Background
Unilateral laminotomy for bilateral decompression (ULBD) has yielded positive results in the treatment of lumbar spinal stenosis (LSS). Unilateral biportal ULBD (UB-ULBD) and percutaneous endoscopic ULBD (PE-ULBD) are gaining popularity because of the progress that has been made in minimally invasive surgery (MIS). The objective of this study was to evaluate and compare the radiographic and clinical results of UB-ULBD and PE-ULBD.
Methods
This study retrospectively enrolled patients who underwent ULBD surgery for LSS. The patients were categorized into two groups on the basis of the surgical method: the UB-ULBD group and the PE-ULBD group. Data on the general demographic data, surgical details, clinical efficacy, radiography and complications were compared between the two groups were compared. The minimum follow-up duration was 12 months.
Results
A total of 113 LSS patients who had undergone ULBD at our institution were included, of whom 61 patients underwent UB-ULBD surgery and 52 underwent PE-ULBD surgery. The UB-ULBD group had a significantly shorter operation time (
P
< 0.05). The facet was significantly better preserved in the UB-ULBD group than in the PE-ULBD group, and the angle of ipsilateral facet joint resection in the UE-ULBD group was significantly smaller (
P
< 0.05). The ODI score, VAS score and modified Macnab criteria improved postoperatively in both groups. The UB-ULBD group had a 95.08% rate of excellent or good patient outcomes, whereas the PE-ULBD group had a 92.30% rate.
Conclusion
Both UB-ULBD and PE-ULBD can provide favourable clinical outcomes when used to treat LSS. UB-ULBD is beneficial because of its shorter operation time, smaller angle of ipsilateral facet joint resection and better facet preservation, making it a viable and safe option for treating LSS while ensuring spinal stability.

“U” route transforaminal percutaneous endoscopic thoracic discectomy as a new treatment for thoracic spinal stenosis

Purposes
To describe the rationale, surgical technique, and short-term follow-up results of a new minimally invasive treatment for thoracic spinal stenosis (TSS) caused by herniation, ossification of the ligamentum flavum (OLF), and/or ossification of the posterior longitudinal ligament (OPLL) with a “U” route transforaminal percutaneous endoscopic thoracic discectomy (PETD).
Methods
Fourteen patients, including seven males and seven females, underwent “U” route PETD. Myelopathy was caused by OLF in 14 patients, OPLL in one, combined OLF-OPLL in ten, and intervertebral disc herniation (IDH) in five. Decompression was performed in one segment in 12 patients, and in two segments in two patients. The Japanese Orthopedic Association (JOA) scores, visual analog scale (VAS) scores, and complications were documented.
Results
The JOA scores improved from 4.64 ± 2.31 pre-operatively to 7.07 ± 1.59 one day post-operatively and 11.79 ± 1.85 at final follow-up. The difference between pre-operation and post-operation was statistically significant (
P
< 0.05). Moreover, the VAS score was 6.07 ± 2.06 points pre-operatively, decreasing to 3.00 ± 1.24 points at one day post-operatively, and 1.14 ± 0.86 points at last follow-up (
P
< 0.05). Dural tear was observed in two cases during the intervention. No patient had transient worsening of pre-operative paralysis.
Conclusions
This retrospective analysis shows that “U” route PETD for decompression may be a feasible alternative to treat thoracic spinal stenosis.

Surgical results and prognostic factors following percutaneous full endoscopic posterior decompression for thoracic myelopathy caused by ossification of the ligamentum flavum

Minimally invasive surgery (MIS) has shown satisfactory surgical results for the treatment of thoracic myelopathy (TM) caused by ossification of the ligamentum flavum (OLF). This study investigated the prognostic factors following MIS and was based on the retrospective analysis of OLF patients who underwent percutaneous full endoscopic posterior decompression (PEPD). Thirty single-segment OLF patients with an average age of 60.4 years were treated with PEPD under local anaesthesia. Clinical data were collected from the medical and operative records. The surgical results were assessed by the recovery rate (RR) calculated from the modified Japanese Orthopaedic Association (mJOA) score. Correlations between the RR and various factors were analysed. Patients’ neurological status improved from a preoperative mJOA score of 6.0 ± 1.3 to a postoperative mJOA score of 8.5 ± 2.0 (

< 0.001) at an average follow-up of 21.3 months. The average RR was 53.8%. Dural tears in two patients (6.7%, 2/30) were the only observed complications. Multiple linear regression analysis showed that a longer duration of preoperative symptoms and the presence of a high intramedullary signal on T2-weighted MRI (T2HIS) were significantly associated with poor surgical results. PEPD is feasible for the treatment of TM patients with a particular type of OLF. Patients without T2HIS could achieve a good recovery if they received PEPD early.

Positive-Sum Game-Based Capacity Configuration Planning Method of Solar-Wind-Battery System Designed for Black-Start

For achieving the carbon peaking and carbon neutrality goals, the renewable energy and the battery energy storage will be developed more rapidly. The participation of the solar-wind-battery renewable energy system (SWBS) in black-start can improve the resilience of the power grid. Aiming at the balance between the capacity demand of SWBS participating in black-start and the benefit of SWBS, this paper proposes a positive-sum game-based SWBS capacity configuration planning method. First, a levelized cost of energy- (LCOE-) based full life cycle index is presented and constructs an economic benefit model of SWBS considering the participation of wind and solar power plants in carbon trading. Second, a black-start capacity demand model is constructed considering the continuous power demand on the auxiliary devices of the operated thermal power generator. Taking the economic benefit of SWBS and the black-start capacity requirement into account, this paper establishes a positive-sum game-based SWBS capacity optimal configuration model, in which maximizing the economic benefits of SWBS as an objective. Third, the simulation analysis is carried out by using a real power system, and results verify that the proposed method can achieve the optimal balance between the feasibility of black-start and the economic benefit of the SWBS.

Gelatin Methacrylic Acid Hydrogel-Based Nerve Growth Factors Enhances Neural Stem Cell Growth and Differentiation to Promote Repair of Spinal Cord Injury

The challenge in treating irreversible nerve tissue damage has resulted in suboptimal outcomes for spinal cord injuries (SCI), underscoring the critical need for innovative treatment strategies to offer hope to patients.
In this study, gelatin methacrylic acid hydrogel scaffolds loaded with nerve growth factors (GMNF) were prepared and used to verify the performance of SCI. The physicochemical and biological properties of the GMNF were tested. The effect of GMNF on activity of neuronal progenitor cells (NPCs) was investigated in vitro. Histological staining and motor ability was carried out to assess the ability of SCI repair in SCI animal models.
Achieving nerve growth factors sustained release, GMNF had good biocompatibility and could effectively penetrate into the cells with good targeting permeability. GMNF could better enhance the activity of NPCs and promote their directional differentiation into mature neuronal cells in vitro, which could exert a good neural repair function. In vivo, SCI mice treated with GMNF recovered their motor abilities more effectively and showed better wound healing by macroscopic observation of the coronal surface of their SCI area. Meanwhile, the immunohistochemistry demonstrated that the GMNF scaffolds effectively promoted SCI repair by better promoting the colonization and proliferation of neural stem cells (NSCs) in the SCI region and targeted differentiation into mature neurons.
The application of GMNF composite scaffolds shows great potential in SCI treatment, which are anticipated to be a potential therapeutic bioactive material for clinical application in repairing SCI in the future.

Wind speed model based on kernel density estimation and its application in reliability assessment of generating systems

An accurate probability distribution model of wind speed is critical to the assessment of reliability contribution of wind energy to power systems. Most of current models are built using the parametric density estimation (PDE) methods, which usually assume that the wind speed are subordinate to a certain known distribution (e.g. Weibull distribution and Normal distribution) and estimate the parameters of models with the historical data. This paper presents a kernel density estimation (KDE) method which is a nonparametric way to estimate the probability density function (PDF) of wind speed. The method is a kind of data-driven approach without making any assumption on the form of the underlying wind speed distribution, and capable of uncovering the statistical information hidden in the historical data. The proposed method is compared with three parametric models using wind data from six sites. The results indicate that the KDE outperforms the PDE in terms of accuracy and flexibility in describing the long-term wind speed distributions for all sites. A sensitivity analysis with respect to kernel functions is presented and Gauss kernel function is proved to be the best one. Case studies on a standard IEEE reliability test system (IEEE-RTS) have verified the applicability and effectiveness of the proposed model in evaluating the reliability performance of wind farms.

Analysis of Emergency Demand Response Levels of Central Air-Conditioning

Central air-conditioning (CAC) is a flexible load which can be dispatched during an emergency demand response (EDR) program. However, consumers’ trade-off between thermal comfort levels (TCLs) and profits will affect the potential of CAC. Therefore, this paper proposes a method to evaluate CAC’S response levels. An EDR optimization model simulating consumers’ trade-off between TCLs and profits is established and it is solved by a particle swarm optimization (PSO) algorithm. Then the definition of EDR levels is presented to quantitatively analyze the capability of CAC providing the EDR. Furthermore, uncertainty of EDR levels caused by stochastic initial indoor temperature is analyzed through a Monte Carlo (MC) simulation. A case study shows the rationality of the presented method, and the effects of weight coefficients, compensation prices and penalty prices on EDR levels are also analyzed.

A Method for Load Classification and Energy Scheduling Optimization to Improve Load Reliability

With the large amount of distributed generation in use, the structure of the distribution system is increasingly complex. Therefore, it is necessary to establish a method to improve load reliability. Based on the reliability model of distributed generation, this paper investigates the time sequential simulation of a wind/solar/storage combined power supply system under off-grid operation. After classifying the load by power supply region, the load weight coefficient is established, which modifies the reliability index of the load point and system. The modified expected energy not supplied (EENS) is adopted as the objective function, and the particle swarm optimization algorithm is used to solving the optimal energy scheduling for improving the load reliability. Finally, the load reliability is calculated with a hybrid method. Using the IEEE-RBTS Bus 6 system as an example, the correctness and validity of the proposed method are verified as an effective way to improve load reliability.

Mitofusin 2 Protects Hepatocyte Mitochondrial Function from Damage Induced by GCDCA

Mitochondrial impairment is hypothesized to contribute to the pathogenesis of chronic cholestatic liver diseases. Mitofusin 2 (Mfn2) regulates mitochondrial morphology and signaling and is involved in the development of numerous mitochondrial-related diseases; however, a functional role for Mfn2 in chronic liver cholestasis which is characterized by increased levels of toxic bile acids remain unknown. Therefore, the aims of this study were to evaluate the expression levels of Mfn2 in liver samples from patients with extrahepatic cholestasis and to investigate the role Mfn2 during bile acid induced injury in vitro. Endogenous Mfn2 expression decreased in patients with extrahepatic cholestasis. Glycochenodeoxycholic acid (GCDCA) is the main toxic component of bile acid in patients with extrahepatic cholestasis. In human normal hepatocyte cells (L02), Mfn2 plays an important role in GCDCA-induced mitochondrial damage and changes in mitochondrial morphology. In line with the mitochondrial dysfunction, the expression of Mfn2 decreased significantly under GCDCA treatment conditions. Moreover, the overexpression of Mfn2 effectively attenuated mitochondrial fragmentation and reversed the mitochondrial damage observed in GCDCA-treated L02 cells. Notably, a truncated Mfn2 mutant that lacked the normal C-terminal domain lost the capacity to induce mitochondrial fusion. Increasing the expression of truncated Mfn2 also had a protective effect against the hepatotoxicity of GCDCA. Taken together, these findings indicate that the loss of Mfn2 may play a crucial role the pathogenesis of the liver damage that is observed in patients with extrahepatic cholestasis. The findings also indicate that Mfn2 may directly regulate mitochondrial metabolism independently of its primary fusion function. Therapeutic approaches that target Mfn2 may have protective effects against hepatotoxic of bile acids during cholestasis.

The electrical system of a wind farm has a significant influence on the wind farm reliability and electrical energy yield. The disconnect switch installed in an electrical system cannot only improve the operating flexibility, but also enhance the reliability for a wind farm. Therefore, this paper develops a probabilistic transfer technique for integrating the electrical topology structure, the isolation operation of disconnect switch, and stochastic failure of electrical equipment into the reliability assessment of wind farm electrical system. Firstly, as the traditional two-state reliability model of electrical equipment cannot consider the isolation operation, so the paper develops a three-state reliability model to replace the two-state model for incorporating the isolation operation. In addition, a proportion apportion technique is presented to evaluate the state probability. Secondly, this paper develops a probabilistic transfer technique based on the thoughts that through transfer the unreliability of electrical system to the energy transmission interruption of wind turbine generators (WTGs). Finally, some novel indices for describing the reliability of wind farm electrical system are designed, and the variance coefficient of the designed indices is used as a convergence criterion to determine the termination of the assessment process. The proposed technique is applied to the reliability assessment of a wind farm with the different topologies. The simulation results show that the proposed techniques are effective in practical applications.
