Incidents related to the Hickman® catheter: identification of damages
ABSTRACT Objective: to identify damages resulting from incidents with the Hickman® catheter. Method: descriptive, retrospective, qualitative approach. The source of data were the notifications of incidents that occurred between January 2012 and May 2015, as well as the information available on the medical records of patients involved in incidents with the Hickman® catheter. Results: the incidents related to the Hickman® catheter with the greatest impact on patient care were obstruction, fracture and traction. All incidents caused damage to patients, in a greater or lesser degree, in the dimensions of physical damage and subjective damage. Final considerations: damage or potential risk of damage was present in all incidents analyzed. The need to revise cleaning and obstruction protocols for the maintenance of the permeability of Hickman® catheters was demonstrated. RESUMEN Objetivo: identificar el acaecimiento de daños en incidentes relacionados con el catéter de Hickman®. Método: investigación descriptiva, retrospectiva, de abordaje cualitativo. Como fuente de datos, se utilizaron fichas de notificación de incidentes ocurridos entre enero de 2012 y mayo de 2015, además de informaciones relatadas en los prontuarios de los pacientes involucrados en los incidentes relacionados con el catéter de Hickman®. Resultados: los incidentes de impacto más importantes en la atención al paciente, relacionados con el catéter de Hickman®, fueron la obstrucción, la fractura y la tracción. Todos los incidentes generaron, en mayor o menor grado, daños a los pacientes en las dimensiones física y subjetiva. Consideraciones finales: se encontraba presente en todos los incidentes analizados, el daño en sí mismo o el riesgo potencial para el daño. Quedó en evidencia la necesidad de revisar los protocolos de lavado y bloqueo del catéter de Hickman® para el mantenimiento de su permeabilidad. RESUMO Objetivo: identificar a ocorrência de danos nos incidentes relacionados ao cateter de Hickman®. Método: pesquisa descritiva, retrospectiva, de abordagem qualitativa. Como fonte dos dados, foram utilizadas as fichas de notificação de incidentes ocorridos entre janeiro de 2012 e maio de 2015, além de informações dos prontuários dos pacientes envolvidos com incidentes relacionados ao cateter de Hickman®. Resultados: os incidentes, relacionados ao cateter de Hickman®, de maior impacto para a assistência ao paciente foram a obstrução, fratura e tração. Todos os incidentes geraram dano aos pacientes, de maior ou menor grau, nas dimensões de dano físico e danos subjetivos. Considerações finais: dano ou risco potencial para dano esteve presente em todos os incidentes analisados. Evidenciou-se a necessidade de revisar os protocolos de lavagem e bloqueio para a manutenção da permeabilidade do cateter de Hickman®.
Nursing diagnosis after hematopoietic stem cell transplant due to Fanconi anemia
ABSTRACT Objectives: to identify nursing diagnoses in patients who underwent hematopoietic stem-cell transplants due to Fanconi anemia, according to the NANDA-I taxonomy. Methods: exploratory study using a retrospective analysis of 85 records from patients who underwent hematopoietic stem-cell transplants due to Fanconi anemia, developed in a specialize transplant center in the South of Brazil. The results were analyzed using descriptive statistics. Results: 73 different diagnoses were found in 9 out of the 13 domains from the NANDA-I taxonomy. From these, 22 were in 50% or more of the patients investigated, and most of them are related to the domain Safety/Protection. Conclusions: it was possible to identify the nursing diagnosis in the patients who underwent hematopoietic stem cell transplants due to Fanconi anemia, contributing to design a plan for the care of these patients. The same was true for those with other syndromes of chromosomal instability that need to undergo this transplant. RESUMO Objetivos: identificar diagnósticos de enfermagem em pacientes submetidos a transplante de células-tronco hematopoéticas por anemia de Fanconi, segundo a taxonomia da NANDA-I. Métodos: estudo exploratório mediante análise retrospectiva dos registros de 85 prontuários de pacientes submetidos a transplante de células-tronco hematopoéticas por anemia de Fanconi, desenvolvido em um centro transplantador de referência do Sul do Brasil. Analisaram-se os dados utilizando-se estatística descritiva. Resultados: identificaram-se 73 diferentes diagnósticos em 9 dos 13 domínios da taxonomia da NANDA-I. Destes, 22 diagnósticos atingiram 50% ou mais dos pacientes investigados, e o maior número está relacionado ao domínio de Segurança e Proteção. Conclusões: foi possível identificar os diagnósticos de enfermagem presentes em pacientes submetidos a um transplante de células-tronco hematopoéticas por anemia de Fanconi, contribuindo para o delineamento do plano de cuidados desses pacientes, assim como para aqueles com outras síndromes de instabilidade cromossômica que necessitam ser submetidos ao transplante. RESUMEN Objetivos: identificar diagnósticos de enfermería en pacientes sometidos a trasplante de células madre hematopoyéticas por anemia de Fanconi, segundo la taxonomía NANDA-I. Métodos: estudio exploratorio mediante análisis retrospectivo de registros de 85 prontuarios de pacientes sometidos a trasplante de células madre hematopoyéticas por anemia de Fanconi, desarrollado en un centro trasplantador de referencia del Sur Brasileño. Analizados los datos utilizándose estadística descriptiva. Resultados: identificaron 73 diferentes diagnósticos en 9 de los 13 dominios de la taxonomía NANDA-I. De estos, 22 diagnósticos atingieron 50% o más de los pacientes investigados, y el mayor número está relacionado al dominio de Seguridad y Protección. Conclusiones: fue posible identificar los diagnósticos de enfermería presentes en pacientes sometidos a un trasplante de células madre hematopoyéticas por anemia de Fanconi, contribuyendo para el delineamento del plan de cuidados de esos pacientes, incluso para aquellos con otros síndromes de inestabilidad cromosómica que necesitan ser sometidos al trasplante.

Acute inflammation is a predisposing factor for weight gain and insulin resistance
Aim Intense endotoxaemia and infection are able to reduce appetite and induce a catabolic state, therefore leading to weight loss. However, it is underexplored its late effects on energy homeostasis, regulation of body weight and glucose metabolism. Here we addressed whether serial intense endotoxaemia, characterized by an acute phase response and weight loss, could be an aggravating or predisposing factor to diet-induced obesity (DIO) and associated metabolic impairments. Methods Male Swiss Webster mice were submitted to 8 consecutive doses of lipopolysaccharide (LPS - 10 mg/kg), followed by 10 weeks in high-fat diet (HFD). Results After the end of the acute endotoxaemia period, mice under chow diet recovered their weight rapidly, within one-week recovery period, which remained similar to its control counterparts. However, acute endotoxaemia caused a long-lasting adipose tissue expression of the inflammatory markers TLR-4, CD14 and serum amyloid A (SAA) and, when challenged by a HFD, LPS-treated mice gained more weight, showed increased fat depots, leptin and insulin levels, and also impaired insulin sensitivity. Conclusions LPS-treated mice showed a higher susceptibility to the harmful effects of a subsequent HFD. Conditions leading to intense and recurrent endotoxaemia, such as common childhood bacterial infections, may resound for a long time and aggravate the effects of a western diet. If confirmed in humans, infections should be considered an additional factor contributing to obesity and type 2 diabetes epidemics and additionally impose more rigorous dietary recommendations for patients in post-infection recovery.
Development and validation of a standard area diagram set to assess powdery mildew severity on watermelon leaves
The development and validation of a standard area diagram set (SADs) was proposed in this study to assess the severity of powdery mildew (Podosphaera xanthii) in watermelon (Citrullus lanatus) leaves. The SADs proposed has twelve levels of severity, varying from 0.07 to 100%. The SADs were validated by 16 raters who had no previous experience in evaluating plant disease severity. Initially, the estimation of severity was performed without the use of the SADs in leaves with different levels of severity. In a second moment, the same raters estimated the disease severity using the SADs proposed. By Lin’s concordance correlation analysis, there was an improvement in precision (coefficient of correlation, r = 0.878 and r = 0.959, without and with SADs, respectively) and accuracy (bias correction factor, Cb = 0.830 and 0.982, without and with SADs, respectively) using SADs when compared to the non-use of SADs. The agreement (Lin’s concordance correlation coefficient, ρc = 0.734 and 0.952 without and with SADs, respectively) also improved using SADs. Severity estimates inter-rater were more reliable when using SADs (coefficient of determination, R2 = 0.681 without and R2 = 0.864 with SADs; Intra-class correlation coefficient, ρ = 0.759 and ρ = 0.928, without and with SADs, respectively). Therefore, SADs improved precision, accuracy and reliability of powdery mildew severity on watermelon leaves. RESUMO: Neste estudo foi proposto o desenvolvimento e validação de uma escala diagramática (ED) para avaliar a severidade do oídio (Podosphaera xanthii) em folhas de melancia (Citrullus lanatus). A ED proposta possui 12 níveis de severidade, variando de 0,07 a 100%. A ED foi validada por 16 avaliadores inexperientes em avaliação de severidade de doenças de plantas. Inicialmente, as estimativas de severidade foram realizadas sem o uso da ED em folhas com diferentes níveis de severidade. No segundo momento, os mesmos avaliadores estimaram a severidade da doença usando a ED proposta. Pela análise da correlação concordante de Lin, houve melhoria na precisão (coeficiente de correlação, r = 0,787 e r = 0,959, sem e com o uso da ED, respectivamente) e acurácia (fator de correção do desvio, Cb = 0,830 e 0,982, sem e com o uso da ED, respectivamente) usando a ED quando comparado ao não uso da ED. O coeficiente de correlação concordante de Lin, ρc = 0,734 e 0,952 sem e com o uso da ED, respectivamente) também melhorou com o uso da ED. As estimativas de severidade tiveram melhoria na reprodutibilidade quando a ED foi usada (coeficiente de determinação, R2=0,681 e R2 = 0,864 sem e com o uso da ED, respectivamente; coeficiente de correlação intra-classe, ρ = 0,759 e ρ = 0,928, sem e com o uso da ED, respectivamente). Portanto, a ED melhorou a precisão, acurácia e reprodutibilidade das estimativas de severidade do oídio em folhas de melancia.
Beneficial effects of ascorbic acid to treat lung fibrosis induced by paraquat
Paraquat (PQ) is one of the most widely employed herbicides that is used worldwide and it causes severe toxic effects in humans and animals. A PQ exposition can lead to pulmonary fibrosis (PF) and the mechanisms seem to be linked to oxidative stress, although other pathways have been suggested. Antioxidants can be useful as a therapy, although interventions with this kind of system are still controversial. Hence, this study has investigated the role of ascorbic acid (vitamin C) post-treatment on PQ-induced PF in male C57/BL6 mice. Pulmonary fibrosis was induced by a single PQ injection (10mg/kg; i.p.). The control group received a PQ vehicle. Seven days after the PQ or vehicle injections, the mice received vitamin C (150 mg/kg, ip, once a day) or the vehicle, over the following 7 days. Twenty-four hours after the last dose of vitamin C or the vehicle, the mice were euthanized and their bronchoalveolar lavage fluid (BALF) and their lungs were collected. The data obtained showed that vitamin C reduced the cellular recruitment, the secretion of IL-17 -a cytokine involved in neutrophils migration, TGF-β-a pro-fibrotic mediator and the collagen deposition. Moreover, vitamin C elevated the superoxide dismutase (SOD) and catalase levels, both antioxidant enzymes, but it did not alter the tracheal contractile response that was evoked by methacholine. Therefore, the researchers have highlighted the mechanisms of vitamin C as being non-invasive and have suggested it as a promising tool to treat lung fibrosis when it is induced by a PQ intoxication.
Conjugated linoleic acid trans-10, cis-12 increases the expression of genes from cell cycle progression and cis-9, trans-11 stimulates apoptotic genes in different mammary tumor explants of female dogs
Background The conjugated linoleic acid (CLA) isomer cis -9, trans -11 is an anticarcinogen that inhibits cell proliferation and/or induces apoptosis of tumor cells. The objective of this study was to evaluate the expression of genes responsible for cell cycle regulation and apoptosis in tumor explants of mammary anaplastic carcinoma (AC) and mammary tubulopapillary carcinoma (TC) cultured in vitro with the CLA isomers cis -9, trans -11 and trans -10, cis -12. Methods In this study we used mammary explants from two adult female dogs that revealed two types of malignant tumors: (a) anaplastic mammary carcinoma (AC) and (b) mammary tubulopapillary carcinoma (TC). The explants (n = 6 per treatment) had an average weight of 80.0 ± 2.0 mg and were cultured for 24 h in 35 mm culture plates under the following treatments: (a) Control: Culture medium + fatty acid free bovine serum albumin (BSA); (b) Culture medium +  cis -9, trans -11 CLA (75 µM) diluted with fatty acid free bovine serum albumin (BSA), and; (c) Culture medium +  trans -10, cis -12 CLA (75 µM) diluted with fatty acid free bovine serum albumin (BSA). After that, total RNA was extracted, complementary DNA was synthesized (cDNA), and quantitative analysis by real-time polymerase chain reaction (RT-qPCR) was conducted. Data were analyzed using the MIXED procedure of SAS. Results Compared with the Control, the CLA trans -10, cis -12 treatment decreased expression of the gene encoding the p53 by 20% ( P  = 0.02), Caspase-3 by 25% ( P  = 0.06) and Bax by 51% ( P  = 0.001) in AC. The CLA cis -9, trans -11 increased the gene expression of proapoptotic protein Bax in TC by 68% ( P  = 0.01), but increased the expression of the antiapoptotic Bcl2 gene in AC by 72% ( P  = 0.006). Conclusion The CLA cis -9, trans -11 stimulates apoptotic genes in mammary tubulopapillary carcinoma, but has a contrary effect on the anaplastic carcinoma, and the CLA trans -10, cis -12 stimulates cell cycle progression genes and may have an antiapoptotic effect, mainly in mammary anaplastic carcinoma.
Development of Ac2-26 Mesoporous Microparticle System as a Potential Therapeutic Agent for Inflammatory Bowel Diseases
Inflammatory bowel diseases (IBDs) disrupt the intestinal epithelium, leading to severe chronic inflammation. Current therapies cause adverse effects and are expensive, invasive, and ineffective for most patients. Annexin A1 (AnxA1) is a pivotal endogenous anti-inflammatory and tissue repair protein in IBD. Nanostructured compounds loading AnxA1 or its active N-terminal mimetic peptides improve IBD symptomatology. To further explore their potential as a therapeutic candidate, the AnxA1 N-terminal mimetic peptide Ac2-26 was incorporated into SBA-15 ordered mesoporous silica and covered with EL30D-55 to deliver it by oral treatment into the inflamed gut. The systems SBA-Ac2-26 developed measurements revealed self-assembled rod-shaped particles, likely on the external surface of SBA-15, and 88% of peptide incorporation. SBA-15 carried the peptide Ac2-26 into cultured Raw 264.7 macrophages and Caco-2 epithelial cells. Moreover, oral administration of Eudragit-SBA-15-Ac2-26 (200 μg; once a day; for 4 days) reduced colitis clinical symptoms, inflammation, and improved epithelium recovery in mice under dextran-sodium sulfate-induced colitis. The absorption of SBA-15 in gut epithelial cells is typically low; however, the permeable inflamed barrier can enable microparticles to cross, being phagocyted by macrophages. These findings suggest that Ac2-26 is successfully delivered and binds to its receptors in both epithelial and immune cells, aligning with the clinical results. Our findings demonstrate a simple and cost-effective approach to delivering Ac2-26 orally into the inflamed gut, highlighting its potential as non-invasive IBD therapy.
GPCRs overexpression and impaired fMLP-induced functions in neutrophils from chronic kidney disease patients
G-protein coupled receptors (GPCRs) expressed on neutrophils regulate their mobilization from the bone marrow into the blood, their half-live in the circulation, and their pro- and anti-inflammatory activities during inflammation. Chronic kidney disease (CKD) is associated with systemic inflammatory responses, and neutrophilia is a hallmark of CKD onset and progression. Nonetheless, the role of neutrophils in CKD is currently unclear. Blood and renal tissue were collected from non-dialysis CKD (grade 3 - 5) patients to evaluate GPCR neutrophil expressions and functions in CKD development. CKD patients presented a higher blood neutrophil-to-lymphocyte ratio (NLR), which was inversely correlated with the glomerular filtration rate (eGFR). A higher frequency of neutrophils expressing the senescent GPCR receptor (CXCR4) and activation markers (CD18 CD11b CD62L ) was detected in CKD patients. Moreover, CKD neutrophils expressed higher amounts of GPCR formyl peptide receptors (FPR) 1 and 2, known as neutrophil pro- and anti-inflammatory receptors, respectively. Cytoskeletal organization, migration, and production of reactive oxygen species (ROS) by CKD neutrophils were impaired in response to the FPR1 agonist (fMLP), despite the higher expression of FPR1. In addition, CKD neutrophils presented enhanced intracellular, but reduced membrane expression of the protein Annexin A1 (AnxA1), and an impaired ability to secrete it into the extracellular compartment. Secreted and phosphorylated AnxA1 is a recognized ligand of FPR2, pivotal in anti-inflammatory and efferocytosis effects. CKD renal tissue presented a low number of neutrophils, which were AnxA1 . Together, these data highlight that CKD neutrophils overexpress GPCRs, which may contribute to an unbalanced aging process in the circulation, migration into inflamed tissues, and efferocytosis.
Inoculation methods and agressiveness of Macrophomina phaseolina isolates in cowpea
The fungi Macrophomina phaseolina is the charcoal rot causal agent, one of the most important cowpea crop disease in semiarid regions can causes 100% yield losses. The search for resistant genotypes requires efficient phenotyping. In addition, there is the problem of great variation in aggressiveness between isolates. This study aimed to 1) test three methods of inoculation in semiarid conditions, and 2) to evaluate the aggressiveness of isolates of M. phaseolina. In the first experiment carried out in greenhouse, the inoculations methods were evaluated, using two cowpea lines, three inoculation methods and three pathogen isolates. On the second experiment, fifteen M. phaseolina isolates were inoculated in one cultivar to evaluate their aggressiveness. By assessing the length of the lesions and the severity of the disease using an index, we identified the toothpick inoculation method as the most efficient. Toothpick method allowed to discriminate the genotypes and the aggressiveness of the pathogen. RESUMO: O fungo Macrophomina phaseolina é o agente etiológico da podridão cinzenta do caule, uma das mais importantes doenças na cultura do feijão caupi em regiões semiáridas, podendo ocasionar perdas de 100%. A busca por genótipos resistentes exige uma fenotipagem eficiente. Além disso tem o problema da grande variação na agressividade entre isolados. Este trabalho teve como objetivos 1) testar três metodologias de inoculação em condições semiáridas, e 2) avaliar a agressividade de isolados de M. phaseolina. No primeiro experimento, conduzido em ambiente protegido, avaliou-se metodologias de inoculação em duas linhagens de caupi, por três métodos de inoculação e três isolados do patógeno. No segundo experimento, 15 isolados de M. phaseolina foram inoculados em uma cultivar de caupi para avaliar a agressividade do patógeno. Pela avaliação do comprimento das lesões e da severidade da doença por meio de um índice, identificamos o método de inoculação por palito de dente mais eficiente pois permitiu discriminar os genótipos estudados e a agressividade do patógeno.
