This paper investigates how non-cognitive skills relate to the relative labour market performance of immigrants. Using the German Socio-Economic Panel (SOEP) and the Five-Factor Model of personality as a proxy for the non-cognitive skills, we show that these skills matter for the labour market integration of immigrants in the host country. We use two comparison benchmarks. Compared to an average native, immigrants’ non-cognitive skills, e.g., extroversion or emotional stability, can lead to 5–15 percentage points lower lifetime employment probability disadvantage implying a better overall integration. Comparing immigrants and natives with the same type and level of non-cognitive skills suggests that returns of extroversion and openness to experience are higher among immigrants, leading to 3–5 percentage points lower lifetime employment probability disadvantage. These results are robust with respect to self-selection, non-random returns to the home country, stability of personality, and estimators. Our detailed analysis suggests that non-cognitive skills (especially extroversion) are substitutes for the standard human capital measures (e.g., formal education and training) among low educated immigrants, while there is no significant relative return of non-cognitive skills among highly educated immigrants.

Online food ordering delivery strategies based on deep reinforcement learning

With the rapid development of Online to Offline (O2O) business, millions of transactions are performed on the popular online food ordering platforms each day. Efficient dispatching of orders and dynamic adjustment of delivery routes are critical to the success of the O2O platforms. However, the vast volume of transactions and the computational complexity of delivery routes pose significant challenges to the efficient dispatching of orders. The action to dispatch orders and the resulting state transition of couriers form a Markov decision process (MDP). The reinforcement learning technique had proven its capability of dealing with MDP. This paper proposes a Double Deep Q Netwok (DQN) based reinforcement learning framework that gradually tests and learns the order dispatching policy by communicating with an O2O simulation model developed by SUMO. The preliminary experimental results using the real order data demonstrate the effectiveness and efficiency of the proposed Double-DQN based order dispatcher. Also, different state encoding schemes are designed and tested to improve the performance of the Double-DQN based dispatcher.

The objective of this study is to evaluate the effectiveness of the natural flotation process in reducing pollution with reasonable investment and operating costs of an industrial effluent of refining vegetable oils. Flotation tests were carried out in separating funnels and in drums of 30 l. The results obtained have shown that the volume of sludge produced during flotation is related to the pollutant load of the wastewater studied (process wastewater and acidic wastewater). The sludge volume is respectively 600, 12, and 120 ml/l for heavy, light, and medium loads respectively. Therefore, it is essential to find an effective way to remove oils and greases from polluted waters. Natural flotation eliminates on average 88% of COD for acid wastewater and 50% for process wastewater. However, the reduction of BOD5 showed 28 and 43% respectively for acid wastewater and process wastewater. In addition, the yield of fats and oils, TSS, and turbidity varies around 85%, 45%, and 88% respectively for acidic wastewater, while for process wastewater, elimination yields vary around 58%, 46%, and 46% respectively for the grease and oil parameters, the MES, and the turbidity. Flotation allows the elimination of 10659 Kg/day of greases and oils for process wastewater while it eliminates 5765 Kg/day for acidic wastewater. This reduces the cost of treatment related to chemicals and energy. Reducing pollution of wastewater by natural flotation could therefore help reduce the costs of treating wastewater, and recycling would then be more attractive for this purpose for the company.

The Importance of Nursing Homes in the Spread of Methicillin-resistant Staphylococcus aureus (MRSA) Among Hospitals

Background: Hospital infection control strategies and programs may not consider control of methicillin-resistant Staphylococcus aureus (MRSA) in nursing homes in a county. Methods: Using our Regional Healthcare Ecosystem Analyst, we augmented our existing agent-based model of all hospitals in Orange County (OC), California, by adding all nursing homes and then simulated MRSA outbreaks in various health care facilities. Results: The addition of nursing homes substantially changed MRSA transmission dynamics throughout the county. The presence of nursing homes substantially potentiated the effects of hospital outbreaks on other hospitals, leading to an average 46.2% (range, 3.3%—156.1%) relative increase above and beyond the impact when only hospitals are included for an outbreak in OC's largest hospital. An outbreak in the largest hospital affected all other hospitals (average 2.1% relative prevalence increase) and the majority (∼90%) of nursing homes (average 3.2% relative increase) after 6 months. An outbreak in the largest nursing home had effects on multiple OC hospitals, increasing MRSA prevalence in directly connected hospitals by an average 0.3% and in hospitals not directly connected through patient transfers by an average 0.1% after 6 months. A nursing home outbreak also had some effect on MRSA prevalence in other nursing homes. Conclusions: Nursing homes, even those not connected by direct patient transfers, may be a vital component of a hospital's infection control strategy. To achieve effective control, a hospital may want to better understand how regional nursing homes and hospitals are connected through both direct and indirect (with intervening stays at home) patient sharing.

Point-of-care lung ultrasound in children with community acquired pneumonia

To present lung ultrasound findings in children assessed with suspected pneumonia in the emergency department and to show the benefit of lung ultrasound in diagnosing pneumonia in comparison with chest X-rays.
This observational prospective study was performed in the pediatric emergency department of a single center. Point of care lung ultrasound was performed on each child by an independent sonographer blinded to the patient's clinical and chest X-ray findings. Community acquired pneumonia was established as a final diagnosis by two clinicians based on the recommendations in the British Thoracic Society guideline.
One hundred sixty children with a mean age of 3.3±4years and a median age of 1.4years (min–max 0.08–17.5years) were investigated. Final diagnosis in 149 children was community-acquired pneumonia. Lung ultrasound findings were compatible with pneumonia in 142 (95.3%) of these 149 children, while chest X-ray findings were compatible with pneumonia in 132 (88.5%). Pneumonia was confirmed with lung ultrasound in 15 of the 17 patients (11.4%) not evaluated as compatible with pneumonia at chest X-ray. While pneumonia could not be confirmed with lung ultrasound in seven (4.6%) patients, findings compatible with pneumonia were not determined at chest X-ray in two of these patients. When lung ultrasound and chest X-ray were compared as diagnostic tools, a significant difference was observed between them (p=0.041).
This study shows that lung ultrasound is at least as useful as chest X-ray in diagnosing children with community-acquired pneumonia.

Long-Term Care Facilities: Important Participants of the Acute Care Facility Social Network?

Acute care facilities are connected via patient sharing, forming a network. However, patient sharing extends beyond this immediate network to include sharing with long-term care facilities. The extent of long-term care facility patient sharing on the acute care facility network is unknown. The objective of this study was to characterize and determine the extent and pattern of patient transfers to, from, and between long-term care facilities on the network of acute care facilities in a large metropolitan county.
We applied social network constructs principles, measures, and frameworks to all 2007 annual adult and pediatric patient transfers among the healthcare facilities in Orange County, California, using data from surveys and several datasets. We evaluated general network and centrality measures as well as individual ego measures and further constructed sociograms. Our results show that over the course of a year, 66 of 72 long-term care facilities directly sent and 67 directly received patients from other long-term care facilities. Long-term care facilities added 1,524 ties between the acute care facilities when ties represented at least one patient transfer. Geodesic distance did not closely correlate with the geographic distance among facilities.
This study demonstrates the extent to which long-term care facilities are connected to the acute care facility patient sharing network. Many long-term care facilities were connected by patient transfers and further added many connections to the acute care facility network. This suggests that policy-makers and health officials should account for patient sharing with and among long-term care facilities as well as those among acute care facilities when evaluating policies and interventions.

A Retrospective Cohort Study of Traumatic Brain Injury in Children: A Single-Institution Experience and Determinants of Neurologic Outcome

Traumatic brain injury (TBI) has become a significant cause of death and morbidity in childhood since the elucidation of infectious causes within the last century. Mortality rates in this population decreased over time due to developments in technology and effective treatment modalities.This retrospective cohort study aimed to describe the volume, severity and mechanism of all hospital-admitted pediatric TBI patients at a university hospital over a 5-year period.This was a single-center, retrospective cohort study including 90 pediatric patients with TBI admitted to a tertiary care PICU. The patients’ demographic data, injury mechanisms, disease and trauma severity scores, initiation of enteral nutrition and outcome measures such as hospital stay, PICU stay, duration of mechanical ventilation, mortality, and Glasgow Outcome Scale (GOS) were also recorded. Late enteral nutrition was defined as initiation of enteral feeding after 48 hours of hospitalization.Of the 90 patients included in the cohort, 60% had mild TBI, 21.1% had moderate TBI and 18.9% had severe TBI. Their mean age was 69 months (3–210 months). TBI was isolated in 34 (37.8%) patients and observed as a part of multisystemic trauma in 56 (62.2%). The most commonly involved site in multisystemic injury was the thorax (33.3%). The length of hospitalization in the late enteral nutrition group was significantly higher than that in the early nutrition group, while the PICU stay was not significantly different between the two groups. The multiple logistic regression analysis found a significant relationship between GOS-3rd month and PIM3 score, the presence of diffuse axonal injury and the need for CPR in the first 24 h of hospitalization.Although our study showed that delayed enteral nutrition did not affect neurologic outcome, it may lead to prolonged hospitalization and increased hospital costs. High PIM3 scores and diffuse axonal injury are both associated with worse neurologic outcomes.
