Industrial Internet of Things in the production environment of a Shipyard 4.0
Nowadays, we are already involved in the Fourth Industrial Revolution, known as Industry 4.0. Inside this paradigm, IoT is one of fundamental technologies that allows the integration of physical and virtual systems. The important role of the IoT is based on its potential to offer different solutions for the operational of existing industrial system, in order to transform them in digital environments. Following this idea, a proof of concept is presented, applying IIoT (Industrial Internet of Things) in a shipyard. This technology is used as a key enabler to achieved a connected and advanced manufacturing with the aim of improve efficiency in a specific workshop inside the shipyard. This first approach to the transformation of conventional shipyard in a smart factory comprises two parts: the communication between the information systems and the connection with physical devices. The goal is to solve the challenges that arise with the implementation of IIoT and to provide a useful decision tool to operators and managers of the workshop.
Development of new titanium implants with longitudinal gradient porosity by space-holder technique
Bone replacement with conventional biomaterials entails a biomechanical incompatibility with respect to highly specialized and anisotropic bone tissue; stiffness mismatch is the most important example of that event, and it is always present in the components of all prosthetic systems for dental and joint replacements. In the case of titanium implants used for those biomedical applications, the main consequence of that mismatch is the bone resorption around the implants due to stress shielding with respect to bone. Bio-inspired design frameworks have opened a broad field of possibilities for new approaches to the stress-shielding phenomenon in bone replacements systems. To that end, conventional and non-conventional powder metallurgy have emerged as the feasible processing techniques for producing porous samples, which can match both complexity and anisotropy of bone tissue. Complete dental restoration is a good example of biomechanical systems with an important change of longitudinal stiffness once the components are implanted; therefore, development of new prosthetics systems with graded porosity for continuous Young’s modulus change is required. Samples with longitudinal graded porosity (symmetric and non-symmetric) by space-holder technique were developed, fabricated, and characterized in this work. Main findings indicated that the experimental procedure for space-holder elimination was effective, feasible, and reproducible, with better results for a compaction pressure of 800 MPa with low global NaCl content. Three-layer design (30/40/50) allowed stress shielding to be reduced without any important effect on mechanical strength with respect to the cortical bone.
Effect of lorcaserin on prevention and remission of type 2 diabetes in overweight and obese patients (CAMELLIA-TIMI 61): a randomised, placebo-controlled trial
There is a direct relationship between bodyweight and risk of diabetes. Lorcaserin, a selective serotonin 2C receptor agonist that suppresses appetite, has been shown to facilitate sustained weight loss in obese or overweight patients. We aimed to evaluate the long-term effects of lorcaserin on diabetes prevention and remission. In this randomised, double-blind, placebo-controlled trial done in eight countries, we recruited overweight or obese patients (body-mass index ≥27 kg/m2) with or at high risk for atherosclerotic vascular disease. Eligible patients were aged 40 years or older; patients at high risk for atherosclerotic vascular disease had to be aged 50 years or older with diabetes and at least one other risk factor. Patients were randomly assigned to receive either lorcaserin (10 mg twice daily) or matching placebo. Additionally, all patients had access to a standardised weight management programme based on lifestyle modification. The prespecified primary metabolic efficacy endpoint of time to incident diabetes was assessed in patients with prediabetes at baseline. The prespecified secondary outcomes for efficacy were incident diabetes in all patients without diabetes, achievement of normoglycaemia in patients with prediabetes, and change in glycated haemoglobin (HbA1c) in patients with diabetes. Hypoglycaemia was a prespecified safety outcome. Analysis was by intention to treat, using Cox proportional hazard models for time-to-event analyses. This trial is registered with ClinicalTrials.gov, number NCT02019264. Between Feb 7, 2014, and Nov 20, 2015, 12 000 patients were randomly assigned to lorcaserin or placebo (6000 patients in each group) and followed up for a median of 3·3 years (IQR 3·0–3·5). At baseline, 6816 patients (56·8%) had diabetes, 3991 (33·3%) prediabetes, and 1193 (9·9%) normoglycaemia. At 1 year, patients treated with lorcaserin had a net weight loss beyond placebo of 2·6 kg (95% CI 2·3–2·9) for those with diabetes, 2·8 kg (2·5–3·2) for those with prediabetes, and 3·3 kg (2·6–4·0) for those with normoglycaemia (p<0·0001 for all analyses). Lorcaserin reduced the risk of incident diabetes by 19% in patients with prediabetes (172 [8·5%] of 2015 vs 204 [10·3%] of 1976; hazard ratio 0·81, 95% CI 0·66–0·99; p=0·038) and by 23% in patients without diabetes (174 [6·7%] of 2615 vs 215 [8·4%] of 2569; 0·77, 0·63–0·94; p=0·012). Lorcaserin resulted in a non-significant increase in the rate of achievement of normoglycaemia in patients with prediabetes (185 [9·2%] vs 151 [7·6%]; 1·20, 0·97–1·49; p=0·093). In patients with diabetes, lorcaserin resulted in a reduction of 0·33% (95% CI 0·29–0·38; p<0·0001) in HbA1c compared with placebo at 1 year from a mean baseline of 53 mmol/mol (7·0%). In patients with diabetes at baseline, severe hypoglycaemia with serious complications was rare, but more common with lorcaserin (12 [0·4%] vs four [0·1%] events; p=0·054). Lorcaserin decreases risk for incident diabetes, induces remission of hyperglycaemia, and reduces the risk of microvascular complications in obese and overweight patients, supporting the role of lorcaserin as an adjunct to lifestyle modification for chronic management of weight and metabolic health. Peer support telephone dyads for elderly women: Was this the wrong intervention?
Tested a preventive intervention in which peer telephone dyads were developed for low-income, community-living, elderly women with low perceived social support. After an initial assessment, respondents were randomly assigned to either an assessment-only control or received 10 weeks of friendly staff telephone contact. After a second assessment, participants receiving the staff contact were randomly assigned to continue that contact or were paired in dyads to continue phone contact with one another. Dependent variables were measures of perceived social support, morale, depression, and loneliness. All groups, particularly the staff contact group, showed some improvement in mental health scores over time, but there were no significant differences between intervention groups, or between intervention and assessment-only control groups. The results suggest that participation in the study and in personal assessment interviews at home were probably morale enhancing, and that additional telephone contact did not significantly add to that effect. Evidence also indicates that, in this sample, low perceived family support was significantly related to poor mental health, so it is possible that a program designed to increase friend support may have been the wrong intervention.
Vitamin C deficiency in weanling guinea pigs: differential expression of oxidative stress and DNA repair in liver and brain
Neonates are particularly susceptible to malnutrition due to their limited reserves of micronutrients and their rapid growth. In the present study, we examined the effect of vitamin C deficiency on markers of oxidative stress in plasma, liver and brain of weanling guinea pigs. Vitamin C deficiency caused rapid and significant depletion of ascorbate (P < 0·001), tocopherols (P < 0·001) and glutathione (P < 0·001), and a decrease in superoxide dismutase activity (P = 0·005) in the liver, while protein oxidation was significantly increased (P = 0·011). No changes in lipid oxidation or oxidatively damaged DNA were observed in this tissue. In the brain, the pattern was markedly different. Of the measured antioxidants, only ascorbate was significantly depleted (P < 0·001), but in contrast to the liver, ascorbate oxidation (P = 0·034), lipid oxidation (P < 0·001), DNA oxidation (P = 0·13) and DNA incision repair (P = 0·014) were all increased, while protein oxidation decreased (P = 0·003). The results show that the selective preservation of brain ascorbate and induction of DNA repair in vitamin C-deficient weanling guinea pigs is not sufficient to prevent oxidative damage. Vitamin C deficiency may therefore be particularly adverse during the neonatal period.
Design, Processing and Characterization of Materials with Controlled Radial Porosity for Biomedical and Nuclear Applications
The manufacture of graded materials has gained an enormous interest during the last decade due to the diversity of industrial and biological materials systems that require or are actually designed to implement that criterion; those natural or artificial materials offer multiple possibilities of applications. In this work, a novel uniaxial and sequential compaction device has been successfully designed and fabricated, in order to obtain samples with three different layers; this new device is suitable for both conventional and non-conventional powder metallurgy (PM) techniques. In addition, this device allowed us to use different combinations of powders and space-holder particles, irrespective of their nature, sizes, morphologies and proportions. It has no restriction about applying different compaction pressures for every layer, which may result in increasing or decreasing porosity. This compaction device is especially powerful if the aim is obtaining samples with radial graded porosity for biomedical applications (replacement of cortical bone involved in different joints and dental restorations) and nuclear applications (mimicking burnt used nuclear fuel). Specifically in this work, different samples with radial graded porosity were fabricated and then microstructurally and mechanically characterized: i) Commercially pure titanium (CP Ti) samples, starting from blends CP Ti with 20 vol.%, 40 vol.% and 60 vol.% of Sodium Chloride (NaCl) as space holder, which were placed in core, intermediate and external layers, respectively; processing conditions were 800 MPa of compaction pressure and 1250 °C for 2h in high vacuum of sintering; and ii) CeO2 samples, starting from blends CeO2 with 0.5 vol.%, 3.0 vol.% and 7.5 vol.% of Ethylene Bis Stearamide (EBS) as space holder, which were placed in core, intermediate and external layers, respectively; processing conditions were 460 MPa in external layer and 700 MPa in core and intermediate layers of compaction pressure, and 1700 °C during 4h in static air of sintering. This new device has proved to have unique advantages for solving problems of structural integrity in conventional PM manufacturing in a simple, economic and reproducible way.
Support interventions for older adults: Confidante relationships, perceived family support, and meaningful role activity
The rationale and problems encountered in implementing a peer-support telephone intervention are described. The research conducted by Heller, Thompson, Trueba, Hogg, and Vlachos-Weber (1991) was based on epidemiological literature documenting the moral enhancing value of confidante relationships. However, that literature may be insufficiently precise to form the basis for an intervention without prior ethnographic study of the local target population. Furthermore, the positive effects of peer friendships may refer to long established relationships rather than to newly formed social ties. It is concluded that future support intervention research with older adults should concentrate on reinforcing meaningful role activities, and that greater attention should be given to strengthening indigenous ties before attempting to create new ones. While prior process research and formative evaluation are of value, we also emphasize the importance of well-controlled field studies in the ultimate test of intervention hypotheses.

Benefits of different postoperative treatments in patients undergoing knee arthroscopic debridement
To assess the effectiveness of viscosupplementation or platelet-rich plasma (PRP), compared to standard care, for pain relief after knee arthroscopic debridement in patients with meniscal pathology and osteoarthritis (OA), under normal clinical practice conditions. We conducted a prospective, randomized, evaluator-blind, pilot study. After arthroscopy, patients were randomized to receive 1) five injections of HA1 (Suprahyal /Adant ); 2) four injections of HA2 (Orthovisc ); 3) three injections of HA3 (Synvisc ); 4) a single injection of PRP (GPS™ II); or 5) standard care (control). Patients were followed up for 18 months. Clinical outcomes were evaluated using the Western Ontario and McMaster Universities Arthritis Index (WOMAC) at 3, 6, 12, and 18 months. Minimally Clinical Important Improvement (MCII), as relative improvement ≥20 for pain and function, was also calculated. Fifty patients were included. At early follow-up (3 months), total WOMAC scores improved in all groups compared to baseline with reductions of 44.79% (HA1), 24.02% (HA2), 40.38% (HA3), 39.77% (PRP), and 27.64% (control) ( =0.002 HA1 compared to HA2). At 18 months, the higher improvement in total WOMAC was in HA1 with a 65.20% reduction, followed by PRP (55.01%), HA3 (49.57%), and HA2 (29.82%), whereas the control group had a 14.55% increase over baseline ( =0.001 control compared to HA1 and HA3). The percentage of patients achieving the MCII for both pain and function at 18 months was 100% (HA1), 80% (HA3), 60% (HA2), and 60% (PRP), whereas, in the control group, all patients returned to pre-arthroscopy levels. There were no adverse events attributable to surgery or to intraarticular administration. Viscosupplementation following arthroscopy is more effective than PRP in adequately selected patients with meniscal lesions occurring concomitantly with OA. Further controlled studies with a larger sample size and/or alternative regimens would be of interest for the scientific community.
