Contact pathway of coagulation and inflammation

The contact system, also named as plasma kallikrein-kinin system, consists of three serine proteinases: coagulation factors XII (FXII) and XI (FXI), and plasma prekallikrein (PK), and the nonenzymatic cofactor high molecular weight kininogen (HK). This system has been investigated actively for more than 50 years. The components of this system and their interactions have been elucidated from in vitro experiments, which indicates that this system is prothrombotic by activating intrinsic pathway, and proinflammatory by producing bioactive peptide bradykinin. Although the activation of the contact system have been implicated in various types of human disease, in only a few instances is its role clearly defined. In the last 10 years, our understanding of the contact system, particularly its biology and (patho)physiology has greatly increased through investigations using gene-modified animal models. In this review we will describe a revitalized view of the contact system as a critical (patho)physiologic mediator of coagulation and inflammation.

Higgs as heavy-lifted physics during inflation

Signals of heavy particle production during inflation are encoded as nonanalytic momentum scaling in primordial non-Gaussianity. These non-analytic signatures can be sourced by Standard Model particles with a modified Higgs scale uplifted by the slow-roll dynamics of inflation. We show that such a lifting mechanism becomes more efficient with the presence of a strong Higgs-inflaton mixing, where the Higgs mass scale is further increased by a small speed of sound in the effective theory of inflation. As a primary step towards detecting new particles in the cosmological collider program, non-Gaussianity due to heavy Higgs production in the strong-mixing regime can act as important background signals to be tested by future cosmological surveys.

Functional roles and networks of non-coding RNAs in the pathogenesis of neurodegenerative diseases

Recent transcriptome analyses have revealed that noncoding RNAs (ncRNAs) are broadly expressed in mammalian cells and abundant in the CNS, with tissue and cell type-specific expression patterns. Moreover, ncRNAs have been found to intricately and dynamically regulate various signaling pathways in neurodegeneration. As such, some antisense transcripts and microRNAs are known to directly affect neurodegeneration in disease contexts. The functions of ncRNAs in pathogenesis are unique for each disorder, as are the pertinent networks of ncRNA/miRNA/mRNA that mediate these functions. Thus, further understanding of ncRNA biogenesis and effects might aid the discovery of diagnostic biomarkers or development of effective therapeutics for neurodegenerative disorders. Here, we review the ncRNAs that have so far been identified in major neurodegenerative disease etiology and the mechanisms that link ncRNAs with disease-specific phenotypes, such as HTT aggregation in HD, α-synuclein in PD, and Aβ plaques and hyperphosphorylated Tau in AD. We also summarize the known lncRNA/miRNA/mRNA networks that participate in neurodegenerative diseases, and we discuss ncRNA-related treatments shown to delay disease onset and prolong lifespan in rodent models.

Urban–rural health disparity among patients with chronic kidney disease: a cross-sectional community-based study from 2012 to 2019

ObjectivesThe incidence of chronic kidney disease (CKD) is increasing owing to the ageing population, resulting in an increased demand for dialysis and kidney transplantation, which can be costly. Current research lacks clarity regarding the relationship between residence setting and CKD prevalence or its related risk factors. This study explored the urban–rural disparities in CKD prevalence and risk factors in Taiwan. Our findings will aid the understanding of the distribution of CKD and the design of more effective prevention programmes.DesignThis cross-sectional community-based study used the Renal Value Evaluation Awareness and Lift programme, which involves early screening and health education for CKD diagnosis and treatment. CKD prevalence and risk factors including alcohol consumption, smoking and betel nut chewing were compared between urban and rural areas.SettingUrbanisation levels were determined based on population density, education, age, agricultural population and medical resources.ParticipantsA total of 7786 participants from 26 urban and 15 rural townships were included.ResultsThe prevalence of CKD was significantly higher in rural (29.2%) than urban (10.8%) areas, representing a 2.7-fold difference (p<0.0001). Risk factors including diabetes (rural vs urban: 21.7% and 11.0%), hypertension (59.0% vs 39.9%), hyperuricaemia (36.7% vs 18.6%), alcohol consumption (29.0% vs 19.5%), smoking (15.9% vs 12.0%), betel nut chewing (12.6% vs 2.8%) and obesity (33.6% vs 19.4%) were significantly higher (p<0.0001) in rural areas.ConclusionsThe prevalence of CKD is three times higher in rural versus urban areas. Despite >99% National Health Insurance coverage, disparities in CKD prevalence persist between residential areas. Targeted interventions and further studies are crucial for addressing these disparities and enhancing CKD management across different settings.

Constant-rate inflation: primordial black holes from conformal weight transitions

Constant-rate inflation, including ultra-slow-roll inflation as a special case, has been widely applied to the formation of primordial black holes with a significant deviation from the standard slow-roll conditions at both the growing and decaying phases of the power spectrum. We derive analytic solutions for the curvature perturbations with respect to the late-time scaling dimensions (conformal weights) constrained by the dilatation symmetry of the de Sitter background and show that the continuity of conformal weights across different rolling phases is protected by the adiabatic condition of the inflaton perturbation. The temporal excitation of subleading states (with the next-to-lowest conformal weights), recorded as the “steepest growth” of the power spectrum, is triggered by the entropy production in the transition from the slow-roll to the constant-rate phases.
