Impact of BTV-3 Circulation in Belgium in 2024 and Current Knowledge Gaps Hindering an Evidence-Based Control Program
Between 2006 and 2010, northwestern Europe experienced its first significant bluetongue virus (BTV) outbreak, driven by the spread of BTV-8, which had major repercussions on the European livestock sector. While BTV-3 was first identified in Europe in Italy in 2017, a new introduction of the virus was reported in 2023, in the Netherlands, and subsequently spread rapidly across the continent. A limited number of BTV-3 outbreaks were notified in Belgium in 2023, leading to the loss of its BTV-free status. In the following year, 2024, the virus spread throughout the country in a short time period. This study describes the impact of BTV-3 circulation in Belgium in 2024, detailing both its geographic spread and the associated increase in mortality, reduced births recorded, and decline in milk production among ruminants. Furthermore, preliminary results on the effectiveness of field vaccination and maternal immunity transfer are presented, as well as critical gaps that hinder the development of a robust, evidence-based management strategy. As the epidemiological situation is expected to become more complex in the future, due to the co-circulation of multiple BTV serotypes and other Culicoides-borne diseases, such as EHDV, effective collaboration and communication among stakeholders and international authorities will be crucial for implementing measures to mitigate the spread of these diseases.
A nationwide population-based cohort study of peripartum hysterectomy and arterial embolisation in Belgium: results from the Belgian Obstetric Surveillance System
ObjectivesTo assess the prevalence of major obstetric haemorrhage managed with peripartum hysterectomy and/or interventional radiology (IR) in Belgium. To describe women characteristics, the circumstances in which the interventions took place, the management of the obstetric haemorrhage, the outcome and additional morbidity of these women.DesignNationwide population-based prospective cohort study.SettingEmergency obstetric care. Participation of 97% of maternities covering 98.6% of deliveries in Belgium.ParticipantsAll women who underwent peripartum hysterectomy and/or IR procedures in Belgium between January 2012 and December 2013.ResultsWe obtained data on 166 women who underwent peripartum hysterectomy (n=84) and/or IR procedures (n=102), corresponding to 1 in 3030 women undergoing a peripartum hysterectomy and another 1 in 3030 women being managed by IR, thereby preserving the uterus. Seventeen women underwent hysterectomy following IR and three women needed further IR despite hysterectomy. Abnormal placentation and/or uterine atony were the reported causes of haemorrhage in 83.7%. Abnormally invasive placenta was not detected antenatally in 34% of cases. The interventions were planned in 15 women. Three women were transferred antenatally and 17 women postnatally to a hospital with emergency IR service. Urgent peripartum hysterectomy was averted in 72% of the women who were transferred, with no significant difference in need for transfusion. IR procedures were able to stop the bleeding in 87.8% of the attempts. Disseminated intravascular coagulation secondary to major haemorrhage was reported in 32 women (19%).ConclusionThe prevalence in Belgium of major obstetric haemorrhage requiring peripartum hysterectomy and/or IR is estimated at 6.6 (95% CI 5.7 to 7.7) per 10 000 deliveries. Increased clinician awareness of the risk factors of abnormal placentation could further improve the management and outcome of major obstetric haemorrhage. A case-by-case in-depth analysis is necessary to reveal whether the hysterectomies and arterial embolisations performed in this study were appropriate or preventable.
Group B Streptococcus maternal colonization and neonatal sepsis in Belgium between 2012 and 2021: a description of the epidemiological situation and identification of risk factors
Background Group B Streptococcus (GBS) infection is a leading cause of neonatal morbidity. Maternal rectovaginal GBS colonization is a primary risk factor for early onset neonatal GBS infection. In Belgium, pregnant women are screened and, if positive or at risk of transmission, offered intrapartum antibiotic prophylaxis (IAP). We aimed to provide the first comprehensive overview of Belgian data and identify possible risk factors of maternal GBS colonization and neonatal infection. Methods We calculated proportions of maternal screening, colonization and incidence of all neonatal GBS infections and identified their risk factors using log binomial regression from national registries’ data between 2012–2021. Results Of all women, 90.2% were screened and of them, 18.5% were GBS colonized. These proportions were stable over time. Risk factors for maternal GBS colonization included Sub-Saharan (aRR = 1.45, [1.41–1.48]) and North-African origin (aRR = 1.31, [1.28–1.34]) and 5 + parity (aRR = 1.21, [1.18–1.25]). Neonatal GBS sepsis incidence showed a gradually decreasing trend in two individual data sources. Flanders’ overall neonatal GBS sepsis estimates were around 0.49 cases per 1000 livebirths. Risk factors for neonatal GBS infection included preterm birth (aRR=3-27, depending on level of prematurity) and very low birthweight (aRR=3.13, [1.32-7.44]). Babies of colonized women were at higher risk without (aRR=7.21, [4.84-10.75]) than with IAP (aRR=4.40, [3.31-5.85]) compared to non-colonized women. Conclusions The maternal colonization rate and the neonatal GBS sepsis incidence in Belgium is comparable to European data. We identified already known risk factors and effectiveness of IAP. Mothers at risk for GBS colonization and premature/low birth weight deliveries could benefit most from additional maternal vaccination strategy.
Effect of maternal origin on the association between maternal height and risk of preterm birth in Belgium: a retrospective observational cohort study
ObjectivesTo investigate the effect of maternal origin on the association between maternal height and the risk of preterm birth (PTB).DesignRetrospective observational cohort study.SettingTwo of the three Belgian regions, including Brussels-Capital and Walloon regions.ParticipantsA total of 245 204 women spontaneously delivered live singletons between 2009 and 2013. Maternal nationality at the time of birth included Belgium, Congo, French, Italy, Morocco, Poland, Romania and Turkey.Outcomes measuresThe outcome variable was spontaneous PTB, defined as childbirth occurring at less than 37 weeks’ gestation.ResultsAverage height, demographic characteristics and the spontaneous PTB rate differed according to maternal origin, defined as maternal nationality at birth. The pattern of association between maternal height and the risk of PTB was not uniform by maternal nationality at birth. The low maternal height category was associated with a statistically significant increased risk of spontaneous PTB for Belgian (adjusted OR (aOR) 1.23, 95% CI 1.16 to 1.32), Italian (aOR 1.48, 95% CI 1.12 to 1.96) and Polish (aOR 1.76, 95% CI 1.11 to 2.78), respectively. However, this association was not observed for the women from Congo, France, Morocco, Romania and Turkey.ConclusionsThe association between height and the risk of PTB was modified by maternal nationality, even for mothers from the same region of the world. For example, there was a significant inverse association for the Belgians and Italians but not for French women. Our data suggest that PTB risk assessment should take into account the specific height of maternal origin.
Missing information in birth certificates in Brussels after reinforcement of data collection, and variation according to immigration status. A population-based study
Background A problem repeatedly reported in birth certificate data is the presence of missing data. In 2008, a Centre for Perinatal Epidemiology was created inter alia to assist the Health Departments of Brussels-Capital City Region to check birth certificates. The purpose of this study is to assess the changes brought by the Centre in terms of completeness of data registration for the entire population and according to immigration status. Methods Birth certificates from the birth registry of 2008 and 2009 of Brussels were considered. We evaluated the initial missing information in January 2008 (baseline situation) and the corresponding rate at the end of 2008 after oral and written information had been given to the city civil servants and health providers. The data were evaluated again at the end of 2009 where no reinforcement rules were adopted. We also measured residual missing data after correction in socio-economic and medical data, for the entire population and according to maternal nationality of origin. Changes in registration of stillbirths were estimated by comparison to 2007 baseline data, and all multiple births were checked for complete identification of pairs. Results Missing information initially accounted for 64.0%, 20.8% and 19.5% of certificates in January 2008, December 2008, and 2009 respectively. After correction with lists sent back to the hospitals or city offices, the mean residual missing data rate was 2.1% in 2008 and 0.8% in 2009. Education level and employment status were missing more often in immigrant mothers compared to Belgian natives both in 2008 and 2009. Mothers from Sub-Saharan Africa had the highest missing rate of socio-economic data. The stillbirth rate increased from 4.6 ‰ in 2007 to 8.2 ‰ in 2009. All twin pairs were identified, but early loss of a co-twin before 22 weeks was rarely reported. Conclusions Reinforcement of data collection was associated with a decrease of missing information. The residual missing data rate was very low. The stillbirth rate was also improved but the early loss of a co-twin before 22 weeks seems to remain underreported.
