Tiltmeters have the potential to resolve ground deformation due to changes in hydraulic head induced by conduit pressurization. Conduit pressure variations cause groundwater to be stored or released from storage within the surrounding rock matrix. We modeled this process and infer whether the resulting deformation is measurable with tiltmeters and what behavior to expect by fully coupling porous media flow and solid mechanics in a poroelastic, 2D finite element model. Parameter sets globally representative of Paleozoic, Mesozoic, and Cenozoic confined and unconfined aquifers are considered. Our analysis focuses on the impact of the parameterization on pore pressure, vertical displacement, and tilt. We find that the spatial distribution of the poroelastic signal depends on the hydraulic diffusivity, and its magnitude depends on the mechanical and coupling parameters. Additional analysis of the impacts of conduit radius and depth suggests that tilt polarity could be an indicator of conduit location and relative conduit size. We calibrated the model to data observations acquired at the Santa Fe River Sink‐Rise system in north‐central Florida, US. We find that an overlying clay‐rich layer may act to partially confine the aquifer. Although the observed tilt signal is present in radial and transverse components and polarity reversals occur, we were able to recover the magnitude and general trend of the tilt response. Plain Language Summary Our study focused on understanding how changes in ground deformation can be caused by groundwater flow induced by a pressurized karst conduit. We modeled this process in different types of aquifers to determine if the deformation can be measured using tiltmeters, instruments that measure changes in the angle of the ground. We also explored the effect of conduit size and depth. Our findings show that the distribution and magnitude of the ground response depend on the physical parameters of the aquifer. Additionally, we find that tilt data can provide information about conduit location, size, and depth. We tested the model using data from the Santa Fe River Sink‐Rise system in Florida. We find this system may behave as a partially confined aquifer due to the presence of clay in the overlying unit. From the use of our simplified numerical formulation, we were able to recover the magnitude and overall trend of the observed tilt data. Relatively little is currently known about the spatiotemporal variability of land surface conditions during the North American monsoon, in particular for regions of complex topography. As a result, the role played by land–atmosphere interactions in generating convective rainfall over steep terrain and sustaining monsoon conditions is still poorly understood. In this study, the variation of hydrometeorological conditions along a large-scale topographic transect in northwestern Mexico is described. The transect field experiment consisted of daily sampling at 30 sites selected to represent variations in elevation and ecosystem distribution. Simultaneous soil and atmospheric variables were measured during a 2-week period in early August 2004. Transect observations were supplemented by a network of continuous sampling sites used to analyze the regional hydrometeorological conditions prior to and during the field experiment. Results reveal the strong control exerted by topography on the spatial and temporal variability in soil moisture, with distinct landscape regions experiencing different hydrologic regimes. Reduced variations at the plot and transect scale during a drydown period indicate that homogenization of hydrologic conditions occurred over the landscape. Furthermore, atmospheric variables are clearly linked to surface conditions, indicating that heating and moistening of the boundary layer closely follow spatial and temporal changes in hydrologic properties. Land–atmosphere interactions at the basin scale (~100 km²), obtained via a technique accounting for topographic variability, further reveal the role played by the land surface in sustaining high atmospheric moisture conditions, with implications toward rainfall generation during the North American monsoon.

Large‐scale underground storage of CO2 to mitigate climate change is being implemented around the world; this requires additional planning and protections of water resources around the injection sites. In the USA, CO2 storage and the protection of underground sources of drinking water (USDW's) for an injection site are regulated by the Environmental Protection Agency through the Underground Injection Control (UIC) Program and implemented through its Class VI well permit. Technical criteria of the permit application includes detailed site characterization where the extent of the lowest USDW's, the direction of groundwater flow within them, and their depth relative to the injection zone must be defined. Here, we support a UIC Class VI permit application for a large‐scale commercial CO2 storage project within the San Juan Basin. We acquired all publicly available groundwater and produced water data and used ArcGIS Pro and the USGS geologic maps of New Mexico and Colorado to define the areal extent of the lowest USDW's in the area. Results indicate that there are seven formations that qualify as the lowest USDW in the region and that hydrocarbon‐bearing formations which report water production should be reviewed as potential sources of drinking water if not exempt. Because very little literature exists in the analysis of potential USDW's for the Class VI permit application, this study may provide insight for future organizations who intend to apply. Our results will be particularly insightful in oil and gas producing states where production formations also contain large volumes of water.

Infrastructure to factorially manipulate the mean and variance of precipitation in the field

Extensive ecological research has investigated extreme climate events or long‐term changes in average climate variables, but changes in year‐to‐year (interannual) variability may also cause important biological responses, even if the mean climate is stable. The environmental stochasticity that is a hallmark of climate variability can trigger unexpected biological responses that include tipping points and state transitions, and large differences in weather between consecutive years can also propagate antecedent effects, in which current biological responses depend on responsiveness to past perturbations. However, most studies to date cannot predict ecological responses to rising variance because the study of interannual variance requires empirical platforms that generate long time series. Furthermore, the ecological consequences of increases in climate variance could depend on the mean climate in complex ways; therefore, effective ecological predictions will require determining responses to both nonstationary components of climate distributions: the mean and the variance. We introduce a new design to resolve the relative importance of, and interactions between, a drier mean climate and greater climate variance, which are dual components of ongoing climate change in the southwestern United States. The Mean × Variance Experiment (MVE) adds two novel elements to prior field infrastructure methods: (1) factorial manipulation of variance together with the climate mean and (2) the creation of realistic, stochastic precipitation regimes. Here, we demonstrate the efficacy of the experimental design, including sensor networks and PhenoCams to automate monitoring. We replicated MVE across ecosystem types at the northern edge of the Chihuahuan Desert biome as a central component of the Sevilleta Long‐Term Ecological Research Program. Soil sensors detected significant treatment effects on both the mean and interannual variability in soil moisture, and PhenoCam imagery captured change in vegetation cover. Our design advances field methods to newly compare the sensitivities of populations, communities, and ecosystem processes to climate mean × variance interactions.

Cabotegravir (CAB) is a novel strand-transfer integrase inhibitor being developed for HIV treatment and prevention. CAB is formulated both as an immediate-release oral tablet for daily administration and as a long-acting injectable suspension (long-acting CAB [CAB LA]) for intramuscular (IM) administration, which delivers prolonged plasma exposure to the drug after IM injection. HIV Prevention Trials Network study 077 (HPTN 077) evaluated the safety, tolerability, and pharmacokinetics of CAB LA in HIV-uninfected males and females at 8 sites in Brazil, Malawi, South Africa, and the United States.
HPTN 077 was a double-blind, placebo-controlled phase 2a trial. Healthy individuals age 18-65 years at low HIV risk were randomized (3:1) to receive CAB or placebo (PBO). In the initial oral phase, participants received 1 daily oral tablet (CAB or PBO) for 4 weeks. Those without safety concerns in the oral phase continued and received injections in the injection phase (Cohort 1: 3 injections of CAB LA 800 mg or 0.9% saline as PBO IM every 12 weeks for 3 injection cycles; Cohort 2: CAB LA 600 mg or PBO IM for 5 injection cycles; the first 2 injections in Cohort 2 were separated by 4 weeks, the rest by 8 weeks). The primary analysis included weeks 5 to 41 of study participation, encompassing the injection phase. The cohorts were enrolled sequentially. Primary outcomes were safety and tolerability. Secondary outcomes included pharmacokinetics and events occurring during the oral and injection phases. Between February 9, 2015, and May 27, 2016, the study screened 443 individuals and enrolled 110 participants in Cohort 1 and 89 eligible participants in Cohort 2. Participant population characteristics were as follows: 66% female at birth; median age 31 years; 27% non-Hispanic white, 41% non-Hispanic black, 24% Hispanic/Latino, 3% Asian, and 6% mixed/other; and 6 transgender men and 1 transgender woman. Twenty-two (11%) participants discontinued the oral study product; 6 of these were for clinical or laboratory adverse events (AEs). Of those who received at least 1 CAB LA injection, 80% of Cohort 1 and 92% of Cohort 2 participants completed all injections; injection course completion rates were not different from those in the PBO arm. Injection site reactions (ISRs) were common (92% of Cohort 1 and 88% of Cohort 2 participants who received CAB LA reported any ISR). ISRs were mostly Grade 1 (mild) to Grade 2 (moderate), and 1 ISR event (Cohort 1) led to product discontinuation. Grade 2 or higher ISRs were the only AEs reported more commonly among CAB LA recipients than PBO recipients. Two Grade 3 (severe) ISRs occurred in CAB recipients, 1 in each cohort, but did not lead to product discontinuation in either case. Seven incident sexually transmitted infections were diagnosed in 6 participants. One HIV infection occurred in a participant 48 weeks after last injection of CAB LA: CAB was not detectable in plasma both at the time of first reactive HIV test and at the study visit 12 weeks prior to the first reactive test. Participants in Cohort 2 (unlike Cohort 1) consistently met prespecified pharmacokinetic targets of at least 95% of participants maintaining CAB trough concentrations above PA-IC90, and 80% maintaining trough concentrations above 4× PA-IC90. Study limitations include a modest sample size, a short course of injections, and a low-risk study population.
In this study, CAB LA was well tolerated at the doses and dosing intervals used. ISRs were common, but infrequently led to product discontinuation. CAB LA 600 mg every 8 weeks met pharmacokinetic targets for both male and female study participants. The safety and pharmacokinetic results observed support the further development of CAB LA, and efficacy studies of CAB LA for HIV treatment and prevention are in progress.

Evaluation of long‐acting cabotegravir safety and pharmacokinetics in pregnant women in eastern and southern Africa: a secondary analysis of HPTN 084

Introduction Long‐acting injectable cabotegravir (CAB‐LA) for pre‐exposure prophylaxis significantly reduced HIV acquisition in HPTN 084. We report on the safety and CAB‐LA pharmacokinetics in pregnant women during the blinded period of HPTN 084. Methods Participants were randomized 1:1 to either active cabotegravir (CAB) plus tenofovir disoproxil fumarate/emtricitabine (TDF/FTC) placebo or active TDF/FTC plus CAB placebo. Pregnancy testing was performed at each visit; participants with a positive test had study product withheld and were offered open‐label TDF/FTC. Pregnancies were confirmed on two tests at least 4 weeks apart. All participants with a positive pregnancy test prior to November 5, 2020 are included in this analysis. Pregnancy incidence, maternal adverse event (AE) incidence, pregnancy outcomes (including composite outcome of spontaneous abortion <20 weeks, intrauterine foetal death or stillbirth ≥20 weeks, premature birth <37 weeks, or small for gestational age) were assessed. The apparent terminal phase half‐life (t1/2app) of CAB‐LA in pregnant women in HPTN 084 was compared to non‐pregnant women from the phase 2a HPTN 077 trial. Multivariable models assessed associations with t1/2app. Results Fifty‐seven pregnancies (30 CAB‐LA, 27 TDF/FTC) were confirmed over 3845 person‐years [py] (incidence 1.5/100 py, 95% CI 1.1−1.9). CAB‐LA group participants had a median 342 days (IQR 192, 497) of CAB‐LA exposure prior to pregnancy detection. Grade 2 or higher maternal AE incidence did not differ by study arm (CAB 157, 95% CI 91−271 per 100 py vs. TDF/FTC 217, 95% CI 124–380 per 100 py; p = 0.256). Most pregnancies (81%) resulted in live births (25 CAB‐LA, 22 TDF/FTC). Composite poor pregnancy outcomes did not differ significantly by group (CAB 6/30 vs. TDF/FTC 4/27; p = 0.476). No congenital anomalies were observed. The CAB t1/2app geometric mean was 52.8 days (95% CI 40.7−68.4) in pregnant women compared to 60.3 days (95% CI 47.7−76.3; p = 0.66) in non‐pregnant women; neither pregnancy nor body mass index were significantly associated with t1/2app. Conclusions CAB‐LA concentrations post‐cessation of injections were generally well tolerated in pregnant women. The t1/2app was comparable between pregnant and non‐pregnant women. Ongoing studies will examine the safety and pharmacology of CAB‐LA in women who choose to continue CAB‐LA through pregnancy and lactation.

Introduction HPTN 083 demonstrated the superiority of long‐acting cabotegravir (CAB‐LA) versus daily oral emtricitabine/tenofovir disoproxil fumarate (TDF/FTC) as pre‐exposure prophylaxis (PrEP) among cisgender men and transgender women who have sex with men (MSM/TGW). HPTN 083 provided the first opportunity to understand experiences with injectable PrEP in a clinical trial. Methods Participants from two US sites (Chicago, IL and Atlanta, GA) and one international site (Rio de Janeiro, Brazil) were purposively sampled for individual qualitative interviews (N = 40), between November 2019 and March 2020, to explore trial experiences, barriers to adherence and other factors that may have impacted study implementation or outcomes. The blinded phase ended early due to efficacy; this analysis includes interviews conducted prior to unblinding with three groups defined by adherence (i.e. injection visit attendance): adherent (n = 27), non‐adherent (n = 12) and early discontinuers (n = 1). Data were organized using NVivo software and analysed using content analysis. Results Participants (mean age: 27) were primarily cisgender MSM (90%) and Black/African American (60%). Reasons for trial enrolment and PrEP use included a preference for using HIV prevention medication versus treatment in the event of HIV acquisition; the ability to enhance health via study‐related education and services; access to a novel, convenient HIV prevention product at no cost; and contributing to MSM/TGW communities through research. Participants contrasted positive experiences with study staff with their routine clinical care, and emphasized increased scheduling flexibility, thorough communication, non‐judgemental counselling and open, affirming environments (e.g. compassion, less stigma) as adherence facilitators. Injection experiences were positive overall; some described early injection‐related anxiety, which abated with time and when given some measure of control (e.g. pre‐injection countdown), and minimal injection site discomfort. Some concerns and misperceptions about injectable PrEP were reported. Barriers to adherence, across all adherence categories, included structural factors (e.g. financial constraints, travel) and competing demands (e.g. work schedules). Conclusions Respondents viewed injectable PrEP trial participation as a positive experience and a means of enhancing wellbeing. Study site flexibility and affirming clinic environments, inclusive of non‐judgemental counselling, were key facilitators of adherence. To support injection persistence, interventions that address structural barriers and promote flexible means of injection delivery may be most effective.

Phase 2 Study of the Safety and Tolerability of Maraviroc-Containing Regimens to Prevent HIV Infection in Men Who Have Sex With Men (HPTN 069/ACTG A5305)

Background. Maraviroc (MVC) is a candidate for human immunodeficiency virus (HIV) pre-exposure prophylaxis. Methods. Phase 2 48-week safety/tolerability study was conducted, comparing 4 regimens: MVC alone, MVC plus emtricitabine (FTC), MVC plus tenofovir disoproxil fumarate (TDF), and TDF plus FTC. Eligible participants were HIV-uninfected men and transgender women reporting condomless anal intercourse with ≥1 HIV-infected or unknown-serostatus man within 90 days. At each visit, assessments, laboratory testing, and counseling were done. Analyses were intention to treat. Results. Among 406 participants, 84% completed follow-up, 7% stopped early, and 9% were lost to follow-up; 9% discontinued their regimen early. The number discontinuing and the time to discontinuation did not differ among study regimens (P = .60). Rates of grade 3-4 adverse events did not differ among regimens (P = .37). In a randomly selected subset, 77% demonstrated detectable drug concentrations at week 48. Five participants acquired HIV infection (4 MVC alone, 1 MVC + TDF; overall annualized incidence, 1.4% [95% confidence interval, .5%-3.3%], without differences by regimen; P = .32); 2 had undetectable drug concentrations at every visit, 2 had low concentrations at the seroconversion visit, and 1 had variable concentrations. Conclusions. MVC-containing regimens were safe and well tolerated compared with TDF + FTC; this study was not powered for efficacy. Among those acquiring HIV infection, drug concentrations were absent, low, or variable. MVC-containing regimens may warrant further study for pre-exposure prophylaxis. Clinical Trials Registration.
