Impact of tongue fat volume on obstructive sleep apnea in non-obese patients

Evidence suggests excess head and neck adipose tissue contributes to OSA, particularly in obese patients. Surgical treatments are often ineffective in this subset of the population. We sought to investigate the role of tongue fat in the normal and overweight populations. In this prospective cohort study, patients underwent overnight polysomnography and MRI using a Dixon sequence. Volumetric reconstruction evaluated the size and distribution of tongue fat deposits in subjects with and without sleep apnea. The study included 86 patients; mean age of 42.2 (SD 11.2) years, 16% female. Average BMI 27.5 kg/m

= 16) normal BMI, 61.6% (
n
= 53) overweight, 19.8% (
n
= 17) obese. Logistic regression lines showed positive associations for BMI and age with AHI. No significant correlation was found between tongue fat volume or fraction and increased AHI nor presence of OSA. Although tongue volume and fat fraction were higher in patients with AHI ≥ 5 events per hour, the difference was not statistically significant. This study suggests that tongue fat does not play a significant role in the pathophysiology of OSA in the non-obese (BMI < 30) patient population. Therefore, selective treatments targeting tongue fat should focus on obese and morbidly obese patients.

Episodes of racial conflict in Detroit form just one facet of the city's storied and legendary history, and they have sometimes overshadowed the less widely known but equally important occurrence of interracial cooperation in seeking solutions to the city's problems. The conflicts also present many opportunities to analyze, learn from, and interrogate the past in order to help lay the groundwork for a stronger, more equitable future. This astute and prudent history poses a number of critical questions: Why and where have race riots occurred in Detroit? How has the racial climate changed or remained the same since the riots? What efforts have occurred since the riots to reduce racial inequality and conflicts, and to build bridges across racial divides? Unique among books on the subject,Detroitpays special attention to post-1967 social and political developments in the city, and expands upon the much-explored black-white dynamic to address the influx of more recent populations to Detroit: Middle Eastern Americans, Hispanic Americans, and Asian Americans. This book examines an important economic development in East Asia during the first decade of the 21st century. Whereas regional arrangements were, with the sole significant exception of ASEAN, conspicuously absent before 2000, they have proliferated since 2000 in both the monetary and trade areas. The book places this political development in the changing nature of the national economies, especially their increasing integration into regional and global value chains with the fragmentation of production processes.
Surgical Excision of Pilomatrixoma of the Head and Neck: A Retrospective Review of 26 Cases

The objective of this article is to describe our experiences in treating patients for head and neck pilomatrixoma at our institution during a 5-year period and to compare our findings with previously published results. To that end, we conducted a 5-year retrospective chart review to identify those patients who had a confirmed histopathologic diagnosis of pilomatrixoma involving the head and neck area. We reviewed medical records for presenting signs and symptoms, lesion characteristics, treatment rendered, and outcomes.
We identified 26 patients, aged 6 to 77 (mean: 33), who met the criteria for inclusion in our study. All had been treated for solitary tumors with simple surgical excision and closure. We found no reported adverse outcomes and no tumor recurrences at the surgical sites. These findings support the use of simple surgical excision as the treatment of choice for these tumors.

nfertility, impotence, and emasculation - psychosocia :ontexts for abandoning reproduction

From a Darwinian perspective we live to reproduce, but in various situations genetic males elect not to reproduce by choosing medical treatments leading to infertility, impotence, and, in the extreme, emasculation. For many men, infertility can be psychologically distressing. However, for certain genetic males, being infertile may improve their quality of life. Examples include (1) men who seek vasectomy, (2) individuals with Gender Dysphoria (e.g., transwomen, and modern day voluntary eunuchs), (3) most gay men, and (4) men treated for testicular and prostate cancer. Men who desire vasectomy typically have a Darwinian fitness W 〉1 at the time of their vasectomies; i.e., after they have their desired number of offspring or consider themselves past an age for parenting newborns. In contrast, prostate and testicular cancer patients, along with individuals with extreme Gender Dysphoria, do not necessarily seek to be sterile, but accept it as an unavoidable consequence of the treatment for their condition undertaken for survival (in case of cancer patients) or to achieve a better quality of life (for those with Gender Dysphoria). Most gay men do not father children, but they may play an avuncular role, providing for their siblings' offspring's welfare, thus improving their inclusive fitness through kin selection. In a strictly Darwinian model, the primary motivation for all individuals is to reproduce, but there are many situations for men to remove themselves from the breeding populations because they have achieved a fitness W 〉1, or have stronger medical or psychological needs that preclude remaining fertile.
