The 2012 KDIGO Guideline for CKD evaluation, classification, and management has updated the original 2002 KDOQI Guidelines, using newer data and addressing issues raised over the last decade concerning definitions and assessment. This review highlights the key aspects of the CKD guideline, and describes the rationale for specific wording and the scope of the document. A précis of key concepts in each of the five sections of the guideline is presented. The guideline document is intended for general practitioners and nephrologists, and covers CKD evaluation, classification, and management for both adults and children. Throughout the guideline, we have attempted to overtly address areas of controversy or non-consensus, international relevance, and impact on practice and public policy.

Six amendments : how and why we should change the Constitution
By the time of his retirement in June 2010, the author had become the second longest serving Justice in the history of the Supreme Court. Now he draws upon his more than three decades on the Court, during which he was involved with many of the defining decisions of the modern era, to offer a book articulating not only the need for changes, but also what those improvements should be. Pietas in Patriam: Milton’s Classical Patriotism

The subject of this essay is the relation between Milton’s classical patriotism and his English nationalism. It has two principal aims. First, it sets out to examine the degree to which the affective or emotional quality of Milton’s patriotism was shaped by the classics, especially Cicero and Virgil. For all the energy that has gone into studying Milton’s classical republicanism, there has been relatively little interest in that political movement’s central concern with patriotism: few, for instance, have shown much interest in David Norbrook’s acknowledgment that “English republicanism emerged in part as a vehicle for English nationalism.” And second, through this focus on the classical aspect of Milton’s patriotism, it argues that far from being neutralized or undercut, Milton’s nascent nationalism was actually enabled and intensified by his internationalism, an internationalism that is most graphically illustrated by his engagement with Italy and its role in recovering the classics.

Change in the estimated glomerular filtration rate over time and risk of all-cause mortality

Using a community-based cohort we studied the association between changes in the estimated glomerular filtration rate (eGFR) over time and the risk of all-cause mortality. We identified 529,312 adults who had at least three outpatient eGFR measurements over a 4-year period from a provincial laboratory repository in Alberta, Canada. Two indices of change in eGFR were evaluated: the absolute annual rate of change (in ml/min per 1.73m2 per year) and the annual percentage change (percent/year). The adjusted mortality risk associated with each category of change in eGFR was assessed, using stable eGFR (no change) as the reference. Over a median follow-up of 2.5 years there were 32,372 deaths. Compared to the reference participants, those with the greatest absolute annual decline less than or equal to 5ml/min per 1.73m2 per year had significantly increased mortality (hazard ratio of 1.52) adjusted for covariates and kidney function at baseline (last eGFR measurement). Participants with the greatest increase in eGFR of 5ml/min per 1.73m2 per year or more also had significantly increased mortality (adjusted hazard ratio of 2.20). A similar pattern was found when change in eGFR was quantified as an annual percentage change. Thus, both declining and increasing eGFR were independently associated with mortality and underscore the importance of identifying change in eGFR over time to improve mortality risk prediction.

This Review focuses on early detection of chronic kidney disease (CKD) and highlights its associations with cardiac disease and other comorbidities that contribute to poor outcomes. The authors describe the role of estimated glomerular filtration rate and albuminuria in identifying early CKD, examine the benefits of early CKD detection, and describe the limitations of current knowledge with respect to screening, early detection and treatment.
The past decade has seen an increasing focus on chronic kidney disease (CKD) and its attendant complications, which has resulted in improved understanding of their impact on health-care resources. The early detection of CKD has been facilitated by the implementation of routine reporting of estimated glomerular filtration rates (eGFRs) and by education of primary care physicians on the implications of detecting a decreased eGFR with respect to patient safety as well as to cardiovascular and renal outcomes. The goals of early CKD detection are to prevent CKD progression and associated complications, thus improving patient outcomes and reducing the impact of CKD on health-care resources. This Review examines the benefits of the early detection of CKD, and describes the limitations of current knowledge with respect to screening, early detection and treatment, as well as the unintended consequences of detection. In addition, this article highlights what is currently known about cardiovascular and renal outcomes and the effects of intervention in patients with CKD.
