Coconut Coir Fiber Composites for Sustainable Architecture: A Comprehensive Review of Properties, Processing, and Applications
The growing need for sustainable materials in architecture has sparked significant interest in natural-fiber-based composites. Among these, coconut coir, a by-product of the coconut industry, has emerged as a promising raw material owing to its abundance, renewability, and excellent mechanical properties. The promise of coir-based composites in architecture is highlighted in this review, which also looks at their problems, advantages for the environment, manufacturing processes, and mechanical, thermal, and acoustic performances. The fibrous shape of the coir provides efficient thermal and acoustic insulation, while its high lignin concentration guarantees stiffness, biological resistance, and dimensional stability. Fiber-matrix adhesion and durability have improved owing to advancements in treatment and environmentally friendly binders, opening up the use of cement, polymers, and hybrid composites. In terms of the environment, coir composites promote a biophilic design, reduce embodied carbon, and decrease landfill waste. Moisture sensitivity, inconsistent fiber quality, and production scaling are obstacles; however, advancements in hybridization, grading, and nanotechnology hold promise. This review provides comprehensive, architecture-focused review that integrates material science, fabrication techniques, and real-world architectural applications of coir-based composites. Coir-based composites have the potential to be long-lasting, sustainable substitutes for conventional materials in climate-resilient architectural design if they are further investigated and included in green certification programs and the circular economy.
Nasal Carriage of Staphylococcus Aureus in Patients with Pyoderma
BackgroundThe practicing dermatologist is faced with an ever changing epidemiologic spectrum of cutaneous bacterial diseases. They may account for upto 17% of clinical visits. Staphylococcus aureus is one of the most common organisms isolated from skin lesions of pyoderma.Nasal carriage of bacteria may cause recurrence of pyodermas. Approximate rates for resident carriage sites of S. aureus are: anterior nares 35%, perineum 20%, axillae 5- 10% and toe webs 5-10%. Contamination from carriage sites is common, notably of the hands from the nose. Due to an increasing number of infections caused by methicillin resistant S. aureus (MRSA) strains, which are now most often multi drug resistant, therapy has become problematic.Therefore prevention of staphylococcal infections is now more important than ever. Carriage of S. aureus in the nose appears to play a key role in the epidemiology and pathogenesis of the infection. Paucity of studies showing nasal carriage of S. aureus in patients with pyoderma in coastal Karnataka population has motivated us to undertake this study.ObjectivesWe carried out the above study to study the association between nasal carriage of S. aureus and pyodermas and to study the occurrence of Methicillin-resistant Staphylococcus aureus in nasal carriage of healthy controls and in patients with pyoderma.MethodA prospective study carried out in the Department of Dermatology, Venereology and leprosy, Father Muller Medical College, Kankanady, Mangalore over a period of 18 months from October 2015 to March 2017. Sixty four cases with primary or secondary pyoderma and 64 healthy controls were recruited for the study. A detailed clinical history and examination was done for all the cases. Bacteriological examinations were conducted to confirm the presence of S. aureus from the anterior nares as well as from the lesional site. Positive S.aureus isolated from culture were analyzed for presence of methicillin resistance in both the groups.ResultsSixty four cases with primary or secondary pyoderma and 64 healthy controls were recruited for the study. Most of the cases were below 20 years of age (54.1%) with mean age of 17.65 years. Controls were significantly older than the cases with mean age of 25.4 years. Male predominance was seen in cases with male to female ratio of 1.78:1. No difference was observed between cases and controls with respect to gender. Most common affected site was face (48.4%) followed by legs (32.8%). Most of the cases had primary pyoderma (68.8%) while secondary bacterial infection were observed in 31.2%. Most common etiology of primary pyoderma was Impetigo (47.7%) followed by folliculitis (27.3%). Other etiologies were ecthyma (13.6%), furunculosis (6.8%) and bullous impetigo (4.5%). Staph. aureus was isolated from the lesion site in 84.4% of the cases while Strep. Pyogenes was isolated in 4.7% cases. Mixed growth was seen in 3.1% cases while no growth was observed in 7.8% cases. No organism was isolated from anterior nares in 62.5% cases while Staph. aureus was isolated in 34.4% cases.
The tumor microenvironment as driver of stemness and therapeutic resistance in breast cancer: New challenges and therapeutic opportunities
Background: Breast cancer (BC), the second most common cause of cancer-related deaths, remains a significant threat to the health and wellness of women worldwide. The tumor microenvironment (TME), comprising cellular components, such as cancer-associated fibroblasts (CAFs), immune cells, endothelial cells and adipocytes, and noncellular components such as extracellular matrix (ECM), has been recognized as a critical contributor to the development and progression of BC. The interplay between TME components and cancer cells promotes phenotypic heterogeneity, cell plasticity and cancer cell stemness that impart tumor dormancy, enhanced invasion and metastasis, and the development of therapeutic resistance. While most previous studies have focused on targeting cancer cells with a dismal prognosis, novel therapies targeting stromal components are currently being evaluated in preclinical and clinical studies, and are already showing improved efficacies. As such, they may offer better means to eliminate the disease effectively. Conclusions: In this review, we focus on the evolving concept of the TME as a key player regulating tumor growth, metastasis, stemness, and the development of therapeutic resistance. Despite significant advances over the last decade, several clinical trials focusing on the TME have failed to demonstrate promising effectiveness in cancer patients. To expedite clinical efficacy of TME-directed therapies, a deeper understanding of the TME is of utmost importance. Secondly, the efficacy of TME-directed therapies when used alone or in combination with chemo- or radiotherapy, and the tumor stage needs to be studied. Likewise, identifying molecular signatures and biomarkers indicating the type of TME will help in determining precise TME-directed therapies.
Association of blood group types and clinico-pathological features of gynecological cancers (GCs)
Background Gynecological cancers (GCs) affect the reproductive system of females, and are of multiple types depending on the affected organ most common of which are cervical, endometrial, ovarian cancers. Among different risk factors for GCs, ABO blood group system is considered as one of the pivotal contributing factors for increased susceptibility of GCs. The aim of our study was to report on the demographics of GC patients and to investigate the relationship between the ABO blood group system and the risk of acquiring GC in our population. Methods The current retrospective cross-sectional study was carried out between the years of 2016 and 2023. The sample included all the patients having age > 18 with a record of blood group and confirmed histological or cytological diagnosis as per International Federation of Gynecology and Obstetrics (FIGO) guidelines. A comprehensive review of the charts was conducted to gather data including demographics, tumor characteristics, comorbidities, adverse effects, and treatment methods. Results A total of 543 female patients were included in the study. The mean age of patients was 61.6. The three most common BG in our GC cases were as O + (43.8%), followed by A + (26%), and B + (15.5%). Among comorbidities, hypertension (HTN), diabetes mellitus (DM), dyslipidemia were the top three affecting GC patients. A significant association ( p  < 0.05) between ABO BG and serous histology in endometrial cancer was found. ABO blood group and fallopian cancer showed a significant relationship between serous histology and B blood group ( p  < 0.05). For ovarian cancer a significant association between AB blood group and recurrence rates were found ( p  < 0.05). In case of the patient dependent GCs, a significant association between ovarian cancers and recurrence, fallopian tube cancers and adverse events and survival status, vaginal/vulval cancers and TNM stage and mixed GCs and tumor type (carcinoma) was observed ( P  < 0.05). Furthermore, multinomial analysis between various confounding factors and GCs revealed that the risk of Cervical, Endometrial and Ovarian cancers to be significant for Type B BG ( P  < 0.05). Conclusion Our study found that O + BG was the most prevalent among our population. Furthermore, there was a significant association between BG B and endometrial and serous histology in fallopian tube and between BG AB and ovarian malignancies, respectively. Additionally, multinomial analysis revealed higher risk of Cervical, Endometrial and Ovarian cancers for Type B BG ( P  < 0.05).
Synthesis and Biological Evaluation of Novel Uracil Derivatives as Thymidylate Synthase Inhibitors
Cell division is driven by nucleic acid metabolism, and thymidylate synthase (TYMS) catalyzes a rate-limiting step in nucleotide synthesis. As a result, thymidylate synthase has emerged as a critical target in chemotherapy. 5-Fluorouracil (5-FU) is currently being used to treat a wide range of cancers, including breast, pancreatic, head and neck, colorectal, ovarian, and gastric cancers The objective of this study was to establish a new methodology for the low-cost, one-pot synthesis of uracil derivatives (UD-1 to UD-5) and to evaluate their therapeutic potential in BC cells. One-pot organic synthesis processes using a single solvent were used for the synthesis of drug analogues of Uracil. Integrated bioinformatics using GEPIA2, UALCAN, and KM plotter were utilized to study the expression pattern and prognostic significance of TYMS, the key target gene of 5-fluorouracil in breast cancer patients. Cell viability, cell proliferation, and colony formation assays were used as in vitro methods to validate the in silico lead obtained. BC patients showed high levels of thymidylate synthase, and high expression of thymidylate synthase was found associated with poor prognosis. In silico studies indicated that synthesized uracil derivatives have a high affinity for thymidylate synthase. Notably, the uracil derivatives dramatically inhibited the proliferation and colonization potential of BC cells in vitro. In conclusion, our study identified novel uracil derivatives as promising therapeutic options for breast cancer patients expressing the augmented levels of thymidylate synthase.
Outcomes of synchronous and metachronous bilateral small renal masses (< 4 cm): a population-based cohort study
ObjectivesTo report longitudinal outcomes of a population-based cohort of patients diagnosed with bilateral small renal masses from a period of over 11 years.Patients and methodsConsecutive patients diagnosed with bilateral small renal masses (synchronous or metachronous) of a defined geographical area were recorded in a large database (TUCAN database) between January 2005 and December 2016. Patients had a unique identifier number and followed during this period using an agreed upon protocol. Clinicopathological characteristics and outcomes of bilateral small renal masses on active surveillance were analysed and compared to propensity score-matched sporadic unilateral small renal masses. Data were analysed for renal mass growth rate, rate of intervention and development of metastatic disease and patient survival.ResultsA total of 1060 patients were diagnosed with renal cancer, of which bilateral small renal masses accounted for 70 (6.6%) cases. Synchronous SRMs were observed in 63 patients, whereas metachronous lesions were found in seven patients during the study period. Metachronous lesion mean time to appearance was 62 ± 41 months (range 9–149 months). While most cases were sporadic, four were found to be hereditary. Growth rate of bilateral small renal masses did not differ from that of unilateral sporadic small renal masses. Similarly, there were no differences between the groups for rate of interventions and survival.ConclusionsProgression, rate of metastases and survival for patients diagnosed with bilateral small renal masses are similar to those diagnosed with unilateral disease.
SARS-CoV-2 Infection Is at Herd Immunity in the Majority Segment of the Population of Qatar
Abstract Background Qatar experienced a severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) epidemic that disproportionately affected the craft and manual worker (CMW) population, who comprise 60% of the total population. This study aimed to assess ever and/or current infection prevalence in this population. Methods A cross-sectional population-based survey was conducted during July 26 to September 09, 2020, to assess both anti-SARS-CoV-2 positivity through serological testing and current infection positivity through polymerase chain reaction (PCR) testing. Associations with antibody and PCR positivity were identified through regression analyses. Results The study included 2641 participants, 69.3% of whom were <40 years of age. Anti-SARS-CoV-2 positivity was 55.3% (95% CI, 53.3%–57.3%) and was significantly associated with nationality, geographic location, educational attainment, occupation, and previous infection diagnosis. PCR positivity was 11.3% (95% CI, 9.9%–12.8%) and was significantly associated with nationality, geographic location, occupation, contact with an infected person, and reporting 2 or more symptoms. Infection positivity (antibody and/or PCR positive) was 60.6% (95% CI, 58.6%–62.5%). The proportion of antibody-positive CMWs who had a prior SARS-CoV-2 diagnosis was 9.3% (95% CI, 7.9%–11.0%). Only seven infections were ever severe, and only 1 was ever critical—an infection severity rate of 0.5% (95% CI, 0.2%–1.0%). Conclusions Six in every 10 CMWs in Qatar have been infected, suggestive of reaching the herd immunity threshold. Infection severity was low, with only 1 in every 200 infections progressing to be severe or critical. Only 1 in every 10 infections had been previously diagnosed, which is suggestive of mostly asymptomatic or mild infections.
Histomorphology and Immunohistochemistry of the Lower Esophageal Sphincter of the Least Shrew (Cryptotis Parva)
The biochemical and histopathological changes in the lower esophageal sphincter (LES) in the pathogenesis of gastroesophageal reflux disease have gained interest. The least shrew is able to vomit in response to emetogens and provides a good model to study the histology of this phenomenon relative to the published reports in the commonly used but vomit-incompetent laboratory species. The LES is located at the junction of the esophagus and stomach. It typically closes at rest and opens in response to swallowing. Our findings demonstrate that the least shrew does not have a well-defined LES, lacks esophageal glands and has a mucosal valve-like projection from the terminal end of the esophagus before joining the gastric epithelium at the lesser curvature. In addition, the least shrew has thoracic and abdominal components prior to joining the gastric epithelium. The mucosal lining of the esophagus is folded, becoming clearly convoluted and forming a bucket-like structure at the level of the esophageocardiac junction (ECJ). No significant differences are to be found between the structure and thickness of the wall before and after the ECJ. Thus, the ECJ forming the LES is relatively less complex than those of other mammals including man. The distribution of enterochromaffin (EC) cells is confined to the lamina propria of the junction and is not associated with the cardiac glands, suggesting its functional involvement with the smooth muscle in and around the ECJ. In conclusion, the least shrew's anatomical sphincter appears ill-defined and is replaced by a less sturdy valve-like mucosal flap.
