Mental Health Status of Health Care Professionals Working in Public and Private Sectors in Visakhapatnam: A Cross-Section Study
ABSTRACT Introduction: The material that is currently available on the psychological issues that doctors deal with is restricted to a few national surveys and certain particular hospital surveys. The motive of the current study was to define the mental health status of medical as well as surgical professionals working in the public and private sectors of Visakhapatnam, Andhra Pradesh, India. Methodology: A self-reported questionnaire survey was used in an observational cross-sectional study to appraise the mental health state of both private and public medical practitioners. The Kessler Psychological Distress Scale (K10) by William C. Menninger was applied. Results: The majority of study participants experienced psychological anguish, showing a substantial difference in the total level of psychological distress. When the Wellinger's Distress and the Kesslers Distress tools were compared, it was found that there was a substantial difference for all the variables taken into account in the current study, including gender, experience, marital status, children, and occupation, designation, and sector of work. Conclusion: Stress and burnout were more prevalent among medical specialists with lower yearly income, young ages, and fewer years of practice. Clinicians and policymakers must be proactive in creating programes that help healthcare professionals cope with stress and burnout.
Musculoskeletal Disorders and Their Associated Risk Factors in Long-Distance Truck Drivers From Allahabad, India: A Cross-Sectional Study
Occupational driving has often been associated with a high prevalence of pain in the neck, shoulder, wrist/hand, back and knee. The present study was undertaken to find out the prevalence of musculoskeletal disorders and associated risk factors among long-distance truck drivers from Allahabad, Uttar Pradesh. A cross-sectional study was conducted with 108 long-distance truck drivers. They were interviewed and examined as per a pre-designed, pre-tested semi-structured interview questionnaire. Results indicate that 78% of long-distance truck drivers had musculoskeletal disorder. The most common symptoms were low back pain (40%) followed by shoulder pain (26%), knee pain (24%), and neck pain (20%). Age, education, addiction, years of work experience, hours of driving were significantly associated with musculoskeletal morbidity (P<0.001). Long-distance truck drivers with continued driving exposure have a high prevalence of a musculoskeletal disorder.
Liposarcoma of Maxilla- A Rare Case Report
Introduction Sarcomas of the head and neck region account for less than 10% of soft tissue sarcomas, and comprise less than 1% of head and neck malignancies. Approximately 80% of sarcomas arise from soft tissue, with the remaining originating from bone or cartilage. Head and neck sarcomas typically occur more frequently in men. Case report Our patient was a 30 year old male who presented with a ulceroproliferative mass over the soft palate increasing in size over past 6 months. Punch biopsy of the lesion was suggestive of carcinosarcoma. MRI PNS was suggestive of 38 × 39 × 33 mm sized lobulated lesion involving soft palate on right side, crossing midline and displaces uvula. The lesion involved right side of hard palate, right retromolar trigone with abutment of right medial pterygoid. It was also suggestive of bilateral sub centimetric nodes involving bilateral level 2 neck nodes. The patient received 3 cycles neoadjuvant chemotherapy Ifosfamide and Doxorubicin and patient had no subjective response to chemotherapy with subjective increase in the size of lesion. MRI PNS post neoadjuvant chemotherapy was suggestive of 46 × 46 × 41 mm lobulated lesion involving right side of the soft palate crossing midline over to the left, involving right posterior hard palate and right retromolar trigone, tonsillar fossa and TL sulcus. Discussion The patient underwent wide local excision with medial maxillectomy via a bilateral Weber-Ferguson incision with Diffenbach extension and sent for frozen. The margins were negative and free of tumor. Bilateral modified neck dissection type 3 was done for lymph node clearance. A bilateral temporalis flap was done for reconstruction of the defect with a temporary obturator placement. Sarcomas of the sinonasal region may present a diagnostic challenge, as their location in the sinuses or nasal cavity may lead to presenting symptoms such as epistaxis, nasal congestion, or sinus pain and pressure that may be attributed to more benign causes such as chronic sinusitis, sinonasal polyposis, or allergic rhinitis. The less common sinonasal sarcoma subtypes may present a diagnostic challenge as their histopathological characteristics may overlap, especially with variations in tumor grade or with dedifferentiation. Additionally, the ideal treatment modality may present a therapeutic challenge, as the response to radiation and/or chemotherapy may vary according to the sarcoma subtype. Surgery is considered to be the mainstay in the management of carcinosarcomas of head and neck. Based on the evidence presented herein, elective neck dissection should be considered to treat squamous cell carcinomas of the maxillary palate, ginigiva, and alveolus.
Differentiated Thyroid Cancers with Synchronous Second Primary Cancers: Case Series
The incidence of synchronous second primary cancer (SSPC) with DIfferentiated Thyroid Cancers (DTC) is a very rare entity. Very few case reports and case series were published in the literature. To enumerate the clinicopathological data of patients with DTC with SSPC. This is a single-center retrospective study. All the patients diagnosed with DTC and SSPC from January 2016 to July 2023 were included in the study. The demographic, clinicopathological data, and survival data were collected from the institute's database. Twelve patients were found to have DTC with SSPC. All the patients had papillary thyroid cancer. Ten patients had head and neck squamous cell carcinoma, one patient had malignant phyllodes tumor, and another patient had endometrioid adenocarcinoma as the SSPC. Six patients were diagnosed during preoperative evaluation, one patient was diagnosed 3 months after the oral malignancy diagnosis, and five patients were diagnosed with PTC in cervical nodes after neck dissection. Complete thyroid removal was done in six patients and hemithyroidectomy in two patients. The median follow-up was 25.55 months. The median overall survival was 40.97 months. Head and neck SCC is the most common SSPC with DTC. Since these are rare scenarios multi-disciplinary panel discussion can help in deciding management.
Ballistic damage in hybrid composite laminates
Ballistic damage of hybrid woven-fabric composites made of plain-weave E-glass- fabric epoxy and 8H satin-weave T300 carbon-fabric epoxy is studied using a combination of experimental tests, microstructural studies and finite-element (FE) analysis. Ballistic tests were conducted with a single-stage gas gun. Fibre damage and delamination were observed to be dominating failure modes. A ply-level FE model was developed, with a fabric-reinforced ply modelled as a homogeneous orthotropic material with capacity to sustain progressive stiffness degradation due to fibre matrix cracking, fibre breaking and plastic deformation under shear loading. Simulated damage patterns on the front and back faces of fabric-reinforced composite plates provided an insight into their damage mechanisms under ballistic loading.
Thermal Infrared Imaging Spectrometer for Mars Orbiter Mission
Thermal Infrared Imaging Spectrometer (TIS), which operates in the infrared spectral region (7–13 μm), is one of the five instruments on-board the Mars Orbiting Mission (MOM). TIS was designed to detect emitted thermal infrared radiation from the Martian environment, which would enable the estimation of ground temperature of the surface of Mars and also map its surface composition. TIS instrument is a grating-based spectrometer which has spatial resolution of 258 m at periapsis (372 km). TIS hardware was realized with light-weight miniaturized components (total weight 3.2 kg) with power requirement of 6 W. Observations from TIS instrument were carried out during Earth-bound manoeuvres and cruise phase operations of MOM and the results were found to be in agreement with the laboratory measurements.
