Postoperative adjuvant tyrosine kinase inhibitors combined with anti-PD-1 antibodies improves surgical outcomes for hepatocellular carcinoma with high-risk recurrent factors
The clinical value of postoperative adjuvant therapy (PAT) for hepatocellular carcinoma (HCC) remains unclear. This study aimed to explore the effect of PAT with tyrosine kinase inhibitors (TKIs) and anti-PD-1 antibodies on the surgical outcomes of HCC patients with high-risk recurrent factors (HRRFs). HCC patients who underwent radical hepatectomy at Tongji Hospital between January 2019 and December 2021 were retrospectively enrolled, and those with HRRFs were divided into PAT group and non-PAT group. Recurrence-free survival (RFS) and overall survival (OS) were compared between the two groups after propensity score matching (PSM). Prognostic factors associated with RFS and OS were determined by Cox regression analysis, and subgroup analysis was also conducted. A total of 250 HCC patients were enrolled, and 47 pairs of patients with HRRFs in the PAT and non-PAT groups were matched through PSM. After PSM, the 1- and 2-year RFS rates in the two groups were 82.1% vs. 40.0% ( < 0.001) and 54.2% vs. 25.1% ( = 0.012), respectively. The corresponding 1- and 2-year OS rates were 95.4% vs. 69.8% ( = 0.001) and 84.3% vs. 55.5% ( = 0.014), respectively. Multivariable analyses indicated that PAT was an independent factor related to improving RFS and OS. Subgroup analysis demonstrated that HCC patients with tumor diameter > 5 cm, satellite nodules, or vascular invasion could significantly benefit from PAT in RFS and OS. Common grade 1-3 toxicities, such as pruritus (44.7%), hypertension (42.6%), dermatitis (34.0%), and proteinuria (31.9%) were observed, and no grade 4/5 toxicities or serious adverse events occurred in patients receiving PAT. PAT with TKIs and anti-PD-1 antibodies could improve surgical outcomes for HCC patients with HRRFs.
CAGCN: Centrality-Aware Graph Convolution Network for Anomaly Detection in Industrial Control Systems
In industrial control systems, the utilization of deep learning based methods achieves improvements for anomaly detection. However, most current methods ignore the association of inner components in industrial control systems. In industrial control systems, an anomaly component may affect the neighboring components; therefore, the connective relationship can help us to detect anomalies effectively. In this paper, we propose a centrality-aware graph convolution network (CAGCN) for anomaly detection in industrial control systems. Unlike the traditional graph convolution network (GCN) model, we utilize the concept of centrality to enhance the ability of graph convolution networks to deal with the inner relationship in industrial control systems. Our experiments show that compared with GCN, our CAGCN has a better ability to utilize this relationship between components in industrial control systems. The performances of the model are evaluated on the Secure Water Treatment (SWaT) dataset and the Water Distribution (WADI) dataset, the two most common industrial control systems datasets in the field of industrial anomaly detection. The experimental results show that our CAGCN achieves better results on precision, recall, and F 1 score than the state-of-the-art methods.
Impact of intraperitoneal lavage with lobaplatin on survival in patients undergoing hepatectomy for ruptured hepatocellular carcinoma
Background Intraoperative intraperitoneal lavage with lobaplatin is widely used in the treatment of colorectal and gastric cancers. This study aimed to explore the efficacy of lobaplatin lavage in patients with ruptured hepatocellular carcinoma (HCC) administered radical hepatectomy. Methods Ruptured HCC patients who underwent hepatectomy between September 2007 and March 2022 were divided into the distilled water lavage and lobaplatin lavage groups. Survival curves for RFS and OS were generated by the Kaplan-Meier method and compared by the log-rank test using Propensity Score Matching (PSM). Cox regression analysis was conducted to identify independent prognostic factors of RFS and OS. Results This study included 201 patients, with 114 cases (89.1%) in the distilled water lavage group and 55 cases (75.3%) in the lobaplatin lavage group experiencing recurrence, and 95 (74.2%) and 44 (60.3%) patients died in the distilled water lavage and lobaplatin lavage groups, respectively during the follow-up period. After PSM, 1-, 2-, 3-year RFS rates were 38.6%, 21.1%, 10.5% and 57.9%, 40.4%, 26.3% in the distilled water lavage and lobaplatin lavage groups, respectively ( P  = 0.001). 1-, 2-, 3-year OS rates were 56.1%, 40.4%, 33.3% and 89.5%, 63.2%, 43.9% in the distilled water lavage and lobaplatin lavage groups, respectively ( P  = 0.001). Multivariable Cox regression indicated that lobaplatin lavage may improve RFS (HR = 0.524, 95% CI: 0.354–0.775; P  = 0.001) and OS (HR = 0.524, 95% CI: 0.350–0.783; P  = 0.002). Conclusions Intraoperative lobaplatin lavage improves RFS and OS in patients with ruptured HCC after hepatectomy. Trial registration TJ-IRB202403002, 2024-04-26, retrospectively registered.
The Early Cretaceous High-Nb Basalt and Arc Andesite Association in the Eastern Segment of the Altyn Tagh Fault: Petrological Records of Intracontinental Extension
The Altyn Tagh Fault plays a critical role in understanding the tectonic evolution of the northern margin of the Tibetan Plateau. However, considerable debate persists regarding its activity and deformation history. This study investigates volcanic rocks from the Beidayao-Jianquanzi-Hanxia-Hongliuxia area in the eastern segment of the fault. By employing zircon U-Pb dating, whole-rock geochemistry, and Sr-Nd isotope analysis, we aim to elucidate their petrogenesis and tectonic setting, thereby providing new insights into the crustal evolution of the eastern Altyn Tagh Fault. Zircon U-Pb dating of the Hongliuxia rhyolite yields a weighted mean 206Pb/238U age of 106.6 ± 0.6 Ma, indicating an Early Cretaceous eruption. Geochemically, the western part of the study area (Beidayao and Jianquanzi) is dominated by basalts that exhibit significant enrichment in large ion lithophile elements and light rare earth elements, together with high Nb concentrations (>20 ppm), as well as high Nb/La (0.64–1.12) and Nb/U (29.8–35.42) ratios, consistent with the characteristics of high-Nb basalt. In contrast, the eastern area (Hanxia and Hongliuxia) is characterized by andesitic rocks that display typical continental arc affinities, marked by enrichment in Th, U, and Pb and depletion in Nb, Ta, and Ti. Isotopically, the basalts show initial 87Sr/86Sr ratios of 0.706–0.707 and εNd (t) values ranging from −3.2 to 0.8, whereas the andesites possess more radiogenic Sr isotopic compositions, with (87Sr/86Sr)i ratios of 0.710–0.717, and more negative εNd (t) values from −11.4 to −1.5, suggesting derivation from an enriched mantle source. Integrating geochemical data with regional geological records, we propose that the eastern part of the Altyn Fault experienced a significant intracontinental extensional setting during the Early Cretaceous, where asthenospheric mantle upwelling played a key role in the generation of the volcanic rocks. This study provides key petrological and geochemical constraints on Early Cretaceous deformation and activity along the Altyn Tagh Fault, and also offers a valuable reference for understanding the evolution of similar fault systems.
Clinical Characteristics of Visual Dysfunction in Carbon Monoxide Poisoning Patients
Purpose. The aim of the present study was to analyze the clinical characteristics of visual dysfunction in patients with carbon monoxide (CO) poisoning. Methods. A total of 436 patients with CO poisoning were enrolled in our hospital from October 2012 to December 2018, including 193 patients with moderate poisoning (MP group), 165 with severe poisoning (SP group), and 78 with delayed encephalopathy (DE group). The clinical characteristics of visual dysfunction in patients with CO poisoning were analyzed through the collection of medical history, regular physical examination, brain magnetic resonance imaging (MRI), ophthalmological examination, the National Eye Institute Visual Function Questionnaire (NEI-VFQ), and its influencing factors. Results. Some patients in the three groups had visual dysfunction. The main ocular symptoms were local pain, eye movement disorder, and visual field defect. The key pathological factors were keratopathy, retinal nerve cell damage, optic nerve damage, retinal vascular disease, macular disease, and occipital visual center damage. The clinical symptoms of visual dysfunction after CO poisoning lasted for a long time (>12 months) and were not completely consistent with the positive results of the ophthalmological examination. A few sequelae of ophthalmology were still left after the help of medicine. Conclusion. The incidence of visual dysfunction in patients with CO poisoning was high, the clinical symptoms were rich and diverse, the duration of disease was long, and the prognosis was poor. Thus, the relevant ophthalmological examination and intervention treatment should be perfected as soon as possible.
Portal vein velocity predicts portal vein system thrombosis after splenectomy with esophagogastric devascularization
BackgroundPortal vein system thrombosis (PVST) is a potentially fatal complication after splenectomy with esophagogastric devascularization (SED) in cirrhotic patients with portal hypertension. However, the impact of portal vein velocity (PVV) on PVST after SED remains unclear. Therefore, this study aims to explore this issue.MethodsConsecutive cirrhotic patients with portal hypertension who underwent SED at Tongji Hospital between January 2010 and June 2022 were enrolled. The patients were divided into two groups based on the presence or absence of PVST, which was assessed using ultrasound or computed tomography after the operation. PVV was measured by duplex Doppler ultrasound within one week before surgery. The independent risk factors for PVST were analyzed using univariate and multivariate logistic regression analysis. A nomogram based on these variables was developed and internally validated using 1000 bootstrap resamples.ResultsA total of 562 cirrhotic patients with portal hypertension who underwent SED were included, and PVST occurred in 185 patients (32.9%). Multivariate logistic regression analysis showed that PVV was the strongest independent risk factor for PVST. The incidence of PVST was significantly higher in patients with PVV ≤ 16.5 cm/s than in those with PVV > 16.5 cm/s (76.2% vs. 8.5%, p < 0.0001). The PVV-based nomogram was internally validated and showed good performance (optimism-corrected c-statistic = 0.907). Decision curve and clinical impact curve analyses indicated that the nomogram provided a high clinical benefit.ConclusionA nomogram based on PVV provided an excellent preoperative prediction of PVST after splenectomy with esophagogastric devascularization.

Abstract Leptin has been shown to improve insulin sensitivity and glucose metabolism in normoinsulinemic healthy or obese rodents. It has not been determined whether leptin may act independently of insulin in regulating energy metabolism in vivo. The present study was designed to examine the effects of leptin treatment alone on glucose metabolism in insulin-deficient streptozotocin (STZ)-induced diabetic rats. Four groups of STZ-induced diabetic rats were studied: 1) rats treated with recombinant methionine murine leptin subcutaneous infusion with osmotic pumps for 12-14 days (LEP; 4 mg x kg(-1) x day(-1), n = 10); 2) control rats infused with vehicle (phosphate-buffered saline) for 12-14 days (VEH; n = 10); 3) pair-fed control rats given a daily food ration matching that of LEP rats for 12-14 days (PF; n = 8); and 4) rats treated with subcutaneous phloridzin for 4 days (PLZ; 0.4 g/kg twice daily, n = 10). Phloridzin treatment normalizes blood glucose without insulin and was used as a control for the effect of leptin in correcting hyperglycemia. All animals were then studied with a hyperinsulinemic-euglycemic clamp (6 mU x kg(-1) x min(-1). Our study demonstrates that leptin treatment in the insulin-deficient diabetic rats restored euglycemia, minimized body weight loss due to food restriction, substantially improved glucose metabolic rates during the postabsorptive state, and restored insulin sensitivities at the levels of the liver and the peripheral tissues during the glucose clamp. The effects on glucose turnover are largely independent of food restriction and changes in blood glucose concentration, as evidenced by the minimal improvement of insulin action and glucose turnover parameters in the PF and PLZ groups. Our results suggest that the antidiabetic effects of leptin are achieved through both an insulin-independent and an insulin-sensitizing mechanism.
High expression of JMJD6 predicts unfavorable survival in lung adenocarcinoma
The upregulated expression of JMJD6 was observed in various human cancers. However, little was known about JMJD6 expression and its clinicopathological significance in lung adenocarcinoma. The aim of this study was to investigate the expression and significance of JMJD6 in lung adenocarcinoma progression and prognosis. The levels of JMJD6 mRNA and protein in lung adenocarcinoma specimens and corresponding non-tumorous lung tissues were evaluated by quantitative reverse-transcriptase polymerase chain reaction (qRT-PCR) and Western blot. In order to investigate the correlations between JMJD6 and the clinicopathological features of lung adenocarcinoma, the expression of JMJD6 in 154 patients with lung adenocarcinoma was detected by immunohistochemistry. By qRT-PCR and Western blot, the relative expression levels of JMJD6 mRNA and protein were significantly higher in lung adenocarcinoma tissues than in corresponding non-tumorous lung tissues ( P  < 0.001). Immunohistochemical staining revealed that high JMJD6 expression was closely correlated with tumor size ( P  = 0.005), pathological grade ( P  = 0.003), pT status ( P  = 0.012), pN status ( P  = 0.003), and pleural invasion ( P  < 0.001). Moreover, the results of Kaplan–Meier analysis indicated that a high expression level of JMJD6 resulted in a significantly poor prognosis of lung adenocarcinoma patients. Multivariate analysis showed that the status of JMJD6 expression was an independent prognostic factor for lung adenocarcinoma patients. Our results showed that JMJD6 plays a key role in lung adenocarcinoma and therefore may provide an opportunity for developing a novel therapeutic target as well as a prognostic marker in lung adenocarcinoma.
ADAM17 is overexpressed in non-small cell lung cancer and its expression correlates with poor patient survival
The purpose of this study was to assess ADAM17 expression and to explore its contribution to the non-small cell lung cancer (NSCLC). Real-time quantitative reverse transcriptase-polymerase chain reaction was conducted to detect ADAM17 mRNA expression. In addition, ADAM17 expression was analyzed by immunohistochemistry in 124 clinicopathologically characterized NSCLC cases. The correlation of ADAM17 expression with patients’ survival rate was assessed by Kaplan–Meier and Cox regression. The expression levels of ADAM17 mRNA and protein in NSCLC tissues were both significantly higher than those in non-cancerous tissues. In addition, high expression of ADAM17 was significantly correlated with tumor grade ( P  = 0.026), tumor size ( P  = 0.001), clinical stage ( P  = 0.016), and lymph node metastases ( P  < 0.001). Furthermore, multivariate analysis suggested that tumor grade, tumor size, clinical stage, lymph node metastases, and ADAM17 expression were independent prognostic indicators for NSCLC. Our data suggest for the first time that the increased expression of ADAM17 in NSCLC is associated significantly with aggressive progression and poor prognosis. ADAM17 may be an important molecular marker for predicting the carcinogenesis, progression, and prognosis of NSCLC.
