Ultradian rhythmicity of plasma cortisol is necessary for normal emotional and cognitive responses in man
Glucocorticoids (GCs) are secreted in an ultradian, pulsatile pattern that emerges from delays in the feedforward-feedback interaction between the anterior pituitary and adrenal glands. Dynamic oscillations of GCs are critical for normal cognitive and metabolic function in the rat and have been shown to modulate the pattern of GC-sensitive gene expression, modify synaptic activity, and maintain stress responsiveness. In man, current cortisol replacement therapy does not reproduce physiological hormone pulses and is associated with psychopathological symptoms, especially apathy and attenuated motivation in engaging with daily activities. In this work, we tested the hypothesis that the pattern of GC dynamics in the brain is of crucial importance for regulating cognitive and behavioral processes. We provide evidence that exactly the same dose of cortisol administered in different patterns alters the neural processing underlying the response to emotional stimulation, the accuracy in recognition and attentional bias toward/away from emotional faces, the quality of sleep, and the working memory performance of healthy male volunteers. These data indicate that the pattern of the GC rhythm differentially impacts human cognition and behavior under physiological, nonstressful conditions and has major implications for the improvement of cortisol replacement therapy.
Heterologous Expression and Structural Elucidation of a Highly Thermostable Alkaline Serine Protease from Haloalkaliphilic Actinobacterium, Nocardiopsis sp. Mit-7
A highly thermostable alkaline serine protease gene (SPSPro, MN429015) obtained from haloalkaliphilic actinobacteria, Nocardiopsis sp. Mit-7 (NCIM-5746), was successfully cloned and overexpressed in Escherichia coli BL21 under the control of the T7 promoter in the pET Blue1 vector leading to a 20-kDa gene product. The molecular weight of the recombinant alkaline protease, as determined by SDS-PAGE and the Mass Spectrometer (MALDI-TOF), was 34 kDa. The structural and functional attributes of the recombinant thermostable alkaline serine protease were analyzed by Bioinformatic tools. 3D Monomeric Model and Molecular Docking established the role of the amino acid residues, aspartate, serine, and tryptophan, in the active site of thealkaline protease.The activity of the recombinant alkaline protease was optimal at 65 °C, 5 °C higher than its native protease. The recombinant protease was also active over a wide range of pH 7.0–13.0, with a maximal activity of 6050.47 U/mg at pH 9. Furthermore, the thermodynamic parameters of the immobilized recombinant alkaline protease suggested its reduced vulnerability against adverse conditions under which the enzyme has to undergo varied applications.
Six Thinking Hats model of learning-Creative teaching method in physiotherapy-A pilot study
BACKGROUND: The fast growth of science and technology in the 21st century has made it necessary for advances in many sectors, including social, economic, educational, and health. Physiotherapists must have highly developed critical thinking abilities, such as creative, critical, problem-solving, and analytical thinking, to deal with these changes. Creativity is the capacity to view, shape, and organize the world in a unique way, whereas critical thinking is the ability to think outside the box, notice parts that others cannot see, be open to new ideas, and never be afraid to attempt new things. All of the continually developing and changing knowledge cannot be included in educational programs for physiotherapy students. The Six Thinking Hats were designed by Edward de Bono. The \"Six Thinking Hats\" concept is a teaching strategy for encouraging creative thinking by organizing and presenting ideas within a predetermined framework. The goal of the study was to apply the Six Thinking Hats model to physiotherapy themes in order to create a creative teaching technique and get feedback by fostering the critical thinking abilities of physiotherapy students. MATERIALS AND METHODS: During the 2021-2022 academic year, there were 42 students registered for III and IV BPT students enrolled for physiotherapy course; however, the research sample comprised 41 students. The course included 6 h of theory and 18 h of clinical sessions. Students were taught by two instructors via a PowerPoint presentation on SCI, obesity, and for further practice, they were given two case studies taught by Six Thinking Hats model. After the class, each group received a set of hats in one of six colors to use the Six Thinking Hats method effectively, and then students were asked to provide opinions on the Six Thinking Hats model of learning. Data analysis and response reporting were conducted, using literature and student comments. RESULTS: The Six Thinking Hats model was discovered to be more efficient in comprehending topics and provides all-encompassing care. The ability to understand patients' views, see both the positive and negative aspects in conditions, develop their ability to think critically, and create with unique concepts all improved for the students. About 90.2% of students stated that they learned to think from different aspects, 85.4% of the students stated that it ensured they considered the patient holistically, 87.8% of the students stated that the method allowed for sharing different ideas and thoughts, 75.6% that it helped them to produce creative ideas. Overall, 95% of students agreed that the Six Thinking Hats approach was better than conventional lectures and case studies, with 5% disagreeing. CONCLUSION: This study concluded that the Six Thinking Hats model of learning is a creative teaching method that is useful for the development of physiotherapy students' critical thinking. Skills and having this type of study in the physiotherapy literature add to the improvement and enrichment of physiotherapy education.
Stability of Alkaline Proteases from Haloalkaliphilic Actinobacteria Probed by Circular Dichroism Spectroscopy
Two halo-tolerant and alkaliphilic actinomycetes, Nocardiopsis alba OM-4 (GeneBank Number, KC119568) and Nocardiopsis alba TATA-13 (GeneBank Number, KC119569) isolated from the salt-enriched soil of the Coastal Gujarat (India) were studied for their proteases. The low molecular weight (19–20 kDa) alkaline proteases were purified by the hydrophobic interaction chromatography on Phenyl Sepharose 6 FF column. The enzymes were optimally active at 60–70°C and pH 10.0. NaCl enhanced the catalysis and enzyme stability at different temperatures and in the presence of up to 50% concentrations of various solvents. The purified enzymes were resistant against various surfactants and inhibitors, suggesting their potential applications in the detergent industry. The changes in the secondary structures were probed by the circular dichroism spectroscopy at various temperatures and solvents, followed by the K2D analysis. With increasing temperatures, the contents of the α- helices and β-sheets increased in the N. alba OM-4 protease, while a reverse trend was evident for the N. alba TATA-13 protease. On the other hand, the α-helix contents increased accompanied with decreased β-sheets in both proteases in the presence of different solvents.
Clinical and laboratory profile of patients with TB/HIV coinfection: A case series of 50 patients
Tuberculosis (TB) is said to be one of the commonest opportunistic infection in patients with HIV/AIDS. To study the clinical and laboratory profile of patients with HIV/TB coinfection. Fifty adult TB patients having confirmed HIV seropositivity were included in randomized manner. A detailed history and thorough physical examination was done. Laboratory and radiological investigations were carried out as appropriately warranted. Most of the patients were farm workers (30%) followed by manual laborers (22%) and transport drivers (16%). Heterosexual route was found in 86% of patients. Cough was present in 94% while fever and weight loss in 86% and 78% of patients, respectively. Out of 50 patients, 40% had only pulmonary TB (PTB), 46% had pulmonary and extra-pulmonary TB (EPTB), 10% had only EPTB and 4% had multisystemic EPTB. Mediastinal lymphadenopathy was present in 34% while pleural effusion and extra-thoracic lymph nodes was present in 20% and 18% of patients, respectively. Positive smear for acid-fast bacilli (AFB) was found in 25.58% while positive Mantoux test was found in 32.14% of patients. HIV/TB coinfection is more common in sexually active age group and commonest mode of HIV infection is heterosexual transfer. Sputum smear AFB and Mantoux test positivity is low in TB patients having HIV. Disseminated TB is common in HIV. Mediastinal lymphadenopathy is common site among extra-pulmonary tuberculosis.
GM-007 An economic analysis of the introduction of a ward based ‘dispensing for discharge’ pharmacy technician medicines management service
BackgroundMost hospitals throughout the UK now use a ‘dispensing for discharge’ model for issuing medication to inpatients. There is significant evidence of substantial improvements in productivity and service delivery, but little evidence of financial savings currently exists. The data presented here aim to address the lack of financial evidence for this model.PurposeTo elucidate the drug cost benefit of introducing a pharmacy technician led ‘dispensing for discharge’ service. To prove that the service was cost beneficial after accounting for all associated staffing costs.Material and methodsAdmission data derived from the hospital patient administration system were combined with pharmacy drug expenditure data. The data were then analysed to establish the drugs costs for all admissions to the two acute respiratory wards which had implemented the ‘dispensing for discharge’ service, versus two acute respiratory wards not running the ward based technician service. Data were reviewed for a period of 6 months from April 2015 to September 2015.ResultsThe results showed that the two ‘dispensing for discharge’ intervention wards had lower drug expenditure compared with the same months from the previous financial year: ward 1 had spent £9045 less and ward 2 had spent £12 743 less, for a combined total of £21 788 less. The non-intervention wards had higher drug expenditure compared with the same months from the previous financial year: ward 3 had spent £7972 more and ward 4 had spent £ 2225 more, for a combined total of £10 196 more. This resulted in a cumulative saving of £31 984 for the 6 months studied. This would equate to an estimated total year saving of £63 968. This was sufficient to offset all staffing costs associated with implementing the service and resulted in a small financial gain. This analysis does not take into account the wider financial impact, or quality improvements gained from increased patient service provision, safety and efficiency delivered by the ward based service.ConclusionThis analysis shows that the drug cost savings associated with the ward based technician service was sufficient to fully fund all associated staffing and setup costs, resulting in significant patient service improvements with no financial burden to the organisation.No conflict of interest
Pediatric Atypical Mycobacterium Infection Presenting as Wheezing and Concern for Foreign Body Aspiration
Atypical mycobacterium infection most commonly presents as asymptomatic cervical lymphadenitis in immunocompetent children. Over the last several decades, rates of Mycobacterium avium complex (MAC) infection have been increasing in both number and severity, with more cases of pulmonary infection reported in healthy children. However, guidelines on how to treat children with these infections remain unclear. The presentation of this disease is variable and often presents with an indolent course of wheezing that is misdiagnosed as foreign body aspiration. Several case reports have described successful treatment of these children with surgical excision without the need for additional treatment with antimycobacterial agents. We present the case of a healthy 20-month old male with wheezing and concern for foreign body ingestion. Rigid bronchoscopy demonstrated a left bronchus mass. The patient underwent video-assisted thoracoscopic surgery (VATS) with improvement in respiratory symptoms. Final pathology showed necrotizing granulomatous infection consistent with MAC. This report demonstrates the importance of keeping intrathoracic MAC infection in the differential when evaluating an immunocompetent child with wheezing or shortness of breath.
A Balloon-Based Payload for Exposing Microorganisms in the Stratosphere (E-MIST)
The survival and transit of microorganisms in Earth's upper atmosphere is relevant to terrestrial ecology and astrobiology, but the topic is understudied due to a scarcity of suitable flight systems. We designed, built, and flew a self-contained payload, Exposing Microorganisms in the Stratosphere (E-MIST), on a large scientific balloon launched from New Mexico on 24 August 2014. The payload carried SAFR-032, a highly-resilient spore-forming bacterial strain originally isolated from a NASA spacecraft assembly facility. Our test flight evaluated E-MIST functionality in the stratosphere, including microbiological procedures and overall instrument performance. Herein, we summarize features of the E-MIST payload, protocols, and preliminary results that indicate it is possible to conduct a tightly-controlled microbiological experiment in the stratosphere while collecting pertinent environmental data. Additional studies of this nature may permit survival models for microbes traveling through Earth's harsh upper atmosphere. Moreover, measuring the endurance of spacecraft-associated microbes at extreme altitudes may help predict their response on the surface of Mars.
Dietary patterns among a national random sample of British adults
STUDY OBJECTIVES To identify groups within the UK male and female population who report similar patterns of diet. DESIGN National representative dietary survey, using seven day weighed dietary records, of men and women aged 16–64 years living in private households in Great Britain in 1986–7. Cluster analysis was used to aggregate participants into diet groups. SETTING Great Britain. PARTICIPANTS 1087 men and 1110 women. RESULTS 93% of men and 86% of women fell into one of four distinct diet groups. Among men the most prevalent diet group was “beer and convenience food” (34% of the male population); second was “traditional British diet” (18%); third was “healthier but sweet diet” (17.5%) and fourth was “healthier diet “ (17%). Among women, the most prevalent diet group was “ traditional British diet” (32%); second, was “healthy cosmopolitan diet” (25%); third was a “convenience food diet” (21%); and fourth was “healthier but sweet diet” (15%). There were important differences in nutrient profile, sociodemographic and behavioural characteristics between diet groups. CONCLUSIONS Cluster analysis identified four diet groups among men and four among women, which differed not only in terms of reported dietary intakes, but also with respect to nutrient, social and behavioural profiles. The groups identified could provide a useful basis for development, monitoring and targeting of public health nutrition policy in the UK.
