Interstitial photodynamic therapy for newly diagnosed glioblastoma
Purpose Innovative, efficient treatments are desperately needed for people with glioblastoma (GBM). Methods Sixteen patients (median age 65.8 years) with newly diagnosed, small-sized, not safely resectable supratentorial GBM underwent interstitial photodynamic therapy (iPDT) as upfront eradicating local therapy followed by standard chemoradiation. 5-aminolevulinic acid (5-ALA) induced protoporphyrin IX was used as the photosensitizer. The tumors were irradiated with light at 635 nm wavelength via stereotactically implanted cylindrical diffuser fibers. Outcome after iPDT was retrospectively compared with a positively-selected in-house patient cohort (n = 110) who underwent complete tumor resection followed by chemoradiation. Results Median progression-free survival (PFS) was 16.4 months, and median overall survival (OS) was 28.0 months. Seven patients (43.8%) experienced long-term PFS > 24 months. Median follow-up was 113.9 months for the survivors. Univariate regression revealed MGMT-promoter methylation but not age as a prognostic factor for both OS (p = 0.04 and p = 0.07) and PFS (p = 0.04 and p = 0.67). Permanent iPDT-associated morbidity was seen in one iPDT patient (6.3%). Patients treated with iPDT experienced superior PFS and OS compared to patients who underwent complete tumor removal (p < 0.01 and p = 0.01, respectively). The rate of long-term PFS was higher in iPDT-treated patients (43.8% vs. 8.9%, p < 0.01). Conclusion iPDT is a feasible treatment concept and might be associated with long-term PFS in a subgroup of GBM patients, potentially via induction of so far unknown immunological tumor-controlling processes.
Interstitial Photodynamic Therapy of Glioblastomas: A Long-Term Follow-up Analysis of Survival and Volumetric MRI Data
Background: The treatment of glioblastomas, the most common primary malignant brain tumors, with a devastating survival perspective, remains a major challenge in medicine. Among the recently explored therapeutic approaches, 5-aminolevulinic acid (5-ALA)-mediated interstitial photodynamic therapy (iPDT) has shown promising results. Methods: A total of 16 patients suffering from de novo glioblastomas and undergoing iPDT as their primary treatment were retrospectively analyzed regarding survival and the characteristic tissue regions discernible in the MRI data before treatment and during follow-up. These regions were segmented at different stages and were analyzed, especially regarding their relation to survival. Results: In comparison to the reference cohorts treated with other therapies, the iPDT cohort showed a significantly prolonged progression-free survival (PFS) and overall survival (OS). A total of 10 of 16 patients experienced prolonged OS (≥ 24 months). The dominant prognosis-affecting factor was the MGMT promoter methylation status (methylated: median PFS of 35.7 months and median OS of 43.9 months) (unmethylated: median PFS of 8.3 months and median OS of 15.0 months) (combined: median PFS of 16.4 months and median OS of 28.0 months). Several parameters with a known prognostic relevance to survival after standard treatment were not found to be relevant to this iPDT cohort, such as the necrosis–tumor ratio, tumor volume, and posttreatment contrast enhancement. After iPDT, a characteristic structure (iPDT remnant) appeared in the MRI data in the former tumor area. Conclusions: In this study, iPDT showed its potential as a treatment option for glioblastomas, with a large fraction of patients having prolonged OS. Parameters of prognostic relevance could be derived from the patient characteristics and MRI data, but they may partially need to be interpreted differently compared to the standard of care.

In the boom era of the digital advertising, the search engine platforms have rushed to attract consumers and increase the number of subscribers. Google Company has managed to develop digital advertising product named Interactive graphics (Doodles) in order to disseminate its visual identity. Google has shared with the world the details of its events that are not limited to the international public figures but in all the consumer's moments. Google has created from its trade mark the doodles and creative games amid wide electronic space. This research proposes that the Digital Advertisement is a mean to emphasize the Arabic heritage and identity through the presentation of analytical study for the Google search engine. Some believe that the oldest search engine is Google, while the first launch of \"Google\" was in 1998 but It is reported that the first search engine launched on the Internet was in 1990 created by a student at McGill University in Montreal, and the engine was known as \"Archie\" - it is derived from the word \"archive\" with the removal of the letter \"v\" - and it automatically loads a list of all files available on every Internet site. The search was done through this list with names of the files (3, 4). Brief on the evolution of search engines: Then the search process developed in the year 1993 so the Wandex engine appeared where the web started and expanded. The Internet is no longer restricted to files stored on connected computers, but it helps to increase the numbers of actual web pages. Another student from MIT created the search engine (the web) and this engine has the ability to search on all sites connected to the Internet, not just file names as in the (Archie) engine. The big jump came in the history of the search in 1994 with a search engine called (Web Crawler) and this was the first engine that created an index for all the names and locations of different sites, and every single word inside these pages that were passed on. This allowed the user for the first time to search through the actual contents of network sites via the search engine (3, 4).
Towards a circular economy: valorization of banana peels by developing bio-composites thermal insulators
The building construction materials are responsible for a large amount of energy and natural resource consumption. In light of the current challenges of resource scarcity and global climate change, the circular economy (CE) is a promising strategy to mitigate pressure on the environment, improve supplying of raw materials, and increase new market and employment opportunities. Developing eco-friendly thermal insulation materials based on agro-waste is a new waste management trend to achieve the sustainability of the resource and energy consumption in the construction sectors. In this work, banana-polystyrene composites were prepared by mixing the banana peels powder (BP) with polystyrene (PS) in different weight ratios (90:10, 80:20, 70:30, and 60:40). The physical and thermal properties such as thermal conductivity, electrical conductivity, Fourier Transform Infrared (FTIR), crystallographic structures of the fibers, X-Ray Diffraction (XRD), Thermogravimetric Analysis (TGA), and Differential Scanning Calorimetry (DSC) were carried out on BP and BP-PS1 that were prepared with ten wt.% and 20 wt.% of polystyrene powder (BP-PS2). The bio-composites results showed low thermal conductivity ranging from 0.028 to 0.030 W/m.K. The BP-PS2 exhibited a lower thermal conductivity of 0.027 W/m.K, while the pure peel powder demonstrated notable thermal stability, indicated by a total weight loss of 66.4% and a high crystallinity value of 56.1%. Furthermore, the thermal analysis (TGA) and X-Ray Diffraction (XRD) demonstrated that the pure banana peel has the highest thermal stability and crystallinity. These findings indicate that using banana peel-polystyrene composites represents an innovative solution for thermal insulation in buildings as an alternative to conventional materials to reduce energy and resource consumption.
Development and validation of a predictive scoring system for in-hospital mortality in COVID-19 Egyptian patients: a retrospective study
SARS-CoV-2 virus has rapidly spread worldwide since December 2019, causing COVID-19 disease. In-hospital mortality is a common indicator for evaluating treatment outcomes. Therefore, the developing and validating a simple score system from observational data could assist in modulating the management procedures. A retrospective cohort study included all data records of patients with positive PCR for SARS-CoV-2. The factors that associated with mortality were analyzed, then allocation of potential predictors of mortality was executed using different logistic regression modeling, subsequently scoring system was developed from the most weighted predictors. The mortality rate of patients with COVID-19 pneumonia was 28.5% and 28.74%, respectively. The most significant factors that affected in-hospital mortality were old age (> 60 years), delay in hospital admission (> 4 days), high neutrophil/lymphocyte ratio “NLR” (> 3); higher computed tomography severity score; and CT-SS (> 20), in addition to using remdesivir and tocilizumab in the treatment protocol (P < 0.001 for all). The validity of the newly performed score was significant; the AUC was 85%, P < 0.001, and its prognostic utility was good; the AUC was 75%, P < 0.001. The prognostic utility of newly developed score system (EGY.Score) was excellent and could be used to adjust the treatment strategy of highly at-risk patients with COVID-19 pneumonia.
Immobilized lipase enzyme on green synthesized magnetic nanoparticles using Psidium guava leaves for dye degradation and antimicrobial activities
Zinc ferrite nanoparticles (ZnF NPs) were synthesized by a green method using Psidium guava Leaves extract and characterized via structural and optical properties. The surface of ZnF NPs was stabilized with citric acid (CA) by a direct addition method to obtain (ZnF-CA NPs), and then lipase (LP) enzyme was immobilized on ZnF-CA NPs to obtain a modified ZnF-CA-LP nanocomposite (NCs). The prepared sample’s photocatalytic activity against Methylene blue dye (MB) was determined. The antioxidant activity of ZnF-CA-LP NCs was measured using 1,1-diphenyl-2-picryl hydrazyl (DPPH) as a source of free radicals. In addition, the antibacterial and antibiofilm capabilities of these substances were investigated by testing them against gram-positive Staphylococcus aureus ( S. aureus ATCC 25923) and gram-negative Escherichia coli ( E. coli ATCC 25922) bacterial strains. The synthesized ZnF NPs were discovered to be situated at the core of the material, as determined by XRD, HRTEM, and SEM investigations, while the CA and lipase enzymes were coated in this core. The ZnF-CA-LP NCs crystallite size was around 35.0 nm at the (311) plane. Results obtained suggested that 0.01 g of ZnF-CA-LP NCs achieved 96.0% removal of 5.0 ppm of MB at pH 9.0. In-vitro zone of inhibition (ZOI) and minimum inhibitory concentration (MIC) results verified that ZnF-CA-LP NCs exhibited its encouraged antimicrobial activity against S. aureus and E. coli (20.0 ± 0.512, and 27.0 ± 0.651 mm ZOI, respectively) & (1.25, and 0.625 μg/ml MIC, respectively). ZnF-CA-LP NPs showed antibiofilm percentage against S. aureus (88.4%) and E. coli (96.6%). Hence, ZnF-CA-LP NCs are promising for potential applications in environmental and biomedical uses.
Promising photocatalytic and antimicrobial activity of novel capsaicin coated cobalt ferrite nanocatalyst
In this study, CoFe 2 O 4 nanoparticles were prepared by the co-precipitation method then surface modified with Capsaicin ( Capsicum annuum ssp. ). The virgin CoFe 2 O 4 NPs and Capsaicin-coated CoFe 2 O 4 NPs (CPCF NPs) were characterized by XRD, FTIR, SEM, and TEM. The antimicrobial potential and photocatalytic degradation efficiencies of the prepared samples via Fuchsine basic (FB) were investigated. The results revealed that CoFe 2 O 4 NPs have spherical shapes and their diameter varied from 18.0 to 30.0 nm with an average particle size of 25.0 nm. Antimicrobial activity was tested on Gram-positive ( S. aureus ATCC 52923) and Gram-negative ( E. coli  ATCC 52922) by disk diffusion and broth dilution methods to determine the zone of inhibition (ZOI) and minimum inhibitory concentration (MIC), respectively. UV-assisted photocatalytic degradation of FB was examined. Various parameters affecting the photocatalytic efficiency such as pH, initial concentration of FB, and dose of nanocatalyst were studied. The in-vitro ZOI and MIC results verified that CPCF NPs were more active upon Gram-Positive S. aureus ATCC 52923 (23.0 mm ZOI and 0.625 μg/ml MIC) than Gram-Negative E. coli ATCC 52922 (17.0 mm ZOI and 1.250 μg/ml MIC). Results obtained from the photocatalytic activity indicated that the maximum FB removal achieving 94.6% in equilibrium was observed using 20.0 mg of CPCF NPS at pH 9.0. The synthesized CPCF NPs were effective in the removal of FB and also as potent antimicrobial agent against both Gram-positive and Gram-negative bacteria with potential medical and environmental applications.
Antibacterial potential of endophytic Streptomyces spp. isolated from peanut (Arachis hypogaea) roots: bioactiveprofiling and molecular docking studies
The worldwide escalation of antimicrobial resistance (AMR) necessitates the search for new bioactive agents from natural sources. Consequently, this study investigates the antimicrobial activity of endophytic Streptomyces spp. Eighteen Streptomyces isolates were recovered from sixteen peanut root samples using nitrate starch agar at 30 °C for 7 days. Among these, two strains, Streptomyces rochei RSA1 and Streptomyces sp. RSA2, exhibited significant antibacterial activity against both Gram-positive ( Bacillus cereus and Staphylococcus aureus ) and Gram-negative ( Pseudomonas aeruginosa and Escherichia coli ) bacteria, compared with six standard antibiotics. Nine metabolic bioactive compounds were identified using GC-MS. However, two compounds2-(butylthio) pyrimidine-4,6(1 H,5 H)-dione and 2,4-di-tert-butylphenol were particularly prominent (> 96% abundance). Functional groups were confirmed via FT-IR spectra. Molecular docking and dynamics simulations with relevant bacterial protein targets (PDB ID: 6FJH, 1O9G, 1J5E, and 9QT5) revealed strong hydrogen bonding and electrostatic interactions, with S. rochei RSA1 forming the most stable complex. Overall, peanut-derived endophytic Streptomyces represent promising sources of bioactive antibacterial metabolites for combating multidrug-resistant infections.
MoS2-based nanocomposites: synthesis, structure, and applications in water remediation and energy storage: a review
The world is currently facing critical water and energy issues due to the growing population and industrialization, calling for methods to obtain potable water, e.g., by photocatalysis, and to convert solar energy into fuels such as chemical or electrical energy, then storing this energy. Energy storage has been recently improved by using electrochemical capacitors and ion batteries. Research is actually focusing on the synthesis of materials and hybrids displaying improved electronic, physiochemical, electrical, and optical properties. Here, we review molybdenum disulfide (MoS2) materials and hybrids with focus on synthesis, electronic structure and properties, calculations of state, bandgap and charge density profiles, and applications in energy storage and water remediation.
First confirmed case of multisystem inflammatory syndrome in adult in Egypt
Key Clinical Message Our case report demonstrates extremely uncommon data associated with MIS‐A, such as cholestatic jaundice, anemia, and quickly progressing pneumonia. IVIG and pulse steroid medications are the best treatments for improving clinical outcomes. We report a case of multiple organ dysfunctions due to MIS‐A in an adult with a history of suspected COVID‐19. Our case demonstrates extremely uncommon data associated with MIS‐A, such as cholestatic jaundice, anemia, and quickly progressing pneumonia. IVIG and pulse steroid medications are the best treatments for improving clinical outcomes.
