Exploring the perspectives of health care professionals on digital health technologies in pediatric care and rehabilitation

Background
Digital health technologies are increasingly used by healthcare professionals working in pediatric hospital and rehabilitation settings. Multiple factors may affect the implementation and use of digital health technologies in these settings. However, such factors have not been identified in a multidisciplinary, pediatric context. The objective of this study was to describe actual use and to identify the factors that promote or hinder the intention to use digital health technologies (mobile learning applications, virtual/augmented reality, serious games, robotic devices, telehealth applications, computerized assessment tools, and wearables) among pediatric healthcare professionals.
Methods
An online survey evaluating opinions, current use, and future intentions to use digital health technologies was completed by 108 professionals at one of Canada’s largest pediatric institutes. Mann-Whitney U tests were used to compare the attitudes of healthcare professionals who intend to increase their use of digital health technologies and those who do not. Linear regression analyses were used to determine predictors of usage success.
Results
Healthcare professionals reported mostly using mobile and tablet learning applications (
n
= 43, 38.1%), telehealth applications (
n
= 49, 43.4%), and computerized assessment tools (
n
= 33, 29.2%). Attitudes promoting the intention to increase the use of digital health technologies varied according to technology type. Healthcare professionals who wished to increase their use of digital health technologies reported a more positive attitude regarding benefits in clinical practice and patient care, but were also more critical of potential negative impacts on patient-professional relationships. = 0.020) was a significant predictor of more favorable usage success. The range of obstacles encountered was also a significant predictor (

Conclusions
When working with children, incorporating digital health technologies can be effective for motivation and adherence. However, it is crucial to ensure these tools are implemented properly. The findings of this study underscore the importance of addressing training and infrastructure needs when elaborating technology-specific strategies for multidisciplinary adoption of digital health technologies in pediatric settings.

This paper addresses the distribution of Genitives and PPs in deverbal nominalizations with a particular focus on Romanian -tor nominals that express agents, e.g., vânzător ‘seller’, and instruments, e.g., tocător ‘shredder’. The issue of the distribution of Genitives is central to understanding the argumental structure properties of deverbal nominalizations in Romanian, and cross-linguistically. Derived nominals expressing agents and instruments have been and continue to be the subject of much controversy in the literature with respect to whether they (or at least some of them) involve an argument structure. We argue that the existence of an argumental Genitive in Romanian provides strong support for views that propose that (at least a subclass of) agent nominals have argument-structure properties. However, we also show that a proper understanding of the distribution of Genitive case-marked complements and PPs bears upon their interpretive nature, and more particularly on their specific vs. non-specific interpretation.

Accounting for period effect, a reduced occurrence of diarrhoea is significantly associated with improved sanitation facilities in children 24-59 months (OR: 0.62; p < 0.05) and with owning a refrigerator in children 6-11 months (OR: 0.11; p < 0.05). Children aged 24-59 months with two or more commodities have a prevalence of diarrhoea that is statistically lower than that of their disadvantaged counterparts (OR: 0.45; p < 0.05).

We investigate so-called causative psych nominalizations (CPNs), i.e., nominalizations of object experiencer (OE) verbs that realize non-agentive causers as external arguments. While they are ruled out in English (Grimshaw 1990; Iwata 1995; Pesetsky 1995) and have been suggested to be cross-linguistically banned (Landau 2010), we show that Romanian and Greek derive CPNs from the subject experiencer (SE) form of alternating OE verbs. We analyze them as nominalizations of the anticausative SE form of these verbs. Our results suggest a structural difference between Romanian/Greek and English psych nominalizations: the former can nominalize either the SE anticausative or (the passive of) the OE transitive structure (Anagnostopoulou 1999), while the latter only nominalize the OE transitive form, as the verbs lack the anticausative structure.
A case of abnormally dilated and tortuous arc of Buhler and pancreaticoduodenal arteries in the absence of celiac trunk stenosis

IntroductionThe Arc of Buhler is a rare vascular variant describing a persistent remnant of the embryologic ventral anastomosis between the celiac trunk (CT) and superior mesenteric artery (SMA), invariably reported in the context of CT stenosis.PurposeTo report a case of (1) a large and tortuous pancreaticoduodenal arcade and (2) a large and tortuous Arc of Buhler in the absence of celiac axis stenosis.MethodsThe variant was discovered during routine cadaver dissection. We acquired transverse biopsies of variant vessels and evaluated their wall thickness.ResultsThe donor’s anterior PDA, posterior PDA, and Arc of Buhler had larger diameters, and the common hepatic artery had a smaller diameter than the literature-reported values of a standard human body. The posterior PDA had significantly increased wall thickness compared to the other investigated vessels.ConclusionsThe Arc of Buhler is a rare remnant of the embryologic ventral anastomosis that is estimated to be hemodynamically active in only half of cases. Previous reports have documented hemodynamically active Arcs of Buhler only in cases of CT stenosis. To the best of the authors’ knowledge, this is a unique case of a persistent and hemodynamically active Arc of Buhler in the absence of CT stenosis. Clinicians should be aware of this variant as its abnormal position may increase risk of herniation and surgical complications, and its tortuosity may increase risk of clot formation.

Prefrontal cortex dysfunction in patients with suicidal behavior

Background. Abnormal serotonergic neurotransmission has long been demonstrated in suicidal behavior. The dorsal and median raphe nuclei housing the main serotonergic cell bodies and the prefrontal cortex (PFC), particularly the ventral part innervated by the serotonergic system, have therefore been studied extensively in suicidal behavior research. However, only a few studies have described neuropsychological function impairment in suicidal patients. We investigated PFC-related neuropsychological function in patients with suicidal behavior, separating dorsolateral PFC (DLPFC)- and orbitofrontal cortex (OFC)-related functions. Method. We compared 30 euthymic patients with suicidal behavior aged 18–65 years with 39 control subjects, for the following neuropsychological domains: global intellectual functioning, reward sensitivity, initiation, inhibition, and working memory. Patients and controls were compared by means of univariate and multivariate analyses, adjusting for age at interview, level of education and mood state at the time of evaluation. Trait impulsivity, measured with the Barratt Impulsivity Scale version 10 (BIS-10), was also included as a covariate in a subset of analyses. Results. Multivariate comparisons demonstrated significant executive function deficits in patients with suicidal behavior. In particular, we observed impairment in visuospatial conceptualization (p<0·0001), spatial working memory (p=0·001), inhibition (Hayling B–A, p=0·04; go anticipations, p=0·01) and visual attention (or reading fluency) (p=0·002). Similar results were obtained following adjustment for motor impulsivity as a covariate, except for spatial working memory. Conclusions. These deficits are consistent with prefrontal dysfunction in patients with suicidal behavior. Differentiation between DLPFC- and OFC-related neuropsychological functions showed no specific dysfunction of the orbitofrontal region in patients with suicidal behavior in our sample.

Commodités familiales et santé des enfants de moins de cinq ans en Haïti

OBJECTIFS
: Évaluer le rôle des commodités familiales dans l’occurrence de la diarrhée chez les enfants de moins de 5 ans en Haïti, selon leur type (eau potable, toilettes et réfrigérateur) et leur nombre.
MÉTHODE:
Nous avons mis en commun les données de quatre Enquêtes démographiques et de santé réalisées en Haïti entre 1994–1995 et 2012. Nous avons retenu un échantillon de 14 481 enfants de 1–59 mois. Nous avons évalué le rôle des commodités familiales dans l’occurrence de la diarrhée selon le groupe d’âge d’enfants à risque (1–5; 6–11; 12–23; 24–59 mois), au moyen des modèles de régression logistique.
RÉSULTATS:
La prévalence de la diarrhée chez les enfants de 1–59 mois en Haïti est estimée à 29,31 %. Elle atteint 42,14 % chez les enfants de 6–11 mois. Elle est plus faible pour les enfants dont les familles ont accès à une eau potable améliorée, des toilettes améliorées ou un réfrigérateur. Elle est d’autant plus faible que les familles possèdent au moins deux commodités simultanément. Nous ne trouvons pas de différences significatives dans l’occurrence de la diarrhée infanto-juvénile selon l’accès à l’eau potable (
p
< 0,10). Prenant en compte l’effet de période, une occurrence réduite de la diarrhée est significativement associée aux toilettes améliorées chez les enfants de 24–59 mois (RC: 0,62;
p
< 0,05) et au réfrigérateur chez les 6–11 mois (RC: 0,11;
p
< 0,05). Pour le nombre de commodités, seuls les 24–59 mois ayant au moins deux commodités ont une occurrence de la diarrhée statistiquement inférieure de celle des plus défavorisés (RC: 0,45;
p
< 0,05).
CONCLUSION:
Augmenter l’accès aux commodités familiales et leur nombre par ménage demeure essentiel dans la lutte contre la diarrhée en Haïti. Une intervention intégrée serait donc à préconiser dans l’environnement sociosanitaire haïtien.
