He lives in Woodlands St.-Mary, Berkshire, England.

The role of the thymus is vital for orchestration of T-cell development and maturation. With increasing age the thymus undergoes a process of involution which results in a reduction in thymic size, function and output. Until relatively recent it was not feasible to accurately measure the magnitude of age-related loss of thymic function. With the discovery of T-cell receptor excision circles (TRECs), which are the stable by-products of the newly generated T-cells, it is now possible to quantitatively measure the extent of thymic output. This review examines the available data on immune function and zinc deficiency and places them in the context of the aims of the ZINCAGE project which include the evaluation of the role played by zinc in maintaining thymic output in healthy elderly individuals.

Designing Sustainable Community Interfaces

This thesis explores the role of systems and processes to support inter-community communication and relations between communities and public service institutions. I explore two contexts, rural and urban, and the potential of technical systems to support communities organise, but also support better understanding and dialogues between services providers and beneficiaries. This potential depends on the contextual barriers and opportunities and the willingness of stakeholders to adopt systems and processes that inevitably effects change in day-to-day organisational cultures. Drawing from two longitudinal case studies in the NorthEast of England, I describe the requirements of these systems to be used effectively and sustainably by communities and public service institutions. The first case study developed with rural communities of Wooler and Rothbury where, over the course of three years, a communityrun pervasive display network was designed and deployed to bolster intra-community communication. I describe the development of a partnership with Glendale Gateway Trust and the design and deployment of simple hardware and bespoke software packaged alongside a suggested approach for its administration and governance. The study explores the implications of incorporating a governance model with particular focus on how this impacts ownership, moderation and maintenance as part of sustaining technical systems between self-organised configurations of communities. The second case study, developed with Northumbria Police's Neighbourhood Policing team, involved the iterative design, implementation and evaluation of an approach to support police officers communicate more effectively with local communities and better understand local issues. The intervention aimed to strengthen communityinstitutional relations; it included training neighbourhood police officers in alternative modes of community engagement and ways to configure technical processes for public institutions' specific needs and challenges. Learning from these two case studies is synthesized in a discussion of the different ways technical systems and processes can be designed and configured to meet stakeholders' needs and organisational cultures; and to embody important values such as sustainability, scalability, moderation, governance and ownership.

PurposeAnesthetic and surgical considerations for awake craniotomy (AC) include airway patency, patient comfort, and optimization of real-time brain mapping. The purpose of this study is to report our experience of using dexmedetomidine and scalp blocks, without airway intervention, as a means to facilitate and optimize intraoperative brain mapping and brain tumour resection during AC.MethodsWe conducted a retrospective cohort study of 55 patients who underwent AC from March 2012 to September 2016. The incidence of critical airway outcomes, perioperative complications, and successful intraoperative mapping was determined. The primary outcome was the incidence of a failed AC anesthetic technique as defined by the need to convert to general anesthesia with a secured airway prior to (or during) brain mapping and brain tumour resection. Secondary outcomes were the intraoperative incidence of: 1) altered surgical management due to information acquired through real-time brain mapping, 2) interventions to restore airway patency or rescue the airway, 3) hemodynamic instability (> 20% from baseline), 4) nausea and vomiting, 5) new onset neurologic deficits, and 6) seizure activity.ResultsThere were no anesthesia-related critical events and no patients required airway manipulation or conversion to a general anesthetic. Multimodal language, motor, and sensory assessment with direct cortical electrical stimulation was successfully performed in 100% of cases. In 24% (13/55) of patients, data acquired during intraoperative brain mapping influenced surgical decision-making regarding the extent of tumour resection. Nine (16%) patients had intraoperative seizures.ConclusionsDexmedetomidine-based anesthesia and scalp block facilitated AC surgery without any requirement for urgent airway intervention or unplanned conversion to a full general anesthetic. This approach can enable physiologic testing before and during tumour resection facilitating real-time surgical decision-making based on intraoperative brain mapping with patients awake thereby minimizing the risk of neurologic deficit and increasing the opportunity for optimal surgical resection.

Two extraordinary mavericks changed the face of jazz in the 1940s. Saxophonist Charlie Parker changed the way it was played and entrepreneur Norman Granz changed the way it was sold. Parker was the music revolutionary who ushered in a new era of jazz called bebop, while Granz took jazz out of the smoky jazz clubs and dives and put it on to the concert stage. In 1955 Granz formed Verve Records to consolidate his recording activities under one label and promote one artist in particular: Ella Fitzgerald. The label got off to a flying start with \"Ella Fitzgerald Sings the Cole Porter Songbook\", one of the best-selling albums of 1956, and quickly became one of the best-known labels for both jazz and popular music.
