric oxide synthase (nNOS). Quantitative reverse transcription PCR was used to analyze mRNAs for inflammatory markers. Functional motility and permeability analyses were performed in vitro. Phenotypic studies were also performed in 8 newborns with intestinal atresia. In the experimental model, the proportion of nNOS-immunoreactive neurons was similar in proximal and distal segments (6.7±4.6% vs 5.6±4.2%, p = 0.25), but proximal segments contained a higher proportion of ChAT-immunoreactive neurons (13.2±6.2% vs 7.5±4.3%, p = 0.005). Phenotypic changes were associated with a 100-fold lower concentration-dependent contractile response to carbachol and a 1.6-fold higher EFS-induced contractile response in proximal compared to distal segments. Transcellular (p = 0.002) but not paracellular permeability was increased. Comparison with controls showed that modifications involved not only proximal but also distal segments. Phenotypic studies in human atresia confirmed the changes in ChAT expression. Experimental atresia in fetal rat induces differential myenteric plexus phenotypical as well as functional changes (motility and permeability) between the two sides of the obstruction. Delineating these changes might help to identify markers predictive of motility dysfunction and to define guidelines for post-surgical care.
Head injuries in prehospital and Emergency Department settings: a prospective multicenter cross-sectional study in France
Background Head injuries are the leading cause of trauma in Emergency Departments (EDs). Recent studies have shown epidemiological changes in patients consulting ED for head injuries. The main objective of this study was to describe the profile of head injury patients consulting in the EDs in France and assess i) head injury severity across age groups; ii) the delay between the occurrence of head injury and ED arrival; iii) factors associated with traumatic intracranial hemorrhage (ICH). Methods This cross-sectional study collected patient data over a three-day period in March 2023. All adult patients (≥ 18 years old) admitted to the ED with a head injury (defined as a trauma to the head) were included. TBI severity was classified according to patients’ initial Glasgow Coma Scale score in the ED: severe (3–8); moderate (9 -12); mild (13–15); and simple head trauma in the absence of transient or persistent neurological symptoms. Results Among the 71 participating EDs, 26,008 patients visited EDs and a total of 1070 patients (4.1%, IC 95 3.9—4.4) presented a head injury were included in the study, with a median age of 68.5 [37–85] years old. Most of the patients (66.7%) were referred to ED after a call to the Emergency Medical Dispatcher (EMD). The median time from head injury to ED visit was 2 h [1.0 – 5.5]. Ground-level falls were the leading cause of head injury (60.3%). Most of patient presented a simple head trauma ( n  = 715, 66.8%) followed by mild TBI ( n  = 337, 31.5%). CT head scans were performed for 636 patients (59.6%), of which 58 were positive. Traumatic ICH prevalence was 5.4% (95% CI: 4.1–6.9) and three patients (0.3%) required an urgent neurosurgical intervention. Neither preinjury anticoagulant ( p  = 0.97) nor antiplatelet ( p  = 0.93) use was associated with an increased risk of traumatic ICH. Conclusions One head injury patient out of two presenting in the ED is aged over 65 years. Patients referred by EMD were more likely to visit ED promptly. The majority of older patients underwent a head CT scan and preinjury anticoagulant use was not associated with increased risk of traumatic ICH.
FAUST XVII: Super deuteration in the planet forming system IRS 63 where the streamer strikes the disk
Recent observations suggest that planets formation starts early, in protostellar disks of \\(\\le10^5\\) yrs, which are characterized by strong interactions with the environment, e.g., through accretion streamers and molecular outflows. To investigate the impact of such phenomena on disk physical and chemical properties it is key to understand what chemistry planets inherit from their natal environment. In the context of the ALMA Large Program Fifty AU STudy of the chemistry in the disk/envelope system of Solar-like protostars (FAUST), we present observations on scales from ~1500 au to ~60 au of H\\(_2\\)CO, HDCO, and D\\(_2\\)CO towards the young planet-forming disk IRS~63. H\\(_2\\)CO probes the gas in the disk as well as in a large scale streamer (~1500 au) impacting onto the South-East (SE) disk side. We detect for the first time deuterated formaldehyde, HDCO and D\\(_2\\)CO, in a planet-forming disk, and HDCO in the streamer that is feeding it. This allows us to estimate the deuterium fractionation of H\\(_2\\)CO in the disk: [HDCO]/[H\\(_2\\)CO]\\(\\sim0.1-0.3\\) and [D\\(_2\\)CO]/[H\\(_2\\)CO]\\(\\sim0.1\\). Interestingly, while HDCO follows the H\\(_2\\)CO distribution in the disk and in the streamer, the distribution of D\\(_2\\)CO is highly asymmetric, with a peak of the emission (and [D]/[H] ratio) in the SE disk side, where the streamer crashes onto the disk. In addition, D\\(_2\\)CO is detected in two spots along the blue- and red-shifted outflow. This suggests that: (i) in the disk, HDCO formation is dominated by gas-phase reactions similarly to H\\(_2\\)CO, while (ii) D\\(_2\\)CO was mainly formed on the grain mantles during the prestellar phase and/or in the disk itself, and is at present released in the gas-phase in the shocks driven by the streamer and the outflow. These findings testify on the key role of streamers in the build-up of the disk both concerning the final mass available for planet formation and its chemical composition.
Reference-star differential imaging on SPHERE/IRDIS
Reference-star differential imaging (RDI) is a promising technique in high-contrast imaging that is thought to be more sensitive to exoplanets and disks than angular differential imaging (ADI) at short angular separations (i.e., <0.3\"). However, it is unknown whether the performance of RDI on ground-based instruments can be improved by using all the archival data to optimize the subtraction of stellar contributions. We characterize the performance of RDI on SPHERE/IRDIS data in direct imaging of exoplanets and disks. We made use of all the archival data in H23 obtained by SPHERE/IRDIS in the past five years to build a master reference library and perform RDI. In the point-source detection, RDI can outperform ADI at small angular separations (<0.4\") if the observing conditions are around the median conditions of our master reference library. On average, RDI has a gain of ~0.8 mag over ADI at 0.15\" separation for observations under median conditions. We demonstrate that including more reference targets in the master reference library can indeed help to improve the performance of RDI. In disk imaging, RDI can reveal more disk features and provide a more robust recovery of the disk morphology. We resolve 33 disks in total intensity (19 planet-forming disks and 14 debris disks), and 4 of them can only be detected with RDI. Two disks are resolved in scattered light for the first time. Three disks are detected in total intensity for the first time. The master reference library we built in this work can be easily implemented into legacy or future SPHERE surveys to perform RDI, achieving better performance than that of ADI. To obtain optimal RDI gains over ADI, we recommend future observations be carried out under seeing conditions of 0.6\"-0.8\".
Hot methanol in the BHB2007 11 protobinary system: hot corino versus shock origin? : FAUST V
Methanol is a ubiquitous species commonly found in the molecular interstellar medium. It is also a crucial seed species for the building-up of the chemical complexity in star forming regions. Thus, understanding how its abundance evolves during the star formation process and whether it enriches the emerging planetary system is of paramount importance. We used new data from the ALMA Large Program FAUST (Fifty AU STudy of the chemistry in the disk/envelope system of Solar-like protostars) to study the methanol line emission towards the [BHB2007] 11 protobinary system (sources A and B), where a complex structure of filaments connecting the two sources with a larger circumbinary disk has been previously detected. Twelve methanol lines have been detected with upper energies in the range [45-537] K along with one 13CH3OH transition. The methanol emission is compact and encompasses both protostars, separated by only 28 au and presents three velocity components, not spatially resolved by our observations, associated with three different spatial regions, with two of them close to 11B and the third one associated with 11A. A non-LTE radiative transfer analysis of the methanol lines concludes that the gas is hot and dense and highly enriched in methanol with an abundance as high as 1e-5. Using previous continuum data, we show that dust opacity can potentially completely absorb the methanol line emission from the two binary objects. Although we cannot firmly exclude other possibilities, we suggest that the detected hot methanol is resulting from the shocked gas from the incoming filaments streaming towards [BHB2007] 11 A and B, respectively. Higher spatial resolution observations are necessary to confirm this hypothesis.
