A randomized, controlled trial of initial anti-retroviral therapy with abacavir/lamivudine/zidovudine twice-daily compared to atazanavir once-daily with lamivudine/zidovudine twice-daily in HIV-infected patients over 48 weeks (ESS100327, the ACTION Study)
Background Traditional first line regimens containing a non-nucleoside reverse transcriptase inhibitor or protease inhibitor may not be suitable for a subset of antiretroviral-naïve patients such as those with certain co-morbidities, women of child-bearing potential, and intolerability to components of standard first line therapy. This study was conducted to determine if alternate treatment options may meet the needs of both general and special patient populations. The ACTION study was a randomized, open-label, multicenter, 48-week trial that compared the safety and efficacy of a triple nucleoside regimen versus a protease inhibitor plus a dual nucleoside regimen in HIV-1 treatment-naïve subjects. Results 279 HIV-infected subjects with HIV-1 RNA (VL) >5000 but < 200,000 copies/mL (c/mL) and CD4+ count ≥ 100 cells/mm 3 were randomized (1:1) to receive abacavir sulfate/lamivudine/zidovudine (ABC/3TC/ZDV) twice-daily or atazanavir (ATV) once-daily plus lamivudine/zidovudine (3TC/ZDV) twice-daily. Protocol-defined virologic failure was based on multiple failure criteria. Non-inferiority of ABC/3TC/ZDV to ATV+3TC/ZDV was established with 62% vs. 59% of subjects achieving a VL < 50 c/mL at week 48, [ITT(E), M/S = F, 95% CI: -5.9, 10.4]. Similar results were observed in the 230 (82%) subjects with baseline VL<100,000 c/mL (ABC/3TC/ZDV vs. ATV+3TC/ZDV), 66% vs. 59%; 95% CI: -5.6, 19.5. However, ABC/3TC/ZDV did not meet the non-inferiority criterion compared to ATV+3TC/ZDV in the 48 subjects with baseline VL ≥ 100,000 c/mL, 39% vs. 60%; 95% CI: -49.2, 7.4, respectively. Protocol-defined virologic failure was similar between groups. Conclusion ABC/3TC/ZDV demonstrated comparable virologic efficacy to ATV+3TC/ZDV in this population over 48 weeks. In those with a baseline VL ≥ 100,000 c/mL, subjects in the ATV+3TC/ZDV showed better virologic efficacy. Both regimens offer benefits in select therapy-naïve subjects. A Controlled Clinical Trial with a Specifically Configured RNA Drug, Poly(I). POly(C12U), in Chronic Fatigue Syndrome
Chronic fatigue syndrome (CFS) is a physically debilitating illness associated with immunologic abnormalities, viral reactivation, and impairment of cognition. In a randomized, multicenter, placebo-controlled, double-blind study of 92 patients meeting the CFS case definition of the Centers for Disease Control and Prevention, the response of several laboratory and clinical variables to an antiviral and immunomodulatory drug, poly(I) · poly(C12U), was determined. Measures of clinical response included Karnofsky performance score, a cognition scale derived from a self-administered instrument assessing symptomatology (SCL-90-R), an activities of daily living scale, and exercise treadmill performance. After 24 weeks, patients receiving poly(I) · poly(C12U) had higher scores for both global performance and perceived cognition than did patients receiving placebo. In particular, patients given poly(I) · poly(C12U) had increased Karnofsky performance scores (P < .03), exhibited a greater ability to do work during exercise treadmill testing (P = .01), displayed an enhanced capacity to perform the activities of daily living (P < .04), had a reduced cognitive deficit (P = .05), and required less use of other medications (P < .05).
Possible Occupational Infection of Healthcare Workers with Monkeypox Virus, Brazil
We evaluated epidemiologic and molecular characteristics of monkeypox virus (MPXV) infections sampled from 2 healthcare nurses. Five days after collecting samples from an infected patient, the nurses showed typical MPXV manifestations; quantitative PCR and whole-genome sequencing confirmed MPXV infection, most likely transmitted through contact with fomites.
Discovery of extreme quasi-periodic eruptions in a newly accreting massive black hole
Quasi-periodic eruptions (QPEs) are rapid, recurring X-ray bursts from supermassive black holes, believed to result from interactions between accretion disks and surrounding matter. The galaxy SDSS1335+0728, previously stable for two decades, exhibited an increase in optical brightness in December 2019, followed by persistent active galactic nucleus (AGN)-like variability for 5 yr, suggesting the activation of a ~10 6 - M ⊙ black hole. Since February 2024, X-ray emission has been detected, revealing extreme ~4.5-d QPEs with high fluxes and amplitudes, long timescales, large integrated energies and a ~25-d superperiod. Low-significance UV variations are reported, probably related to the long timescales and large radii from which the emission originates. This discovery broadens the possible formation channels for QPEs, suggesting that they are linked not solely to tidal disruption events but more generally to newly formed accretion flows, which we are witnessing in real time in a turn-on AGN candidate. In February 2024, rapid, recurring X-ray bursts (quasi-periodic eruptions) were detected from the black hole within galaxy SDSS1335+0728. Named Ansky, the event features day-and-a-half-long flares and extreme energy levels, challenging existing models.
Multicentric Genome-Wide Association Study for Primary Spontaneous Pneumothorax
Despite elevated incidence and recurrence rates for Primary Spontaneous Pneumothorax (PSP), little is known about its etiology, and the genetics of idiopathic PSP remains unexplored. To identify genetic variants contributing to sporadic PSP risk, we conducted the first PSP genome-wide association study. Two replicate pools of 92 Portuguese PSP cases and of 129 age- and sex-matched controls were allelotyped in triplicate on the Affymetrix Human SNP Array 6.0 arrays. Markers passing quality control were ranked by relative allele score difference between cases and controls (|RASdiff|), by a novel cluster method and by a combined Z-test. 101 single nucleotide polymorphisms (SNPs) were selected using these three approaches for technical validation by individual genotyping in the discovery dataset. 87 out of 94 successfully tested SNPs were nominally associated in the discovery dataset. Replication of the 87 technically validated SNPs was then carried out in an independent replication dataset of 100 Portuguese cases and 425 controls. The intergenic rs4733649 SNP in chromosome 8 (between LINC00824 and LINC00977) was associated with PSP in the discovery (P = 4.07E-03, ORC[95% CI] = 1.88[1.22-2.89]), replication (P = 1.50E-02, ORC[95% CI] = 1.50[1.08-2.09]) and combined datasets (P = 8.61E-05, ORC[95% CI] = 1.65[1.29-2.13]). This study identified for the first time one genetic risk factor for sporadic PSP, but future studies are warranted to further confirm this finding in other populations and uncover its functional role in PSP pathogenesis.
Serum of COVID-19 patients changes neuroinflammation and mitochondrial homeostasis markers in hippocampus of aged rats
Patients affected by COVID-19 present mostly with respiratory symptoms but acute neurological symptoms are also commonly observed. Furthermore, a considerable number of individuals develop persistent and often remitting symptoms months after infection, characterizing the condition called long-COVID. Since the pathophysiology of acute and persistent neurological manifestations is not fully established, we evaluated the expression of different genes in hippocampal slices of aged rats exposed to the serum of a post-COVID (sPC) individual and to the serum of patients infected by SARS-CoV-2 [Zeta (sZeta) and Gamma (sGamma) variants]. The expression of proteins related to inflammatory process, redox homeostasis, mitochondrial quality control and glial reactivity was determined. Our data show that the exposure to sPC, sZeta and sGamma differentially altered the mRNA levels of most inflammatory proteins and reduced those of antioxidant response markers in rat hippocampus. Furthermore, a decrease in the expression of mitochondrial biogenesis genes was induced by all serum samples, whereas a reduction in mitochondrial dynamics was only caused by sPC. Regarding the glial reactivity, S100B expression was modified by sPC and sZeta. These findings demonstrate that changes in the inflammatory response and a reduction of mitochondrial biogenesis and dynamics may contribute to the neurological damage observed in COVID-19 patients.
A Relação entre Confiança e Custos de Transação em Relacionamentos Interorganizacionais
Based on the assumption that trust is a mechanism of relational governance, which can influence the costs associated with the formation, development and maintenance of interorganizational relationships, in this article one aims to test the relation between trust and transaction costs in these relationships. Based on the literature on trust and on transaction costs, one puts forward four new hypotheses, which suggest a negative relation between trust and costs of information, of negotiation, of implementation and of monitoring in interorganizational relationships. These hypotheses were tested using multiple regression analysis in a quantitative case study where the buyer is illustrated by an automaker and its suppliers are represented by a sample of the major ones. The results suggest that there are significant, yet negative relations between trust and costs of negotiation and of monitoring. The relations between trust and costs of information and of implementation, in turn, are not statistically significant. We believe that these results represent a more-fined grained understanding of the relation between trust and transaction costs in interorganizational relationships to the degree that it is suggested that the four types of transaction costs are not perfectly correlated.

A relação entre confiança e custos de transação em relacionamentos interorganizacionais
Partindo do pressuposto de que a confiança é um mecanismo relacional de governança que pode interferir nos custos associados à formação, desenvolvimento e manutenção de relacionamentos interorganizacionais, busca-se, neste trabalho, testar a relação entre confiança e custos de transação nesses relacionamentos. A partir da literatura de confiança e de custos de transação, foram formuladas quatro novas hipóteses, que sugerem uma relação negativa entre confiança e custos de informação, de negociação, de implementação e de monitoramento em relacionamentos interorganizacionais. Utilizando análise de regressão múltipla, tais hipóteses foram testadas em um estudo de caso de natureza quantitativa, em que o comprador é representado por uma montadora de automóveis e os fornecedores por uma amostra dos seus principais fornecedores. Os resultados sugerem que a confiança está relacionada negativamente com os custos de negociação e de monitoramento. Não foram encontradas, por sua vez, relações estatísticas significativas entre confiança e custos de informação e de implementação. Acredita-se que tais resultados representam uma compreensão mais refinada da relação entre confiança e custos de transação em relacionamentos interorganizacionais à medida que sugerem que os quatro tipos de custos de transação não são perfeitamente correlacionados.
