Effect of different doses of esketamine on the median effective concentration of propofol for inhibiting body movement during hysteroscopy

The objective of this study is to investigate the effects of various doses of esketamine on the median effective concentration (EC

) of propofol required for inhibiting body movement during hysteroscopy. Additionally, this research aims to explore the pharmacodynamic interactions between esketamine and propofol. ) group. For the initial patient in each group, the starting effector target concentration of propofol was set at 4 µg/ml. Each patient received an initial intravenous injection of 0.04 mg/kg midazolam, followed by the administration of the appropriate dose of esketamine. Ten seconds after the esketamine injection, propofol was administered intravenously to achieve the target concentration. In accordance with the sequential method principle, the concentration of propofol for the subsequent patient was adjusted based on the response of the previous patient. Effective inhibition of body movement was defined as the absence of any involuntary body movements throughout the entire surgical process. If the previous patient exhibited body movements, the propofol concentration for the next patient was increased by 0.5 µg/ml; conversely, if no movements were observed, it was decreased by 0.5 µg/ml. Chloroplast Genome Evolution in Four Montane Zingiberaceae Taxa in China

Chloroplasts are critical to plant survival and adaptive evolution. The comparison of chloroplast genomes could provide insight into the adaptive evolution of closely related species. To identify potential adaptive evolution in the chloroplast genomes of four montane Zingiberaceae taxa ( Cautleya , Roscoea , Rhynchanthus , and Pommereschea ) that inhabit distinct habitats in the mountains of Yunnan, China, the nucleotide sequences of 13 complete chloroplast genomes, including five newly sequenced species, were characterized and compared. The five newly sequenced chloroplast genomes (162,878–163,831 bp) possessed typical quadripartite structures, which included a large single copy (LSC) region, a small single copy (SSC) region, and a pair of inverted repeat regions (IRa and IRb), and even though the structure was highly conserved among the 13 taxa, one of the rps19 genes was absent in Cautleya , possibly due to expansion of the LSC region. Positive selection of rpoA and ycf2 suggests that these montane species have experienced adaptive evolution to habitats with different sunlight intensities and that adaptation related to the chloroplast genome has played an important role in the evolution of Zingiberaceae taxa.

Selection on grain shattering genes and rates of rice domestication

Molecular cloning of major quantitative trait loci (QTLs) responsible for the reduction of rice grain shattering, a hallmark of cereal domestication, provided opportunities for in-depth investigation of domestication processes. Here, we studied nucleotide variation at the shattering loci, sh4 and qSH1, for cultivated rice, Oryza sativa ssp. indica and Oryza sativa ssp. japonica, and the wild progenitors, Oryza nivara andOryza rufipogon. The nonshattering sh4 allele was fixed in all rice cultivars, with levels of sequence polymorphism significantly reduced in both indica and japonica cultivars relative to the wild progenitors. The sh4 phylogeny together with the neutrality tests and coalescent simulations suggested that sh4 had a single origin and was fixed by artificial selection during the domestication of rice. Selection on qSH1 was not detected in indica and remained unclear in japonica. Selection on sh4 could be strong enough to have driven its fixation in a population of cultivated rice within a period of c. 100 yr. The slow fixation of the nonshattering phenotype observed at the archeological sites might be a result of relatively weak selection on mutations other than sh4 in early rice cultivation. The fixation of sh4 could have been achieved later through strong selection for the optimal phenotype.

Multi‐genic pattern found in rare type of hypopituitarism: a whole‐exome sequencing study of Han Chinese with pituitary stalk interruption syndrome

Pituitary stalk interruption syndrome (PSIS) is a rare type of hypopituitarism manifesting various degrees of pituitary hormone deficiency. Although mutations have been identified in some familial cases, the underpinning mechanisms of sporadic patients with PSIS who are in a vast majority remain elusive, necessitating a comprehensive study using systemic approaches. We postulate that other genetic mechanisms may be responsible for the sporadic PSIS. To test this hypothesis, we conducted a study in 24 patients with PSIS of Han Chinese with no family history using whole‐exome sequencing (WES) and bioinformatic analysis. We identified a group of heterozygous mutations in 92% (22 of 24) of the patients, and these genes are mostly associated with Notch, Shh, Wnt signalling pathways. Importantly, 83% (20 of 24) of the patients had more than one mutation in those pathways suggesting synergy of compound mutations underpin the pathogenesis of sporadic PSIS.

Introduction Very few studies have investigated the specific relationship between neutrophil‐to‐lymphocyte ratio (NLR) and the short‐term outcomes of patients suffering from mild acute ischemic stroke (AIS) and receiving intravenous thrombolysis (IVT). This study aimed to investigate whether a high NLR is associated with a poor short‐term outcome in patients with mild AIS after IVT. Methods We retrospectively analyzed data that were prospectively acquired from patients with AIS treated with IVT. Mild AIS was defined as a National Institutes of Health Stroke Scale (NIHSS) score ≤ 7 on admission. The NLR was based on a blood test performed prior to IVT and was classified as ‘high’ when exceeding the 75th percentile. Follow‐ups were performed at discharge and 3 months after onset. A poor outcome was defined as a modified Rankin scale (mRS) ≥3. Results A total of 192 patients were included in this study. The median NLR was 3.0 (interquartile range [IQR]: 2.0–3.9). Fifty‐one patients (26.6%) had a high NLR (≥3.9) on admission. Forty‐one patients (21.4%) had a poor outcome at discharge, while 34 patients (17.7%) had a poor outcome at 3 months. Patients with a poor outcome at discharge, and at 3 months after onset, were more likely to have a high NLR at discharge (42.9% vs. 21.9%; p = .005) and at 3 months (44.1% vs. 22.8%; p = .011), compared with those with a better outcome. After adjustment for NIHSS score on admission, ipsilateral severe intracranial large artery occlusion, and atrial fibrillation, logistic regression analyses revealed that a high NLR was a significant predictor of poor outcome at discharge and at 3 months after onset. Conclusions A high NLR on admission could be a useful marker for predicting poor short‐term outcome in patients with mild AIS following IVT. To the best of our knowledge, few studies have focused on the specific relationship between neutrophil‐to‐lymphocyte ratio (NLR) and the short‐term prognosis of patients with mild acute ischemic stroke (AIS) and treated with intravenous thrombolysis (IVT). Our study evaluate whether an increased NLR was predictive of a poor short‐term outcome in patients suffering from mild AIS and treated with IVT. A high NLR (≥3.9) was associated with a poor outcome in patients with mild AIS treated with IVT, at both discharge and 3 months after onset.

Seasonal dynamics of the juvenile fish community structure in the Maowei Sea mangroves

More than 50% of Chinese mangroves were lost between 1950 and 2000 to habitat destruction, prompting an urge for conservation. To assess the importance of the protected Maowei Gulf mangrove estuary for fish population assemblage in the Beibu Gulf (China), we studied species composition and abundance of juvenile fish (including larvae) from July 2012 to June 2013. A total of 11 691 specimens were collected, which belonged to 24 species and 15 families. Six perciform species constituted 93% of the total sample. Pseudogobius javanicus (53.29%) was the dominant species from August to November, Omobranchus elegans (28.49%) from April to July, non-identified species in December and January, and Liza carinata in February and March. A number of commercially important fish species were also identified. Abundance was the highest in summer/early autumn (max 162.4 in Sep), and lowest in winter/early spring (Mar = 4.5). Diversity (H') and richness (Dma) indices (both max. in May: 1.67 and 1.95 respectively) were generally positively correlated with tide and temperature, and negatively with salinity. Seasonal variations play a more important role in the fish assemblage structure than tidal rhythm, with differences particularly pronounced between colder and warmer months. Despite the prominent seasonal differences in abiotic factors, this study indicates that Maowei mangroves provide habitat and food for juvenile fish throughout the year and thus are indispensable for the fish diversity in the Beibu Gulf.

Organization, Phylogenetic Marker Exploitation, and Gene Evolution in the Plastome of Thalictrum (Ranunculaceae)

Thalictrum is a phylogenetically and economically important genus in the family Ranunculaceae, but is also regarded as one of the most challengingly difficult in plants for resolving the taxonomical and phylogenetical relationships of constituent taxa within this genus. Here, we sequenced the complete plastid genomes of two Thalictrum species using Illumina sequencing technology via de novo assembly. The two Thalictrum plastomes exhibited circular and typical quadripartite structure that was rather conserved in overall structure and the synteny of gene order. By updating the previously reported plastome annotation of other nine Thalictrum species, we found that the expansion or contraction of the inverted repeat region affect the boundary of the single-copy regions in Thalictrum plastome. We identified eight highly variable noncoding regions— infA-rps8 , ccsA-ndhD , trnS UGA -psbZ , trnH GUG -psbA , rpl16-rps3 , ndhG-ndhI , ndhD-psaC , and ndhJ-ndhK —that can be further used for molecular identification, phylogenetic, and phylogeographic in different species. Selective pressure and codon usage bias of all the plastid coding genes were also analyzed for the 11 species. Phylogenetic relationships showed Thalictrum is monophyly and divided into two major clades based on 11 Thalictrum plastomes. The availability of these plastomes offers valuable genetic information for accurate identification of species and taxonomy, phylogenetic resolution, and evolutionary studies of Thalictrum , and should assist with exploration and utilization of Thalictrum plants.

Semi-extended intramedullary nailing of the tibia using an infrapatellar approach: a retrospective cohort study

Purpose
The semi-extended tibial intramedullary nailing method would enable easier and improved reductions for tibial fractures as well as facilitate fluoroscopic imaging; however, its in-articular nature remains controversial. The aim of this study was to compare the clinical and functional outcomes of the semi-extended infrapatellar (SEIP) approach and hyper-flexed infrapatellar (HFIP) approach for intramedullary nailing to treat tibial shaft fractures.
Methods
This study involved the retrospective analysis of the medical records of patients with tibial shaft fractures that were fixed through either the SEIP approach or the HFIP approach of intramedullary nailing and who were admitted to a level 1 trauma centre. The minimum patient follow-up period was 12 months and the clinical and functional outcomes were estimated at the 12-month visit.
Results
Of the 80 patients whose medical records were analyzed, 40 (50%) underwent SEIP nailing and the remaining 40 (50%) underwent the traditional HFIP nailing. Compared with the HFIP group, patients in the SEIP group had a higher Lysholm knee score (median, 92 [interquartile range, 88–95] vs median, 88 [interquartile range, 81–92];
p
= .01), a shorter intra-operative fluoroscopy time (median, 93 [interquartile range, 78–105] s, vs median, 136 [interquartile range, 110–157] s;
p
< .001), and operation time (mean, 88.1 [SD, 17.8] min vs mean, 98.7 [SD, 19.3] min;
p
= .01). The VAS score was significantly lower in the SEIP group (median, 0; interquartile range, 0–0) than in the HFIP group (median, 0; interquartile range, 0–2) (

= .03). There were two cases (5%) in the SEIP group and 10 cases (25%) in the HFIP group of moderate AKP (

= .03). Meanwhile, there was no significant difference in malalignment, nonunion, delayed union, infection, and other complications, as well as SF-36 physical and mental scores.
Conclusion
We found that the SEIP approach to tibia intramedullary nailing was superior to the HFIP approach based on the intra-operative and post-operative outcomes. Thus, this novel technique provides an infrapatellar option for semi-extended tibial nailing.

Proteasome Subunits Regulate Reproduction in Nilaparvata lugens and the Transovarial Transmission of Its Yeast-like Symbionts

The brown planthopper, Nilaparvata lugens, a major rice pest, harbors yeast-like symbionts (YLSs) that form mutualistic relationships with the host, significantly influencing its development and reproduction. As proteasome subunits play major roles in the assembly and functional maintenance of the proteasome, but their regulation on the YLSs in N. lugens are unclear. In this study, we analyzed the spatiotemporal and temporal expression patterns of five N. lugens proteasome subunits (NlPSMA2, NlPSMB5, NlPSMC4, NlPSMD10, NlPSMD13), and further verified their functions on the transovarial transmission of YLSs, in addition to the reproduction of N. lugens, based on RNA interference (RNAi). The results showed that NlPSMA2, NlPSMB5, NlPSMC4, NlPSMD10, and NlPSMD13 were highly expressed in ovarian follicular cells of N. lugens upon sexual maturation. After suppressing the expression of these genes by RNAi, N. lugens exhibited a shortened lifespan, abnormal pear-shaped follicles, and impaired oviposition capacity, but the number of YLSs in the whole body and the oocyte of N. lugens were significantly increased. These results indicate that the proteasome subunits play crucial roles in the reproduction of N. lugens and the transovarial transmission of its YLSs.

ObjectiveThe spinal block was limited to the operative side during unilateral spinal anesthesia, which has less physiological interference and fewer complications for the patient. The optimal dose of ropivacaine for unilateral spinal anesthesia is still unclear. The aim of this trial was to investigate the ED50 and ED95 of hypobaric ropivacaine during unilateral spinal anesthesia in older patients undergoing hip arthroplasty.MethodsAll patients were administered hypobaric ropivacaine at a spinal anesthetic drug concentration of 0.4%. The trial was conducted using the Dixon sequential method with an initial dose of 10 mg of ropivacaine and a dose difference of 0.5 mg between two consecutive patients. If the subject’s post-block non-surgical leg Bromage score is greater than 0, the next subject’s dose of ropivacaine is reduced by 0.5 mg. If the subject’s post-block non-surgical leg Bromage score is equal to 0, the next patient’s dose of ropivacaine is increased by 0.5 mg. The trial was terminated when alternating positive–negative results were obtained for 7 pairs of patients. ED50, ED95, and the corresponding 95% confidence intervals (CI) for unilateral spinal anesthesia with 0.4% hypobaric ropivacaine were calculated using probit regression analysis. Patients with motor block in the non-surgical leg were identified as the positive group, and patients without motor block in the non-surgical leg were identified as the negative group. Anesthesia onset time, sensory block duration, duration of motor block, the highest dermatomes blocked on the surgical side, and postoperative complications were compared between the two groups.ResultsThe ED50 and ED95 of hypobaric ropivacaine during unilateral spinal anesthesia in older patients undergoing hip arthroplasty were 11.13 mg (95% CI: 10.85–11.42 mg) and 10.30 mg (95% CI: 9.04–10.65 mg), respectively. The drug dosage was higher in the positive group than in the negative group. There was no differences in the onset of anesthesia, block plane of the affected side, the sensory and motor block time of affected side, and the incidences of low blood pressure, nausea and vomiting, chills, urinary retention between groups ( p > 0.05).ConclusionThe ED50 and ED95 of hypobaric ropivacaine during unilateral spinal anesthesia in older patients undergoing hip arthroplasty were 11.13 mg (95% CI: 10.85–11.42 mg) and 10.30 mg (95% CI: 9.04–10.65 mg), respectively. Unilateral spinal anesthesia limits the level of block to the operative side with less physiologically disruptive to the patient, more stable perioperative hemodynamics and fewer complications.
