Temperature and mental health–related emergency department and hospital encounters among children, adolescents and young adults
We examine the association between high ambient temperature and acute mental health-related healthcare encounters in New York City for children, adolescents and young adults. This case-crossover study included emergency department (ED) visits and hospital encounters with a primary diagnosis of any mental health disorder during warm-season months (June-August) in New York City from 2005 to 2011 from patients of three age groups (6-11, 12-17 and 18-25 years). Using a distributed lag non-linear model over 0-5 lag days, by fitting a conditional logistic regression for each age group, we calculated the cumulative odds ratios of mental health encounters associated with an elevated temperature. Analyses were stratified by race/ethnicity, payment source and mental health categories to elucidate vulnerable subpopulations. In New York City, there were 82,982 mental health-related encounters for young people aged 6 to 25 years during our study period months. Elevated temperature days were associated with higher risk of mental health-related ED and hospital encounters for the 6- to 11-year-olds (odds ratio [OR]: 1.28, 95% confidence interval [CI]: 1.13-1.46), for the 12- to 17-year-olds (OR: 1.17, 95% CI: 1.09-1.25) and for the 18- to 25-year-olds (OR: 1.09, 95% CI: 1.04-1.15). Children with reaction disorders, adolescents with anxiety and bipolar disorders, young adults with psychosis and reaction disorders and Black and non-Hispanic children and adolescents showed vulnerability to elevated temperature. We found that elevated ambient temperatures were associated with acute mental health ED or hospital encounters across childhood, adolescence and young adulthood.
A Time Series Analysis of Associations between Daily Temperature and Crime Events in Philadelphia, Pennsylvania
Urban crime may be an important but overlooked public health impact of rising ambient temperatures. We conducted a time series analysis of associations between temperature and crimes in Philadelphia, PA, for years 2006–2015. We obtained daily crime data from the Philadelphia Police Department, and hourly temperature and dew point data from the National Centers for Environmental Information. We calculated the mean daily heat index and daily deviations from each year’s seasonal mean heat index value. We used generalized additive models with a quasi-Poisson distribution, adjusted for day of the week, public holiday, and long-term trends and seasonality, to estimate relative rates (RR) and 95% confidence intervals. We found that the strongest associations were with violent crime and disorderly conduct. For example, relative to the median of the distribution of mean daily heat index values, the rate of violent crimes was 9% (95% CI 6–12%) higher when the mean daily heat index was at the 99th percentile of the distribution. There was a positive, linear relationship between deviations of the daily mean heat index from the seasonal mean and rates of violent crime and disorderly conduct, especially in cold months. Overall, these analyses suggest that disorderly conduct and violent crimes are highest when temperatures are comfortable, especially during cold months. This work provides important information regarding the temporal patterns of crime activity.
Associations of greenspace use and proximity with self-reported physical and mental health outcomes during the COVID-19 pandemic
Research has shown that the COVID-19 pandemic affected individual’s mental and physical health. The aim of this study was to estimate associations between greenspace use and proximity with perceived mental and physical health during the COVID-19 pandemic. We surveyed metropolitan Philadelphia residents, October 20–December 1, 2020, about walking time to the nearest greenspace from their home, frequency of greenspace use in the past 30 days, change in frequency of greenspace use during the COVID-19 pandemic, and perceived physical and mental health outcomes. We ran unadjusted and adjusted log-binomial regression models to derive Risk Ratio (RR) and 95% Confidence Intervals (CI) estimates of associations of loneliness, physical and mental health outcomes with: (1) self-reported walking time to nearest greenspace; (2) reported greenspace use frequency; and (3) changes in greenspace use frequency. Of 485 survey participants, 244 (51.4%) reported feeling lonelier, 147 (31.37%) reported higher perceived stress, 261 (54.9%) reported worsened mental health, and 137 (28.7%) reported worsened physical health during vs. before the start of pandemic-restrictions in mid-March of 2020. After adjustment for gender, age, and change in financial status, RR estimates suggested modest protective associations between visiting greenspaces more frequently during vs. before the pandemic and worsened mental (RR: 0.84, 95% CI: 0.70–1.00), and physical health (RR 0.77, 95% CI: 0.56–1.10), and loneliness (RR: 0.91, 95% CI: 0.75–1.1) and perceived stress (RR 0.83, 95% CI: 0.61–1.13). Shorter walking distances to the nearest greenspace were associated with reduced risk of reporting worsened physical health and higher perceived stress; however, living shorter walking distances from greenspace were not associated with protection against worsened mental health or loneliness. These results suggest that active greenspace use may provide mental and physical health protection, particularly during a stressful public health crisis.
Projected changes in heatwaves over Central and South America using high-resolution regional climate simulations
Heatwaves (HWs) pose a severe threat to human and ecological systems. Here we assess the projected changes in heatwaves over Latin America using bias corrected high-resolution regional climate simulations under two Representative Concentration Pathway scenarios (RCPs). Heatwaves are projected to be more frequent, long-lasting, and intense in the mid-century under both RCP2.6 and RCP8.5 scenarios, with severe increases under the RCP8.5 scenario. Even under the low emissions scenario of RCP2.6, the frequency of heatwaves doubles over most of the region. A three- to tenfold rise in population exposure to heatwave days is projected over Central and South America, with climate change playing a dominant role in driving these changes. Results show that following the low emission pathway would reduce 57% and 50% of heatwave exposure for Central and South American regions respectively, highlighting the need to control anthropogenic emissions and implement sustainable practices.
City-level impact of extreme temperatures and mortality in Latin America
Climate change and urbanization are rapidly increasing human exposure to extreme ambient temperatures, yet few studies have examined temperature and mortality in Latin America. We conducted a nonlinear, distributed-lag, longitudinal analysis of daily ambient temperatures and mortality among 326 Latin American cities between 2002 and 2015. We observed 15,431,532 deaths among ≈2.9 billion person-years of risk. The excess death fraction of total deaths was 0.67% (95% confidence interval (CI) 0.58–0.74%) for heat-related deaths and 5.09% (95% CI 4.64–5.47%) for cold-related deaths. The relative risk of death was 1.057 (95% CI 1.046–1.067%) per 1 °C higher temperature during extreme heat and 1.034 (95% CI 1.028–1.040%) per 1 °C lower temperature during extreme cold. In Latin American cities, a substantial proportion of deaths is attributable to nonoptimal ambient temperatures. Marginal increases in observed hot temperatures are associated with steep increases in mortality risk. These risks were strongest among older adults and for cardiovascular and respiratory deaths. An ecological analysis of 326 cities in 9 countries across Latin America found that changes in ambient temperature have a substantial contribution to all-cause mortality, with small increases in extreme heat associated with steep increases in mortality risk.
Invited Perspective: Important New Evidence for Glyphosate Hazard Assessment
Glyphosate is the most widely used pesticide in the world. It is also, arguably, the most controversial. Since being classified as \"probably carcinogenic to humans\" by the International Agency for Research on Cancer (IARC) in 2015, glyphosate has received close scrutiny from scientists and regulatory bodies. A new study in this issue of Environmental Health Perspectives by Chang et al. provides important new evidence to support glyphosate hazard assessment. The IARC conclusion was driven by the results of animal cancer bioassays and mechanistic studies--mostly produced in vitro or, in nonhumans, in vivo--indicating DNA and chromosomal damage and oxidative stress induced by glyphosate and glyphosate formulations. The evidence from human studies of cancer was considered \"limited\" by IARC given that it included positive associations between glyphosate and incidence of non-Hodgkin's lymphoma from several retrospective case-control studies but no such association from the Agricultural Health Study, the only prospective cohort study published at the time of the agency's review.
Associations Between Historical Redlining and Present-Day Heat Vulnerability Housing and Land Cover Characteristics in Philadelphia, PA
Historical, institutional racism within the housing market may have impacted present-day disparities in heat vulnerability. We quantified associations between historically redlined areas with present-day property and housing characteristics that may enhance heat vulnerability in Philadelphia, PA. We used color-coded Home Owners Loan Corporation (HOLC) maps and tax assessment data to randomly select 100 present-day (2018–2019) residential properties in each HOLC grade area (A = Best; B, C, and D = Most hazardous; N = 400 total). We conducted virtual inventories of the properties using aerial and streetview imagery for land cover and housing characteristics (dark roof color, flat roof shape, low or no mature tree canopy, no recently planted street trees) that may enhance heat vulnerability. We used modified Poisson regression models to estimate associations of HOLC grades with the property characteristics, unadjusted and adjusted for historical and contemporary measures of the neighborhood sociodemographic environment. Compared to grade A areas, higher proportions of properties in grade B, C, and D areas had dark roofs, low/no mature tree canopy, and no street trees. Adjusting for historical sociodemographics attenuated associations, with only associations with low or no tree canopy remaining elevated. Adjusting for present-day concentrated racial and socioeconomic deprivation did not substantially impact overall findings. In Philadelphia, PA, HOLC maps serve as spatial representations of present-day housing and land cover heat vulnerability characteristics. Further analyses incorporating longitudinal data on urban redevelopment, reinvestment, and neighborhood change are needed to more fully represent complex relationships among historical racism, residential segregation, and heat vulnerability.
Pooled study of occupational exposure to aromatic hydrocarbon solvents and risk of multiple myeloma
ObjectivesTo investigate the association between occupational exposure to aromatic hydrocarbon solvents and risk of multiple myeloma (MM) in a large, consortium-based study.MethodsWe pooled data on 2854 cases and 10 743 controls from nine studies participating in the InterLymph consortium. Occupational exposures to benzene, toluene and xylene were assigned by a job–exposure matrix, coupled with ‘correction’ of exposure probability by self-reported or expert-assessed exposure from the individual studies. Cumulative intensity was calculated as the job-specific exposure intensity multiplied by job duration, summed across jobs. Associations were estimated using logistic regression, with inclusion of covariates for study matching factors and other potential confounders. We repeated our main analysis using random-effects meta-analysis to evaluate heterogeneity of effect.ResultsBenzene, toluene and xylene were each associated with MM. For the three solvents, the highest quartile of high-probability cumulative intensity exposure (vs unexposed) was associated with 42% to 63% increased risks of MM. Associations with toluene and xylene exposures were fairly consistent and robust to sensitivity analyses. The estimated effect for benzene was moderately heterogeneous between the studies. Each solvent’s association with MM was stronger for exposure occurring within 20 years of diagnosis than with exposure lagged by more than 20 years.ConclusionsOur study adds important evidence for a role of aromatic hydrocarbon solvents in causation of MM. The difficulty in disentangling individual compounds in this group and a lack of data on potential carcinogenicity of toluene and xylene, in widespread current use, underscore a need for further epidemiological evaluation.
High Ambient Temperature and Infant Mortality in Philadelphia, Pennsylvania: A Case–Crossover Study
Objective. To quantify the association between heat and infant mortality and identify factors that influence infant vulnerability to heat. Methods. We conducted a time-stratified case–crossover analysis of associations between ambient temperature and infant mortality in Philadelphia, Pennsylvania, during the warm months of 2000 through 2015. We used conditional logistic regression models to estimate associations of infant mortality with daily temperatures on the day of death (lag 0) and for averaging periods of 0 to 1 to 0 to 3 days before the day of death. We explored modification of associations by individual and census tract–level characteristics and by amounts of green space. Results. Risk of infant mortality increased by 22.4% (95% confidence interval [CI] = 5.0%, 42.6%) for every 1°C increase in minimum daily temperature over 23.9°C on the day of death. We observed limited evidence of effect modification across strata of the covariates. Conclusions. Our results contribute to a growing body of evidence that infants are a subpopulation that is particularly vulnerable to climate change effects. Further research using large data sets is critically needed to elucidate modifiable factors that may protect infants against heat vulnerability.
Associations between high ambient temperatures and asthma exacerbation among children in Philadelphia, PA: a time series analysis
ObjectivesHigh ambient temperatures may contribute to acute asthma exacerbation, a leading cause of morbidity in children. We quantified associations between hot-season ambient temperatures and asthma exacerbation in children ages 0–18 years in Philadelphia, PA.MethodsWe created a time series of daily counts of clinical encounters for asthma exacerbation at the Children’s Hospital of Philadelphia linked with daily meteorological data, June–August of 2011–2016. We estimated associations between mean daily temperature (up to a 5-day lag) and asthma exacerbation using generalised quasi-Poisson distributed models, adjusted for seasonal and long-term trends, day of the week, mean relative humidity,and US holiday. In secondary analyses, we ran models with adjustment for aeroallergens, air pollutants and respiratory virus counts. We quantified overall associations, and estimates stratified by encounter location (outpatient, emergency department, inpatient), sociodemographics and comorbidities.ResultsThe analysis included 7637 asthma exacerbation events. High mean daily temperatures that occurred 5 days before the index date were associated with higher rates of exacerbation (rate ratio (RR) comparing 33°C–13.1°C days: 1.37, 95% CI 1.04 to 1.82). Associations were most substantial for children ages 2 to <5 years and for Hispanic and non-Hispanic black children. Adjustment for air pollutants, aeroallergens and respiratory virus counts did not substantially change RR estimates.ConclusionsThis research contributes to evidence that ambient heat is associated with higher rates of asthma exacerbation in children. Further work is needed to explore the mechanisms underlying these associations.
