This contemporary, international, evidence-informed guidance aims to achieve the greatest good for the greatest number of people with familial hypercholesterolaemia (FH) across different countries. FH, a family of monogenic defects in the hepatic LDL clearance pathway, is a preventable cause of premature coronary artery disease and death. Worldwide, 35 million people have FH, but most remain undiagnosed or undertreated. Current FH care is guided by a useful and diverse group of evidence-based guidelines, with some primarily directed at cholesterol management and some that are country-specific. However, none of these guidelines provides a comprehensive overview of FH care that includes both the lifelong components of clinical practice and strategies for implementation. Therefore, a group of international experts systematically developed this guidance to compile clinical strategies from existing evidence-based guidelines for the detection (screening, diagnosis, genetic testing and counselling) and management (risk stratification, treatment of adults or children with heterozygous or homozygous FH, therapy during pregnancy and use of apheresis) of patients with FH, update evidence-informed clinical recommendations, and develop and integrate consensus-based implementation strategies at the patient, provider and health-care system levels, with the aim of maximizing the potential benefit for at-risk patients and their families worldwide.Familial hypercholesterolaemia (FH) is a preventable cause of premature coronary artery disease and death. Fashioning the Victorians : a critical sourcebook
Offering a unique anthology of primary texts, this sourcebook opens a window on the writing that shaped and mirrored Victorian fashion, taking us from corsets to crinolines, dandies to decadent 'New Women'. Surface-to-Space Atmospheric Waves From Hunga Tonga–Hunga Ha’apai Eruption

The January 2022 Hunga Tonga–Hunga Ha’apai eruption was one of the most explosive volcanic events of the modern era, producing a vertical plume that peaked more than 50 km above the Earth. The initial explosion and subsequent plume triggered atmospheric waves that propagated around the world multiple times. A global-scale wave response of this magnitude from a single source has not previously been observed. Here we show the details of this response, using a comprehensive set of satellite and ground-based observations to quantify it from surface to ionosphere. A broad spectrum of waves was triggered by the initial explosion, including Lamb waves propagating at phase speeds of 318.2 ± 6 m s^(−1) at surface level and between 308 ± 5 to 319 ± 4 m s^(−1) in the stratosphere, and gravity waves propagating at 238 ± 3 to 269 ± 3 m s^(−1) in the stratosphere. Gravity waves at sub-ionospheric heights have not previously been observed propagating at this speed or over the whole Earth from a single source. Latent heat release from the plume remained the most significant individual gravity wave source worldwide for more than 12 h, producing circular wavefronts visible across the Pacific basin in satellite observations. A single source dominating such a large region is also unique in the observational record. The Hunga Tonga eruption represents a key natural experiment in how the atmosphere responds to a sudden point-source-driven state change, which will be of use for improving weather and climate models.

The southern stratospheric gravity wave hot spot: individual waves and their momentum fluxes measured by COSMIC GPS-RO

Nearly all general circulation models significantly fail to reproduce the observed behaviour of the southern wintertime polar vortex. It has been suggested that these biases result from an underestimation of gravity wave drag on the atmosphere at latitudes near 60° S, especially around the \"hot spot\" of intense gravity wave fluxes above the mountainous Southern Andes and Antarctic peninsula. Here, we use Global Positioning System radio occultation (GPS-RO) data from the COSMIC satellite constellation to determine the properties of gravity waves in the hot spot and beyond. We show considerable southward propagation to latitudes near 60° S of waves apparently generated over the southern Andes. We propose that this propagation may account for much of the wave drag missing from the models. Furthermore, there is a long leeward region of increased gravity wave energy that sweeps eastwards from the mountains over the Southern Ocean. Despite its striking nature, the source of this region has historically proved difficult to determine. Our observations suggest that this region includes both waves generated locally and orographic waves advected downwind from the hot spot. We describe and use a new wavelet-based analysis technique for the quantitative identification of individual waves from COSMIC temperature profiles. This analysis reveals different geographical regimes of wave amplitude and short-timescale variability in the wave field over the Southern Ocean. Finally, we use the increased numbers of closely spaced pairs of profiles from the deployment phase of the COSMIC constellation in 2006 to make estimates of gravity wave horizontal wavelengths. We show that, given sufficient observations, GPS-RO can produce physically reasonable estimates of stratospheric gravity wave momentum flux in the hot spot that are consistent with measurements made by other techniques. We discuss our results in the context of previous satellite and modelling studies and explain how they advance our understanding of the nature and origins of waves in the southern stratosphere.

Computational methods have made substantial progress in improving the accuracy and throughput of pathology workflows for diagnostic, prognostic, and genomic prediction. Still, lack of interpretability remains a significant barrier to clinical integration. We present an approach for predicting clinically-relevant molecular phenotypes from whole-slide histopathology images using human-interpretable image features (HIFs). Our method leverages >1.6 million annotations from board-certified pathologists across >5700 samples to train deep learning models for cell and tissue classification that can exhaustively map whole-slide images at two and four micron-resolution. Cell- and tissue-type model outputs are combined into 607 HIFs that quantify specific and biologically-relevant characteristics across five cancer types. We demonstrate that these HIFs correlate with well-known markers of the tumor microenvironment and can predict diverse molecular signatures (AUROC 0.601–0.864), including expression of four immune checkpoint proteins and homologous recombination deficiency, with performance comparable to ‘black-box’ methods. Our HIF-based approach provides a comprehensive, quantitative, and interpretable window into the composition and spatial architecture of the tumor microenvironment.

Conservation, acquisition, and functional impact of sex-biased gene expression in mammals

In mammals, many species exhibit sex-specific phenotypes that differ between males and females. Although attention has been directed to the effects of the X and Y sex chromosomes, we do not understand how sex affects the rest of the genome. Naqvi et al. examined gene expression in 12 tissues in male and female humans, mice, rats, dogs, and cynomolgus macaques and identified diversity in gene expression between the sexes. Examining sex-biased gene expression in human height identified opposing male or female bias. Although conservation of differential sex-specific gene expression among species was observed, specific genes differed in the sexes among species and lineages suggesting the evolution of species- or lineage-specific sex-biased expression. Science , this issue p. eaaw7317 Mammalian genome-wide gene expression varies between males and females. Sex differences abound in human health and disease, as they do in other mammals used as models. The extent to which sex differences are conserved at the molecular level across species and tissues is unknown. We surveyed sex differences in gene expression in human, macaque, mouse, rat, and dog, across 12 tissues. In each tissue, we identified hundreds of genes with conserved sex-biased expression—findings that, combined with genomic analyses of human height, explain ~12% of the difference in height between females and males. We surmise that conserved sex biases in expression of genes otherwise operating equivalently in females and males contribute to sex differences in traits. However, most sex-biased expression arose during the mammalian radiation, which suggests that careful attention to interspecies divergence is needed when modeling human sex differences.
