Risk stratification in patients with structurally normal hearts: Does fibrosis type matter?
The study sought to assess the prognostic significance of nonischemic myocardial fibrosis (MF) on cardiovascular magnetic resonance (CMR)-both macroscopic MF assessed by late gadolinium enhancement (LGE) and diffuse microscopic MF quantified by extracellular volume fraction (ECV)-in patients with structurally normal hearts. The clinical relevance of tissue abnormalities identified by CMR in patients with structurally normal hearts remains unclear. Consecutive patients undergoing CMR were screened for inclusion to identify those with LGE imaging and structurally normal hearts. ECV was calculated in patients with available T1 mapping. The associations between myocardial fibrosis and the outcomes of all-cause mortality, new-onset heart failure [HF], and an arrhythmic outcome were evaluated. In total 525 patients (mean age 43.1±14.2 years; 30.5% males) were included. Over a median follow-up of 5.8 years, 13 (2.5%) patients died and 18 (3.4%) developed new-onset HF. Nonischemic midwall /subepicardial LGE was present in 278 (52.9%) patients; isolated RV insertion fibrosis was present in 80 (15.2%) patients. In 276 patients with available T1 mapping, the mean ECV was 25.5 ± 4.4%. There was no significant association between LGE and all-cause mortality (HR: 1.36, CI: 0.42-4.42, p = 0.61), or new-onset HF (HR: 0.64, CI: 0.25-1.61, p = 0.34). ECV (per 1% increase) correlated with all-cause mortality (HR: 1.19, CI: 1.04-1.36, p = 0.009), but not with new-onset HF (HR: 0.97, CI: 0.86-1.10, p = 0.66). There was no significant association between arrhythmic outcomes and LGE (p = 0.60) or ECV (p = 0.49). In a multivariable model after adjusting for covariates, ECV remained significantly associated with all-cause mortality (HR per 1% increase in ECV: 1.26, CI: 1.06-1.50, p = 0.009). Nonischemic LGE in patients with structurally normal hearts is common and does not appear to be associated with adverse outcomes, whereas elevated ECV is associated with all-cause mortality and may be an important risk stratification tool.
Impairment in quantitative microvascular function in non-ischemic cardiomyopathy as demonstrated using cardiovascular magnetic resonance
Microvascular dysfunction (MVD) is present in various cardiovascular diseases and portends worse outcomes. We assessed the prevalence of MVD in patients with non-ischemic cardiomyopathy (NICM) as compared to subjects with preserved ejection fraction (EF) using stress cardiovascular magnetic resonance (CMR). We retrospectively studied consecutive patients with NICM and 58 subjects with preserved left ventricular (LV) EF who underwent stress CMR between 2011-2016. MVD was defined visually as presence of a subendocardial perfusion defect and semiquantitatively by myocardial perfusion reserve index (MPRI<1.51). MPRI was compared between groups using univariate analysis and multivariable linear regression. In total, 41 patients with NICM (mean age 51 ± 14, 59% male) and 58 subjects with preserved LVEF (mean age 51 ± 13, 31% male) were identified. In the NICM group, MVD was present in 23 (56%) and 11 (27%) by semiquantitative and visual evaluation respectively. Compared to those with preserved LVEF, NICM patients had lower rest slope (3.9 vs 4.9, p = 0.05) and stress perfusion slope (8.8 vs 11.7, p<0.001), and MPRI (1.41 vs 1.74, p = 0.02). MPRI remained associated with NICM after controlling for age, gender, hypertension, ethnicity, diabetes, and late gadolinium enhancement (log MPR, [beta] coefficient = -0.19, p = 0.007). MVD-as assessed using CMR-is highly prevalent in NICM as compared to subjects with preserved LVEF even after controlling for covariates. Semiquantitative is able to detect a greater number of incidences of MVD compared to visual methods alone. Further studies are needed to determine whether treatment of MVD is beneficial in NICM.
Exploring sex differences in myocardial fibrosis in patients with structurally normal hearts
Myocardial fibrosis as evaluated by cardiovascular magnetic resonance (CMR) plays a central role in pathophysiology of cardiovascular diseases and exhibits sex-specific differences. The goal of our study was to assess sex-related differences in fibrosis in patients with structurally normal hearts. We retrospectively studied patients undergoing clinical CMR at 1.5T with preserved cardiac function and no structural abnormalities. Standardized CMR protocols included cine imaging, T1 mapping, and late gadolinium enhancement (LGE) imaging. LGE presence, extent, and pattern, and extracellular volume fraction (ECV) were evaluated according to sex. The primary outcomes were all cause mortality and new-onset heart failure. Of 525 patients studied (43.1 ± 14.2 years, 69.5% female), 258 (49.1%) exhibited nonischemic LGE. Nonischemic LGE was more common in males (61.3% vs. 43.8%, p  < 0.001), and remained so in multivariable analysis after adjustment for clinical covariates (OR 2.2, CI 1.17–4.13, p  = 0.015). Women had higher ECV values (26.5 ± 4.3% vs. 23.2 ± 3.7, p  < 0.001), and these sex differences remained significant in multivariable analysis (β coefficient = 1.29, p  = 0.02). During 5.8 years follow-up there was no association between primary outcomes and nonischemic LGE. There was a trend towards association between ECV increase and all-cause mortality. The optimal ECV cutoff for all-cause mortality was 31.3% for females and 27.5% for males. Among patients with structurally normal hearts with preserved systolic function referred for clinical CMR exams, nonischemic LGE is more frequently observed in males, whereas ECV values are higher in females. ECV increase may be associated with all-cause mortality, both in female and male cohorts.
Occult RV systolic dysfunction detected by CMR derived RV circumferential strain in patients with pectus excavatum
To investigate the right ventricular (RV) strain in pectus excavatum (PE) patients using cardiac magnetic resonance tissue tracking (CMR TT). Fifty consecutive pectus excavatum patients, 10 to 32 years of age (mean age 15 ± 4 years), underwent routine cardiac magnetic resonance imaging (CMR) including standard measures of chest geometry and cardiac size and function. The control group consisted of 20 healthy patients with a mean age of 17 ± 5 years. RV longitudinal and circumferential strain magnitude was assessed by a dedicated RV tissue tracking software. Fifty patients with images of sufficient quality were included in the analysis. The mean right and left ventricular ejection fractions were 55 ± 5% and 59 ± 4%. The RV global longitudinal strain was -21.88 ± 4.63%. The RV circumferential strain at base, mid-cavity and apex were -13.66 ± 3.09%, -11.31 ± 2.79%, -20.73 ± 3.45%, respectively. There was no statistically significant decrease in right ventricular or left ventricular ejection fraction between patients and controls (p > 0.05 for each). There was no significant difference in RV global longitudinal strain between two groups (-21.88 ± 4.63 versus -21.99 ± 3.58; p = 0.93). However, there was significant decrease in mid-cavity circumferential strain magnitude in pectus patients compared with controls (-11.31 ± 2.79 versus -16.19 ± 2.86; p < 0.001). PE patients had a significantly higher basal circumferential strain (-13.66 ± 3.09% versus -9.76 ± 1.79; p < 0.001) as well as apical circumferential strain (-20.73 ± 3.45% versus -12.07 ± 3.38) than control group. Mid-cavity circumferential strain but not longitudinal strain is reduced in pectus excavatum patients. Basal circumferential strain as well as apical circumferential strain were increased as compensatory mechanism for reduced mid-cavity circumferential strain. Further studies are needed to establish clinical significance of this finding.
Atrial function and geometry differences in transthyretin versus immunoglobulin light chain amyloidosis: a cardiac magnetic resonance study
To determine the differences in left atrial (LA) function and geometry assessed by cardiac magnetic resonance (CMR) between transthyretin (ATTR) and immunoglobulin light chain (AL) cardiac amyloidosis (CA). We performed a retrospective analysis of 54 consecutive patients (68.5% male, mean age 67 ± 11 years) with confirmed CA (24 ATTR, 30 AL) who underwent comprehensive CMR examinations. LA structural and functional assessment including LA volume, LA sphericity index, and LA strain parameters were compared between both subtypes. In addition, 15 age-matched controls were compared to all groups. Patients with ATTR-CA were older (73 ± 9 vs. 62 ± 10 years, p < 0.001) and more likely to be male (83.3% vs. 56.7%, p = 0.036) when compared to AL-CA. No significant difference existed in LA maximum volume and LA sphericity index between ATTR-CA and AL-CA. LA minimum volumes were larger in ATTR-CA when compared with AL-CA. There was a significant difference in LA function with worse strain values in ATTR vs AL: left atrial reservoir [7.4 (6.3–12.8) in ATTR vs. 13.8 (6.90–24.8) in AL, p = 0.017] and booster strains [3.6 (2.6–5.5) in ATTR vs. 5.2 (3.6–12.1) in AL, p = 0.039]. After adjusting for age, LA reservoir remained significantly lower in ATTR-CA compared to AL-CA (p = 0.03), but not LA booster (p = 0.16). We demonstrate novel differences in LA function between ATTR-CA and AL-CA despite similar LA geometry. Our findings of more impaired LA function in ATTR may offer insight into higher AF burden in these patients.
Echocardiographic Reference Ranges of Non-invasive Myocardial Work Indices in Children
Global myocardial work (GMW) is an emerging method to characterize left ventricle (LV) function with potential advantages over both ejection fraction and global longitudinal strain (GLS). We aimed to determine the feasibility and reproducibility for echocardiographic-derived GMW in a healthy pediatric population; establish normal reference values; and investigate the influence of age, gender, and other clinical factor on normal reference ranges. We prospectively enrolled 212 individuals (median age of 9 years; interquartile range, 6 to 12 years, 112 female). Global work index (GWI), global constructive work (GCW), global wasted work (GWW), and global work efficiency (GWE) were measured from LV pressure–strain loops. Quantification of GMW was performed using a GE Vivid E95 system and available software package (Echopac V.203, GE). The mean LV EF was 64 ± 3% with GLS of -21.3 ± 1.5%. GWI was 1688 ± 219 mmHg% with mean GWE of 96.5 ± 1.4%. The GCW was 1959 ± 207 mmHg%, and the mean GWW of 61.1 ± 30.9 mmHg%. No significant difference was found in MW indices across age group and gender ( p  > 0.05 for all). There were significant correlations between both GWI and GCW with GLS and systolic blood pressure ( p  < 0.001), but not with GWE and GWW. Linear regression model revealed that GWI and GCW were more closely correlated with systolic blood pressure than GLS. LV MW indices had good intra-observer and inter-observer reproducibility. This study establishes both the feasibility and reference ranges for non-invasive echocardiographic indices of GMW in healthy children. Myocardial work appears to be a complementary modality to assess LV performance in children.
Shaping cardiac diagnostics: The role of myocardial tissue mapping in unraveling ring-like fibrosis
Patients with non-ischemic cardiomyopathy exhibit a range of myocardial fibrosis (MF) patterns on cardiovascular magnetic resonance (CMR) late gadolinium enhancement (LGE) imaging. Data suggests that ring-like MF is associated with worse prognosis. In the present study it was sought to analyze the prevalence of parametric mapping abnormalities in ring-like MF and their prognostic value for arrhythmic events. Patients undergoing clinical CMR at 1.5T/3T were evaluated for ring-like MF defined as midwall/subepicardial fibrosis involving ≥ 3 contiguous left ventricular segments. CMR protocol included cine imaging, T1 and T2 mapping, and LGE. Mean native T1, ECV, and T2 values and a number of mid short axis segments with elevated values were calculated. LGE extent was assessed segmentally. Arrhythmic outcomes were defined as appropriate device shock, premature ventricular contractions ≥ 10%, non-sustained/sustained ventricular tachycardia, or ventricular fibrillation. In total 49 patients (53 ± 17 years, 26.5% female) were analyzed. Many patients had elevated global/segmental mapping values: 45%/76% in native T1, 57%/57% in T2, and 57%/78% in ECV. During median follow-up of 12 months, arrhythmic events occurred in 65% of patients. There was no association between native T1/T2 elevation or number of LGE segments and arrhythmic outcomes. There was a significant association between ECV and arrhythmic outcomes, both septal ECV (p = 0.036) and any segmental ECV elevation (p = 0.03). T1 and T2 myocardial tissue abnormalities are common in patients with ring-like MF. ECV elevation was associated with arrhythmic events in this cohort. Further studies are needed to establish the diagnostic and prognostic value of parametric mapping in patients with ring-like MF.
Stunting and Overweight among 12-24-Month-Old Children Receiving Vaccination in Ho Chi Minh City, Vietnam
Introduction. Malnutrition and obesity are a double burden on children in developing countries and could induce higher risks of noncommunicable diseases in the long term. In the big cities of Vietnam, both issues are present and share the issue of nutrition problems; the prevalence of malnutrition in children is gradually decreasing while the prevalence of obesity is increasing rapidly. The paper aims to identify the prevalence of stunting and overweight/obesity in apparently healthy young children in Ho Chi Minh City (HCMC). Methods. A prospective cross-sectional study recruited 12-24-month-old children receiving national vaccination in community health centers in HCMC from February 2016 to July 2017. Sixteen healthcare centers were randomly selected among 8 districts of HCMC. Stunting and overweight were defined by height-for-age z-score <-2 SD and BMI z-score ≥+2 SD. Results. A total of 768 children had mean age of 16.8±4.2 months old, 51.7% boys. The prevalence of stunting and overweight/obesity was 8.2% and 10.7%, respectively. Stunting was associated with older age, boys, and low birth weight of children and occupation of mothers (P <0.05). No associated risk factor was observed for overweight/ obesity status. Conclusion. The prevalence of overweight/obesity was higher than the prevalence of stunting in 12-24-month-old children in HCMC. Overweight/obesity would be a public health problem for children in big cities.
Valve involvement in infective endocarditis among intravenous drug users: a systematic review and meta-analysis
Background Intravenous drug use (IVDU) is a major risk factor for infective endocarditis (IE), with right-sided infective endocarditis (RSIE) most frequently observed. However, precise estimates of valve involvement and the relative risk (RR) of RSIE within the IVDU population remain unclear. This systematic review and meta-analysis aimed to quantify the prevalence of RSIE, left-sided IE (LSIE), both-sided IE, and valve-specific involvement in IVDUs while assessing the relative risk of right-sided versus left-sided IE within the IVDU population. Methods A systematic search was conducted in PubMed, Embase, and Web of Science to identify studies reporting IE in IVDUs. Inclusion criteria followed the PICO framework, ensuring clear definitions of IVDU status and valve involvement. A random-effects model was used to calculate pooled prevalence estimates and RR ratios with 95% confidence intervals (CIs). Heterogeneity was assessed using the I² statistic, and sensitivity analyses were performed. Results A total of 14 studies involving 1,783 patients met the inclusion criteria. The pooled prevalence of right-sided infective endocarditis (RSIE) was 59% (95% CI: 54%–65%), left-sided infective endocarditis (LSIE) was 39% (95% CI: 33%–46%), and both-sided infective endocarditis was 8% (95% CI: 6%–12%). Among individual valves, the tricuspid valve was most commonly affected (59% (95% CI: 53%–64%)), followed by the mitral valve (21% (95% CI: 18%–26%)) and the aortic valve (16% (95% CI: 12%–20%)). Pulmonic valve involvement was rare (1% (95% CI: 0%–3%)). Overall, individuals who inject drugs (IVDUs) had a 1.49-fold increased risk of developing right-sided IE compared to left-sided IE (RR = 1.49 (95% CI: 1.15–1.95), p  = 0.008). Conclusion This SRMA confirms the predominance of right-sided IE, particularly tricuspid valve involvement, in IVDUs, with a significantly higher risk compared to left-sided IE. The findings underscore the need for targeted screening, early intervention, and IVDU-specific management strategies in IE care. Future research should focus on regional variations, microbiological patterns, and long-term outcomes in this high-risk population. Clinical trial number Not applicable. Key points Right-sided infective endocarditis is the most common presentation in IVDUs, with the tricuspid valve involved in over half of cases; IVDUs have a 1.49-fold higher risk of RSIE than LSIE. Left-sided IE (LSIE) occurs in nearly 40% of IVDU cases, often involving the mitral and aortic valves, and is associated with higher rates of complications, surgeries, and mortality. Routine transesophageal echocardiography (TEE) and comprehensive valve evaluation are essential in IVDU-IE to avoid underdiagnosing LSIE and guide timely surgical interventions.
