Comparison of spontaneous versus operative vaginal delivery using Obstetric Quality of Recovery-10 (ObsQoR-10): An observational cohort study

We aimed to determine whether patient-reported quality of recovery differed between spontaneous and operative vaginal delivery. We also aimed to psychometrically evaluate the Obstetric Quality of Recovery-10 scoring tool (ObsQoR-10) for use in this setting.
Single center observational cohort study.
Labour and delivery ward at a peripheral general hospital within the United Kingdom, over a 10-month period.
123 women delivering via either spontaneous (n = 68) or operative vaginal delivery (n = 55).
Women were asked to complete the ObsQoR-10 and global health visual analogue scale (0−100) on postpartum day 1. A convenience sample of consenting parturients delivering via spontaneous or operative vaginal delivery (forceps or vacuum assisted), were included. In total, 123 deliveries were included (68 via spontaneous and 55 via operative vaginal delivery), with no dropouts.
Primary outcome was ObsQoR-10 score and secondary outcomes included measures of validity, reliability and feasibility of ObsQoR-10.
Quality of recovery was better following spontaneous vaginal delivery. ObsQoR-10 scores were 80.2 (95% CI 76.4–83.9) and 72.1 (95% CI 67.3–76.9], (a difference in score of 8.1 [95% CI 2.1–14.0]) following spontaneous and operative vaginal delivery respectively (p = 0.008).
Validity: ObsQoR-10 correlated with global health visual analogue scale score (R = 0.52; p = 0.01) and scores were higher in women requiring <36 h compared to ≥36 h postpartum hospital stay (81.3 (95% CI 77.9–84.7) versus 72.6 (95% CI 67.9–77.2] hours respectively, (a difference in score of 8.7 [95% CI 2.8–14.6]; p = 0.004).
Reliability: ObsQoR-10 demonstrated good internal consistency (Cronbach's alpha = 0.82 and inter-item correlation = 0.32) good split-half reliability (Spearman-Brown Prophesy Reliability Estimate = 0.88) and excellent test-re-test reliability (intra-class correlation coefficient of 0.86 [95% CI 0.72–0.93]).
Feasibility: All women completed the survey with a median completion time of 2.5 min.
Quality of recovery appears to be better following spontaneous compared to operative vaginal delivery. This study also demonstrates that ObsQoR-10 is a valid and reliable tool for use following these delivery modes.

Intermittent annular crash during the current ramp-up phase in JT-60SA

Intermittent sawtooth-like crash in the soft X-ray (SX) emissions was observed during the plasma current ramp-up in the initial operation phase of JT-60SA. In particular, this behavior was seen in plasmas with electron cyclotron (EC) heating which has a resonance layer at off-axis position. These events are thought to be magnetic reconnection between multiple rational surfaces with the same value of the safety factor q , which is known as the double tearing modes (DTMs). From the experimental viewpoint, this hypothesis is supported by the fact that the annular crashes were observed only in plasmas with off-axis EC heating, which facilitates the formation of a local off-axis minimum in q due to reduced Ohmic current penetration through locally increased electron temperature T e . This hypothesis is supported by numerical simulations using the integrated transport code TOPICS and the reduced two-fluid code R2F. It has also been shown that the period between the crashes mainly consists of the recovery of the T e profile by both EC and Ohmic heating, and the recovery of the current profile by the resistive current diffusion. Above discussions imply that the annular crashes related to the DTMs, which can reduce plasma performance, should be taken into account in cases where the off-axis EC heating power is used to assist with the plasma current ramp-up.

Clinical pattern of primary systemic therapy and outcomes of estrogen receptor-positive, HER2-negative metastatic breast cancer: a review of a single institution

Purpose
In the management of estrogen receptor-positive, human epidermal growth factor receptor 2-negative metastatic breast cancer (ER+HER2−MBC) patients, endocrine therapy (ET) is preferred to chemotherapy (CT) as a primary systemic therapy (PST) when tumor burden is not high. However, there are no definite criteria for choosing a PST, transitioning from ET to CT or using maintenance ET subsequent to CT.
Methods
We reviewed the medical records of 311 ER+HER2−MBC patients who underwent CT from September 2002 to December 2016 and assessed their outcomes.
Results
Of the 311 patients, 178 (57%) received ET as a PST (ET-first group), and 133 (43%) received CT prior to ET (CT-first group). The ET-first group showed a median overall survival (OS) from the diagnosis of MBC (OS
MBC
) of 1593 days, and the median OS from the initiation of CT (OS
CT
) was 938 days. Patients with visceral involvement, liver metastasis, soft tissue metastasis, ≥3 organ involvement, or primary advanced BC at the MBC diagnosis showed a significantly higher tendency to be assigned to the CT-first group (

< 0.01 for any visceral involvement,

Conclusion
Our findings suggest the possibility for changing the treatment paradigm of patients with ER+HER2−MBC, so a randomized prospective study is warranted to determine the optimum sequence of systemic therapies.

78Ni revealed as a doubly magic stronghold against nuclear deformation

Nuclear magic numbers correspond to fully occupied energy shells of protons or neutrons inside atomic nuclei. Doubly magic nuclei, with magic numbers for both protons and neutrons, are spherical and extremely rare across the nuclear landscape. Although the sequence of magic numbers is well established for stable nuclei, experimental evidence has revealed modifications for nuclei with a large asymmetry between proton and neutron numbers. Here we provide a spectroscopic study of the doubly magic nucleus

Ni, which contains fourteen neutrons more than the heaviest stable nickel isotope. We provide direct evidence of its doubly magic nature, which is also predicted by ab initio calculations based on chiral effective-field theory interactions and the quasi-particle random-phase approximation. Our results also indicate the breakdown of the neutron magic number 50 and proton magic number 28 beyond this stronghold, caused by a competing deformed structure. State-of-the-art phenomenological shell-model calculations reproduce this shape coexistence, predicting a rapid transition from spherical to deformed ground states, with

Regulation of intestinal T-cell responses is crucial for immune homeostasis and prevention of inflammatory bowel disease (IBD). A vital cytokine in regulating intestinal T cells is transforming growth factor-β (TGFβ), which is secreted by cells as a latent complex that requires activation to function. However, how TGFβ activation is regulated in the human intestine, and how such pathways are altered in IBD is completely unknown. Here we show that a key activator of TGFβ, integrin αvβ8, is highly expressed on human intestinal dendritic cells (DCs), specifically on the CD1c+ but not the CD141+ intestinal DC subset. Expression was significantly upregulated on intestinal DC from IBD patients, indicating that inflammatory signals may upregulate expression of this key TGFβ-activating molecule. Indeed, we found that the Toll-like receptor 4 ligand lipopolysaccharide upregulates integrin αvβ8 expression and TGFβ activation by human DC. We also show that DC expression of integrin αvβ8 enhanced induction of FOXP3 in CD4+ T cells, suggesting functional importance of integrin αvβ8 expression by human DC. These results show that microbial signals enhance the TGFβ-activating ability of human DC via regulation of integrin αvβ8 expression, and that intestinal inflammation may drive this pathway in patients with IBD.

Non-destructive operando measurements of muonic x-rays on Li-ion battery

We have developed a non-destructive technique to observe Li in an electrode and metallic Li deposition on an anode using muonic x-rays. With high intent negative muon beam at J-PARC, we have attempted operando measurements of Li and metallic Li deposition in LIBs for the first time. Both increase and decrease of Li in a cathode during charging and discharging processes were observed. We also observed significant increase of muonic x-rays of Li with cycles, which may correspond to the degradation of charge capacity with the cycles.

Measuring the β-decay Properties of Neutron-rich Exotic Pm, Sm, Eu, and Gd Isotopes to Constrain the Nucleosynthesis Yields in the Rare-earth Region

The β-delayed neutron-emission probabilities of 28 exotic neutron-rich isotopes of Pm, Sm, Eu, and Gd were measured for the first time at RIKEN Nishina Center using the Advanced Implantation Detector Array (AIDA) and the BRIKEN neutron detector array. The existing β-decay half-life (T 1/2) database was significantly increased toward more neutron-rich isotopes, and uncertainties for previously measured values were decreased. The new data not only constrain the theoretical predictions of half-lives and β-delayed neutron-emission probabilities, but also allow for probing the mechanisms of formation of the high-mass wing of the rare-earth peak located at A ≈ 160 in the r-process abundance distribution through astrophysical reaction network calculations. An uncertainty quantification of the calculated abundance patterns with the new data shows a reduction of the uncertainty in the rare-earth peak region. The newly introduced variance-based sensitivity analysis method offers valuable insight into the influence of important nuclear physics inputs on the calculated abundance patterns. The analysis has identified the half-lives of 168Sm and of several gadolinium isotopes as some of the key variables among the current experimental data to understand the remaining abundance uncertainty at A = 167–172.

Noise simulation of an industrial axial fan has been conducted by implicit large-eddy simulation with Lattice Boltzmann simulation which is based on Cumulant Lattice Boltzmann method with wall model and diffuse-interface method for a moving boundary condition. Simulation results are compared with experimental results of a factory test of an industrial fan for a thermal power generation. The result shows a good agreement and overall value is within 3dB error.

Glycome characterization of immunoglobulin G from buffalo (Bubalus bubalis) colostrum

Immunoglobulin G (IgG) is a major glycoprotein in ruminant colostrum. First day buffalo colostrum protein was purified on Sephadex G-100 and its mass was determined by MALDI-TOF as 147.848 KDa. The PMF data of protein subunits revealed its homology to IgG, which was supported by the identification of peptide sequences LLIYGATSR and VYNEYLPAPIVR corresponding to light and heavy chains of IgG by CID MS/MS analysis. The N-glycan microheterogeneity was established based on chemoselective glycoblotting technique with the identification of high mannose, neutral complex/hybrid and sialylated complex/hybrid glycans. A complete structural assignment of 54 N-linked oligosaccharides were identified and the ratio of sialyl oligosaccharides was found to be higher compared to neutral saccharides. The fucosylation observed in more than 20 oligosaccharides, high mannose and trisialyl oligosaccharides were present in diminutive amount. The high non-fucosyl and sialyl oligosaccharides in buffalo colostrum IgG provide ample scope for its utilization in targeted therapies to elicit effective ADCC and anti-inflammatory responses.
