Open intraperitoneal onlay mesh repair with anterior component separation as a bail-out procedure in the management of complex hernias
Purpose Surgical repair of complex abdominal wall hernias remains technically demanding and is widely recognized as a risk factor for unfavorable outcomes with high recurrence and morbidity rates. The objective is to assess short- and long-term complications after open intraperitoneal onlay mesh (IPOM) repair combined with bilateral anterior component separation (ACS) for large and difficult incisional hernias, alongside evaluating hernia recurrence rates. Methods This retrospective analysis utilized data sourced from Hospital electronic health records and a prospective database at an academic tertiary referral center. Data collection was carried out from patients operated between January 2006 and December 2017. Eligible patients had complex incisional hernias measuring at least 10 cm in their transverse diameter and had an open IPOM repair with bilateral ACS. Results In our study group of 45 patients, the 30-day surgical site occurrence (SSO) rate was high (37.8%), primarily consisting of superficial postoperative complications as seroma (17.8%) and wound dehiscence (6.7%). Among six patients (13.3%), wound complications escalated to chronic infected mesh-related problems, leading to complete mesh removal in four cases (8.9%) and partial mesh removal in two cases (4.4%). Regarding long-term complications, five patients (11.1%) developed enterocutaneous fistula. The recurrence rate was modest [5 out of 41 (12.2%)] over a median follow-up period of 99 months. Conclusions Despite a high SSO rate, application of the open IPOM technique with ACS could serve as a valuable rescue option for managing large and complex hernias, with acceptable hernia recurrence rates at long-term follow-up.
Road traffic and landscape characteristics predict the occurrence of native halophytes on roadside verges
Road management practices, such as winter de-icing create ideal habitats and competitive advantage for salt-tolerant species. We aimed to map the occurrences of halophytes along roads in Hungary. Furthermore, we tested factors that might play a role in the roadside occurrences of five chosen native halophytes from rare to common, we encountered during our field surveys. These were Festuca pseudovina , Limonium gmelinii subsp. hungaricum , Podospermum canum , Puccinellia distans and Spergularia media . We found, that at least one halophyte species was documented in 71% of the total sampling points. Germination experiments indicated that substrate salt concentration significantly decreased germination rates in each of the five species, but in case of L. gmelinii subsp. hungaricum , or P. distans germination occurred on extremely high salt concentrations. Traffic intensity, the presence of other halophytes at the sampling point and the presence of a given species in the surrounding landscape had a significant positive effect on the occurrence of four of the five model species. Our results suggest that the studied species are mostly in the early stage of their roadside spread, colonizing roadsides close to their native distribution ranges. The possibility of a future range expansion along roads cannot be excluded.
Adaptive law-based feature representation for time series classification
Time series classification (TSC) underpins applications across finance, healthcare, and environmental monitoring, yet real-world series often contain noise, local misalignment, and multiscale patterns. We introduce adaptive law-based transformation (ALT), a multiscale generalization of the earlier linear law-based transformation (LLT). ALT scans each series with variable-length, shifted windows, constructs symmetric delay embeddings, and extracts eigenvectors associated with the eigenvalue of minimal magnitude (“shapelet laws”) that capture locally stable patterns. These laws are assembled into class-specific dictionaries, and new series are projected onto them to yield compact, transparent features that enhance linear separability while remaining compatible with standard classifiers. On the BasicMotions dataset with synthetic Gaussian noise, ALT sustained test accuracy roughly 15–20 percentage points (pp) above raw inputs and 5–10 pp above LLT at moderate noise levels. Across ten datasets from the UCR Time Series Classification Archive—spanning motion, biomedical, spectroscopy, and industrial domains—ALT improved median test accuracy by + 7.6 pp with k-nearest neighbors (KNN) and + 4.8 pp with support vector machines (SVMs), with particularly large gains on long, noisy industrial series (FordA/B: + 23.1–25.3 pp). In addition, ALT often reduced SVM training time (median reductions of 340.6 s on FordB and 717.5 s on FordA) while maintaining or improving accuracy. ALT thus offers a lightweight and transparent alternative to heavyweight TSC pipelines: it requires only a small hyperparameter set, produces stable and discriminative features, and delivers competitive or superior accuracy under challenging conditions.
Regional and institutional trends in assessment for academic promotion
The assessment of research performance is widely seen as a vital tool in upholding the highest standards of quality, with selection and competition believed to drive progress. Academic institutions need to take critical decisions on hiring and promotion, while facing external pressure by also being subject to research assessment 1 , 2 , 3 – 4 . Here we present an outlook on research assessment for career progression with specific focus on promotion to full professorship, based on 314 policies from 190 academic institutions and 218 policies from 58 government agencies, covering 32 countries in the Global North and 89 countries in the Global South. We investigated how frequently various promotion criteria are mentioned and carried out a statistical analysis to infer commonalities and differences across policies. Although quantitative methods of assessment remain popular, in agreement with what is found in more geographically restricted studies 5 , 6 , 7 , 8 – 9 , they are not omnipresent. We find differences between the Global North and the Global South as well as between institutional and national policies, but less so between disciplines. A preference for bibliometric indicators is more marked in upper-middle-income countries. Although we see some variation, many promotion policies are based on the assumption of specific career paths that become normative rather than embracing diversity. In turn, this restricts opportunities for researchers. These results challenge current practice and have strategic implications for researchers, research managers and national governments. Analysis of policies for promotion criteria to full professor from academic institutions and government agencies worldwide reveals considerable variation in assessment practices, particularly between the Global North and South.
Sequential and choice feeding in laying hens: adapting nutrient supply to requirements during the egg formation cycle
Conventional feeding systems for laying hens rely on a complete feed available ad libitum in mash, pelleted or crumble form. When complete feeds are used, intake is mainly controlled by the hens’ energy requirement and feed presentation, but the birds cannot adjust their consumption to other nutritional needs and thus over-consume to cover the calcium needed for egg shell formation. Sequential, loose-mix and choice feeding offer birds the opportunity to select different diets in the short term. These feeding strategies have been proposed as alternative feeding systems whose main objectives are to match nutrient supply to individual requirements during the daily changes induced by the temporal sequence of the egg formation. This review discusses some findings related to the use of whole cereals and of alternating low and high energy or protein diets, conditions which may improve feed utilisation efficiency. In addition, the adjustment of calcium and phosphorus levels during the day can have benefits in terms of egg production and quality. This review explores the physiological basis for sequential, loose-mix and choice feeding and evaluates the impact of these systems on egg production and quality.
Novel dissolved organic 14C analyses method applied in a case study at a LILW waste repository
A routine chemical procedure was developed at the Ede Hertelendi Laboratory of Environmental Studies (HEKAL), in Debrecen which can measure the dissolved organic radiocarbon content of groundwater as well as the inorganic and total fraction. The typical background of this non-purgeable dissolved organic radiocarbon preparation is 0.73 ± 0.14 percent modern carbon (pMC), using a carbon contamination correction on fossil dissolved material (potassium hydrogen phthalate) samples.
In breast cancer patients sentinel lymph node metastasis characteristics predict further axillary involvement
The aim of the study was to correlate various primary tumor characteristics with lymph node status, to examine sentinel lymph node (SLN) metastasis size and non-SLN axillary involvement, to look for a cut-off size/number value possibly predicting additional axillary involvement with more accuracy and to examine the relationship of SLN metastasis size to overall survival. Of 301 patients who underwent SLN biopsy, 75 had positive SLNs. The size of the metastases was measured. For different size categories, association with the prevalence of non-SLN metastases was assessed. Associations between metastasis size and tumor characteristics and overall survival (OS) were studied. The prevalence of axillary lymph node (ALN) involvement was not significantly different between cases with micrometastasis or macrometastasis in SLNs ( p  = 0.124). However, for metastases larger than 6, 7, and 8 mm, the prevalence of ALN involvement was significantly higher ( p  = 0.046, 0.022, and 0.025). OS was significantly lower in SLN-positive than in SLN-negative cases ( p  = 0.0375). Primary tumor size larger than 20 mm was associated with a significantly higher incidence of SLN metastasis ( p  < 0.001), and primary tumor size over 26 mm was associated with additional positive non-SLN ( p  < 0.001). Higher mitotic index (≥7) in primary tumors was significantly ( p  < 0.001) associated with ALN involvement in SLN-positive cases, whereas higher Ki67 labeling index was not significantly correlated with SLN or ALN involvement. Lymphovascular invasion (LVI) in primary tumors was significantly correlated with SLN positivity ( p  < 0.001) but not with further ALN involvement or OS. Tumor size and LVI are predictive for SLN metastasis. Mitotic index, primary tumor size, and larger volume SLN involvement are determinants of further ALN involvement. SLN metastasis size over 6 mm is a strong predictor of further axillary involvement. OS is shorter in the presence of positive SLN.
Resveratrol improves insulin sensitivity, reduces oxidative stress and activates the Akt pathway in type 2 diabetic patients
Although resveratrol has widely been studied for its potential health benefits, little is known about its metabolic effects in humans. Our aims were to determine whether the polyphenol resveratrol improves insulin sensitivity in type 2 diabetic patients and to gain some insight into the mechanism of its action. After an initial general examination (including blood chemistry), nineteen patients enrolled in the 4-week-long double-blind study were randomly assigned into two groups: a resveratrol group receiving oral 2 × 5 mg resveratrol and a control group receiving placebo. Before and after the second and fourth weeks of the trial, insulin resistance/sensitivity, creatinine-normalised ortho-tyrosine level in urine samples (as a measure of oxidative stress), incretin levels and phosphorylated protein kinase B (pAkt):protein kinase B (Akt) ratio in platelets were assessed and statistically analysed. After the fourth week, resveratrol significantly decreased insulin resistance (homeostasis model of assessment for insulin resistance) and urinary ortho-tyrosine excretion, while it increased the pAkt:Akt ratio in platelets. On the other hand, it had no effect on parameters that relate to β-cell function (i.e. homeostasis model of assessment of β-cell function). The present study shows for the first time that resveratrol improves insulin sensitivity in humans, which might be due to a resveratrol-induced decrease in oxidative stress that leads to a more efficient insulin signalling via the Akt pathway.
