Cardiac involvement in eosinophilic granulomatosis with polyangiitis (churg-strauss disease) : the role of cardiovascular magnetic resonance
Eosinophilic granulomatosis with polyangiitis (EGPA), previously known as Churg-Strauss disease, is a rare vasculitis that affects small- to medium-sized vessels and has a propensity to involve the heart. Patients with cardiac involvement have a poor prognosis and usually require immunosuppressive treatment along with corticosteroids. Cardiovascular magnetic resonance (CMR) is a non-invasive diagnostic tool for detecting cardiac involvement and guiding the management plan. We report a 39-year-old male patient with a known history of bronchial asthma who was referred to the chest clinic at a tertiary hospital in 2019 for further assessment of persistent lung parenchymal changes on chest computed tomography. Given the clinical context of the patient and the radiological findings, EGPA was suspected and confirmed with a lung biopsy. CMR was performed for further assessment, which confirmed cardiac involvement. The patient was started on prednisolone and azathioprine and showed significant radiological and clinical improvement.
واقع ممارسة العمليات الإدارية في المنظمات غير الحكومية :‏ ‏‏دراسة ميدانية مطبقة على جمعيات المرأة العمانية بسلطنة عمان
هدفت الدراسة للوقوف على أبرز خصائص أعضاء مجالس الإدارة بالمنظمات غير الحكومية بسلطنة عمان، وتعرف واقع ممارسة عمليات الإدارة في هذه المنظمات وهي التخطيط التنسيق، التنظيم التدريب التمويل). وقد سعت الدراسة كذلك إلى تحديد أبرز التحديات العامة المؤثرة على كفاءة ممارسة هذه العمليات، وكذلك الآليات الممكنة لتفعيل ممارستها لدى المنظمات.وتعد الدراسة الراهنة دراسة وصفية تعتمد على المسح الاجتماعي بنوعيه الشامل والعينة؛ حيث طبقت الدراسة على جميع جمعيات المرأة العمانية كإحدى المنظمات غير الحكومية بالسلطنة في محافظتي مسقط وظفار . وقد شارك في الدراسة ۱۲ جمعية من أصل ١٤ بنسبة مشاركة بلغت حوالي ٨٦% ، كما طبقت الدراسة على ۹۸ من عضوات مجالس الإدارة بتلك الجمعيات من أصل ۱۲٥ بنسبة مشاركة بلغت حوالي ۷۸ ، وقد اشتملت أدوات جمع البيانات على أداتي : الاستبيان والاستبار لتحقيق أهداف الدراسة.وقد خرجت الدراسة بنتائج عديدة أبرزها أن نسبة كبيرة من المبحوثات يحملن مؤهل دبلوم عام (ثانوي) فما دون بنسبة وصلت إلى ۷۹ ، وأن متوسط أعمار المبحوثات بلغ ٣٤,٣ سنة، كما أظهرت الدراسة وجود ارتفاع نسبي لخبرات المبحوثات بمتوسط خبرة بلغت 5 سنوات، وأن أغلبية المبحوثات قد حصلن على دورات تدريبية بنسبة وصلت إلى ٦٩,٤% بمتوسط دورات بلغ ۳,۱۳ دورة. كذلك أكدت نتائج الدراسة أن أغلبية المبحوثات يندرجن تحت فئة (لا أعمل) بنسبة ٠٦٨,٤وفيما يخص نتائج ممارسة العمليات الإدارية، فقد أكدت نتائج الدراسة أن الأداء العام للمنظمات في هذه العمليات بلغ ٧٤,٧% وهي درجة تقع ضمن مستوى \"الأداء المتوسط\" وفق معيار التصنيف المحدد . وقد أكدت النتائج أيضاً أن أفضل أداء للمنظمات كان في عمليتي التنسيق والتخطيط على التوالي، بينما جاءت عمليتي التنظيم والتمويل على التوالي كأقل العمليات أداء لدى المنظمات. وقد أظهرت نتائج اختبار (T-test) وجود فروق دالة إحصائيا في ممارسة بعض عمليات الإدارة بين المنظمات في محافظتي مسقط وظفار ولصالح الأخيرة، كما كشفت نتائج اختبار (ONE WAY ANOVA) تأثيرا إحصائيا لمتغيرات: (العمر - ومستوى التعليم والحالة الوظيفية على ممارسة بعض العمليات، كما أظهرت النتائج أن ضعف الدعم المادي يعد أكبر التحديات المؤثرة على كفاءة عمليات الإدارة. وخلصت الدراسة إلى مجموعة من المقترحات مقرونة بآليات لتنفيذها بحيث يمكن أن تسهم في تفعيل ممارسة عمليات الإدارة في المنظمات غير الحكومية بالسلطنة.
Safety of transurethral resection of large prostate
Background: Benign prostatic hyperplasia is a common benign disease occurs in older men. Some patients can be treated medically but eventually, most of them will need a surgical intervention, and the most commonly applied procedure is transurethral resection of the prostate (TURP). Objectives: The objective of this study is to assess the feasibility and safety of performing transurethral resection of large prostate (80 g and more). Methodology: Out of 153 patients reviewed 48 cases included in this study. The main data collected from patients' files and patient interview. The criteria of exclusion were prostate size <80 g and previous history of TURP. The collected data were analyzed by the Statistical Package for the Social Sciences (SPSS). Results: The main results showed that 93.7% of patients did not experience major bleeding postoperatively, neither major drop in hemoglobin level. Moreover, the patient's distribution according to the presence of TUR syndrome was only 2.1% with mild symptoms. No patient had an episode of retention during the hospital stay or in the follow-up. Conclusion: Surgeon experience, systematic resection approach, and strict time of resection are important factors to assure the safety of TURP in large prostate. In cases of huge prostate size >100 g, staged TURP can be offered safely or if patients' obstructive symptoms do not resolve after the first procedure.
Postoperative complications in living liver donors : a retrospective study, single-center experience in Saudi Arabia
Living donor liver transplantation (LDLT) offers life to patients with end-stage liver disease. The balance between the benefit to the recipient and the risk to the donor plays a central role in justifying LDLT. However, the incidence rates of complications posttransplant differ widely. This study is designed to identify postoperative complications in LDLT in a tertiary care center King Abdulaziz Medical City (KAMC). This was a retrospective cohort study. All donors at KAMC between January 2003 and December 2015 were reviewed through a hospital database and patient charts to determine the postoperative complications based on the modified Clavien classification system. All donors were relatives of the recipients and assessed before the surgery. A total of 101 donors underwent LDLT: 75 were male and 26 were female, with a mean age of 27.7 ± 6.6. The breakdown of specific surgical procedures was as follows: 65 (64.3%) donors underwent right hepatic lobectomy, 31 (30.6%) underwent left lateral hepatectomy, three (2.97%) underwent extended right hepatectomy with the inclusion of the middle hepatic vein, and two (1.98%) underwent left hepatectomy. Postoperative complications were determined in 20 patients (19.8%), but no mortality was observed. Complications were reported in 14 (21.5%) right and six (19.4%) left lateral hepatectomy donors. A total of 12 patients had Grade I complications, six patients had Grade II complications, and Grade III complications were reported in two cases. The most frequent complications were upper limb weakness to brachial plexus neuropathy and mild bile leak. Life-threatening compli- cations in our center have not been reported in LDLT; however, some donors may experience postoperative morbidity, which usually were mild and had a good prognosis.
Age of initiation and the factors associated with toilet training in healthy Saudi children
With the appropriate knowledge of the age to initiate toilet training and acknowledging the factors that influence it, parents can prevent many health issues related to voiding. Our study aimed to determine the average age of the initiation of toilet training among Saudi children and the factors that may affect this training. This study was carried out by distributing questionnaires to mothers who attended various clinics in our institute. A sample included 1000 healthy Saudi children. The mean age of the initiation of toilet training in healthy children was 25.6 ± 9.4 months while the median was 24 months, the mean duration of training was 6.3 ± 9.1 months. Most children (92%) were trained in the summer months. Mother’s educational level, number of siblings, and socioeconomic status were significant factors that affected the age of initiation. However, mother’s occupation, caregiver, and the order and gender of the child did not have a significant effect on the initiation of toilet training. The median age of toilet training among our children matched other studies. Summer seems to be the preferred season to initiate training.
