Statistical analysis of enhanced SDEx encryption method based on BLAKE3 hash function
This paper presents a statistical analysis of the enhanced SDEx (Secure Data Exchange) encryption method, using a version that incorporates two session keys. This method has not previously been combined with the BLAKE3 hash function. The statistical analysis was conducted using the NIST Statistical Test Suite. Several real-world sample files were encrypted using the proposed method and then subjected to statistical analysis through selected tests from the NIST suite. These tests aimed to determine whether the resulting ciphertexts meet the criteria for pseudorandomness. Additionally, compression tests were performed using WinRAR, which confirmed that the ciphertexts are not compressible.
Consensus on international guidelines for management of groin hernias
BackgroundGroin hernia management has a significant worldwide diversity with multiple surgical techniques and variable outcomes. The International guidelines for groin hernia management serve to help in groin hernia management, but the acceptance among general surgeons remains unknown. The aim of our study was to gauge the degree of agreement with the guidelines among health care professionals worldwide.MethodsForty-six key statements and recommendations of the International guidelines for groin hernia management were selected and presented at plenary consensus conferences at four international congresses in Europe, the America’s and Asia. Participants could cast their votes through live voting. Additionally, a web survey was sent out to all society members allowing online voting after each congress. Consensus was defined as > 70% agreement among all participants.ResultsIn total 822 surgeons cast their vote on the key statements and recommendations during the four plenary consensus meetings or via the web survey. Consensus was reached on 34 out of 39 (87%) recommendations, and on six out of seven (86%) statements. No consensus was reached on the use of light versus heavy-weight meshes (69%), superior cost-effectiveness of day-case laparo-endoscopic repair (69%), omitting prophylactic antibiotics in hernia repair, general or local versus regional anesthesia in elderly patients (55%) and re-operation in case of immediate postoperative pain (59%).ConclusionGlobally, there is 87% consensus regarding the diagnosis and management of groin hernias. This provides a solid basis for standardizing the care path of patients with groin hernias.
Beaver Dams as a Significant Factor in Shaping the Hydromorphological and Hydrological Conditions of Small Lowland Streams
Beavers play a key role in creating temporary water reservoirs that significantly impact the natural environment and local river hydrology. The primary aim of this study was to assess the potential of increasing the number of beaver dams (Castor spp.), as an alternative method of water retention in the environment. Research conducted on three small lowland streams in central Poland revealed that beaver dams, even in modified riverbeds, enable the formation of shallow floodplains and ponds. Innovative analyses considered the structural materials of the dams and their impact on river hydromorphology and sediment transport. The findings emphasise the importance of beavers in water retention processes, the stabilisation of water levels during low flows and the protection of biodiversity. The study also demonstrated that beaver dams play a critical role in storing surface- and groundwater, mitigating drought impacts, reducing surface runoff, and stabilising river flows. These constructions influence local hydrology by increasing soil moisture, extending water retention times, and creating habitats for numerous species. The collected data highlight the potential of beaver dams as a tool in water resource management in the context of climate change. Further research could provide guidance for the sustainable utilisation of beavers in environmental conservation strategies and landscape planning.
The Influence of Temperature on Rheological Parameters and Energy Efficiency of Digestate in a Fermenter of an Agricultural Biogas Plant
This investigation specifically aims to enhance the understanding of digestate flow and mixing behavior across typical temperatures in bioreactors in agricultural biogas plants, facilitating energy-efficient mixing. Experimental tests confirmed that digestate exhibits non-Newtonian characteristics, allowing its flow behavior to be captured by rheological models. This study validated that digestate rheology significantly varies with temperature, which influences flow resistance, mixing efficiency and overall energy requirements. Two rheological models—the Bingham and Ostwald models—were applied to characterize digestate behavior, with the Ostwald model emerging as the most effective for Computational Fluid Dynamic (CFD) simulations, given its balance between predictive accuracy and computational efficiency. Specifically, results suggest that, while three-parameter models, like the Herschel–Bulkley model, offer high precision, their computational intensity is less suitable for large-scale modeling where efficiency is paramount. The small increase in the accuracy of the shearing process description does not compensate for the significant increase in CFD calculation time. Higher temperatures were found to reduce flow resistance, which in turn enables increased flow rates and more extensive mixing zones. This enhanced mass transfer and mixing potential at elevated temperatures are especially pronounced in peripheral areas of the bioreactor, farthest from the agitators. By contributing a model for rheological behavior under realistic bioreactor conditions, this study supports the optimization of energy use in biogas production. These findings emphasize that temperature adjustments within bioreactors could serve as a reliable control strategy to maintain optimal production conditions while minimizing operational costs.
Comparison of two different concepts of mesh and fixation technique in laparoscopic ventral hernia repair: a randomized controlled trial
Background Patients’ need to improve outcomes and to reduce the number of complications triggers the development of new materials and surgery concepts. Currently, there are many implants and fixation systems dedicated for intraperitoneal onlay mesh procedure. The aim of this study was to compare two different mesh/fixation system concepts (PH: Physiomesh/Securestrap and VS: Ventralight ST/SorbaFix) for laparoscopic ventral hernia repair with respect to pain. Methods A single-center, prospective, randomized study was designed to include 50 patients per group with a planned interim analysis for safety after 25 patients. The endpoints were pain occurrences and intensity, which was measured with the visual analogue scale 7 days, 30 days, 3 months and 6 months after surgery. The safety parameters included the number of recurrences and postoperative complications. Results During the interim analysis, the study was stopped due to safety reasons. We observed five (20 %) recurrences in the PH group in first 6 months and none in the VS group. We observed a significantly higher pain rate in the PH group after 3 months ( p  < 0.0001) and no difference after 7 days ( p  = 0. 7019). The pain intensity decreased significantly over time ( p  < 0.0001) and was significantly higher in the PH group ( p  < 0.0001). Conclusions Although this clinical trial was terminated prior to the preplanned recruitment goal, the obtained results from the enrolled patients indicate that the PH system associated with significantly greater hernia recurrences and postoperative pain compared with the VS system. This confirms the superiority of the elastic mesh concept, which may be a safer and more efficacious option for laparoscopic ventral hernia repairs.
Preparation of Samples for the Study of Rheological Parameters of Digested Pulps in a Bioreactor of an Agricultural Biogas Plant
The studies of the rheology of digested pulp from agricultural biogas plants have often been fragmentary and non-standardised due to their complexity and time-consuming nature. As a result of measurements, it was possible to develop a procedure and range of measurements for the correct determination of the parameters of the carrier substance. The applicability of the coaxial cylinder measurement system was demonstrated for assessing the rheological parameters of digested pulp from a fermenter that utilises agricultural biomass. To determine the characteristics of solid particles, the Zingg diagram was used, inter alia, allowing the comparison of particles from each fraction. The analysis of the shape and size of solid particles may help to describe the onset of motion of this phase, flow type, or sedimentation type. The authors propose a completely new research approach to obtain an appropriate, repeatable test conditions of medium, which is the carrier liquid from the biogas plant reactor. The proposed methodology and the scenario of the entire study make it possible to achieve scalable and comparable test results in any laboratory. The proposed solution eliminates the influence of most external factors on the sample and rheological measurements, and the effectiveness of the presented procedure was confirmed in tests.
Do Diabetes and Genetic Polymorphisms in the COMT and OPRM1 Genes Modulate the Postoperative Opioid Demand and Pain Perception in Osteoarthritis Patients After Total Knee and Hip Arthroplasty?
Background: Osteoarthritis (OA) of the hip and knee is a common age-related degenerative disease characterized by joint pain, stiffness, and gait disturbances. This study investigated the influence of genetic polymorphisms in the OPRM1 (rs1799971) and COMT (rs4633, rs4680, rs4818, and rs6269) genes on the postoperative analgesic requirements in 195 diabetic and non-diabetic patients undergoing total hip or knee arthroplasty. Methods: The prospective study included all patients who were admitted between January and September 2020 and agreed to participate. Postoperative pain management was assessed based on acetaminophen, ketoprofen, and morphine consumption on the first and second postoperative day. Results: Multilevel regression analyses revealed a significant three-way interaction between diabetes, type of analgesic, and OPRM1rs1799971 polymorphism, indicating different analgesic dosing patterns in diabetic and non-diabetic patients. Two-way interactions between diabetes and COMT polymorphisms rs4633, rs4680, and rs6269 further influenced the analgesic requirements. No significant associations were found for COMT rs4818. The results show that diabetes and genetic factors significantly influence opioid requirements and pain perception. Conclusions: Given the complexity of pain management in diabetic patients, personalized analgesic strategies tailored to genetic and metabolic profiles could be useful in postoperative pain management and reducing opioid consumption.
Individual Characteristics and Pain Sensitivity during Pregnancy—A Cross-Sectional Study in Pregnant and Non-Pregnant Women
The aim of the study was to describe the characteristics and factors related to pain perception in pregnant women, such as optimism, personality traits, and fear of developing COVID-19 consequences. Sixty-six pregnant women aged 23 to 42 years participated in the study, and the comparison group consisted of n = 59 non-pregnant female students aged 19 to 23 years. Pressure pain threshold and pain tolerance were measured with an algometer. To assess psychological characteristics, the Life-Orientation Test-Revised was used to assess optimism, the Fear of COVID-19 Scale was used to assess COVID-19 anxiety, and the Ten-Item Personality Inventory was applied to assess personality traits in a five-factor model. The main results of the study showed that pain tolerance was significantly lower in both dominant and non-dominant hand pregnant women than in the comparison group. The studied pregnant women had higher scores for conscientiousness, fear of COVID-19, and optimism compared with the non-pregnant women. Regression analysis showed that the variability in pain perception among pregnant women could not be explained by individual differences in personality traits, optimism, and fear of COVID-19.
Individual Factors Modifying Postoperative Pain Management in Elective Total Hip and Total Knee Replacement Surgery
Total hip and knee replacements are the most common orthopedic procedures performed due to osteoarthritis. Pain is an intrinsic symptom accompanying osteoarthritis, persisting long before surgery, and continuing during the preoperative and postoperative periods. Appropriate pain management after surgery determines the comfort, duration, and cost of hospitalization, as well as the effectiveness of postoperative rehabilitation. Individual differences in pain perception and tolerance in orthopedic patients remain an important research topic. Therefore, the aim of this study was to investigate the predictors of analgesic requirements (morphine, acetaminophen, and ketoprofen), including individual pain threshold and tolerance, body mass index (BMI), diabetes, and beliefs about pain control in patients undergoing elective hip or knee arthroplasty using a multilevel regression model (N = 147, 85 women, 62 men, 107 after hip replacement, and 40 after knee replacement). Results: Higher pain tolerance was associated with a lower dose of morphine per kg after surgery. Patients undergoing hip surgery received a lower dose of ketoprofen than patients undergoing knee surgery. The more the patient believed in personal pain control, the stronger the negative relationship between pain tolerance and morphine requirement. The lowest doses were given to patients with the highest pain tolerance and the greatest belief in personal control. Factors such as belief in pain control and pain tolerance should be considered in comprehensive postoperative pain management in orthopedic patients to reduce opioid doses and, thus, side effects.
Water Softener Regeneration Effects on the Operation of Domestic Wastewater Treatment Plants: A Preliminary Study
There has been a large amount of scientific research carried out to date on the impact of salty backwash brine from domestic water softeners (WS) on domestic wastewater treatment plants (DWTPs). Experts and practitioners agree that the impact is harmful and there is still a need to look for new technologies. The study of the effect of an increased sodium chloride (NaCl) concentration after softener regeneration is important from the point of view of the operation of DWTPs and soil properties. This paper presents the results of a field study of the concentration of NaCl at the septic tank (ST) drainage point, into which the grey water from the regeneration of the water softener flowed. During the six-month measurements (recorded every 1 min), an increase in NaCl concentration was observed in the septic tank outflow, from an average NaCl concentration of 1.5 g·L−1—between regenerations—to an average concentration of 4.5 g·L−1—after water softener regeneration. The increased NaCl concentration decreased significantly up to 2 days after the water softener regeneration. Temperature changes in the treated wastewater were also measured—during the winter period, temperature differences of up to 10 °C per day were recorded. In the second part of the study, conducted on a semi-technical scale, the effect of brine from the regeneration of the water softener on the hydraulic conductivity (Ks) of the soil from the infiltration drain of the DWTPs studied was assessed. The Ks was determined by analysing the time it took the water to soak into the soil, using the Van Hoorn equation. The results and statistical analysis indicate an increased salt content in the soil absorbing the brine, which may have been influenced by the reduced absorption and capacity of the drain due to adverse physico-chemical changes.
