An Analysis of Household Vulnerability to Multidimensional Poverty and Implications for Poverty Reduction Policies in Vietnam

Purpose: This study analyzes households' vulnerability to multidimensional poverty in Vietnam and provides implications for poverty reduction policies.
Design/methodology/approach: This study uses the vulnerability as expected poverty approach, with the welfare threshold being the deprivation threshold of essential social services, to analyze households' vulnerability to multidimensional poverty. The method used is three-step feasible generalized least squares (FGLS).
Findings: The results show that demographic characteristics, economic potential, and how households face shocks affect the level of deprivation of essential social services and fluctuations in household welfare. These factors also allow for the estimation of household vulnerability through the probability of falling into multidimensional poverty.
Research limitations/implications: The study results imply that identifying the vulnerability of households to poverty is necessary to design early and appropriate support policies.
Originality/value: This study is a rare one that analyzes the risk of falling into multidimensional poverty in Vietnam in the post-COVID-19 context. The results show that poverty issues must be analyzed not only at the present time but also pay attention to households' vulnerability in order to have appropriate and timely support measures. Motivation or demotivation of health workers providing maternal health services in rural areas in Vietnam: findings from a mixed-methods study

Background
Motivation is an important driver for health professionals to maintain their professional competencies, continue in the workforce and make a positive contribution to their workplace. While there is some research about the motivation of health workers in low- to middle-income countries, maternal morbidity and mortality remains high and this can be lowered by improving the quality of maternal health services and the training and maintenance of the skills of maternal health workers. This study examines the impact of motivation on maintenance of professional competence among maternal health workers in Vietnam using mixed methods.
Methods
The study consisted of a survey using a self-administered questionnaire of 240 health workers in five districts across two Vietnamese provinces and in-depth interviews with 43 health workers and health managers at the commune, district and provincial level to explore external factors that influenced motivation. The questionnaire includes a 23-item motivation instrument based on the Kenyan health context, modified for Vietnamese language and culture.
Results
The 240 responses represented an estimated 95% of the target sample. Multivariate analysis showed that three factors contributed to the motivation of health workers: access to training (

= 0.03), ability to perform key tasks (

= 0.001) and shift schedule (

= 0.05). Motivation was higher in health workers self-identifying as competent or who were enabled to provide more maternal care services. Motivation was lower in those who worked more frequent night shifts and those who had received training in the last 12 months. The interviews identified that the latter was because they felt the training was irrelevant to them, and in some cases, they do not have the opportunity to practice their learnt skills. The qualitative data also showed other factors relating to service context and organisational management practices contributed to motivation.
Conclusions
The study demonstrates the importance of understanding the motivations of health workers and the factors that contribute to this and may contribute to more effective management of the health workforce in low- and middle-income countries.

In Vivo Study of the Antibacterial Chitosan/Polyvinyl Alcohol Loaded with Silver Nanoparticle Hydrogel for Wound Healing Applications

Silver nanoparticles have attracted great interests widely in medicine due to its great characteristics of antibacterial activity. In this research, the antibacterial activity and biocompatibility of a topical gel synthesized from polyvinyl alcohol, chitosan, and silver nanoparticles were studied. Hydrogels with different concentrations of silver nanoparticles (15 ppm, 30 ppm, and 60 ppm) were evaluated to compare their antibacterial activity, nanoparticles’ sizes, and in vivo behaviors. The resulted silver nanoparticles in the hydrogel were characterized by TEM showing the nanoparticles’ sizes less than 22 nm. The in vitro results prove that the antibacterial effects of all of the samples are satisfied. However, the in vivo results demonstrate the significant difference among different hydrogels in wound healing, where hydrogel with 30 ppm shows the best healing rate.

Objectives
Numerous studies have demonstrated the safety and efficacy of percutaneous MitraClip transcatheter interventional therapy in treating mitral regurgitation. This study reports the largest series of outcomes in Vietnamese patients who underwent the MitraClip procedure.
Methods and results
Between September 2014 and December 2024, 23 patients (20 males and 3 females; mean age, 66.3 ± 10.2 years) underwent the MitraClip procedure at five hospitals across Vietnam. Key findings included a high procedural success rate, with 91.3% of patients receiving successful implantations. Postprocedural complications were observed in 33.3% of patients. During the 30-day follow-up, two patients (9.5%) were readmitted; however, no mortality was reported. All patients achieved a mitral regurgitation grade of ≤2+, and 92.9% showed improvement to New York Heart Association Class I/II within 30 days postprocedure.
Conclusions
Multicenter data from hospitals across Vietnam indicate that the MitraClip procedure is effective in reducing mitral regurgitation and demonstrate favorable short-term safety outcomes.

In recent days in Vietnam, the amount of fake news spreading online about the Covid-19 epidemic has shown signs of increasing, causing information confusion and complicating the situation. This fact has received significant attention from scientists. To supplement the evidence of previous studies, enrich the research literature and make policy recommendations to the Government, this study explores the factors influencing the sharing of fake news on social networks. This study was conducted through a cross-sectional survey using an intentional sampling technique (n = 200) multivariate linear regression analysis technique was applied to prove the hypotheses. Research results show that the factors of altruism, entertainment, socialization, self-promotion, and instant information sharing have a positive and meaningful impact on sharing fake news about Covid_19 on social networks.

Genomic alterations involved in fluoroquinolone resistance development in Staphylococcus aureus

Fluoroquinolone (FQ) is a potent antibiotic class. However, resistance to this class emerges quickly which hinders its application. In this study, mechanisms leading to the emergence of multidrug-resistant (MDR) Staphylococcus aureus (S. aureus) strains under FQ exposure were investigated.
S. aureus ATCC 29213 was serially exposed to ciprofloxacin (CIP), ofloxacin (OFL), or levofloxacin (LEV) at sub-minimum inhibitory concentrations (sub-MICs) for 12 days to obtain S. aureus -1 strains and antibiotic-free cultured for another 10 days to obtain S. aureus-2 strains. The whole genome (WGS) and target sequencing were applied to analyze genomic alterations; and RT-qPCR was used to access the expressions of efflux-related genes, alternative sigma factors, and genes involved in FQ resistance.
A strong and irreversible increase of MICs was observed in all applied FQs (32 to 128 times) in all S. aureus-1 and remained 16 to 32 times in all S. aureus-2. WGS indicated 10 noticeable mutations occurring in all FQ-exposed S. aureus including 2 insdel mutations in SACOL0573 and rimI; a synonymous mutation in hslO; and 7 missense mutations located in an untranslated region. GrlA, was found mutated (R570H) in all S. aureus-1 and -2. Genes encoding for efflux pumps and their regulator (norA, norB, norC, and mgrA); alternative sigma factors (sigB and sigS); acetyltransferase (rimI); methicillin resistance (fmtB); and hypothetical protein BJI72_0645 were overexpressed in FQ-exposed strains.
The emergence of MDR S. aureus was associated with the mutations in the FQ-target sequences and the overexpression of efflux pump systems and their regulators.

Characteristics of moderate-to-severe chronic pain and its association with daily functional impairment among older outpatients in Vietnam

To describe the characteristics of moderate-to-severe chronic pain, and its association with daily functional decline among older individuals attending a large outpatient geriatric clinic in Hanoi, Vietnam.
A cross-sectional study was undertaken between November, 2019 and March, 2020. In-person structured interviews were conducted in 518 patients 60 years and older with chronic pain. Patient's self-reported pain was categorised into two levels of mild (0-3), moderate to severe (4-10) pain based on a 0-10 scale.
The median age of the study sample was 69 years, women accounted for 74.1%. The knee-foot-leg region was the most common site of chronic pain in both groups (39.5% in mild pain vs. 45.1% in moderate-to-severe pain. Participants experiencing moderate-to-severe pain were more likely to describe their pain as dull (65.5%) or burning (11.3%) and to report pain occurring independently of specific activities (48.7%), compared to those with mild pain (53.9%, 1.2%, and 32.4%, respectively). In a multivariable adjusted model, dependency in activities of daily living (OR: 3.73, 95% CI: 1.82-7.68) and dependency in instrumental activities of daily living (OR: 9.41, 95% CI: 3.94-22.5) were significantly associated with moderate-to-severe pain.
Moderate to severe chronic pain was reported in a high proportion of older Vietnamese outpatients and was associated with impaired daily functional impairment.

The Efficacy of Silver-Based Electrospun Antimicrobial Dressing in Accelerating the Regeneration of Partial Thickness Burn Wounds Using a Porcine Model

(1) Background: Wounds with damages to the subcutaneous are difficult to regenerate because of the tissue damages and complications such as bacterial infection. (2) Methods: In this study, we created burn wounds on pigs and investigated the efficacy of three biomaterials: polycaprolactone-gelatin-silver membrane (PCLGelAg) and two commercial burn dressings, Aquacel® Ag and UrgoTulTM silver sulfadiazine. In vitro long-term antibacterial property and in vivo wound healing performance were investigated. Agar diffusion assays were employed to evaluate bacterial inhibition at different time intervals. Minimum inhibitory concentration (MIC), minimum bactericidal concentration (MBC) and time-kill assays were used to compare antibacterial strength among samples. Second-degree burn wounds in the pig model were designed to evaluate the efficiency of all dressings in supporting the wound healing process. (3) Results: The results showed that PCLGelAg membrane was the most effective in killing both Gram-positive and Gram-negative bacteria bacteria with the lowest MBC value. All three dressings (PCLGelAg, Aquacel, and UrgoTul) exhibited bactericidal effect during the first 24 h, supported wound healing as well as prevented infection and inflammation. (4) Conclusions: The results suggest that the PCLGelAg membrane is a practical solution for the treatment of severe burn injury and other infection-related skin complications.

Combretum quadrangulare Kurz is widely used in folk medicine in Eastern Asia and is associated with various ethnopharmacological properties including hepatoprotective, antipyretic, analgesic, antidysenteric, and anthelmintic activities. Previous phytochemical investigations reported the presence of numerous triterpenes (mostly cycloartanes, ursanes, lupanes, and oleananes) along with dozens of flavonoids. However, the extracts of C. quadrangulare and isolated flavonoids have not been evaluated for their alpha-glucosidase inhibition. In the frame of our efforts dedicated to the chemical investigation of Vietnamese medicinal plants and their biological activities, a phytochemical study of the MeOH extract of the leaves of C. quadrangulare using bioactive guided isolation was undertaken. In this paper, the isolation and structure elucidation of twelve known compounds, 5-hydroxy-3,7,4′-trimethoxyflavone (1), ayanin (2), kumatakenin (3), rhamnocitrin (4), ombuin (5), myricetin-3,7,3′,5′-tetramethyl ether (6), gardenin D (7), luteolin (12), apigenin (13), mearnsetin (14), isoorientin (15), and vitexin (16) were reported. Bromination was applied to compounds 2 and 3 to provide four new synthetic analogues 8–11. All isolated and synthesized compounds were evaluated for alpha-glucosidase inhibition and antibacterial activity. Compounds 4 and 5 showed moderate antibacterial activity against methicillin-resistant Staphylococcus aureus while others were inactive. All compounds failed to reveal any activity toward extended spectrum beta-lactamase-producing Escherichia coli. Compounds 2, 4, 6–9, and 11–14 showed good alpha-glucosidase inhibition with IC50 values in the range of 30.5–282.0 µM. The kinetic of enzyme inhibition showed that 8 and 11 were noncompetitive type inhibition against alpha-glucosidase. In silico molecular docking model indicated that compounds 8 and 11 were potential inhibitors against enzyme α-glucosidase.

In silico and in vitro studies on the anti-cancer activity of artemetin, vitexicarpin and penduletin compounds from Vitex negundo

Vitex negundo L. (V. negundo) is one of the important medicinal and anticancer enhancer herbs. This plant is commonly used in the preparation of traditional drugs to treat numerous diseases. Inspired by the medicinal properties of this plant, the current study aimed to investigate antiproliferative potential and the primary molecular mechanisms of the apoptotic induction against human HepG2 and MCF-7 cell lines, by pure compounds isolated from targeted fractions of V. negundo which were characterized by NMR, FTIR and HRMS analysis and identified as artemetin (FLV1), vitexicarpin (FLV2), and penduletin (FLV3) compounds. The FLV1, FLV2, and FLV3 compounds were evaluated for the antiproliferative potential against HepG2 and MCF-7 cell lines by cell viability assay and exhibited IC50 values of 2.3, 23.9 and 5.6 µM and 3.9, 25.8, and 6.4 µM, respectively. In addition, those compounds increased the level of reactive oxygen species production, induced cell death occurred via apoptosis, demonstrated by Annexin V-staining cells, contributed significantly to DNA damage, and led to the activation of caspase3/caspase8 pathways.Additionally, molecular docking was also conducted to rationalize the cancer cells inhibitory and to evaluate the ability of the FLV1, FLV2, and FLV3 compounds to be developed as good drug candidates for cancers treatment.
