Association Between Maternal Physical Activity From Pre-pregnancy to Child-rearing and Their Children’s Physical Activity in Early Childhood Among Japanese
Introduction: This study aimed to determine the association between cumulative maternal physical activity level and their children’s physical activity in early childhood. We also compared the influence of each maternal physical activity on children’s physical activity in early childhood.Methods: We analyzed the data from 1,067 Japanese mother-child pairs. Maternal physical activity was assessed using the International Physical Activity Questionnaire. Cumulative physical activity level in mothers was computed based on the categories (low, moderate, and high) of physical activity from five time points (pre-pregnancy, during pregnancy, 1.5, 3.5, and 5.5 years postpartum). Children’s physical activity level was measured at age 5.5 years using the WHO Health Behaviour School-aged Children questionnaire and defined as engaging in physical activity for at least 60 minutes per day for more than 5 days. Logistic regression analysis was used to determine the association between maternal and children’s physical activity levels.Results: The results showed the positive association between cumulative maternal physical activity and children’s physical activity level (P for trend < 0.001). Furthermore, maternal physical activity during pregnancy (P for trend = 0.031) and 5.5 years postpartum (P for trend < 0.001) was positively associated with children’s physical activity.Conclusion: A positive association was observed between the cumulative maternal physical activity level and the physical activity level of their children at 5.5 years of age. Furthermore, maternal physical activity during pregnancy and at 5.5 years postpartum were positively associated with the level of children’s physical activity.
Exposure profile of mercury, lead, cadmium, arsenic, antimony, copper, selenium and zinc in maternal blood, cord blood and placenta: the Tohoku Study of Child Development in Japan
Background The effects of prenatal exposure to toxic elements on birth outcomes and child development have been an area of concern. This study aimed to assess the profile of prenatal exposure to toxic elements, arsenic (As), bismuth (Bi), cadmium (Cd), mercury (total mercury (THg), methylmercury (MHg), inorganic mercury (IHg)), lead (Pb), antimony (Sb) and tin (Sn), and essential trace elements, copper (Cu), selenium (Se) and zinc (Zn), using the maternal blood, cord blood and placenta in the Tohoku Study of Child Development of Japan ( N  = 594–650). Methods Inductively coupled plasma mass spectrometry was used to determine the concentrations of these elements (except mercury). Levels of THg and MeHg were measured using cold vapour atomic absorption spectrophotometry and a gas chromatograph-electron capture detector, respectively. Results Median concentrations (25th–75th) of As, Cd, Pb, Sb, Sn and THg in the maternal blood were 4.06 (2.68–6.81), 1.18 (0.74–1.79), 10.8 (8.65–13.5), 0.2 (0.06–0.40) and 0.2 (0.1–0.38) ng mL −1 and 5.42 (3.89–7.59) ng g −1 , respectively. Median concentrations (25th–75th) of As, Cd, Pb, Sb, Sn and THg in the cord blood were 3.68 (2.58–5.25), 0.53 (0.10–1.25), 9.89 (8.02–12.5), 0.39 (0.06–0.92) and 0.2 (0.2–0.38) ng mL −1 and 9.96 (7.05–13.8) ng g −1 , respectively. Conclusions THg and Sb levels in the cord blood were twofold higher than those in the maternal blood. Cord blood to maternal blood ratios for As, Cd and Sb widely varied between individuals. To understand the effects of prenatal exposure, further research regarding the variations of placental transfer of elements is necessary.
Higher prevalence of hypertensive disorders of pregnancy in women who smoke: the Japan environment and children’s study
Smoking during pregnancy is a risk factor for various adverse birth outcomes but lowers the risk of preeclampsia. Cardiovascular adaptations might underlie these associations. We examined the association of maternal smoking with the risk of hypertensive disorders of pregnancy (HDP) in a low-risk population-based cohort of 76,303 pregnant women. This study was a part of the Japan Environment and Children's Study. Smoking status was assessed using questionnaires completed by participants. Information about HDP was assessed using questionnaires completed by doctors. Compared with that for women who did not smoke, women who continued smoking >10 cigarettes per day during pregnancy had a significantly higher risk of developing HDP (odds ratio: 1.58, 95% confidence interval: 1.11-2.25). In multivariate analyses with adjustment for possible confounding factors, the association still remained (odds ratio: 1.51, 95% confidence interval: 1.04-2.19). When we regarded the number of cigarettes as a continuous variable, there was a linear association between the number of cigarettes and risk of HDP, with an odds ratio of 1.02 per cigarette per day (95% confidence interval: 1.00-1.04). Smoking a greater number of cigarettes was associated with a higher risk of HDP after adjustment for possible confounding factors. Cigarette smoking cessation may avoid the complications of HDP. Our findings suggest that, in addition to the risk of small-for-gestational-age children, an increased risk of HDP should be considered in the management of pregnant women who smoke cigarettes.
Health Risk Assessment and Source Apportionment of Mercury, Lead, Cadmium, Selenium, and Manganese in Japanese Women: An Adjunct Study to the Japan Environment and Children’s Study
Toxic element pollution is a serious global health concern that has been attracting considerable research. In this study, we elucidated the major routes of exposure to three toxic elements (mercury, cadmium, and lead) and two essential elements (manganese and selenium) through diet, soil, house dust, and indoor air and assessed the potential health risks from these elements on women from the coastal area of Miyagi prefecture, Japan. Twenty-four-hour duplicate diet, house dust, soil, and indoor air samples were collected from 37 participants. Cd, Pb, Mn, and Se concentrations were measured using inductively coupled plasma mass spectrometry, and Hg concentrations using cold vapor atomic absorption spectrometry. We found that soil and house dust were the primary reservoirs of these elements. Diet contributed most strongly to the daily intake of these elements, with mean values of 0.72, 0.25, 0.054, 47, and 0.94 μg/kg/day for Hg, Cd, Pb, Mn, and Se, respectively. The mean hazard quotient of Hg was 1.53, indicating a high potential health risk from Hg exposure in daily lives. The intakes of other elements were below the tolerable limits. Future studies with a larger sample size are warranted to confirm our findings.
Health Impacts and Biomarkers of Prenatal Exposure to Methylmercury: Lessons from Minamata, Japan
The main chemical forms of mercury are elemental mercury, inorganic divalent mercury, and methylmercury, which are metabolized in different ways and have differing toxic effects in humans. Among the various chemical forms of mercury, methylmercury is known to be particularly neurotoxic, and was identified as the cause of Minamata disease. It bioaccumulates in fish and shellfish via aquatic food webs, and fish and sea mammals at high trophic levels exhibit high mercury concentrations. Most human methylmercury exposure occurs through seafood consumption. Methylmercury easily penetrates the blood-brain barrier and so can affect the nervous system. Fetuses are known to be at particularly high risk of methylmercury exposure. In this review, we summarize the health effects and exposure assessment of methylmercury as follows: (1) methylmercury toxicity, (2) history and background of Minamata disease, (3) methylmercury pollution in the Minamata area according to analyses of preserved umbilical cords, (4) changes in the sex ratio in Minamata area, (5) neuropathology in fetuses, (6) kinetics of methylmercury in fetuses, (7) exposure assessment in fetuses.
Direct and indirect effects of smoking during pregnancy on child development: The Japan Environment and Children’s Study
Maternal smoking during pregnancy is known to have an adverse impact on child development. This study aimed to elucidate whether maternal smoking during pregnancy has a direct adverse effect on the child development or whether it exerts an indirect effect through birth weight reduction or cadmium exposure. Mothers and infants enrolled in the Japan Environment and Children’s Study were evaluated. A total of 4794 mother–child pairs were included in the analysis. They were classified into four groups according to their smoking status during pregnancy (non-smokers [Q1, n = 2966], ex-smokers who quit before pregnancy [Q2, n = 575], ex-smokers who quit during pregnancy [Q3, n = 505] and smokers [Q4, n = 168)]). Children’s development was assessed at 2 and 4 years of age using a standardized tool, the Kyoto Scale of Psychological Development. Among male children, Q1 exhibited higher scores in all domains except posture-motor than Q4. Female children did not show lower scores in Q4 relative to the other groups. Path analysis revealed that maternal smoking increased cadmium exposure, a direct negative effect of maternal smoking was observed. The findings of this study confirm that maternal smoking during pregnancy has a direct negative impact on the development of children, particularly males.
Association between alcohol consumption during pregnancy and hypertensive disorders of pregnancy in Japan: the Japan Environment and Children’s Study
This study examined the association between maternal alcohol consumption during pregnancy and hypertensive disorders of pregnancy in the Japan Environment and Children's Study, a nationwide birth cohort study. A total of 76 940 pregnant women were included in the analysis. Information about alcohol consumption during pregnancy was obtained using two questionnaires: T1 and T2. The mean (standard deviation) gestational age in the T1 and T2 questionnaires were 16.5 (5.8) and 27.9 (3.7) weeks, respectively. Alcohol consumption was considered as an exposure, hypertensive disorders of pregnancy as an outcome, and possible confounding factors were included in a generalized linear mixed-effects model with a logit link function. Among the study subjects, 2 348 (3.1%) women developed hypertensive disorders of pregnancy. Compared with 25 300 women who never drank alcohol, 43 women who drank alcohol according to the T1 questionnaire and continued to drink ≥150 g ethanol/week according to the T2 questionnaire had significantly higher odds of hypertensive disorders of pregnancy. The adjusted odds ratio was 3.98 (95% confidence interval [CI], 1.33-11.9). In conclusion, alcohol consumption of ≥150 g ethanol/week during pregnancy is better avoided because of the high odds of developing hypertensive disorders of pregnancy. It may be meaningful that healthcare providers confirm information about alcohol consumption during pregnancy. Moreover, discontinuation of alcohol consumption is recommended to prevent the onset of hypertensive disorders of pregnancy in Japan.
A cross‐sectional study of the association between effort‐reward imbalance and psychologic distress among Japanese dietitians
Objectives This study aims to clarify dietitians’ effort‐reward imbalance (ERI) and examine its association with psychologic distress. Methods A cross‐sectional survey was conducted. A total of 3593 questionnaires were distributed to dietitians in about 110 organizations and 1890 responses were received (response rate 52.6%). Hence, a total of 1743 valid questionnaires were used in the analysis. Effort‐reward (ER) ratio was measured by a subscale of the ERI Questionnaire, and psychologic distress was measured by the Kessler Psychological Distress Scale (K6). The association between the ER ratio and psychologic distress was analyzed by multiple logistic regression analysis with covariates. Results The mean ER ratio was 0.83 (SD = 0.53) and ERI (ER ratio >1) prevalence was 26.3%. The mean K6 score was 7.1 (5.3), and psychologic distress (K6 score ≥5) prevalence was 62.4%. The increased psychologic distress was associated with a higher ER ratio, less support from supervisors and coworkers, and lower age and household income. ERI was significantly associated with psychologic distress, even after being adjusted for covariates. Conclusions Dietitians experience high stress, as shown by their high ER ratio and K6 scores. Their ERI was greatly associated with psychologic distress.
Nutritional Status and Associated Factors Among Women in the Third Trimester of Pregnancy
Objective Pregnant women in their third trimester require adequate nutritional intake for a healthy delivery. However, nutritional intake status during this period remains poorly understood. This study aimed to clarify the nutritional intake of pregnant women during the third trimester of pregnancy and identify related factors. Materials and Methods This study examined the dietary status of 291 women who gave birth in the Tohoku region of Japan in the third trimester of pregnancy. Results The study found that compared to the Japanese Dietary Intake Standards (2020), 99.7% of pregnant women consumed excessive amounts of salt, 82.5% consumed saturated fat and 89.0% did not consume enough iron. It was also found that pregnant women with higher education levels and a normal pre‐pregnancy body mass index (BMI) tend to have ideal eating habits. A negative correlation was observed between pre‐pregnancy BMI and weight gain during pregnancy (partial r = −0.336, p < 0.001), indicating that the necessary nutrients are insufficient to suppress excessive weight gain. Conclusion This study shows that pregnant women in their third trimester require personalized and appropriate nutritional support to ensure both weight gain control and adequate nutritional intake.
The risk of secondary sex ratio imbalance and increased monozygotic twinning after blastocyst transfer: data from the Japan Environment and Children’s Study
Background Some studies have suggested that blastocyst transfer is associated with i) imbalance in the secondary sex ratio (SSR) (which favors male offspring), ii) increased incidence of monozygotic twins (MZT). In contrast, others have not found these changes. In this study, we evaluated the association between blastocyst transfer and SSR and MZT, considering potential parental confounders. Methods The Japan Environment and Children’s Study is a large, nationwide longitudinal birth cohort study funded by the Ministry of the Environment of Japan. We used this large dataset, including 103,099 pregnancies, to further investigate the association between blastocyst transfer, SSR and MZT, using spontaneously conceived pregnancies, non-assisted reproductive technology (non-ART) treatment (intrauterine insemination and ovulation induction with timed intercourse) and cleavage stage embryo transfer for comparison. We evaluated the association with each group, the SSR, and the frequency of MZT, calculating the adjusted odds ratio (AOR) using multivariable logistic regression analyses, adjusting for potential parental confounders such as basic health and socioeconomic status. Results For each group (spontaneous conception vs. non-ART treatment vs. cleavage stage embryo transfer vs. blastocyst transfer), the percentages of males were 51.3% vs 50.7% vs 48.9% vs 53.4% and the monozygotic twinning rates per pregnancy were 0.27% vs 0.11% vs 0.27% vs 0.99% respectively. Multivariate logistic regression analyses indicated that blastocyst transfer was significantly associated with a higher SSR and higher incidence of MZT than the other three groups (SSR: AOR 1.095, 95% CI1.001–1.198; MZT: AOR 4.229, 95% CI 2.614–6.684). Conclusions There are significant relationships between blastocyst transfer and SSR imbalance and a higher occurrence of MZT.
