Impact of high-power short-duration atrial fibrillation ablation technique on the incidence of silent cerebral embolism: a prospective randomized controlled study

Background
High-power short-duration (HPSD) ablation strategy has emerged as a popular approach for treating atrial fibrillation (AF), with shorter ablation time. The utilized Smart Touch Surround Flow (STSF) catheter, with 56 holes around the electrode, lowers electrode-tissue temperature and thrombus risk. Thus, we conducted this prospective, randomized study to investigate if the HPSD strategy with STSF catheter in AF ablation procedures reduces the silent cerebral embolism (SCE) risk compared to the conventional approach with the Smart Touch (ST) catheter.
Methods
From June 2020 to September 2021, 100 AF patients were randomized 1:1 to the HPSD group using the STSF catheter (power set at 50 W) or the conventional group using the ST catheter (power set at 30 to 35 W). Pulmonary vein isolation was performed in all patients, with additional lesions at operator’s discretion. High-resolution cerebral diffusion-weighted magnetic resonance imaging (hDWI) with slice thickness of 1 mm was performed before and 24–72 h after ablation. The incidence of new periprocedural SCE was defined as the primary outcome. Cognitive performance was assessed using the Montreal Cognitive Assessment (MoCA) test.
Results
All enrolled AF patients (median age 63, 60% male, 59% paroxysmal AF) underwent successful ablation. Post-procedural hDWI identified 106 lesions in 42 enrolled patients (42%), with 55 lesions in 22 patients (44%) in the HPSD group and 51 lesions in 20 patients (40%) in the conventional group (

= 0.685). No significant differences were observed between two groups regarding the average number of lesions (

= 0.751), maximum lesion diameter (

= 0.405), and total lesion volume per patient (

Conclusions
Involving a small sample size of 100 AF patients, this study reveals a similar incidence of SCE in AF ablation procedures, comparing the HPSD strategy using the STSF catheter to the conventional approach with the ST catheter.

TGF‐β isoforms inhibit hepatitis C virus propagation in transforming growth factor beta/SMAD protein signalling pathway dependent and independent manners

Transforming growth factor beta (TGF‐β) plays an important role in the viral liver disease progression via controlling viral propagation and mediating inflammation‐associated responses. However, the antiviral activities and mechanisms of TGF‐β isoforms, including TGF‐β1, TGF‐β2 and TGF‐β3, remain unclear. Here, we demonstrated that all of the three TGF‐β isoforms were increased in Huh7.5 cells infected by hepatitis C virus (HCV), but in turn, the elevated TGF‐β isoforms could inhibit HCV propagation with different potency in infectious HCV cell culture system. TGF‐β isoforms suppressed HCV propagation through interrupting several different stages in the whole HCV life cycle, including virus entry and intracellular replication, in TGF‐β/SMAD signalling pathway–dependent and TGF‐β/SMAD signalling pathway–independent manners. TGF‐β isoforms showed additional anti‐HCV activities when combined with each other. However, the elevated TGF‐β1 and TGF‐β2, not TGF‐β3, could also induce liver fibrosis with a high expression of type I collagen alpha‐1 and α‐smooth muscle actin in LX‐2 cells. Our results showed a new insight into TGF‐β isoforms in the HCV‐related liver disease progression.

The Stability, Sustained Release and Cellular Antioxidant Activity of Curcumin Nanoliposomes

Curcumin is a multifunctional and natural agent considered to be pharmacologically safe. However, its application in the food and medical industry is greatly limited by its poor water solubility, physicochemical instability and inadequate bioavailability. Nanoliposome encapsulation could significantly enhance the solubility and stability of curcumin. Curcumin nanoliposomes exhibited good physicochemical properties (entrapment efficiency = 57.1, particle size = 68.1 nm, polydispersity index = 0.246, and zeta potential = −3.16 mV). Compared with free curcumin, curcumin nanoliposomes exhibited good stability against alkaline pH and metal ions as well as good storage stability at 4 °C. Curcumin nanoliposomes also showed good sustained release properties. Compared with free curcumin, curcumin nanoliposomes presented an equal cellular antioxidant activity, which is mainly attributed to its lower cellular uptake as detected by fluorescence microscopy and flow cytometry. This study provide theoretical and practical guides for the further application of curcumin nanoliposomes.

Treatment with direct-acting antivirals (DAAs) cures most patients infected with hepatitis C virus (HCV) in the real world. However, some patients, especially those with the underlying advanced liver disease, have a limited reduction of liver injury after achieving a sustained viral response (SVR). Bicyclol was widely used in clinics for the treatment of a variety of liver injuries but with an unknown mechanism for the treatment of hepatitis C. We investigated the anti-inflammatory effects and mechanisms of bicyclol in HCV-infected hepatocytes and further confirmed the putative results in a mouse hepatitis model induced by the coinjection of polyinosinic-polycytidylic acid [poly (I:C)] and D-galactosamine (D-GalN). The results showed that the activation of nuclear factor kappa B (NF-κB) and the subsequent increase of inflammatory factors were directly induced by HCV infection and were persistent after clearance of the virus in Huh7.5 cells. Bicyclol decreased the activation of NF-κB and the levels of inflammatory factors in HCV-infected hepatocytes by inhibiting the activation of the ROS-MAPK-NF-κB pathway, and the effect was synergistic with DAAs in HCV-infected hepatocytes. Bicyclol attenuated the ROS-MAPK-NF-κB axis
recovering mitochondrial function without a dependence on dihydronicotinamide adenine dinucleotide phosphate oxidase and superoxide dismutases. The anti-inflammatory effects and mechanism of bicyclol were verified in mouse hepatitis induced by the coinjection of poly(I:C)/D-GalN. Bicyclol directly ameliorates the chronic inflammation caused by HCV infection and might be used with DAAs or after DAA therapy for ultimately curing chronic hepatitis C.

To overcome the deficiencies of weak local search ability in genetic algorithms (GA) and slow global convergence speed in ant colony optimization (ACO) algorithm in solving complex optimization problems, the chaotic optimization method, multi-population collaborative strategy and adaptive control parameters are introduced into the GA and ACO algorithm to propose a genetic and ant colony adaptive collaborative optimization (MGACACO) algorithm for solving complex optimization problems. The proposed MGACACO algorithm makes use of the exploration capability of GA and stochastic capability of ACO algorithm. In the proposed MGACACO algorithm, the multi-population strategy is used to realize the information exchange and cooperation among the various populations. The chaotic optimization method is used to overcome long search time, avoid falling into the local extremum and improve the search accuracy. The adaptive control parameters is used to make relatively uniform pheromone distribution, effectively solve the contradiction between expanding search and finding optimal solution. The collaborative strategy is used to dynamically balance the global ability and local search ability, and improve the convergence speed. Finally, various scale TSP are selected to verify the effectiveness of the proposed MGACACO algorithm. The experiment results show that the proposed MGACACO algorithm can avoid falling into the local extremum, and takes on better search precision and faster convergence speed.

Nonlinear association between hematocrit levels and short-term all-cause mortality in ICU patients with acute pancreatitis: insights from a retrospective cohort study

Objectives
The purpose of this study was to investigate the relationship between hematocrit levels and the mortality of patients with acute pancreatitis (AP), since limited research has examined this association in intensive care unit (ICU).
Methods
In this study, clinical data were retrieved from Medical Information Mart for Intensive Care database for patients diagnosed with AP. Nonlinear relationships between hematocrit and prognosis were examined through Locally Estimated Scatterplot Smoothing (LOESS) regression, restricted cubic splines (RCS), and U-test analyses. The impact of hematocrit on prognosis was further explored using with a binomial generalized linear model with a logit link, while adjusting for potential confounding factors.
Results
The study encompassed 1,914 patients with AP, revealing a significant difference in hematocrit levels between survivors and non-survivors (33.6 (29.5, 38.1) vs. 32.1 (28.1, 37.4),
P
< 0.001). Hematocrit emerged as an independent prognostic indicator for mortality in both univariate and multivariate logistic regression analyses (all
P
< 0.05). Findings from LOESS regression, RCS regression, and the U-test indicated a U-shaped correlation between hematocrit levels and 28-day mortality, with both elevated and decreased hematocrit levels leading to increased mortality risk (
P
for overall < 0.001). Tertile grouping revealed that lower hematocrit levels (< 30.8%) were associated with heightened 28-day mortality risk (Crude model: Odds ratio (OR) (95%Confidence Interval (CI)) = 1.665 (1.198–2.314); fully adjusted model: adjusted OR = 1.474 (1.005–2.161), all

< 0.05). Survival analyses further supported the adverse prognosis associated with low hematocrit levels.
Conclusions
The findings of this study indicate that in AP patients in the intensive care unit, only low HCT levels were identified as a risk factor for 28-day mortality, despite the presence of a U-shaped correlation between HCT levels and 28-day all-cause mortality.

A comprehensive review on microbial production of 1,2-propanediol: micro-organisms, metabolic pathways, and metabolic engineering

1,2-Propanediol is an important building block as a component used in the manufacture of unsaturated polyester resin, antifreeze, biofuel, nonionic detergent, etc. Commercial production of 1,2-propanediol through microbial biosynthesis is limited by low efficiency, and chemical production of 1,2-propanediol requires petrochemically derived routes involving wasteful power consumption and high pollution emissions. With the development of various strategies based on metabolic engineering, a series of obstacles are expected to be overcome. This review provides an extensive overview of the progress in the microbial production of 1,2-propanediol, particularly the different micro-organisms used for 1,2-propanediol biosynthesis and microbial production pathways. In addition, outstanding challenges associated with microbial biosynthesis and feasible metabolic engineering strategies, as well as perspectives on the future microbial production of 1,2-propanediol, are discussed.

Fostering Solidarity and Encouraging Shared Commitment: Chongqing Daily’s Narratives about the Front-line Workers and the Role of these Narratives in Containing Covid-19

This article analyses Chongqing Daily’s narratives about the front-line workers during the pandemic outbreak and their role in containing the Covid-19. These narratives foreground the disruption of filial connections and family life of those on the front lines as well as the physical toll their work takes on the bodies of these workers. By highlighting the suffering of front-line workers through familiar experiences relating to the body and family connections, this article argues that these narratives enabled inter-subjective connections between the front-line workers and the rest of the population and, therefore, helped foster a feeling of solidarity among the population in controlling the virus. The descriptions about the practices of front-line workers in the quarantined areas and their social moral meaning also functioned as action models and explanatory frameworks for the public to rationalize and assign meaning to the behaviour required of them during the lockdown period, and thus encouraged a shared commitment to combating the virus. Thus, the Chongqing Daily narratives strengthened the social cohesion in Chongqing and helped unite the population for a total war against the pandemic.
