Management of Common Infections in German Primary Care: A Cross-Sectional Survey of Knowledge and Confidence among General Practitioners and Outpatient Pediatricians
Outpatient antibiotic use is closely related to antimicrobial resistance and in Germany, almost 70% of antibiotic prescriptions in human health are issued by primary care physicians (PCPs). The aim of this study was to explore PCPs, namely General Practitioners’ (GPs) and outpatient pediatricians’ (PDs) knowledge of guideline recommendations on rational antimicrobial treatment, the determinants of confidence in treatment decisions and the perceived need for training in this topic in a large sample of PCPs from southern Germany. Out of 3753 reachable PCPs, 1311 completed the survey (overall response rate = 34.9%). Knowledge of guideline recommendations and perceived confidence in making treatment decisions were high in both GPs and PDs. The two highest rated influencing factors on prescribing decisions were reported to be guideline recommendations and own clinical experiences, hence patients’ demands and expectations were judged as not influencing treatment decisions. The majority of physicians declared to have attended at least one specific training course on antibiotic use, yet almost all the participating PCPs declared to need more training on this topic. More studies are needed to explore how consultation-related and context-specific factors could influence antibiotic prescriptions in general and pediatric primary care in Germany beyond knowledge. Moreover, efforts should be undertaken to explore the training needs of PCPs in Germany, as this would serve the development of evidence-based educational interventions targeted to the improvement of antibiotic prescribing decisions rather than being focused solely on knowledge of guidelines.
Can smoking cessation be taught online? A prospective study comparing e-learning and role-playing in medical education
We compared the effect of different didactic formats - e - learning and role-playing - on medical students' knowledge and counselling skills in smoking cessation training. At a German medical school, 145 third-year students were randomly allocated to attend an online course with video examples or an attendance course with role-playing. Students were trained in smoking cessation counselling according to the 5A's (ask, advise, assess, assist, arrange) for approximately 90 minutes. Practical skills were measured in an objective structured clinical examination (OSCE) and represent the primary endpoint of this prospective comparative study. Additionally, changes in theoretic knowledge were assessed by pre - and post - interventional questionnaires and a final written exam. In the OSCE, overall scores were higher in the attendance group (Mdn=70.8 % vs. 62.8 %; U=119; p=.087, n=36), but a statistical advantage was only found in one single counselling sequence (\"Assist\": Mdn=66.7 % vs. 51.4 %; p = .049) and the rating of the standardised patients (M=4.7 vs. 4.2 out of 5 points, t =2.0, p=.028). Students' results (n=130) from self-assessment and written exams suggest that both approaches are equally well suited to increase theoretical knowledge. The online course was more time efficient (90 vs. 73 minutes). Seminar and web-based training seem equally well suited for transferring knowledge and skills on tobacco cessation counselling. Considering their particular strengths, these two teaching approaches could be combined.
Upper-ocean temperature characteristics in the subantarctic southeastern Pacific based on biomarker reconstructions
As remnants of living organisms, alkenones and isoprenoid glycerol dialkyl glycerol tetraether lipids (isoGDGTs) are widely used biomarkers for determining ocean water temperatures from the past. The organisms that these proxy carriers stem from are influenced by a number of environmental parameters, such as water depth, nutrient availability, light conditions, or seasonality, which all may significantly bias the calibration to ambient water temperatures. Reliable temperature determinations thus remain challenging, especially in higher latitudes and for undersampled regions. We analyzed 33 sediment surface samples from the southern Chilean continental margin and the Drake Passage for alkenones and isoGDGTs and compared the results with gridded instrumental reference data from the World Ocean Atlas 2005 (WOA05) and previously published data from an extended study area covering the central and western South Pacific towards the Aotearoa / New Zealand continental margin. We show that for alkenone-derived sea surface temperatures (SSTs), the widely used global core-top calibration of Müller et al. (1998) yields the smallest deviation of the WOA05-based SSTs. On the contrary, the calibration of Sikes et al. (1997), determined for higher latitudes and summer SSTs, overestimates modern WOA05-based SSTs in both the annual mean and summer. Our alkenone SSTs show a slight seasonal shift of ∼ 1 ∘C at the southern Chilean margin and up to ∼ 2 ∘C in the Drake Passage towards austral summer SSTs. Samples in the central South Pacific, on the other hand, reflect an annual mean signal. We show that for isoGDGT-based temperatures, the subsurface calibration of Kim et al. (2012a) best reflects temperatures from the WOA05 in areas north of the Subantarctic Front (SAF). Temperatures south of the SAF are, in contrast, significantly overestimated by up to 14 ∘C, irrespective of the applied calibration. In addition, we used the GDGT [2]/[3] ratios, which give an indication of the production depth of the isoGDGTs and/or potential influences from land. Our samples reflect a subsurface (0–200 m water depth) rather than a surface (0–50 m water depth) signal in the entire study area and show a correlation with the monthly dust distribution in the South Pacific, indicating terrigenous influences. The overestimation of isoGDGT surface and subsurface temperatures south of the SAF highlights the need for a reassessment of existing calibrations in the polar Southern Ocean. Therefore, we suggest a modified Southern Ocean tetraether index (TEX86)-based calibration for surface and subsurface temperatures, which shows a lower temperature sensitivity and yields principally lower absolute temperatures, which align more closely with WOA05-derived values and also OH–isoGDGT-derived temperatures.
Crowdsourcing Relative Rankings of Multi-Word Expressions: Experts versus Non-Experts
In this study we investigate to which degree experts and non-experts agree on questions of difficulty in a crowdsourcing experiment. We ask non-experts (second language learners of Swedish) and two groups of experts (teachers of Swedish as a second/foreign language and CEFR experts) to rank multi-word expressions in a crowdsourcing experiment. We find that the resulting rankings by all the three tested groups correlate to a very high degree, which suggests that judgments produced in a comparative setting are not influenced by professional insights into Swedish as a second language.
Grandma Karl is 27 years old -- research agenda for pseudonymization of research data
Accessibility of research data is critical for advances in many research fields, but textual data often cannot be shared due to the personal and sensitive information which it contains, e.g names or political opinions. General Data Protection Regulation (GDPR) suggests pseudonymization as a solution to secure open access to research data, but we need to learn more about pseudonymization as an approach before adopting it for manipulation of research data. This paper outlines a research agenda within pseudonymization, namely need of studies into the effects of pseudonymization on unstructured data in relation to e.g. readability and language assessment, as well as the effectiveness of pseudonymization as a way of protecting writer identity, while also exploring different ways of developing context-sensitive algorithms for detection, labelling and replacement of personal information in unstructured data. The recently granted project on pseudonymization Grandma Karl is 27 years old addresses exactly those challenges.
