The Tropical Andes Biodiversity Hotspot: A Comprehensive Dataset for the Mira-Mataje Binational Basins
We present a flora and fauna dataset for the Mira-Mataje binational basins. This is an area shared between southwestern Colombia and northwestern Ecuador, where both the Chocó and Tropical Andes biodiversity hotspots converge. We systematized data from 120 sources in the Darwin Core Archive (DwC-A) standard and geospatial vector data format for geographic information systems (GIS) (shapefiles). Sources included natural history museums, published literature, and citizen science repositories across 13 countries. The resulting database has 33,460 records from 6,821 species, of which 540 have been recorded as endemic, and 612 as threatened. The diversity represented in the dataset is equivalent to 10% of the total plant species and 26% of the total terrestrial vertebrate species in both hotspots. The dataset can be used to estimate and compare biodiversity patterns with environmental parameters and provide value to ecosystems, ecoregions, and protected areas. The dataset is a baseline for future assessments of biodiversity in the face of environmental degradation, climate change, and accelerated extinction processes.
Conservation Agriculture Increases Yields and Economic Returns of Potato, Forage, and Grain Systems of the Andes
Physical and environmental vulnerability analysis conducted for the Illangama watershed located in Ecuador's Andean highlands, shows deteriorated soil quality and declining crop productivity. These problems are caused by soil erosion in steep slopes and inappropriate soil management practices. Research projects conducted from 2011 to 2014 adopted and examined the feasibility of conservation agriculture practices for potato (Solanum tuberosum L.), oat (Avena sativa L.) and vetch (Vicia sativa L.), barley (Hordeum vulgare L.), bean (Vicia faba L.), and a mixture of pastures (annual ryegrass [Lolium multiflorum Lam.], perennial ryegrass [L. perenne L.], orchardgrass [Dactylis glomerata L.], white clover[Trifolium repens L.], and red clover [T. pratense L.]). The practices included surface water deviation ditches, reduced tillage, residue retention, and application of N, all within an improved rotation. The study examined crop yields and economic returns in an effort to identify the best practices. Results indicate that crop productivity and net (of cost of production) benefits of the system were increased as much as 25 and 24%, respectively, using a feasible conservation agriculture system compared to conventional practices. This study shows that conservation agriculture increases yields and saves on production costs due to less tillage. We also found that N fertilization with these conservation agriculture practices increases yields and net returns. This study concludes that conservation agriculture practices are good alternatives for these high‐altitude Andean soils. The practices should be promoted among Andean producers to increase the productivity and sustainability of their potato–grain–pasture systems. Core Ideas The Andean region of Ecuador is critical for the country's food security. Cultivation of high‐slope mountainous agriculture is accelerating erosion. Conservation agriculture is an attractive management alternative. Implementation of reduced tillage could contribute to higher net income for farmers. These practices could benefit nearly 200,000 Ecuadorean farms.
Primary systemic therapy in HER2-positive operable breast cancer using trastuzumab and chemotherapy: efficacy data, cardiotoxicity and long-term follow-up in 142 patients diagnosed from 2005 to 2016 at a single institution
The aim of this study was to evaluate the efficacy, cardiotoxicity profile and long-term benefits of neoadjuvant therapy in human epidermal growth factor receptor 2-positive operable breast cancer patients. A total of 142 patients diagnosed from 2005 to 2016 were included in the study. The treatment consisted of a sequential regimen of taxanes and anthracyclines plus trastuzumab. The clinical and pathological responses were evaluated and correlated with clinical and biological factors. The cardiotoxicity profile and long-term benefits were analyzed. The median age was 49 years, and 4%, 69% and 27% of patients had stage I, II and III breast cancer, respectively, while 10% had inflammatory breast cancer at diagnosis. Hormone receptor (HR) status was negative in 43%, and 62% had grade III breast cancer. The clinical complete response rate was 49% and 63% as assessed using ultrasound and magnetic resonance imaging, respectively, and this allowed a high rate of conservative surgery (66%). The pathological complete response (pCR) rate was 52%, and it was higher in HR-negative (64%) patients than in HR-positive (41%) patients and in grade III breast cancer (53%) patients than in grade I-II breast cancer (45%) patients. Patients who achieved pCR had longer disease-free survival and a trend toward improved overall survival. A total of 2% of patients showed a 10% decrease in left ventricular ejection fraction to <50% during treatment. All patients except one recovered after discontinuation of trastuzumab. A sequential regimen of taxanes and anthracyclines plus trastuzumab was effective, with high pCR rates and long-term benefit, and had a very good cardiotoxicity profile.
Conservation Agriculture Increases Profits in an Andean Region of South America
Core Ideas Erosion is decreasing the potential for food security in the Andean region. Zero tillage would be a viable and economical practice for this region. Adding N fertilizer increased the net economic returns by 22%. These practices could potentially increase the income of 200,000 farmers. The Andean region of Ecuador is critical for the country's food security; however, cultivation of high‐slope mountainous agricultural systems that experience significant precipitation is accelerating erosion of the soils and reducing the productivity and sustainability of these systems. For 5 yr we monitored tillage and crop residue management practices using a 2 × 2 factorial randomized block (Phase 1) and a 2 × 2 factorial randomized block with split plot (Phase 2) to assess the effects of tillage, crop residue management, and N fertilization on yields and economic returns. Our study found in the initial phase that for three out of the four crops zero tillage (ZT) had higher average yields than minimum tillage, and for one of these three crops, the increase was significant. Our study found in Phase 2 that when N fertilizer was added as a treatment, compared with crops that were not fertilized, yields were significantly higher in four out of five crops. Leaving the crop residue at the surface was a practice that increased the yields of one of the five crops. The higher net economic returns for Phase 1 were with ZT and with harvesting crop residue. When N was added as a treatment in Phase 2, higher net economic returns were found with ZT and residue removed and with N fertilization. Nitrogen fertilizer, crop residue removal, and zero tillage increased net economic returns by 22, 45.1, and 31.8%, respectively. There is potential to use ZT in this region of South America.
Adalimumab or Infliximab for the Prevention of Early Postoperative Recurrence of Crohn Disease: Results From the ENEIDA Registry
Abstract Background Anti–tumor necrosis factor agents (anti-TNFs) are efficacious at preventing the postoperative recurrence (POR) of Crohn disease, as demonstrated in 2 randomized controlled trials. However, real-life data for infliximab or adalimumab in this setting are scarce. Our aim was to assess both the efficiency of anti-TNFs at preventing early POR of Crohn disease in clinical practice and the associated risk factors for POR. Methods Patients in whom anti-TNFs were prescribed for the prevention of POR within 3 months after ileocolonic resection and who had an endoscopic assessment within 18 months were identified from the ENEIDA registry. Clinical and endoscopic features were collected within 18 months after surgery. Results In total, 152 patients were included (55 treated with infliximab, 97 with adalimumab, and 39% with concomitant immunosuppressants). Anti-TNF treatment was started after a median time of 29 days (IQR 13–44) after surgery. Eighty-two percent of patients had at least one risk factor for POR, and 82% had been exposed to anti-TNFs before the index surgery. Overall, 34% had endoscopic POR (as defined using a Rutgeerts endoscopic score > i1); 14% had advanced endoscopic POR (>i2); and 20% had clinical POR, with no differences between infliximab and adalimumab. In the multivariate analysis, only perianal disease (odds ratio 2.73, 95% confidence interval [CI] 1.26–5.91) and rectal involvement (odds ratio 2.79, 95% CI 1.09–7.14) were independent predictors of endoscopic POR. Conclusions In clinical practice, anti-TNFs for the prevention of POR of Crohn disease are frequently used in patients experienced with anti-TNFs and with concomitant immunosuppressants. The efficacy of infliximab and adalimumab for POR prevention is similar and in accordance with the results obtained in randomized controlled trials. The efficacy of infliximab and adalimumab for POR prevention is similar and are in accordance with the results obtained in RCTs. Perianal disease and rectal involvement were independent predictors of endoscopic POR.
Cancer Histology and Natural History of Patients with Lung Cancer and Venous Thromboembolism
Background: In patients with lung cancer and venous thromboembolism (VTE), the influence of cancer histology on outcome has not been consistently evaluated. Methods: We used the RIETE registry (Registro Informatizado Enfermedad TromboEmbólica) to compare the clinical characteristics and outcomes during anticoagulation in patients with lung cancer and VTE, according to the histology of lung cancer. Results: As of April 2022, there were 482 patients with lung cancer and VTE: adenocarcinoma 293 (61%), squamous 98 (20%), small-cell 44 (9.1%), other 47 (9.8%). The index VTE was diagnosed later in patients with squamous cancer than in those with adenocarcinoma (median, 5 vs. 2 months). In 50% of patients with adenocarcinoma, the VTE appeared within the first 90 days since cancer diagnosis. During anticoagulation (median 106 days, IQR: 45–214), 14 patients developed VTE recurrences, 15 suffered major bleeding, and 218 died: fatal pulmonary embolism 10, fatal bleeding 2. The rate of VTE recurrences was higher than the rate of major bleeding in patients with adenocarcinoma (11 vs. 6 events), and lower in those with other cancer types (3 vs. 9 events). On multivariable analysis, patients with adenocarcinoma had a non-significantly higher risk for VTE recurrences (hazard ratio [HR]: 3.79; 95%CI: 0.76–18.8), a lower risk of major bleeding (HR: 0.29; 95%CI: 0.09–0.95), and a similar risk of mortality (HR: 1.02; 95%CI: 0.76–1.36) than patients with other types of lung cancer. Conclusions: In patients with lung adenocarcinoma, the rate of VTE recurrences outweighed the rate of major bleeding. In patients with other lung cancers, it was the opposite.
Perioperative mortality in Colombia: perspectives of the fourth indicator in The Lancet Commission on Global Surgery – Colombian Surgical Outcomes Study (ColSOS) – a protocol for a multicentre prospective cohort study
IntroductionDeath following surgical procedures is a global health problem, accounting for 4.2 million deaths annually within the first 30 postoperative days. The fourth indicator of The Lancet Commission on Global Surgery is essential as it seeks to standardise postoperative mortality. Consequently, it helps identify the strengths and weaknesses of each country’s healthcare system. Accurate information on this indicator is not available in Colombia, limiting the possibility of interventions applied to our population. We aim to describe the in-hospital perioperative mortality of the surgical procedures performed in Colombia. The data obtained will help formulate public policies, improving the quality of the surgical departments.Methods and analysisAn observational, analytical, multicentre prospective cohort study will be conducted throughout Colombia. Patients over 18 years of age who have undergone a surgical procedure, excluding radiological/endoscopic procedures, will be included. A sample size of 1353 patients has been projected to achieve significance in our primary objective; however, convenience sampling will be used, as we aim to include all possible patients. Data collection will be carried out prospectively for 1 week. Follow-up will continue until hospital discharge, death or a maximum of 30 inpatient days. The primary outcome is perioperative mortality. A descriptive analysis of the data will be performed, along with a case mix analysis of mortality by procedure-related, patient-related and hospital-related conditionsEthics and disseminationThe Fundación Cardioinfantil-Instituto de Cardiología Ethics Committee approved this study (No. 41–2021). DNA sequence and transcript mapping of a soybean gene encoding a small heat shock protein
The DNA sequence of a gene (Gmhsp 17.5-E) encoding a small heat shock protein of soybean, Glycine max, has been determined. Nuclease S1 mapping of the 5′terminus of the corresponding RNA indicates that the start site for transcription is located 82 bases upstream from the coding region and 24 bases downstream from a ``TATA''-like region (-T-T-T-A-A-A-T-A-). The 5′flanking region of Gmhsp 17.5-E contains two imperfect dyads that closely resemble regulatory elements present in the promoters of heat-inducible genes of Drosophila. One, positioned 18 bases upstream from the TATA-like region, shows 90% homology to the Drosophila heat shock consensus sequence. The other overlaps an upstream TATA sequence and is located at position -213. Analysis of the derived amino acid sequence indicates that the protein encoded by Gmhsp 17.5-E is related structurally to the four small heat shock proteins of Drosophila. This relationship is most evident by comparison of hydropathy profiles; they show conservation of several major hydrophilic and hydrophobic regions, which suggests that these proteins have common structural features.
