Greater pre-operative anxiety, pain and poorer function predict a worse outcome of a total knee arthroplasty

Purpose
Around 10–30 % of patients are dissatisfied with the results of their total knee arthroplasty (TKA). This review aimed to identify and evaluate the predictors of outcome measured by the three domains of health-related quality of life (pain, stiffness and function). The focus was on pre-operative psychological factors as related to other patient-related variables.
Methods
A systematic search was performed using the following databases: MEDLINE, PubMed, AMED, CINAHL, PsychINFO, SciFinder, Scopus, EMBASE, Cochrane, Lilacs, Web of Science and ScienceDirect. The quality of identified studies was assessed using the Critical Appraisal Skills Programme Cohort checklist.
Results
Ten studies met the eligibility criteria. From these, nine patient-related predictors of outcome were identified (depression, anxiety, age at surgery, gender (being female), medical co-morbidities, BMI, level of education, pre-operative pain severity and pre-operative knee function). Greater anxiety, pre-operative pain and function were the most significant factors to predict a poorer outcome of a TKA. The results of depression, gender (female), medical co-morbidities, BMI and level of education were variable among the included studies. There was very little evidence to support older age at operation as a predictor of poorer outcome.
Conclusion
Patients experiencing high levels of pain before surgery should be informed of the chances of improvement by having a TKA. A validated psychological screening tool that separates depression and anxiety is recommended as part of the pre-operative assessment stage. Patients presenting with symptoms of depression and anxiety should be identified and consulted before a TKA.

Robust Regression on Clustered Data and Signature Based Online Arabic Handwriting Recognition
In this thesis, we present two different methodologies; one for processing time series data for use in machine learning and the other, a robust linear regression for clustered data. The foundation in both of these methodologies is attempting to utilise time series data in ways in which have traditionally been prohibitive, owing to the ragged nature of such data. In order to use standard machine learning tools to classify online Arabic handwritten characters, we develop a dyadic iterated integral path signature approach to processing the underlying time series data. The process developed transforms raw online Arabic handwritten character data in the form of multiple time series, into a single set of features that can be used as features for machine learning. When applied to the Online KHATT segmented character data set, the methodology combined with both random forests and long short term memory (LSTM) neural networks demonstrates a dramatic improvement in recognition performance over the previously published best (using hidden Markov models). Furthermore, this processing methodology can be applied to any number of similar scenarios including other online handwritten scripts and even drawings on tablets. Secondly, with the aim of carrying out polynomial regression using the iterated integral pathlogsignature, we present a robust eigenvalue polynomial regression. This new form of regression is designed to significantly reduce the impact of clustered data on the fitting of apolynomial approximation to the data. Using knowledge of the location of the clusters of data in space, combined with the region over which we wish to obtain a robust estimate, this eigen- value based method can be seen to have vast improvements over standard least squares linear regression. The methodology is demonstrated to result in a large decrease in the L2 error of polynomial approximations to a number of functions.

On the Complexity and Behaviour of Cryptocurrencies Compared to Other Markets

We show that the behaviour of Bitcoin has interesting similarities to stock and precious metal markets, such as gold and silver. We report that whilst Litecoin, the second largest cryptocurrency, closely follows Bitcoin's behaviour, it does not show all the reported properties of Bitcoin. Agreements between apparently disparate complexity measures have been found, and it is shown that statistical, information-theoretic, algorithmic and fractal measures have different but interesting capabilities of clustering families of markets by type. The report is particularly interesting because of the range and novel use of some measures of complexity to characterize price behaviour, because of the IRS designation of Bitcoin as an investment property and not a currency, and the announcement of the Canadian government's own electronic currency MintChip.

Creatures from the Black Saloon is a long and merry romp through fairy tales and the singles scenes. It sounds incongruous, but it works. The Grimm Brothers-based fairy tales are a hoot, featuring the comic skills of one Damien Sunners as assorted frogs and oafs. They illustrate some of the finer, or less fine, points of the human condition - as do the young denizens of the saloon bar in their complex quests to hook up with the opposite sex. There are some nice characterisations, especially Elspeth Treacy as Sharon, the jargonesque girl from the personnel training course and Jed Odermatt as the tormented \"beast\" with the poet's soul. And prevailing over all is the Hostess, Narrah Luks, who is not only drop-dead gorgeous but talented with it.
