Digital Medicine in the Management of Heart Failure: From Reactive Care to Predictive, Pathophysiology-Driven Strategies

Background: Heart failure (HF) is a progressive, multisystem syndrome characterized by recurrent decompensation, high hospitalization rates, and substantial mortality. Conventional HF management is mainly episodic and often fails to detect worsening conditions in advanced disease. Digital medicine and remote patient monitoring (RPM) hold promise for advancing HF care by enabling earlier detection, proactive action, and personalized care. Methods: We conduct a narrative review to summarize evidence from randomized clinical trials, real-world registries, and emerging digital health technologies regarding the present and future utility of digital medicine in HF care. There is greater emphasis on pathophysiology-based surveillance, personalized care models, and integration into planned health care pathways. Results: Integrated digital interventions, such as implantable hemodynamic monitoring, organized telemedicine programs, or device-based diagnostic technologies, can minimize HF hospitalizations, prolong life, improve quality of life, and optimize resource utilization in health care systems when incorporated into coordinated care. Crucially, trials emphasize that clinical benefit depends not on technology but on a prompt clinical response, multidisciplinary cooperation, and ongoing interaction between the patient and the doctor. New technologies—including voice-based biomarkers, smartphone-derived photoplethysmography, ballistocardiography, and artificial intelligence–driven data integration—may help transition RPM from a hardware-based system to a scalable, “deviceless” approach. Conclusions: Digital medicine is a game-changer for reimagining HF care, involving not only continuous monitoring of physiological changes but also personalized, proactive clinical decision-making. To implement truly patient-centered, predictive HF management in the years to come, technological innovation must be combined with human connection, ethical governance, and health-system readiness.

The 2024 guidelines of the European Society of Cardiology (ESC) refer to four very important therapeutic areas: arterial hypertension, atrial fibrillation, chronic coronary syndromes and peripheral artery disease. The ESC countries and their institutional members share a common goal of reducing the burden of cardiovascular diseases, which remain the leading cause of death worldwide. Experts from these six countries - three from Europe and three from Asia - gathered at the annual meeting to briefly present the epidemiological situation regarding selected issues addressed in the 2024 ESC guidelines, and to create a document highlighting the significant progress in pharmacotherapy. The new guidelines allow us to identify particularly important therapeutic areas and unmet pharmaceutical needs within the four treatment guidelines developed by the ESC in 2024. While discussing each of these four documents, 10 subjectively selected points were chosen to highlight what should be kept in mind in daily clinical practice. Altogether, a range of all-risk categories are represented, and the observations made in this position paper are of a universal nature.

Takotsubo syndrome (TTS) still remains as an enigmatic phenomenon. In particular, long-term challenges (including clinical recurrence and persistent symptoms) and specific entities in the setting of TTS have been the evolving areas of interest. On the other hand, a significant gap still exists regarding the proper risk-stratification of this phenomenon in the short and long terms. The present paper, the second part (part-2) of the consensus report, aims to discuss less well-known aspects of TTS including specific entities, challenges after recovery and risk-stratification.

In the recent years, there has been a burgeoning interest in Takotsubo syndrome (TTS), which is renowned as a specific form of reversible myocardial dysfunction. Despite the extensive literature available on TTS, clinicians still face several practical challenges associated with the diagnosis and management of this phenomenon. This potentially results in the underdiagnosis and improper management of TTS in clinical practice. The present paper, the first part (part-1) of the consensus report, aims to cover diagnostic and therapeutic challenges associated with TTS along with certain recommendations to combat these challenges.

Artificial intelligence (AI) is being intensively applied to cardiology, particularly in diagnostics, risk prediction, treatment planning, and invasive procedures. While AI-driven advancements have demonstrated promise, their real-world implementation remains constrained by critical challenges. Current AI applications, such as electrocardiogram interpretation and automated imaging analysis, have improved diagnostic accuracy and workflow efficiency, yet generalizability, regulatory hurdles, and integration into existing clinical workflows remain major obstacles. Algorithmic bias and the lack of explainable AI further complicate widespread adoption, potentially leading to disparities in healthcare outcomes. In interventional cardiology, robotic-assisted percutaneous coronary intervention has emerged as a technological innovation, but comparative clinical evidence supporting its superiority (or even non-inferiority) over conventional approaches is still limited. Additionally, AI-based decision support systems in high-risk cardiovascular procedures require rigorous validation to ensure safety and reliability. Ethical considerations, including patient data security and region-specific regulatory frameworks, also pose significant barriers. Addressing these challenges requires interdisciplinary collaboration, robust external validation, and the development of transparent, interpretable AI models. This review provides a critical appraisal of the current role of AI in cardiology, emphasizing both its potential and its limitations, and outlines future directions to facilitate its responsible integration into clinical practice.
