The factor structure of the Strength and Difficulties Questionnaire in a national sample of Swedish adolescents: Comparing 3 and 5-factor models
The Strength and Difficulties Questionnaire (SDQ) is one of the most common screening instruments for emotional and behavioral problems in children and adolescents. Although exploratory factor analyses support the originally proposed 5-factor structure of SDQ as well as a 3-factor model, the evidence from confirmatory factor analyses is more mixed. Some of the difficulties items in SDQ are positively worded and it has been proposed that this leads to method effects, i.e. these items share variance that is due to the method used rather than to a substantive construct. Also, there seems to be minor factors in some subscales. This study tests a series of 3- and 5- factor models pertaining to the factor structure of SDQ, also considering method effects and minor factors. The sample consists of a nationally representative cohort of Swedish adolescents born in 2001 (n = 5549). Results show a relatively better fit of the 5-factor model compared with the 3-factor model although neither of these had a satisfactory fit. Model fit was improved when specifying cross-loadings of the positively worded difficulties items on the prosocial scale as well as minor factors on the hyperactivity scale. Although no model provided a completely satisfactory fit to the data, the results show that the 5-factor model performs better than the 3-factor model and has an acceptable fit. We conclude that for the purposes of epidemiological research, SDQ has acceptable factorial validity, provided that researchers consider method effects and minor factors.
Landscape trajectory of natural boreal forest loss as an impediment to green infrastructure
Loss of natural forests by forest clearcutting has been identified as a critical conservation challenge worldwide. This study addressed forest fragmentation and loss in the context of the establishment of a functional green infrastructure as a spatiotemporally connected landscape-scale network of habitats enhancing biodiversity, favorable conservation status, and ecosystem services. Through retrospective analysis of satellite images, we assessed a 50- to 60-year spatiotemporal clearcutting impact trajectory on natural and near-natural boreal forests across a sizable and representative region from the Gulf of Bothnia to the Scandinavian Mountain Range in northern Fennoscandia. This period broadly covers the whole forest clearcutting period; thus, our approach and results can be applied to comprehensive impact assessment of industrial forest management. The entire study region covers close to 46,000 km² of forest-dominated landscape in a late phase of transition from a natural or near-natural to a land-use modified state. We found a substantial loss of intact forest, in particular of large, contiguous areas, a spatial polarization of remaining forest on regional scale where the inland has been more severely affected than the mountain and coastal zones, and a pronounced impact on interior forest core areas. Salient results were a decrease in area of the largest intact forest patch from 225,853 to 68,714 ha in the mountain zone and from 257,715 to 38,668 ha in the foothills zone, a decrease from 75% to 38% intact forest in the inland zones, a decrease in largest patch core area (assessed by considering 100-m patch edge disturbance) from 6114 to 351 ha in the coastal zone, and a geographic imbalance in protected forest with an evident predominance in the mountain zone. These results demonstrate profound disturbance of configuration of the natural forest landscape and disrupted connectivity, which challenges the establishment of functional green infrastructure. Our approach supports the identification of forests for expanded protection and conservation-oriented forest landscape restoration. La pérdida de bosques naturales por causa de la tala uniforme de árboles en los mismos ha sido identificada como unreto muyimportantepara la conservación global. Esteestudioabordó la fragmentación y pérdida de bosques en el contexto del establecimiento de una infraestructura verde funcional como una red de hábitats a escala de paisaje conectados espacio-temporalmente que mejoren la biodiversidad, los estados favorables de conservación y los servicios ambientales. Por medio de un análisis retrospectivo de imágenes satelitales evaluamos una trayectoria de impacto espacio-temporal de 50 a 60 años de tala uniforme sobre bosques boreales naturales y casi naturales en una región considerable y representativa desde el Golfo de Botnia hasta la Cordillera Escandinava en el norte de Fenoescandia. Este rango cubre todo el periodo de tala uniforme en el bosque en términos generales; por lo tanto, nuestra metodología y resultados pueden aplicarse a la evaluación completa del impacto del manejo industrial de bosques. Toda la región de estudio cubría hasta 46,000 km² de paisaje dominado por bosque en una etapa tardía de la transición entre el estado natural o casi natural y el estado de uso de suelo modificado. Encontramos una pérdida sustancial de bosque intacto, particularmente para áreas grandes y contiguas, una polarización espacial del bosque restantea una escala regional en la que tierra adentro hay mayores afectaciones que en las zonas montañosas y costeras, y un impacto pronunciado sobre las áreas nucleares de los bosques interiores. Los resultados salientes fueron una disminución en el área del fragmento más grande de bosque intacto de 225, 853 a 68, 714 ha en la zona montañosa y de 257, 715 a 38, 668 ha en la zona de pie de monte, una disminución del 75% al 38% de bosque intacto en las zonas tierra adentro, una disminución en el área nuclear del fragmento más grande (valorada al considerar 100-m de perturbación al borde del fragmento) de 6, 114 a 351 ha en la zona costera, y un desbalance geográfico en los bosques protegidos con una evidente mayoría en la zona montañosa. Increased cerebrospinal fluid soluble TREM2 concentration in Alzheimer’s disease
Background The discovery that heterozygous missense mutations in the gene encoding triggering receptor expressed on myeloid cells 2 (TREM2) are risk factors for Alzheimer’s disease (AD), with only the apolipoprotein E (APOE) ε4 gene allele conferring a higher risk, has led to increased interest in immune biology in the brain. TREM2 is expressed on microglia, the resident immune cells of the brain and has been linked to phagocytotic clearance of amyloid β (Aβ) plaques. Soluble TREM2 (sTREM2) has previously been measured in cerebrospinal fluid (CSF) by ELISA but in our hands commercial kits have proved unreliable, suggesting that other methods may be required. We developed a mass spectrometry method using selected reaction monitoring for the presence of a TREM2 peptide, which can be used to quantify levels of sTREM2 in CSF. Findings We examined CSF samples from memory clinics in Sweden and the UK. For all samples the following were available: clinical diagnosis, age, sex, and measurements of the CSF AD biomarkers Aβ42, T-tau and P-tau 181 . AD patients ( n  = 37) all met biomarker (IWG2) criteria for AD. Control individuals ( n  = 22) were cognitively normal without evidence for AD in CSF. We found significantly higher sTREM2 concentration in AD compared to control CSF. There were significant correlations between CSF sTREM2 and T-tau as well as P-tau 181 . CSF sTREM2 increase in AD was replicated in a second, independent cohort consisting of 24 AD patients and 16 healthy volunteers. Conclusion CSF concentrations of sTREM2 are higher in AD than in controls, and correlate with markers of neurodegeneration. CSF sTREM2 may be used to quantify glial activation in AD.
Sex and age differences in cortisol levels during glucagon stimulation test in children
Background Previous studies of glucagon stimulation test (GST) in children have shown variable results regarding the utility and reliability of the cortisol response to this test and its correlation with clinical parameters. The aim of this study was to assess cortisol levels at GST and to evaluate how clinical parameters, such as age, sex, pubertal status and Body Mass Index (BMI), correlate to cortisol levels in children. Methods A retrospective study of children evaluated for short stature with the GST. Cortisol, glucose and growth hormone (GH) levels at GST, as well as clinical parameters (age, sex, pubertal status, BMI), were collected from medical records. A peak cortisol of ≥  450 nmol/L was used as a cut-off indicative of a sufficient response. Non-parametric tests were applied in the statistical analysis, and linear regression was used to examine factors affecting cortisol max at the GST. Results In total, 171 children were included; median age 7.8 years (1.0–18.0), 60 (35.1%) female, 23 (13.5%) pubertal. Of all children, 145 (84.8%) achieved a peak cortisol  ≥  450 nmol/L. There was a negative correlation between peak cortisol levels and age (Spearman’s rho − 0.26, p = < 0.001). Peak cortisol levels were higher in females vs. males: 667.5 nmol/L (range 400–995) vs. 602 nmol/L (range 202–1008), p  = 0.005. A higher number of boys than girls did not reach the cortisol cut-off value of 450 nmol/L ( p  = 0.022). The difference in maximum stimulated cortisol levels between the sexes remained after adjusting for age with a linear regression model (β (95% CI) 65.3 (15.9–114.6), p  = 0.01). Conclusion GST is a reliable test of the hypothalamic-pituitary-adrenal (HPA) axis in children. Girls and younger children had higher peak cortisol at GST. The results support a need for sex- and age-dependent reference values for cortisol.
Parents’ drinking, childhood hangover? Parental alcohol use, subjective health complaints and perceived stress among Swedish adolescents aged 10–18 years
Background Alcohol abuse is not only harmful to the consumer but may also negatively impact individuals in the drinker’s social environment. Alcohol’s harm to others is vital to consider when calculating the true societal cost of alcohol use. Children of parents who have alcohol use disorder tend to have an elevated risk of negative outcomes regarding, e.g., health, education, and social relationships. Research on the general youth population has established a link between parental drinking and offspring alcohol use. However, there is a lack of knowledge regarding other outcomes, such as health. The current study aimed to investigate the associations between parental drinking and children’s psychological and somatic complaints, and perceived stress. Methods Data were derived from a nationally representative sample, obtained from the 2010 Swedish Level-of-Living survey (LNU). Parents and adolescents (ages 10–18) living in the same households were interviewed independently. The final study sample included 909 adolescents from 629 households. The three outcomes, psychological and somatic complaints and perceived stress, were derived from adolescents’ self-reports. Parents’ self-reports of alcohol use, both frequency and quantity, were used to categorise adolescents as having abstaining, low-consuming, moderate-drinking, or heavy-drinking parents. Control variables included adolescents’ gender, age, family structure, and household socioeconomic status. Linear and binary logistic regression analyses were performed. Results Parental heavy drinking was more common among adolescents living in more socioeconomically advantaged households and among adolescents living with two custodial parents or in reconstituted families. Adolescents with heavy-drinking parents reported higher levels of psychological and somatic complaints and had an increased likelihood of reporting stress, compared with those having moderate-drinking parents. These associations remained statistically significant when adjusting for all control variables. Conclusion The current study’s results show that parental alcohol consumption is associated with poorer offspring adolescent health. Public health policies that aim to reduce parental drinking or provide support to these adolescents may be beneficial. Further studies investigating the health-related outcomes among young people living with heavy-drinking parents in the general population are needed to gain more knowledge about these individuals and to implement adequate public health measures.
Wind power distribution across subalpine, boreal, and temperate landscapes
Onshore wind power is increasingly expanding to meet global and national goals to increase renewable, clean, and fossil-free energy production. In many countries and regions, however, historical and current land use is extensive, and the expansion of wind power has to be well-tuned to avoid risking irreversible legacy losses of existing and traditional land uses, landscape values, and cultures. Hence, assessments of the siting premises of current and forecasted expansion of wind power are strongly needed as a basis for sustainable planning. We present a study from alpine to temperate biomes in Sweden, where an ambitious onshore wind power expansion strategy has been put in place and will result in Swedish landscapes that are typified by wind power. We explored the existing legal framework—i.e., the national interest for wind power according to the Swedish Environmental Code—concerning the spatial interaction with other national interests for nature conservation, landscape values, and other land uses, and the land cover, landowner, and formally protected areas distribution within wind power sites and in their proximity. We found that the national interest framework does not provide sufficient guidance for locating wind power to avoid spatial overlap with conflicting interests and values. Furthermore, our analysis revealed that wind power is located mainly in forest-dominated landscapes, and on lands where private forest companies are the dominant owners but where the proportion of public and non-industrial private ownership increases in the near surroundings. Finally, we found that large areas of formally protected areas are within the proximate areas influenced by wind power. As an extensive onshore wind power expansion is already going on, and an even more extensive expansion is projected, the ways forward toward a sustainable wind power expansion calls for integrated landscape planning approaches that are based on comprehensive assessments of existing interests and values.
Long-term glycemic variability and the risk of mortality in diabetic patients receiving peritoneal dialysis
The large amount of glucose in the dialysate used in peritoneal dialysis (PD) likely affects the glycemic control. The aim of this study was to investigate the association between HbA1c variability, as a measure of long-term glycemic variability, and the risk of all-cause mortality in diabetic patients with PD. 325 patients with diabetes and ESRD were followed (2008-2018) in the Swedish Renal Registry. Patients were separated in seven groups according to level of HbA1c variability. The group with the lowest variability was denoted the reference. The ratio of the standard deviation (SD) to the mean of HbA1c, HbA1c (SD)/HbA1c (mean), i.e. the coefficient of variation (CV), was defined as HbA1c variability. Hazard ratios (HR) and 95% confidence intervals (CI) were examined using Cox regression analyses. During follow-up, 170 (52%) deaths occurred. The highest mortality was among patients with the second highest HbA1c variability, CV≥2.83 [n = 44 of which 68% patients died]. In the multivariate analyses where lowest HbA1c variability (CV≤0.51) was used as the reference group, HbA1c CV 2.83-4.60 (HR 3.15, 95% CI 1.78-5.55; p<0.001) and CV> 4.6 (HR 2.48, 95% CI 1.21-5.11; p = 0.014) were associated with increased risk of death. The high risk of all-cause mortality in patients with diabetes and PD increased significantly with elevated HbA1c variability, as measure of long-term glycemic control. This indicates that stable glycemia is associated with an improvement of survival; whereas more severe glycemic fluctuations, possibly caused by radical changes in dialysis regimes or peritonitis, are associated with a higher risk of mortality in diabetic patients with PD.
Cerebrospinal fluid biomarker panel for synaptic dysfunction in Alzheimer's disease
Introduction Synaptic dysfunction and degeneration is one of the earliest events in Alzheimer's disease (AD) and the best correlate of cognitive decline. Thus, identification and validation of biomarkers reflecting synaptic degeneration to be used as prognostic biomarkers are greatly needed. Method Solid‐phase extraction and parallel reaction monitoring mass spectrometry were used to quantify 17 synaptic proteins in CSF, in two cross‐sectional studies including AD (n = 52) and controls (n = 37). Results Increased concentrations of beta‐synuclein, gamma‐synuclein, neurogranin, phosphatidylethanolamine‐binding protein 1, and 14‐3‐3 proteins were observed in AD patients compared to controls, while neuronal pentraxin‐2 and neuronal pentraxin receptor were decreased. Discussion We have established a method with a novel panel of synaptic proteins as biomarkers of synaptic dysfunction. The results indicate that several of the proteins included in the panel may serve as synaptic biomarkers for AD.
Protection gaps and restoration opportunities for primary forests in Europe
Aims Primary forests are critical for forest biodiversity and provide key ecosystem services. In Europe, these forests are particularly scarce and it is unclear whether they are sufficiently protected. Here we aim to: (a) understand whether extant primary forests are representative of the range of naturally occurring forest types, (b) identify forest types which host enough primary forest under strict protection to meet conservation targets and (c) highlight areas where restoration is needed and feasible. Location Europe. Methods We combined a unique geodatabase of primary forests with maps of forest cover, potential natural vegetation, biogeographic regions and protected areas to quantify the proportion of extant primary forest across Europe's forest types and to identify gaps in protection. Using spatial predictions of primary forest locations to account for underreporting of primary forests, we then highlighted areas where restoration could complement protection. Results We found a substantial bias in primary forest distribution across forest types. Of the 54 forest types we assessed, six had no primary forest at all, and in two‐thirds of forest types, less than 1% of forest was primary. Even if generally protected, only ten forest types had more than half of their primary forests strictly protected. Protecting all documented primary forests requires expanding the protected area networks by 1,132 km2 (19,194 km2 when including also predicted primary forests). Encouragingly, large areas of non‐primary forest existed inside protected areas for most types, thus presenting restoration opportunities. Main conclusion Europe's primary forests are in a perilous state, as also acknowledged by EU's “Biodiversity Strategy for 2030.” Yet, there are considerable opportunities for ensuring better protection and restoring primary forest structure, composition and functioning, at least partially. We advocate integrated policy reforms that explicitly account for the irreplaceable nature of primary forests and ramp up protection and restoration efforts alike.
