Purpose
No study has compared 30-day and 90-day adverse postoperative outcomes between old-age patients with and those without sarcopenia.
Patients and methods
We categorize elderly patients receiving major surgery into two groups according to the presence or absence of preoperative sarcopenia that were matched at a 1:4 ratio through propensity score matching (PSM). We analyzed 30-day or 90-day adverse postoperative outcomes and mortality in patients with and without sarcopenia receiving major surgery.
Results
Multivariate logistic regression analyses revealed that the patients with preoperative sarcopenia were at significantly higher risk of 30-day postoperative mortality (adjusted odds ratio [aOR]. = 1.25; 95% confidence interval [CI]. = 1.03–1.52) and 30-day major complications such as postoperative pneumonia (aOR = 1.15; 95% CI = 1.00-1.40), postoperative bleeding (aOR = 2.18; 95% CI = 1.04–4.57), septicemia (aOR = 1.31; 95% CI = 1.03–1.66), and overall complications (aOR = 1.13; 95% CI = 1.00-1.46). In addition, surgical patients with sarcopenia were at significantly higher risk of 90-day postoperative mortality (aOR = 1.50; 95% CI = 1.29–1.74) and 90-day major complications such as pneumonia (aOR = 1.27; 95% CI = 1.10–1.47), postoperative bleeding (aOR = 1.90; 95% CI = 1.04–3.48), septicemia (aOR = 1.52; 95% CI = 1.28–1.82), and overall complications (aOR = 1.24; 95% CI = 1.08–1.42).
Conclusions
Sarcopenia is an independent risk factor for 30-day and 90-day adverse postoperative outcomes such as pneumonia, postoperative bleeding, and septicemia and increases 30-day and 90-day postoperative mortality among patients receiving major surgery.
Condensed abstract
No study has compared 30-day and 90-day adverse postoperative outcomes between patients with and those without sarcopenia. We conducted a propensity score?matched (PSM) population-based cohort study to investigate the adverse postoperative outcomes and mortality in patients undergoing major elective surgery with preoperative sarcopenia versus those without preoperative sarcopenia. We demonstrated that sarcopenia is an independent risk factor for 30-day and 90-day adverse postoperative outcomes, such as postoperative pneumonia, bleeding, septicemia, and mortality after major surgery. Therefore, surgeons and anesthesiologists should attempt to correct preoperative sarcopenia, swallowing function, and respiratory muscle training before elective surgery to reduce postoperative complications that contribute to the decrease in surgical mortality.

Comparison of surgical complications after curative surgery in patients with oral cavity squamous cell carcinoma and sarcopenia

Background The study aims to clarify the association of sarcopenia with perioperative and postoperative complications in oral cavity squamous cell carcinoma (OCSCC) patients undergoing curative surgery and to understand the reasons causing the poor oncologic outcomes for OCSCC. Methods We conducted a propensity score matching study to investigate the association of perioperative and postoperative outcomes in OCSCC patients with sarcopenia and without sarcopenia. A retrospective analysis of a large national data set from the Taiwan Cancer Registry Database was conducted. At least two claims for patients with a principal diagnosis of sarcopenia within the 12‐month preoperative period were defined as the criteria for sarcopenia diagnosis (ICD‐10‐CM code M62.84). Sarcopenia was diagnosed through the measurement of low muscle strength and low muscle mass by any one of the patient's attending orthopaedic physician, rehabilitation physician, family medicine specialist or geriatrician. A multivariate logistic regression model was used to calculate the perioperative, and postoperative major complications. Results Our final cohort included 16 293 patients with OCSCC (10 862 and 5 431 in the sarcopenia and nonsarcopenia groups, respectively) who were eligible for further analysis. The sarcopenia group was 10.40% female and 89.60% male, and the nonsarcopenia group was 9.74% female and 90.26% male. The mean age ± standard deviation (SD) were 56.44 ± 11.14 and 56.22 ± 11.29 for sarcopenia and nonsarcopenia groups. OCSCC patients with sarcopenia undergoing curative surgery had a significantly higher blood transfusion rate and volume; longer intensive care unit (ICU) stay, and hospital stay; higher postoperative 30‐day mortality (adjusted odds ratio [aOR]: 1.12, 95% confidence interval [CI] [1.07, 1.56]) and rates of pneumonia (aOR: 1.34, 95% CI [1.20, 1.50]), acute renal failure (aOR: 1.45, 95% CI [1.12, 1.87]) and septicaemia (aOR: 1.29, 95% CI [1.15, 1.45]); higher postoperative first‐year mortality (aOR: 1.18, 95% CI [1.13, 1.51]) and rates of pneumonia (aOR: 1.43, 95% CI [1.30, 1.56]), acute myocardial infarction (aOR: 1.52, 95% CI [1.06, 2.18]) and septicaemia (aOR: 1.29, 95% CI [1.15, 1.45]). Conclusions OCSCC patients with sarcopenia might exhibit more perioperative and surgical complications than those without sarcopenia.

Long-term mortality impact of postoperative hyperactive delirium in older hip fracture surgery patients

Background
Postoperative hyperactive delirium is a common and serious complication in older patients undergoing surgery, but the association between delirium and mortality remains controversial. Compared to other delirium subtypes, hyperactive delirium is characterized by more overt clinical manifestations, facilitating accurate detection and evaluation. This study aimed to clarify this association by comparing long-term mortality between patients with and without postoperative hyperactive delirium, using propensity score matching for robust analysis.
Methods
We conducted a cohort study to evaluate the association between postoperative hyperactive delirium and long-term mortality in older patients undergoing emergency hip fracture surgery. We used the Taiwan National Health Insurance Service database to identify patients aged 65 years or older who underwent emergency hip fracture surgery between 2008 and 2018. The primary outcome was all-cause mortality.
Results
A total of 270,437 patients were included in the analysis, with 6,795 patients in the postoperative hyperactive delirium group and 263,642 patients in the no postoperative hyperactive delirium group. After PSM, both groups contained 6,795 patients, ensuring balanced baseline characteristics for comparison. Postoperative hyperactive delirium was an independent risk factor for all-cause death, with an adjusted hazard ratio of 1.62 (95% confidence interval, 1.51–1.74;

< 0.0001) after PSM. Subgroup analysis revealed that older patients with postoperative hyperactive delirium consistently exhibited significantly higher adjusted hazard ratios of all-cause death compared with those without postoperative hyperactive delirium, regardless of age, sex, income levels, or ASA scores. < 0.0001) was statistically significant, the high survival rates in both groups suggest a modest absolute clinical impact.
Conclusion
Postoperative hyperactive delirium is an independent risk factor for long-term mortality in older patients undergoing emergency hip fracture surgery. While the statistical association is evident, it is important to carefully consider the modest absolute difference in survival rates and its implications for clinical application.

The classical Lotka–Volterra model is mainly a two-dimensional configuration. The two-dimensional form limits the application scenarios of the Lotka–Volterra model. The research objective of this paper is to develop a feasible three population equilibrium model to analyze the industry competition. In order to expand the Lotka–Volterra model to empirical analysis, this paper proposes a symbiosis model of enterprise population based on a three-dimensional Lotka–Volterra model. This paper analyzes the symbiotic system from two aspects: the balanced development of three species and the competitive evolution of the three species, taking the sales volume of three local automobile enterprises in China as the sample for empirical analysis. The symbiotic optimization under the equilibrium state of three populations indicates that there is the possibility of equilibrium and reciprocity in the growth of three automobile product populations. The empirical analysis fully explains the feasibility of this research paradigm. The analysis of competition evolution shows that cooperative action is better than competitive strategy and that comprehensive cooperation is the ideal state of a three-population symbiotic system.

Optimal statin use for prevention of sepsis in type 2 diabetes mellitus

Purpose
To investigate the dose-dependent protective effects of statins, specific classes of statins, and different intensities of statin use on sepsis risk in patients with type 2 diabetes mellitus (T2DM).
Methods
We included patients with T2DM aged ≥ 40 years. Statin use was defined as the use of statin on most days for > 1 months with a mean statin dose of ≥ 28 cumulative defined daily doses (cDDDs) per year (cDDD-year). An inverse probability of treatment-weighted Cox hazard model was used to investigate the effects of statin use on sepsis and septic shock while considering statin use status as a time-dependent variable.
Results
From 2008 to 2020, a total of 812 420 patients were diagnosed as having T2DM. Among these patients, 118,765 (27.79%) statin nonusers and 50 804 (12.03%) statin users developed sepsis. Septic shock occurred in 42,755 (10.39%) individuals who did not use statins and 16,765 (4.18%) individuals who used statins. Overall, statin users had a lower prevalence of sepsis than did nonusers. The adjusted hazard ratio (aHR) of statin use was 0.37 (95% CI 0.35, 0.38) for sepsis compared with no statin use. Compared with the patients not using statins, those using different classes of statins exhibited a more significant reduction in sepsis, with aHRs (95% CIs) of sepsis being 0.09 (0.05, 0.14), 0.32 (0.31, 0.34), 0.34 (0.32, 0.36), 0.35 (0.32, 0.37), 0.37 (0.34, 0.39), 0.42 (0.38, 0.44), and 0.54 (0.51, 0.56) for pitavastatin, pravastatin, rosuvastatin, atorvastatin, simvastatin, fluvastatin, and lovastatin use, respectively. In the patients with different cDDD-years of statins, multivariate analysis indicated a significant reduction in sepsis, with aHRs of 0.53 (0.52, 0.57), 0.40 (0.39, 0.43), 0.29 (0.27, 0.30), and 0.17 (0.15, 0.19) for Q1, Q2, Q3, and Q4 cDDD-years (

for trend < 0.0001). The optimal daily statin dose of 0.84 DDD was associated with the lowest aHR. Similar trends of higher cDDD-year and specific statin types use were associated with a decrease in septic shock when compared to statin non-users.
Conclusion
Our real-world evidence demonstrated that the persistent use of statins reduced sepsis and septic shock risk in patients with T2DM and a higher cDDD-year of statin use was associated with an increased reduction of sepsis and septic shock risk in these patients.

Chronic pain following elective surgery under general anesthesia in older adults

Background
The purpose of this study was to investigate the association between age and chronic postsurgical pain (CPSP) in patients who underwent elective surgery under general anesthesia, with a focus on long-term postsurgical analgesic use. To our knowledge, no previous study has examined this relationship in detail between older and younger patients.
Methods
We conducted a propensity score-matched (PSM) study to compare the rates of long-term (3 or 6 months) postoperative analgesic use between older adult (≥ 65 years) and younger (< 65 years) patients. Multivariate logistic regression was used to assess the use of analgesics as a surrogate indicator of CPSP.
Results
The PSM analysis included 62,784 surgical patients (31,392 in each group). Three months after surgery, the rates of analgesic use were significantly higher in the older age group (adjusted odds ratio [aOR], 1.45; 95% confidence interval [CI], 1.41–1.49) as well as for opioid use specifically (aOR, 1.34; 95% CI, 1.29–1.39). Six months after surgery, the rates of analgesic use remained higher in the older age group (aOR, 1.52; 95% CI, 1.47–1.58), and similarly for opioid use specifically (aOR, 1.42; 95% CI, 1.36–1.48).
Conclusions
Our findings suggest that older adults have higher rates of long-term analgesic use for CPSP after elective surgery under general anesthesia. This study highlights the importance of addressing CPSP in older adult patients and considering age-related factors when managing postoperative pain.

Purpose
To enhance the predictive risk model for all-cause mortality in individuals with Type 2 Diabetes (T2DM) and prolonged Atherosclerotic Cardiovascular Disease (ASCVD) risk factors. Despite the utility of the Coronary Artery Calcium (CAC) score in assessing cardiovascular risk, its capacity to predict all-cause mortality remains limited.
Methods
A retrospective cohort study included 1929 asymptomatic T2DM patients with ASCVD risk factors, aged 40–80. Variables encompassed demographic attributes, clinical parameters, CAC scores, comorbidities, and medication usage. Factors predicting all-cause mortality were selected to create a predictive scoring system. By using stepwise selection in a multivariate Cox proportional hazards model, we divided the patients into three risk groups.
Results
In our analysis of all-cause mortality in T2DM patients with extended ASCVD risk factors over 5 years, we identified significant risk factors, their adjusted hazard ratios (aHR), and scores: e.g., CAC score > 1000 (aHR: 1.57, score: 2), CAC score 401–1000 (aHR: 2.05, score: 2), and more. These factors strongly predict all-cause mortality, with varying risk groups (e.g., very low-risk: 2.0%, very high-risk: 24.0%). Significant differences in 5-year overall survival rates were observed among these groups (log-rank test < 0.001).
Conclusion
The Poh-Ai Predictive Scoring System excels in forecasting mortality and cardiovascular events in individuals with Type 2 Diabetes Mellitus and extended ASCVD risk factors.

Pre-CCRT 18-fluorodeoxyglucose PET-CT improves survival in patients with advanced stages p16-negative oropharyngeal squamous cell carcinoma via accurate radiation treatment planning

PurposeNo large-scale prospective randomized study with a long-term follow-up period has evaluated the survival outcomes of preconcurrent chemoradiotherapy (CCRT) 18-fluorodeoxyglucose positron emission tomography–computed tomography (18FDG PET–CT) in patients with non–human papillomavirus (HPV)-associated oropharyngeal squamous cell carcinoma (OPSCC).Patients and MethodsWe included patients with stage I–IVA p16-negative OPSCC receiving definitive CCRT and categorized them into two groups according to pre-CCRT 18FDG PET–CT and compared their outcomes: the case group consisted of patients who underwent pre-CCRT 18FDG PET–CT, whereas the comparison group consisted of patients who did not receive pre-CCRT 18FDG PET–CT.ResultsThe final cohort consisted of 3942 patients (1663 and 2279 in the case and comparison groups, respectively). According to multivariable Cox regression analysis, pre-CCRT 18FDG PET–CT was not a significant prognostic factor for overall survival in patients with stages I–II of p16-negative OPSCC receiving standard CCRT. The adjusted hazard ratio (95% confidence interval) of all-cause death for the patients with advanced stages (III–IVA) of p16-negative OPSCC receiving pre-CCRT 18FDG PET–CT was 0.75 (0.87–0.94, P = 0.0236).ConclusionsRoutine use of pre-CCRT 18FDG PET–CT is not necessary for each patient with p16-negative OPSCC. Evaluating GLP-1 receptor agonists versus metformin as first-line therapy for reducing dementia risk in type 2 diabetes

IntroductionNo direct comparisons have evaluated glucagon-like peptide-1 receptor agonists (GLP-1 RAs) versus metformin as first-line antidiabetic therapy for preventing dementia in patients with type 2 diabetes mellitus (T2DM). This study aimed to assess the comparative effectiveness of GLP-1 RAs and metformin in reducing dementia risk.Research design and methodsThis retrospective cohort study used data from a global health research network between 2004 and 2024. Patients with T2DM initiating GLP-1 RAs or metformin as first-line monotherapy were included. Propensity score matching was employed to balance baseline characteristics. Dementia incidence was analyzed using Cox proportional hazards models, with sensitivity analyses to confirm robustness.ResultsAmong 87,229 matched patients per cohort, GLP-1 RA use was associated with a significantly lower risk of overall dementia (adjusted HR (AHR) 0.90; 95% CI 0.85 to 0.95), Alzheimer’s disease (AD) (AHR 0.88; 95% CI 0.83 to 0.94), and non-vascular dementias (non-VaDs) (AHR 0.75; 95% CI 0.70 to 0.81) compared with metformin. No significant difference was observed for VaD. Subgroup analyses showed consistent benefit across age and sex, with the strongest effect among older adults and females.ConclusionsGLP-1 RAs were more effective than metformin in reducing the risk of dementia—especially AD and non-vascular types—highlighting their potential as a preferred first-line treatment in T2DM. Further randomized trials are warranted to validate these findings.

Impact of perioperative dexmedetomidine on recurrence and survival outcomes in oral cavity squamous cell carcinoma

ObjectivesTo investigate the association between perioperative dexmedetomidine (DEX) use and oncological outcomes—including locoregional recurrence (LRR) and distant metastasis (DM)—in patients undergoing curative surgery for oral cavity squamous cell carcinoma (OCSCC).MethodsThis retrospective cohort study used data from the Taiwan Cancer Registry Database and included patients with stage I–IVB OCSCC who underwent curative surgery between 2007 and 2019. Patients were categorised by DEX exposure status and matched 1:1 using propensity score matching (PSM) based on key clinical and demographic variables. Cox proportional hazards models and competing risk analyses were used to estimate the association between DEX use and oncological outcomes.ResultsAfter PSM, 8024 patients (4012 per group) were included. Multivariable Cox regression showed that perioperative DEX use was significantly associated with increased risks of LRR (adjusted HR (aHR) 1.67; 95% CI 1.55 to 1.80; p<0.001) and DM (aHR 1.30; 95% CI 1.19 to 1.42; p<0.001).DiscussionThese findings suggest a potential oncological risk associated with perioperative DEX administration. Possible mechanisms include immune modulation and enhanced metastatic potential, as reported in preclinical studies. Further investigation is needed to clarify causal pathways and identify patient subgroups most affected.ConclusionsPerioperative DEX use is independently associated with increased risks of LRR and DM in OCSCC patients. These results underscore the importance of cautious perioperative management and the need for prospective validation in randomised clinical trials.
