Unusual Presentation of Wünderlich Syndrome
Background: Wünderlich syndrome is a rare but important condition because it involves a sudden blood collection in the renal fossa that can cause hemodynamic instability.Case Report: A 38-year-old female with a history of type 2 diabetes mellitus and hypertension with poor adherence to treatment presented to the emergency department with abdominal pain of 2 weeks’ duration accompanied by irritative lower urinary symptoms. Abdominal computed tomography (CT) scan showed bilateral pyelonephritis and an abscess in the lower pole of the right kidney. A second CT scan, performed because of the patient's abrupt decrease in hemoglobin and hematocrit, showed active bleeding secondary to the infectious process in the right kidney. The patient was hemodynamically unstable, so a nephrectomy was performed.Conclusion: Wünderlich syndrome is a spontaneous renal hemorrhage, in most cases attributed to a tumorous etiology and rarely of infectious origin. The clinical picture is varied but can present with the Lenk triad of acute onset flank pain, flank mass, and hypovolemic shock. It is diagnosed principally via an imaging study such as abdominal CT scan. Treatment is conservative in principle, but urgent surgical intervention is sometimes necessary depending on the clinical situation of the patient.
Tumor of the epididymis: an uncommon presentation of disseminated coccidioidomycosis
Coccidioidomycosis is an endemic disease of arid regions in the Western hemisphere. Its clinical presentation varies from asymptomatic nodules on chest x-rays to disseminated disease. We present the case of a 48-year-old man with a hard and heterogeneous tumor in the posterior aspect of the right testis. Color flow doppler testicular ultrasonography was performed and two nodular masses in the tail of the right epididymis were identified. An epididymectomy was performed and histopathological examination revealed coccidioidomycosis. After diagnosis, the patient was successfully treated with fluconazol.
Comparison of imaging based single-cell resolution spatial transcriptomics profiling platforms using formalin-fixed paraffin-embedded tumor samples
Imaging-based spatial transcriptomics (ST) is evolving as a pivotal technology in studying tumor biology and associated microenvironments. However, the strengths of the commercially available ST platforms in studying spatial biology have not been systematically evaluated using rigorously controlled experiments. We use serial 5 μm sections of formalin-fixed, paraffin-embedded surgically resected lung adenocarcinoma and pleural mesothelioma samples in tissue microarrays to compare the performance of the ST platforms (CosMx, MERFISH, and Xenium (uni/multi-modal)) in reference to bulk RNA sequencing, multiplex immunofluorescence, GeoMx, and hematoxylin and eosin staining data. In addition to an objective assessment of automatic cell segmentation and phenotyping, we perform a manual phenotyping evaluation to assess pathologically meaningful comparisons between ST platforms. Here, we show the intricate differences between the ST platforms, reveal the importance of parameters such as probe design in determining the data quality, and suggest reliable workflows for accurate spatial profiling and molecular discovery. Spatial cell distribution within a tissue microenvironment is a rapidly advancing field. Here, authors assess three commercially available single-cell resolution spatial transcriptomics approaches (CosMx, MERFISH, and Xenium) to inform which technology outperforms for immune profiling of solid tumors using patient samples.
A pilot translational study of neoadjuvant fulvestrant plus abemaciclib in women with advanced low-grade serous carcinoma
Low-grade serous carcinoma of the ovary (LGSOC) is relatively resistant to chemotherapy. Given its biological parallels to hormone receptor-positive breast cancer, including responsiveness to anti-estrogen therapies, we conducted a pilot phase II study to assess clinical benefit of neoadjuvant fulvestrant and abemaciclib in women with advanced, unresectable LGSOC (NCT03531645). Imaging assessments were performed every 8 weeks until resectable. The primary endpoint was clinical benefit rate (CBR). Exploratory objectives included evaluation of safety profile, accessing biomarkers related to clinical benefit, and description of tumor phenotypic changes. Fifteen patients were enrolled and evaluable for efficacy. CBR was 100%, with 1/15 patients (7%) achieving complete response (CR), 8/15 patients (53%) achieving partial response (PR), and 6/15 (40%) exhibiting stable disease (SD). Interval cytoreductive surgery (ICS) was performed in 9 patients (60%), with 7 (78%) achieving complete or optimal resection. Fourteen tumor samples underwent transcriptomic and proteomic profiling. Tumors from long-term survivors showed significantly higher baseline expression of cell-cycle-related programs and estrogen signaling-related genes, both suppressed upon treatment. Neoadjuvant fulvestrant and abemaciclib was well tolerated, achieved high response rates, and enabled optimal surgical resection in most patients. Tumor proliferative activity and estrogen signaling dependency may predict therapy response.
Collision Tumor of the Kidney: Renal Cell Carcinoma Hidden in a Giant Angiomyolipoma in a Patient With Tuberous Sclerosis Complex
Background: A renal angiomyolipoma is a mixed mesenchymal benign tumor composed of smooth muscle, adipose tissue, and blood vessels. Malignant transformation of angiomyolipomas is anecdotal. To our knowledge, only 6 cases have been reported, and 4 of the patients had tuberous sclerosis complex diagnosed.Case Report: We present the case of a 29-year-old male with tuberous sclerosis complex who arrived at the emergency room with gross hematuria and a painful right-sided abdominal mass. Imaging studies revealed active bleeding from a giant angiomyolipoma. An emergency nephrectomy was performed. Histopathology evaluation revealed an angiomyolipoma with a focal lesion and clear cell renal carcinoma within the tumor.Conclusion: Limited evidence is available to dictate management of collision tumors of the kidney in the scenario of tuberous sclerosis complex, so a multidisciplinary approach that includes urology, oncology, genetics, and nephrology intervention needs to be considered. No standardized follow-up modality has been established for angiomyolipomas, so patients should be placed under active surveillance, similar to that carried out in cases of renal cell carcinoma.
Gouty Tophus in the Scrotum: An Unusual Extra-Articular Manifestation of Gout
Gout is a chronic disorder caused by the deposition of monosodium urate crystals in soft tissues. Tophi are granulomatous inflammatory responses to the deposited crystals and manifest as subcutaneous nodules, typically in the first metatarsophalangeal joint but also in the olecranon bursa, Achilles tendon, ears, and finger pulps. A 56-year-old male presented to an outpatient clinic with an 8-month history of an expanding scrotal lesion. The patient had no significant family history but had a history of high blood pressure and gout, diagnosed at age 24 years, without current treatment. Excisional biopsy from the ulcerated area of the scrotum was performed for confirmatory diagnosis, and pathology reported gouty tophus. To our knowledge, this case is the first report of a scrotal manifestation of gouty tophus and the second of genital involvement. Awareness of the possibility of genital manifestations of this disease is important because although gouty tophi are rare, they can present in patients with long-term uncontrolled gout.

Our study detailed the intricate differences between the ST platforms, revealed the importance of parameters such as tissue age and probe design in determining the data quality, and suggested reliable workflows for accurate spatial profiling and molecular discovery.
Decline of Fine Suspended Sediments in the Madeira River Basin (2003–2017)
The Madeira River is the second largest Amazon tributary, contributing up to 50% of the Amazon River’s sediment load. The Madeira has significant hydropower potential, which has started to be used by the Madeira Hydroelectric Complex (MHC), with two large dams along the middle stretch of the river. In this study, fine suspended sediment concentration (FSC) data were assessed downstream of the MHC at the Porto Velho gauging station and at the outlet of each tributary (Beni and Mamoré Rivers, upstream from the MHC), from 2003 to 2017. When comparing the pre-MHC (2003–2008) and post-MHC (2015–2017) periods, a 36% decrease in FSC was observed in the Beni River during the peak months of sediment load (December–March). At Porto Velho, a reduction of 30% was found, which responds to the Upper Madeira Basin and hydroelectric regulation. Concerning water discharge, no significant change occurred, indicating that a lower peak FSC cannot be explained by changes in the peak discharge months. However, lower FSCs are associated with a downward break in the overall time series registered at the outlet of the major sediment supplier—the Beni River—during 2010.
On the Relationship between Suspended Sediment Concentration, Rainfall Variability and Groundwater: An Empirical and Probabilistic Analysis for the Andean Beni River, Bolivia (2003–2016)
Fluvial sediment dynamics plays a key role in the Amazonian environment, with most of the sediments originating in the Andes. The Madeira River, the second largest tributary of the Amazon River, contributes up to 50% of its sediment discharge to the Atlantic Ocean, most of it provided by the Andean part of the Madeira basin, in particular the Beni River. In this study, we assessed the rainfall (R)-surface suspended sediment concentration (SSSC) and discharge (Q)-SSSC relationship at the Rurrenabaque station (200 m a.s.l.) in the Beni Andean piedmont (Bolivia). We started by showing how the R and Q relationship varies throughout the hydrological year (September to August), describing a counter-clockwise hysteresis, and went on to evaluate the R–SSSC and Q–SSSC relationships. Although no marked hysteresis is observed in the first case, a clockwise hysteresis is described in the second. In spite of this, the rating curve normally used ( SSSC = aQ b ) shows a satisfactory R2 = 0.73 (p < 0.05). With regard to water discharge components, a linear function relates the direct surface flow Qs–SSSC, and a hysteresis is observed in the relationship between the base flow Qb and SSSC. A higher base flow index (Qb/Q) is related to lower SSSC and vice versa. This article highlights the role of base flow on sediment dynamics and provides a method to analyze it through a seasonal empirical model combining the influence of both Qb and Qs, which could be employed in other watersheds. A probabilistic method to examine the SSSC relationship with R and Q is also proposed.
